Objective To Analyze the high risk factors correlated to the perioperative complications of patients with lung cancer, aiming to study their predictive value on surgical safety and guide the perioperative treatment. Methods The clinical experience of 452 lung cancer patients undergone operation from June 2000 to May 2006 were retrospectively study. The risk factors which closely related to the occurrence of postoperative complications were analyze by multivariate statistical analysis with logistic regression. Results The operative mortality was 0.66%(3/452), and the incidence rate of postoperative complications was 8.85%(40/452), including pulmonary infection, pulmonary atelectasis, arrhythmia, respiratory function failure, empyema, bronchopleural fistula, thoracic bleeding, etc. Multivariate statistical analysis with logistic regression demonstrated that the risk factors which closely related to the occurrence of postoperative complications were age over 70 year (OR=17.823), smoking index over 400 piece year(OR=5.666), concomitance with diseases of vital organs(OR=8.290), modus operandi of pneumonectomy(OR=7.991), forced expiratory volume in one second (FEV1.0%)≤60% (OR=0.922). Conclusion To evaluate above factors of lung cancer patients before operation, the probability of postoperative complications occurrence can be roughly forecasted. This can provide actively clinical guide to help the patients overcome the perioperation successfully and to increase the safety of surgery.
To introduce the basic research and cl inical appl ications of the vascularized epi physeal transplantation. Methods The appl ied anatomy, experimental researches, and cl inical appl ication about the vascularized epi physeal transplantation were summarized in the past two decades. The effectiveness of epi physeal transplantation were discussed in the article. Results The epiphysis flap of fibular head with inferior lateral genicular artery and the epiphysis flap of il iac with deep superior branch of superior gluteal artery can be used as the donor sites of epiphyseal transplantation. Animal experiments proved that the vascularized epi physis survived and maintained growth after transplantation. In a typical caseundergoing distal ulnar reconstruction by the graft of peroneal epiphyseal, the 18-year follow-up results showed that the repaired ulna was nearly as long as the contralateral side and the function of the forearm was good. Conclusion It is an effective way to repair epiphyseal defects of long bones in children with vascularized epiphyseal transplantation.
OBJECTIVE: To investigate the anatomic basis for transposition of the distal dorsal ulna bone flap pedicled with dorsal metacarpal artery to repair the defect of the 3rd or 4th and 5th metacarpal bone head. METHODS: In 30 adult cadaveric upper limbs, the branches and constitutions of the dorsal carpal arterial networks were observed. RESULTS: The dorsal carpal arterial networks were consisted of the dorsal carpal branches of ulnar and radial arteries, the terminal branches of posterior interosseous artery and the dorsal carpal branch of anterior interosseous artery, and then the 2nd, 3rd, 4th dorsal metacarpal branches were originated from the networks. The dorsal metacarpal branches were anastomosed with the deep branches of deep palmar arch to constitute the dorsal metacarpal artery. CONCLUSION: Transposition of the distal dorsal ulna bone flap pedicled with the 3rd, 4th dorsal metacarpal arteries can be used in repairing the defect of 3rd, 4th and 5th metacarpal bone head.
