Traditional medical education is to develop uni-professionals,and is lacking of opportunities for interprofessional communication and interprofessional collaboration.The failure of communication and collaboration is an important cause of medical errors.Interprofessional education is a new strategy to remove professional prejudice,promote interprofessional collaborative competency and improve patients'outcomes.From September 2012,we began to provide Interprofessional Education Program for healthcare undergraduates,and achievements have already been made.
ObjectiveTo explore the status of clinical learning environment in undergraduate nursing students in the late clinical practice stage in Sichuan Province. MethodThe sampling was performed in a layered-randomized and geometric proportional way. A total of 1 027 nursing students from 6 nursing schools in Sichuan Province between May and July 2012 were included. Data were collected by the self-rating questionnaires, including demographic sheet, and the nursing clinical learning environment scale. ResultsThe general score of evaluation for clinical learning environment by the nursing undergraduates was 144.99±26.60, and the average score of the items was 3.45±0.63. The average scores were 3.41±0.74, 3.59±0.64, 3.43±0.69, 3.45±0.73, 3.35±0.77, and 3.49±0.69, representing the personality, task orientation, working atmosphere and team spirit, innovation, interpersonal relation, and student participation, respectively. ConclusionsThe assessments about clinical learning environment of nursing undergraduates in Sichuan province are medium grade and the evaluation dimensions of personality and interpersonal relationships low. It is necessary to improve clinical learning environment in the future.
ObjectiveTo analyze the clinical effect of sodium chlorophyllin in the treatment of geriatric oncology-related anemia and its significance. MethodsWe retrospectively analyzed the clinical data of elderly cancer patients with similar treatment background between December 2011 and February 2013, and the patients were divided into sodium chlorophyllin treatment group and no special treatment group. Then, parallel comparative study was carried out for anemia-related indexes and scores of quality of life. ResultsHemoglobin, red blood cells and quality of life of sodium chlorophyllin treatment group were improved after treatment, and these indexes were also better than those in the no special treatment group (P<0.05). ConclusionSodium chlorophyllin can ameliorate anemia in elderly patients with oncology-related anemia, which is worthy of being popularized.
ObjectiveTo discuss clinical manifestation, laboratory examination, imaging and pathological features of intrahepatic cholangiocellular carcinoma(ICC) patients with fever as initial symptom accompanied with liver area pain, in order to improve the clinicians' acquaintance for ICC under similar circumstances. MethodThe case informations including medical history, clinical manifestation, laboratory examination, imaging finding, pathological examination, and treatment of 4 patients diagnosed with ICC by pathological biopsy from july 2013 to October 2014 in the First hospital of Lanzhou University were analyzed retrospectively. ResultsAll of four cases showed the fever as the initial symptom accompanied with the liver area pain. All of them had got chronic HBV infection. The WBC, neutrophil percentage, and procalcitonin were increased on admission in 3 cases. the levels of serum ALP and GGT were elevated in 3 patients. The AFP was obviously increased in 1 patient. The serum CA19-9 had moderately elevated in 2 patients. the ferroprotein was obviously increased in 2 patients. All the patients were confirmed under the abdominal CT scans and the liver pathological biopsy. ConclusionPatients with fever and liver area pain as intial symptoms, and with chronic hepatitis B and space-occupying lesions, who should be alert for ICC.
ObjectiveTo evaluate the clinical efficacy and safety of telbivudine (TEV) combined with adefovir dipivoxil (ADV) for chronic hepatitis B (CHB), so as to provide references for clinical practice and research. MethodsWe electronically searched databases including The Cochrane Library (Issue 7, 2013), PubMed, EMbase, Web of Science, CBM, CNKI, VIP, and WanFang Data from inception to August 21st, 2013, for the relevant randomized controlled trials (RCTs). Other sources were also retrieved. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data, and assessed the quality of included studies. Then, meta-analysis was performed using RevMan 5.1 software. ResultsA Total of 11 RCTs involving 1 010 patients were included. The trial group were given TEV combined with ADV, while the control group were given TEV alone or ADV alone. The results of metaanalysis showed that, the combined use was superior to TEV alone or ADV alone in improving HBV-DNA negative rates at 12-, 24-, 48-weeks, HBeAg negative rates at 12-, 24-, 48-weeks, and ALT recovery rates at 12-, 24-weeks (P < 0.05). The results of qualitative analysis showed that, the trial group had a lower drug resistance rate, and both were alike in the incidence of adverse reaction. ConclusionCompared with TEV alone or ADV alone, TEV combined with ADV could improve the clinical efficacy of treating CHB which is also fairly safe. Due to the limited quantity and quality of the included studies, the aforementioned conclusion still needs to be further verified by conducting more large-scale and high quality RCTs.
