  • Research Progress on The Mechanism of ARDl Resistance The Colorectal Cancer Cell

    ObjectiveTo summarize the recent research progress on pathogenesis of human arrest defective 1(ARD1) protein in colorectal cancer and treatment process. MethodsSearched the related literatures from the databases such as CNKI, PubMed and so on, the relevant ARD1 in the development, diagnosis and treatment of colorectal cancer were reviewed. ResultsARD1 has effect of anti colorectal cancer, it can inhibit the proliferation and promote apoptosis of colorectal cancer cells, and improve the sensitivity of colorectal cancer cells to anticancer drugs at the cellular level. The treatment is mainly through the induction of cancer cell apoptosis or (and) decreased the proliferation ability of cancer cells, thus delaying the disease process. However, it is still in the research stage of animal experiments, which can not be directly applied to clinical practice. Conciusions ARDl study on the mechanism of anti colorectal cancer cells has become the focus of research with animal research and promotion, and provide new therapy concepts and measures for diagnosis and treatment of colorectal cancer.

  • REPAIR OF THUMB PULP DEFECTS WITH SIDE ISLAND FLAP COINCIDING DORSAL BRANCH OF DIGITAL NERVE

    ObjectiveTo explore the effectiveness of the side island flap coinciding dorsal branch of the digital nerve for repairing thumb pulp defects. MethodsBetween May 2008 and July 2012,36 cases of thumb pulp defects were treated with the side island flap coinciding dorsal branch of the digital nerve.There were 26 males and 10 females,aged 21-51 years (mean,32.4 years).The injury causes included electric saw injury in 14 cases,punch press injury in 8 cases,machine twist injury in 5 cases,door crushing injury in 5 cases,and glass cutting injury in 4 cases.The left hand was involved in 12 cases and the right hand in 24 cases.Combined injuries included tendon and bone exposure in all cases,fracture of the distal phalanx in 3 cases,and nail bed lacerations in 2 cases.The defect size ranged from 1.4 cm×1.2 cm to 2.5 cm×2.1 cm;and the flap size ranged from 1.8 cm×1.4 cm to 3.0 cm×2.5 cm. ResultsTwo flaps with distal skin flap tension blisters and skin scabbing,which were cured after dressing changes;the wound healed by first intension in the other 34 cases.The skin grafts at donor site survived,and primary healing of incision was obtained.The patients were followed up 3-15 months (mean,8 months).The appearance and function restored well.Two-point discrimination of the flap was 5.2 mm on average (range,4-8 mm) at last follow-up.The finger joint had no stiff.According to the upper extremity function evaluation criteria issued by the Hand Surgery Society of Chinese Medical Association,the sensation was S4 in 33 and S3+ in 3 cases.No ectopic feeling was observed.The extension and flexion activity of fingers at donor site was normal,the sensation reached S4 with no atrophy of the finger. ConclusionWith constant anatomy,reliable blood supply,and safe operation,the anastomoses of the finger side island flap with dorsal branch of digital nerve is a better method to repair thumb pulp defects because it also effectively overcome the ectopic feeling with traditional methods and protect feel function of the fingers at donor site.

  • CHOICE AND EFFECTIVENESS EVALUATION OF PROXIMAL FEMORAL NAIL ANTIROTATION AND InterTAN IN TREATMENT OF ELDERLY INTERTROCHANTERIC FRACTURES

    ObjectiveTo compare the effectiveness and complications of proximal femoral nail antirotation (PFNA) and InterTAN nail in treatment of elderly intertrochanteric fractures. MethodsA total of 178 patients with intertrochanteric fractures between January 2011 and June 2013 were enrolled. PFNA was used in 100 cases (PFNA group) and InterTAN in 78 cases (InterTAN group). No significant difference was found in gender, age, side of fracture, cause of injury, fracture classification, and time between injury and operation between 2 groups (P>0.05). The operation time, intraoperative blood loss, intraoperative fluoroscopy time, postoperative complications, hospitalization time, fracture healing time, and Harris score were compared between 2 groups to evaluate the effectiveness. ResultsThe operation time, intraoperative blood loss, intraoperative fluoroscopy time in PFNA group were significantly less than those in InterTAN group (P<0.05). The hospitalization time had no significant difference between the 2 groups (t=1.270, P=0.206). Primary healing was obtained in the others except 2 cases having red and swollen in each group respectively. Ninety-two cases of PFNA group and 63 cases of InterTAN group were followed up 11 months on average (range, 8-16 months). In the patients who were lost, there were 4 deaths in PFNA group and 3 deaths in InterTAN group. The complication rate was 4.2% in PFNA group (2 cases of deep venous thrombosis and 2 cases of hip varus) and was 4.5% in InterTAN group (1 case of deep venous thrombosis and 2 cases of hip varus), showing no significant difference between 2 groups (χ2=0.077, P=0.782). X-ray showed that the fracture line disappeared; no complications of malunion, bone nonunion, infection, and loosening of internal fixation occurred. The patients could walk normally. The healing time and Harris hip scores at last follow-up showed no significant difference between 2 groups (t=1.324, P=0.188; t=1.594, P=0.113). ConclusionPFNA is suitable for elderly patients with osteoporosis in treatment of femoral intertrochanteric fracture; InterTAN can be a better choice for younger patients or patients with unstable fracture.
