  • Autologous Transplantation of Stem Cells for Diabetic Peripheral Neuropathy: A Systematic Review

    Objective To systematically review the effectiveness and safety of autologous implantation of stem cells for diabetic peripheral neuropathy (DPN). Methods Randomized controlled trials on relevant studies were retrieved in databases including CBM (1978-2011.6), CNKI (1979-2011.6), MEDLINE (1950-2011.6), PubMed (1950-2011.6), EMbase (1970-2011.6) and The Cochrane Library (Issue 3, 2011). References of the included studies were also retrieved. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data, and assess the methodological quality of the included studies. Then, meta-analysis was performed using RevMan 5.0 software.Results Four RCTs involving 68 patients (136 limbs) were included, most of which were low in methodological quality. The results of meta-analysis indicated that, autologous stem cell therapy improved or even eliminated DPN symptoms including pain, numbness, and cold sensation in the limbs, intermittent limping, and rest pain. Compared with the routine therapy, autologous stem cell therapy improved tibial sensory nerve conduction velocity (MD=5.75, 95%CI 3.86 to 7.64, Plt;0.000 01), tibial motor nerve conduction velocity (MD=4.04, 95%CI 0.90 to 7.18, P=0.001), sural sensory nerve conduction velocity (MD=7.47, 95%CI 4.00 to 10.94, Plt;0.000 1), and sural motor nerve conduction velocity (MD=3.38, 95%CI 0.07 to 7.58, P=0.05), with no adverse reaction reported. Conclusion Current evidence shows that, autologous stem cell therapy is effective in treating DPN. Due to the lack of high quality studies, more high quality RCTs are needed to verify the above conclusion.

  • Expression and Significance of Early Growth Response Gene-1 in Autogenous Vein Graft in Rats

    Objective To investigate the development and significance of the expression of early growth response gene-1 (EGR-1) in autogenous vein graft in rats and detect the role of it in intimal hyperplasia. Methods Autogenous vein graft model was established in 90 Wistar rats, transplanting the right jugular vein to infra renal abdominal aorta by microsurgical technique. The vein graft samples were harvested at hour 1, 2, 6 and 24, day 3, 7,14, 28 and 42 after procedure. Normal vein as control group. Egr-1 mRNA was measured by reverse transcription-PCR and in situ hybridization. Western blot and immunohistochemistry were used to detect the protein expression of Egr-1. Results Intimal hyperplasia reached peak at day 28 after autogenous vein graft surgery. Egr-1 mRNA and Egr-1 protein hadn’t been found in the normal vein. The expressions of Egr-1 mRNA and Egr-1 protein had biphasic changes. By reverse transcription-PCR and in situ hybridization, we found that the level of Egr-1 mRNA rose at 1 hour after graft, the expression of Egr-1 mRNA was (35±7)%. Decline at hour 6, 24 and day 3, the positive rates of Egr-1 mRNA were (8±2)%, (8±6)% and (8±4)% respectively. Reincrease at day 7, a peak at day 28, the positive rate of Egr-1 mRNA was (45±6)% (compared with other phase, P<0.01). At day 42, the expression of Egr-1 mRNA declined again. Immunohistochemical staining and Western blot revealed Egr-1 protein had expressed at hour 2 early phase, the expression of Egr-1 protein was (30±5)%, and until to hour 6. The level of Egr-1 protein was decrease at hour 24 and day 3, the positive rates were (7±3)% and (7±8)% respectively. A peak at day 28, the positive rate of Egr-1 protein was (40±9)% (compared with other phase, P<0.01). We found that immu-noreative Egr-1 located vascular smooth muscle cells (VSMCs) and monocytes/macrophages in tunica media at the early phase of day 7 and 14, and in neointimal and medial VSMCs at later phase of day 28. Egr-1 was also present in the endoluminal endothelial cells. Conclusion In autogenous vein graft, Egr-1 plays an important role in the proliferation of VSMCs. Egr-1 may become a new target for the prevention and therapy of intimal hyperplasia, stenosis and emphraxis after vein graft.

