  • Adult Diameter Artificial Vascular for Right Pulmonary Artery Originated from Ascending Aorta

    ObjectiveTo summarize our experience of surgical treatment for right pulmonary artery originated from the ascending aorta by using adult diameter artificial vascular and study the operative indication, design, method, and therapeutic efficacy. MethodsWe retrospectively analyzed clinical data of 11 patients with right pulmonary artery originated from ascending aorta in The Second Affiliated Hospital of Harbin Medical University from May 2008 through December 2013, who were treated by using adult diameter artificial vascular. The patients ranged from 4 months to 25 months old, weighted 4-15 kg. Among of them, 4 patients had persistent truncus arteriosus and 7 had aortopulmonary septal defect. All patients were complicated with moderate pulmonary hypertension. All the patients underwent one stage surgical repair under extracorporeal circulation and cardiac arrest. During the surgery, end to side anastomosis was done between the right pulmonary artery and 16-18 mm diameter artificial blood vessels. And artificial blood vessel was connected to the main pulmonary artery or right ventricle outflow tract incision from the aorta above. ResultsThe average operation time was 179-325 (224±68) min. The average cardiopulmonary bypass (CPB) time was 81-208 (117±54) min. The average aortic clamping time was 29-63(42±21) min. The mean residence time in ICU was 71-197 (109±42) hours. The average assisted mechanical ventilator time was 59-191 (91±26) hours. The average length of stay in hospital was 21-39 (28±11) days. Low cardiac output syndromes caused by pulmonary arterial hypertension occurred in 5 patients including 2 deaths and 3 patients with good recovery by reducing the pulmonary arterial pressure and peritoneal dialysis. The result of postoperative cardiac color ultrasound examination of 9 survival patients showed vascular prosthesis, no distortion, no stenosis of the anastomosis, deformity correction satisfaction. Nine patients were followed up for 3-60 months. The results of echocardiography showed no anastomosis and artificial vascular stenosis, and the pulmonary arterial pressure decreased significantly. ConclusionThe right pulmonary artery originated from the ascending aorta in children should be operated as soon as possible. Compared the adult diameter artificial vascular treatment for one stage repair of right pulmonary artery from the ascending aorta with other operation methods, both short-term and long-term effects are good. Postoperative low cardiac output syndrome is a common complication.

  • Effectiveness and Safety of Misoprostol versus Gemeprost Before the First and Second-trimester Induction of Abortion Operation:A Systematic Review

    Objective To evaluate the effectiveness and safety of misoprostol versus gemeprost for the first and second-trimester induction of abortion. Methods We searched randomized controlled trials(RCTs) and quasi-randomized controlled trials of misoprostol versus gemeprost for the first and second-trimester induction of abortion operation. MEDLINE (1974 - 2003 ), EMBASE (1974 - 2003 ), The Cochrane Library (Issue 3, 2003 ), CBM (1979 - 2003 ), CNKI (1989 - 2003 ), CMCC (1994 - 2003 ), VIP (1989 - 2003 ) were searched. Twelve journals and reference lists of eligible studies were handsearched. Two reviewers independently screened the studies for eligibility, evaluated the quality, and extracted data from eligible studies with confirmation by cross checking. Any disputes were decided by a third person. Meta-analysis was conducted using RevMan 4.2 software. Results Seven RCTs were included, three for the first-trimester and four for the second- trimester. Statictical differences were tested between the misoprostol and gemeprost groups with OR 4. 79,95% CI 1.35 to 16.94 in laxation and dilation rate,with WMD -1.50,95% CI -3.00 to 0.00 in operation time for the first-trimester abortion. However, there was no statistical difference in the amount of bleeding during operation with WMD - 14. 40, 95% CI 30.30 to 1.30. For the second-trimester abortion, compared with gemeprost, no consistent results of gravid excluding time were obtained. Compared with gemeprost, the rate of complete abortion was higher and digest system morbidity was lower in misoprostol alone group. But there was no statistical difference between the two groups when they were both combined with mifestone.Conclusions Some evidence indicates that compared with gemeprost, misoprostol may increase laxation and dilation rate of the cervix, shorten gravid exclusion time, improve the rate of complete abortion and make the laxation and dilation range of the cerix larger than gemeprost before the first and second-trimester abortive surgery. Due to the hmited evidence indentified, we can not draw a b conclusion. More radomiased controlled trails of high quahty are required.

  • Clinical Application of Artificial Pneumoperitoneum and Gastrointestinal Contrast CT Imaging in Diagnosis of Abdominal Wall Adhesion to Intestine after Operation

    ObjectiveTo explore the feasibility and safety of the artificial pneumoperitoneum and gastrointestinal contrast CT imaging, and imaging diagnostic value on abdominal wall adhesion to intestine after operation. MethodsThirtynine patients with adhesive intestinal obstruction after operation relieved by conservative therapy were included from January 2008 to November 2009. After the artificial pneumoperitoneum established by injection of gas into abdominal cavity and gastrointestinal comparison by oral administration low concentration of meglucamine diatrizoate, CT scan imaging was performed and the radiographic results were compared with surgical findings. ResultsFour patients refused surgery and discharged, so enterolysis was performed in the remaining patients. The surgical findings were consistent with radiographic results. It was showed by laparoscopic operation that intestinal obstruction caused by the fibrous adhesions and the intestine did not adhere to the abdominal wall in eight patients with fibrous adhesion diagnosed by CT. Of eighteen patients with the abdominal wall septally adhered to the intestinal, the surgical findings showed the intestine and the abdominal wall formed “M”type adhesions and omentum adhesions in sixteen patients underwent open operation, and clear fat space was showed in eight patients and close adhesion was found in another eight patients between the intestine and abdominal wall. Of thirteen patients with the abdominal wall tentiformly adhered to the intestinal, the surgical findings showed the intestine and the abdominal wall formed continuous and tentiform adhesions and omentum adhesions to the intestine in eleven patients. After the followup of 6-18 months (mean 9 months), incomplete intestinal obstruction occurred in one patient and was relieved by conservative treatment. One patient with discontinuous discomfort in abdomen after operation did not receive any treatment. The other patients were cured. ConclusionThe artificial pneumoperitoneum and gastrointestinal contrast CT imaging can accurately show the location, area, and structure composition of the postoperative abdominal wall adhesion to intestine, which is safety, simple, and bly repeatable, and a better imaging method for the diagnosing of abdominal wall adhesion to intestine after operation.

  • Effect of a High Positive End-Expiratory Pressure Ventilation Strategy in Patients with ARDS: A Systematic Review

    Objective To compare the effects of high and low positive end-expiratory pressure( PEEP) levels on mortality and risk of barotrauma in patients with acute respiratory distress syndrome ( ARDS) . Methods Randomized controlled trials ( RCTs) were recruited from PubMed( 1966-2008. 9) ,EMBASE( 1980-2008. 9) , Cochrane Database ( Issue 2, 2008) , Chinese Cochrane Centre Database and CBMdisc ( 1978-2008. 9) . Related published and unpublished data and attached references were hand searched. All RCTs about ventilation with PEEP for patients with ARDS were included, then a systematic review were performed. Results Five eligible trials were enrolled in the systematic review. According to ventilation strategy, all trials were divided into subgroup A( low tidal volumes + high PEEP vs traditional tidal volumes + low PEEP) and subgroup B( low tidal volumes + high PEEP vs low tidal volumes + low PEEP) . In subgroup A, high PEEP was associated with a lower mortality[ RR 0. 59, 95%CI( 0. 43, 0. 82) ] and a lower prevalence of barotraumas [ RR 0. 24, 95% CI( 0. 09, 0. 70) ] in patients with ARDS. In subgroup B, the difference in mortality[ RR 0. 97, 95%CI( 0. 83, 1. 13) ] and barotraumas[ RR 1. 13, 95% CI( 0. 78, 1. 63) ]were not significant. Conclusions As compared with conventional ventilation, low tidal volumes and high PEEP ventilation strategy is associated with improved survival and a lower prevalence of barotraumas in patients with ARDS. It is necessary to further confirm the role of sole high PEEP in the ventilation strategy.