  • Effect of Radix Salviae Miltiorrhizae on Patency and Proliferation Lesion of Autologous Vein to Artery Grafts in the EarlyMiddle Stage

    ObjectiveTo assess the effects of Radix Salviae Miltiorrhizae (RSM) on patency and proliferation lesion of autologous vein to artery grafts in the earlymiddle stage.MethodsAutologous jugular vein was grafted into abdominal artery in the rats. The rats were divided into two groups: RSM group and control group. The rats in RSM group were fed with RSM [24 g/(kg·d )],which began 1 day before operation and continued until harvesting. Vein grafts were harvested at 1,3 days, 1, 2, 4 and 8 weeks after surgery for examining the patency, thickness of intimamedia and expression of proliferating cell nuclear antigen (PCNA). ResultsNo significant differences existed in patency of vein grafts between the two groups (Pgt;0.05). The intimamedia thickness of the vein grafts in RSM group decreased 1/3 compared with control group at 2, 4 and 8 weeks (P<0.01). The PCNA positive cells in RSM group reduced significantly as compared to the control group (P<0.01). ConclusionRSM can inhibit proliferation lesion of vein grafts but has no influence on patency of vein grafts in the earlymiddle stage.

  • Choice of Conduit Size and Material for Palliative Right Ventricle-to-Pulmonary Artery Shunt

    Objective To assess clinical results of three different conduit materials (Gore-Tex synthetic graft,bovinejugular vein and autologous pericardium)for palliative right ventricle-to-pulmonary artery (RV-PA) shunt,and explore the correlation between suitable conduit size and patients’ body weight and McGoon ratio. Methods We retrospectively analyzed clinical data of 24 patients with congenital heart diseases who underwent palliative RV-PA shunt in Department of Pediatric Cardiovascular Surgery of Fu Wai Cardiovascular Hospital from July 2010 to July 2012. There were 11 males and 13 females with their age ranging from 60 days to 6 years and body weight of 10.22±7.41 kg. There were 22 patients with pulmonary atresia and ventricular septal defect (PAVSD),1 patient with tetralogy of Fallot (TOF) and 1 patient with doubleoutlet right ventricle (DORV). Among different conduit materials,autologous pericardium was used for 17 patients,Gore-Texsynthetic graft was used for 5 patients,and bovine jugular vein was used for 2 patients. Conduit size and children’s body weight were analyzed with linear regression,then the equation was corrected with McGoon ratio. Results There was no perioperative death. Postoperative percutaneous saturation (SpO2)of the 24 children was 20.37%±28.33% higher than preoperative SpO2 . Electrocardiogram showed sinus rhythm in all the patients. Twenty-three patients were NYHA classⅡ,and 1 patient was NYHA classⅢ. Postoperative mechanical ventilation time of patients with autologous pericardium were significantly shorter than those of patients with other 2 materials (P=0.017). Sixteen patients were followed up from 10 months to 2 years after discharge,including 12 patients with autologous pericardium,3 patients with Gore-Tex synthetic graft and 1 patient with bovine jugular vein. During follow-up,McGoon ratio of patients with autologous pericardium,Gore-Tex synthetic graft and bovine jugular vein were 1.98±0.46,1.83±0.33 and 1.68 respectively,all of which weresignificantly higher than preoperative McGoon ratio (P<0.05). Six patients underwent radical corrective surgery,including5 patients with autologous pericardium and 1 patient with Gore-Tex synthetic graft. There was no complication directly related to surgery during follow-up. Linear regression was performed to form an equation between suitable conduit size and patients’ body weight:conduit diameter (mm)=0.327×body weight (kg)+4.599. McGoon ratio,conduit size and equationresult were compared to find a practical choice of conduit size. If McGoon ratio<0.8,the first integer greater than the equation result could be chosen. If McGoon ratio>1.2,the first integer less than the equation result could be chosen. If 1.2>McGoon ratio>0.8,the first integer either less or greater than the equation result could be chosen. Group analysis showed that patients who recovered better postoperatively were those whose conduit sizes were closer to equation results as well as equation results corrected with McGoon ratio. Conclusion All the 3 materials can be conventionally chosen for RV-PA shunt. Appropriate conduit size can be decided upon patients’ body weight and McGoon ratio for RV-PA shunt.

  • Clinical Outcomes of Tricuspid Annuloplasty Using a C-type Ring Made of Autologous Pericardium

    Abstract: Objective To evaluate clinical outcomes of tricuspid annuloplasty using a C-type ring made of autologous pericardium for the treatment of functional tricuspid regurgitation (TR). Methods Eleven patients underwent tricuspid annuloplasty in Guizhou Provincial People’s Hospital between March 2009 and January 2011, including 5 male patients and 6 female patients with their age of 32-57 (43.80±12.20) years. There were 3 patients with mild TR, 7 patients with moderate TR, and 1 patient with severe TR. Concomitant procedures included mitral valve replacement and/or aortic valve replacement and/or left atrial thrombectomy. The C-type ring was created using a strip of pericardium after 0.8% glutaraldehyde fixation for 15 minutes. Interrupted horizontal mattress suture was used to secure the C-type ring to the tricuspid annulus. Hear function and echocardiography were examined during follow-up. Results There was no in-hospital death, and the hospital stay was 15-28 (21.10±3.80) days. All the patients were followed up for 8-28 (18.50±7.00)months. There was no death or reoperation because of TR or tricuspid stenosis during follow-up. Ten patients had TR during follow-up, including 9 patients with mild TR and 1 patient with mild to moderate TR, but there was no patient with severe TR. The degree of TR during follow-up was significantly reduced than preoperative degree (Z =-2.81,P<0.05). Preoperative and postoperative right ventricular dimension (19.95±5.11 mm vs. 21.57±12.81 mm,P=0.705) and right atrial dimension(37.55±6.79 mm vs. 35.55±5.22 mm,P=0.317)were not statistically different. Conclusion Tricuspid annuloplasty using a C-type ring made of autologous pericardium has satisfactory clinical outcomes for patients with functional TR.

  • Application of Autologous Plateletpheresis Technology in Cardiovascular Surgery

    Cardiopulmonary bypass(CPB) is associated with thrombocytopenia and platelet dysfunction. The primary cause of acquired platelet defect is thought to be activation and release of alpha granules during CPB. Before CPB, platelet-rich plasma (PRP) was prepared by obtaining the required amount of patient’s whole blood by autologous plateletpheresis. PRP could be reinfused after operation in order to protect the function and quantities of the platelets. On the other hand, PRP could be made into autologous platelet gel (APG). APG contains supraphysiologic amounts of growth factors, and has adequate tensile strength and adhesive ability. Therefore, it can be used for hemostasis in operation, sealing wound and enhancing incision or dehiscent sternal wounds healing.

  • ADVANCEMENT OF ADIPOSE-DERIVED STEM CELLS ASSISTED AUTOLOGOUS LIPOTRANSFER IN BREAST REPAIR AND RECONSTRUCTION

    Objective To review the latest progress in the major biological properties of adipose-derived stem cells (ADSCs) and ADSCs assisted autologous lipotransfer in breast repair and reconstruction. Methods Recent literature about ADSCs assisted autologous lipotransfer in breast repair and reconstruction was reviewed. Results ADSCs have multipotential differentiation capacity, and they could promote angiogenesis and regulate immune reactions. ADSCs assisted autologous lipotransfer can obtain satisfactory effectiveness in breast repair and reconstruction with few complications, but more studies are needed to confirm the long-term safety. Conclusion ADSCs assisted autologous lipotransfer has good effectiveness in breast repaired and reconstruction. But further clinical trials are needed to confirm the long-term safety.

  • EFFECTIVENESS EVALUATION OF COMPOSITE SKIN GRAFTS IN REPAIRING DEEP BURN WOUNDS INFUNCTIONAL REGIONS

    【Abstract】 Objective To evaluate the long-term effectiveness of composite grafts of acellular dermal matrix (ADM)and autologous spl it-thickness skin in repairing deep burn wounds. Methods Between June 2002 and December 2008, 30 patients (42 wound) were treated. There were 25 males and 5 females, aged 3-52 years with a median age of 31 years. Of them, 24 burned patients had 35 wounds, including 23 deep second degree and 12 third degree wounds with a mean disease duration of 24 days (range, 3-45 days); 6 patients with hyperplastic scar had 7 wounds with a mean disease duration of 16 days (range, 9-21 days). The wound locations were neck (2 wound), hand (4 wounds), forearm and elbow (8 wounds), shoulder (3 wounds), poples (6 wounds), laps (4 wounds), ankle and legs (15 wounds), and the area of wounds ranged from 10 cm × 10 cm to 30 cm × 20 cm. After thorough debridement, tangential excision, and scar excision, ADM and autologous spl it-thickness skin graft were used to repair the wounds by one-step method. Results After operation, composite skin graft survived completely in 39 wounds of 27 patients, with a survival rate of 92.9%; partial necrosis occurred in 3 wounds of 3 patients (7.1%), and healed after dressing change and secondary skin graft. The patients were followed up 30-34 months (mean, 32 months) postoperatively. The appearance of the composite grafts were smooth and soft with good elasticity and low pigmentation. The activity and function of limbs recovered well. No scar hyperplasia was observed at the donor sites. Conclusion It can achieve good outcomes in appearance and function to use ADM and autologous spl it-thickness skin graft for repairing deep burn wounds in functional regions.