  • Use of Reactance to Assess Airway Obstruction in Severe COPD Patients and Effect of Noninvasive ventilation

    Objective To assess the effect of continuous positive airway pressure ( CPAP) on expiratory flow limitation in severe chronic obstructive pulmonary disease ( COPD) patients during noninvasive ventilation by oscillatory reactance ( Xrs ) . Methods Eight patients with stable COPD and chronic hypercapnic respiratory failure( type II) received noninvasive ventilation with a traditional CPAP ventilator through a nasal mask were enrolled. The CPAP were successively set as 4, 8 and 12 cm H2O respectively. The forecd oscillation( 5 Hz, 2 cm H2O) was imposed into the mask and the flow and nasal pressure were measured at the airway opening. The difference between inspiratory and expiratory Xrs( ΔXrs)were calculated for each breathing cycle and average ΔXrs was calculated at different pressure levels according to the established algorithm. Meanwhile, the oesophageal pressure was also measured by a balloontipped catheter and transpulmonary pressure was calculated. The breathing cycles above were analyzed subsequently and classified as expiratory flow-limited( EFL) and non-EFL breath. In addition, flow and nasal pressure when breathing naturally( CPAP = 0 cmH2O) was also collected for each patient and the EFL breath cycles was identified as baseline. Then, the percentage of EFL breathing cycles and ΔXrs were calculated for each CPAP level and their relationship was analyzed. The threshold value of ΔXrs with maximum sensitivity and specificity to detect EFL and the optimal CPAP to suppress the development of EFL were computed. Results ①CPAP increased from4 to 8 and 12 cm H2O resulted in fall of mean values of ΔXrs from2. 67 to 1. 62 and 1. 31 cm H2O· s- 1 · L- 1 , respectively( ΔXrs at CPAP 0 cm H2O was not detected) , and the decrease of ΔXrs when CPAP up to 8 cm H2O from 4 cm H2 O was significant ( Z = - 2. 68, P = 0. 01) . ②CPAP significantly suppressed the development of EFL, when CPAP increased from0 cm H2O to 4,8 and12 cmH2O resulted in decrease in the percentage of breathing cycle from 29. 8% to 9. 9% , 8. 1% and 4. 4%, respectively(  2 = 15. 6, P = 0. 01) . ③ ΔXrs was related to the degree of EFL and the mean value of ΔXrs in EFL breathing cycles was significantly higher than that in non-EFL’s. When ΔXrs decreased to 1. 83 cm H2O· s- 1 · L- 1, the majority of breath showed non-EFL, with a sensitivity of 94% and specificity of 97% for detecting EFL, respectively. Conclusions ΔXrs is an indicator of the occurrence of EFL. Appropriate CPAP to render the value of ΔXrs equal to or slightly less than 1. 83 cm H2O·s - 1 ·L- 1 may effectively suppress the development of EFL in severe COPD patients during noninvasive ventilation.

  • Clinical Analysis of Patients Undergoing Mitral Valvuloplasty for Anterior Mitral Leaflet Prolapse with Looped Artificial Chordae

    Objective To summarize surgical techniques,advantages and clinical outcomes of mitral valvuloplasty for anterior mitral leaflet prolapse with looped artificial chordae. Methods Clinical data of 13 patients with anteriormitral leaflet prolapse and severe mitral regurgitation (MR) who underwent mitral valvuloplasty with looped artificial chordaefrom January 2009 to December 2011 in Beijing Anzhen Hospital were retrospectively analyzed. There were 8 male and 5 female patients with their age of 21-61 (39.5±12.9) years. There were 10 patients with anterior mitral leaflet chordal rupture and 3 patients with anterior mitral leaflet elongation. Preoperative left ventricular end-diastolic diameter (LVEDD) was 52-65 (58.3±1.7) mm,and left ventricular ejection fraction (LVEF) was 53%-65% (58.8%±2.8%). All the patients underwent mitral valvuloplasty. We measured the neighboring normal chordae with a caliper for reference and constructed the artificial chordal loops on the caliper with expended polytetrafluoroethylene(ePTFE) CV4 Gore-Tex suture lines. Three to five loops were made and fixed to the papillary muscle with a Gore-Tex suture line and the free edge of the prolapsedanterior mitral leaflet with another Gore-Tex suture line,with the intervals between the loops of 5 mm. Left ventricular watertesting was performed to evaluate MR status,annuloplasty ring implantation or “edge to edge” technique was used if nece-ssary,and left ventricular water testing was performed again to confirm satisfactory closure of the mitral valve. Patientsreceived re-warming on cardiopulmonary bypass and the heart incision was closed. The effect of mitral annuloplasty was alsoassessed by transesophageal echocardiography (TEE) after heart rebeating. Warfarin anticoagulation was routinely used for 3 months after discharge. Results There was no perioperative death in this group. Twelve patients received satisfactory outcomes after 1-stage mitral valvuloplasty with looped artificial chordae and annuloplasty ring implantation. One patient didn’t receive satisfactory outcomes in the left ventricular water testing after mitral valvuloplasty with looped artificial chordae,but satisfactory outcome was achieve after “edge to edge” technique was used,and annuloplasty ring was not used for this patient. Postoperative echocardiography showed trivial to mild MR in all the patients,their LVEDD was significantly reducedthan preoperative LVEDD (47.5±2.1 mm vs. 58.3±1.7 mm,P<0.05),and there was no statistical difference between postoperative and preoperative LVEF(58.5%±2.6% vs. 58.8%±2.8%,P>0.05). All the patients were followed up for 3-36 (19.5±3.7) months. Echocardiography showed mild MR in 4 patients and none or trivial MR in 9 patients during follow-up.Conclusion Mitral valvuloplasty with looped artificial chordae is an effective surgical technique for the treatment of anterior mitral leaflet prolapse with satisfactory clinical outcomes,and this technique is also easy to perform.

  • Mitral Valve Repair with Artificial Chordae in 78 Patients with Mitral Valve Prolapse

    Objective To analyze early and midterm outcomes and summarize clinical experience of mitral valve repair with artificial chordae for degenerative mitral regurgitation (MR). Methods Clinical data of 78 patients with degenerative MR who underwent mitral valve repair with Gore-Tex artificial chordae from October 2008 to December 2011 in General Hospita1 of Shenyang Military Command were retrospectively analyzed. There were 47 male patients and 31female patients with their age of 52.7±9.6 years,who all had degenerative MR. Operation techniques included simple GoreTex artificial chordae replacement in 15 patients,artificial chordae replacement plus quadrangular resection of the posterior leaflet in 58 patients,artificial chordae replacement plus quadrangular resection of the posterior leaflet and Sliding technique in 5 patients. One to three (2.15±1.05) Gore-Tex artificial chordae were used for each patient,and annuloplasty ring was used for all the patients. Thirty-nine patients underwent concomitant tricuspid valvuloplasty. Intraoperative transesophagealechocardiography showed none obvious MR in 62 patients,trivial MR in 13 patients and mild MR in 3 patients. All thepatients were followed up after discharge. Echocardiography was used to evaluate heart function and MR degree duringfollow-up. Results There was no in-hospital death in this group. Postoperative complications included sinus bradycardiain 5 patients,supraventricular tachycardia in 8 patients,late cardiac tamponade in 1 patient,and permanent pacemakerimplantation in 1 patient. Seventy patients were followed up for 1-2 years with the follow-up rate of 89.74% (70/78). Duringfollow-up,1 patient died of cerebral embolism 13 months after discharge,and all the other patients remained alive. There were 60 patients with NYHA classⅠand 9 patients with NYHA classⅡ. Echocardiography at 1 year after dischargeshowed that left atrial diameter,left ventricular end-diastolic diameter,left ventricular end-systolic diameter,and pulmonary artery systolic pressure were significantly smaller or lower than preoperative values (P<0.05),left ventricular ejection fraction (68.00%±7.00% vs. 55.00%±6.00%) and cardiac output were significantly higher than preoperative values(P<0.05),and MR degree (ratio of regurgitation beam area and left atrial area) was significantly reduced compared with preoperative MR degree (3.45%±5.56% vs. 39.55%±9.86%,P<0.05). No artificial chordae rupture was found. There were47 patients without MR and 22 patients with trivial MR during follow-up. Conclusion Gore-Tex artificial chordae replacement is a safe and effective surgical technique for the treatment of degenerative MR.