  • AN EXPERIMENTAL STUDY ON EFFECT OF AUTOLOGOUS PLATELET-RICH PLASMA ON TREATMENT OF EARLY INTERVERTEBRAL DISC DEGENERATION

    Objective Platelet-rich plasma (PRP) can stimulate intervertebral disc cell proliferation, promote extracellular matrix synthesis, and inhibit annulus fibrosus cell apoptosis. To investigate the effects of autologous PRP on the treatment of the early intervertebral disc degeneration (IDD) so as to provide the experimental basis for its clinical application. Methods Forty-five healthy New Zealand white rabbits (male or female, weighing 2.5-3.0 kg) were randomly divided into the experimental group (n=15), the control group (n=15), and the sham group (n=15). PRP was prepared from the arterial blood of rabbit’s ears of the experimental group with Landesberg’s method. The platelet concentrations in both whole blood and PRP were detected. The rabbit model of early IDD was established by annulus fibrosus puncture (L4, 5, L5, 6) in both the experimental group and the control group; 100 ?L autologous PRP and 100 ?L PBS were injected into the degenerative intervertebral discs respectively after 2 weeks of models creation. In sham group, intervertebral discs were separated and exposed without treatment. The general conditions of the rabbits were observed after building models; at 2 weeks after degeneration, 1 and 2 weeks after intervention, 5 rabbits were selected randomly from each group respectively for MRI observation, histological observation by using HE staining and collagen type II immunohistochemical staining. The signal of lumbar MRI was assessed and the contents of collagen type II were detected. Results The platelet concentration of PRP was about 4.92 times as much as that of the whole blood. All the animals survived to the end of the experiment. At 2 weeks after degeneration, a lower T2 signal was observed in both the experimental group and the control group; the nucleus pulposus cells decreased and extracellular matrix degenerated; and the expression of collagen type II decreased in both the experimental group and control group. The degenerative grade of lumbar MRI in the experimental group and control group were significantly higher than that in the sham group (P lt; 0.05), and the content of collagen type II were significantly lower than that in the sham group (P lt; 0.05). At 1, 2 weeks after intervention, disc degeneration in the experimental group was significantly lower than that in control group (P lt; 0.05), and significant difference was found between experimental group and sham group (P lt; 0.05). The nucleus pulposus cells and chondroid matrix in the experimental group were more than those in the control group, showing slight stromal fibrosis; but the expression of collage type II was significantly higher than that in the control group (P lt; 0.05). Conclusion The disc injection of autologous PRP may terminate or even reverse the progress of rabbit early IDD, which may be associated with the role of multiple growth factors of PRP in regulating cell function, improving the tissue microenvironment, and promoting tissue regeneration.

  • A PROSPECTIVE CLINICAL STUDY ON AUTOLOGOUS PERIOSTEUM WRAPPING TENDON ALLOGRAFT FOR ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTION

    Objective To study the effectiveness of anterior cruciate l igament (ACL) reconstruction using autologous periosteum wrapping tendon allograft by comparing with using simple tendon allograft. Methods Between March 2008 and November 2008, 68 patients with ACL injury were treated, who were in accordance with the inclusion criteria. They were divided into 2 groups randomly according to different treatment methods: ACL was reconstructed with autologous periosteum wrapping tendon allograft in 31 patients (test group) and with simple tendon allograft (control group) in 37 patients. There was no significant difference in gender, age, disease duration, the cause of injury, and functional score preoperatively between 2 groups (P gt; 0.05). Anatomic single-bundle ACL reconstruction was performed in 2 groups. Results Little exudation at tibial tunnel incision was found in 1 case respectively in both groups at 2 weeks after operation and was cured by dressing change and antibiotics. The other incisions healed by first intention. The patients were followed up 24-29 months (mean, 26 months) in the test group and 24-32 months (mean, 27 months) in the control group. CT showed bone tunnel enlargement in both groups at 2 years after operation, but the rate of the tunnel enlargement was less inthe test group (5/31, 16.1%) than in the control group (14/37, 37.8%), showing significant difference (χ2=3.948, P=0.047). At 2 years after operation, the results of Lachman test and pivot shift test were negative in 23 cases (74.2%) and 25 cases (80.6%) of the test group, and in 26 cases (70.3%) and 30 cases (81.1%) of the control group, respectively. KT-1000 examination showed the displacement of the test group [(1.74 ± 0.88) mm] was less than that of the control group [(2.36 ± 0.83) mm], showing significant difference (t= —2.979, P=0.004). There was no significant difference in Lysholm score, Hospital for Special Surgery (HSS) score, Tegner score, and International Knee Documentation Committee (IKDC) score between 2 groups at 2 years after operation (P gt; 0.05). Conclusion Compared with simple tendon allograft, ACL reconstruction with autologous periosteum wrapping tendon allograft can improve tendon-bone heal ing, and decrease the rate of bone tunnel enlargement, so it has good short-term outcome.