  • Observation on Therapeutic Effect of Warfarin on 103 Pregnant Women with Prosthetic Mechanical Heart Valves Throughout Pregnancy

    Abstract: Objective To investigate the anticoagulation effect of warfarin on pregnant women with prosthetic mechanical heart valves during the whole course of pregnancy and their fetuses. Methods Followup survey was carried out on 103 pregnant women with prosthetic mechanical heart valves treated in the Second Xiangya Hospital of Central South University from April 1998 to June 2010. Their age ranged from 19 and 38 years (26.4±3.8 years). All the 103 pregnant women were given oral administration of warfarin during the whole course of pregnancy. The average dose of domestic warfarin was 3.30±0.43 mg/d (87 cases), while the average dose of imported warfarin was 2.90±1.05 mg/d (16 cases). Results None of the patients suffered from serious embolic events. One patient suffered from spontaneous peritoneal hemorrhage. There were 4 cases of intrauterine deaths, and 5 cases of fetal malformation including 1 case of Down’s syndrome and 4 cases of hydrocephalus. Six cases of low birth weight infants and 1 case of ABO hemolytic disease were also found. All the other neonates were healthy with normal weight. No pregnant women suffered from postpartum hemorrhage. Conclusion Oral administration of low dose warfarin (lt;5 mg/d) during the whole course of pregnancy is a relative safe and effective anticoagulation protocol.

  • Analysis of Mid Term Results of Total Thoracoabdominal Aortic Replacement: Report of 63 Cases

    Objective To investigate the further results of thoracoabdominal aortic aneurysm (TAAA) repair, and analyze the midterm results of 63 cases treated by total thoraco abdominal aortic replacement with a tetrafurcate graft. Methods From August 2003 to October 2007,total thoracoabdominal aortic replacement with a tetrafurcate graft was performed in 63 consecutive patients with Crawford Ⅱ TAAA in Fu Wai Hospital. There were 46 male and 17 female with a mean age of 39.98 years (17-71 years). All the procedures were performed through combined thoracoabdominal incision via the retroperitoneal approach and underwent profound hypothermia with shorttime interval circulatory arrest. T6 to T12 intercostal arteries were reconstructed by arterial tube technique. The celiac artery, superior mesenteric artery and right renal artery were joined into a patch and anastomosed to the end of the main graft. Left renal artery was anastomosed to an 8 mm branch or joined to the visceral arterial patch. The other 10 mm branches were anastomosed to iliac arteries. KaplanMeier method was used to perform survival analysis. Results All the cases were followed,and the mean followup time was 36.57(8-57) months. No patient died during the operation. Early mortality rate was 7.94%(5/63). Among them, 4 patients died of multiple organ failure. Two of them were caused by neurological complications, and the other 2 of them were caused by renal failure. One patient died of low cardiac output syndrome after surgery because of coronary artery disease. This patient underwent coronary artery bypass grafting (CABG) emergently, but couldn’t wean from cardiopulmonary bypass. The incidence of stroke and temporary neurological dysfunction was 9.52%(6/63), 4 of them were temporary neurological dysfunction and were cured before discharged from hospital. Paraplegia and paraparesis occurred in 2 and 1, respectively. They were all [CM(158.3mm]cured before leaving hospital. Pulmonary complication was 25.40%(16/63), and12 of them were cured. Pseudoaneurysmal change was observed in reconstructed intercostal arteries in 2 patients with Marfan syndrome, but neither of them underwent paraplegia or paraparesis. One patient died at 20th, 23rd, 30th month after discharge, respectively. The survival time of this group was 50.64±2.13 months(95%CI:46.47,54.84 months) with a survival rate of 92.06% after 1 year, 88.38% after 2 years, 86.11% after 3 years. Conclusion Using tetrafurcate graft is a reliable method in total thoracoabdominal aortic replacement and has a satisfactory midterm survival rate. The intercostal arteries reconstruction by arterial tube technique in total thoracoabdominal replacement is simple, and it is helpful in spinal cord protection.

  • Progress in Studies of Seeding Genetically Engineered Endothelial Cell on Artificial Vascular Graft

    The autograft and non-autograft cannot meet the needs of clinical vascular surgery. Since there are possibilities of thrombus formation in artificial vascular grafts, the methods for deposing the graft using physical and chemical ways or simply seeding with endothelial cells cannot produce satisfactory grafts for vascular operations until now. In order to increase the anticoagulative capacity of artificial vascular graft, it is rational to use genetic engineering methods modifying the endothelial cells to make it express anticoagulative factors stably. Although seeding artificial graft with the genetically engineered endothelial cells can possibly produce a satisfactory graft for vascular surgery, some problems still need to be solved.