ObjectiveTo systematically review the clinical efficacy and effects on pregnancy outcomes of S-adenosy-L-methionine combined with ursodesoxycholic acid in the treatment of intrahepatic cholestasis of pregnancy. MethodsDatabases such as PubMed, The Cochrane Library, CNKI, VIP, WanFang Data were searched for the studies about the clinical efficacy and effects on pregnancy outcomes of S-adenosy-L-methionine combined with ursodesoxycholic acid in the treatment of intrahepatic cholestasis of pregnancy up to December 31st, 2013. Two reviewers independently screened literature, extracted data and evaluated methodological quality. Then meta-analysis was conducted using RevMan 5.0.24 software. ResultsA total of 11 RCTs involving 776 patients were included. The results of meta-analysis showed that, combined medication reduced blood biochemical indexes inlcuding ALT (MD=3.63, 95%CI 0.63 to 6.64, P=0.02), TB (MD=3.70, 95%CI 1.45 to 5.96, P=0.001), and AST (MD=7.61, 95%CI 2.47 to 12.75, P=0.004). Combined therapy significantly decreased the rates of amniotic fluid contamination (OR=0.29, 95%CI 0.19 to 0.45, P=0.000 01), cesarean section (OR=0.53, 95%CI 0.36 to 0.79, P=0.002), postpartum hemorrhage (OR=0.32, 95%CI 0.12 to 0.90, P=0.03), preterm birth (OR=0.36, 95%CI 0.24 to 0.55, P < 0.000 01), fetal distress (OR=0.33, 95%CI 0.19 to 0.58, P=0.000 1) and neonates asphyxia (OR=0.30, 95%CI 0.19 to 0.47, P < 0.000 01). Combined therapy was also beneficial to improving pruritus symptoms (MD=0.20, 95%CI 0.08 to 0.31, P=0.000 08) and benefiting fetus growth (MD=0.45, 95%CI 0.23 to 0.66, P < 0.000 1). ConclusionThe combination of S-adenosy-L-methionine and ursodesoxycholic acid is superior to ursodesoxycholic acid alone in improving clinical symptoms and pregnant outcomes of patients with intrahepatic cholestasis of pregnancy.
ObjectiveTo explore clinical outcomes of papaverine via the aortic root under cardiopulmonary bypass (CPB) in coronary artery bypass grafting. MethodsA total of 263 patients with coronary artery disease underwent CPB in coronary artery bypass grafting in Beijing Anzhen Hospital from August 2003 through December 2014 were included. According to whether or not they received papaverine perfusion via the aortic root in the first perfusion during CPB, all the 263 patients were divided into two groups including an intervention group and a control group. There were 176 patients in the intervention group including 109 males and 67 females with their average age of 64.37±23.54 years. They received aortic root perfusion of 60 mg papaverine diluted into 20 ml during the first perfusion, and repeated aspiration was performed 8 to 10 times in CPB when the heart beat weak. In the control group, there were 87 patients including 55 males and 32 females with their average age of 65.98±29.13 years. Patients in the control group received routine procedures of CPB except papaerine perfusion. Clinical effectiveness was compared between the two groups. ResultsThere was no in-hospital death. CPB supporting time after heart re-beating (9.58±3.21 min vs. 19.74±5.67 min, t=-1.133, P=0.000), dopamine usage 3.12±1.27 μg/(kg·min) vs. 4.98±2.53 μg/(kg·min),t=-0.913, P=0.031), epinephrine usage 0.018±0.009 μg/(kg·min) vs. 0.047±0.011 μg/(kg·min), t=-0.811, P=0.018) and nor epinephrine usage 0.021±0.011 μg/(kg·min) vs. 0.037±0.010 μg/(kg·min), t=-0.823, P=0.019) of the intervention group were significantly shorter or lower than those of the control group. Automatic heart re-beating rate of the intervention group was significantly higher than that of the control group (98.86% vs. 70.11%,t=-1.372, P=0.000). A total of 176 patients in the nitroglycerin group were followed up for 6-121 months, and 16 patients were lost during follow-up. During the follow-up period, 6 patients were hospitalized for cardiac events, and 2 died in 29 months and 103 months after surgery, and 9 died from cardiac events. ConclusionFor the patients undergoing on-pump coronary artery bypass grafting, some myocardial protection strategies including papaverine perfusion via the aortic root in the first perfusion, use of norepinephrine, and aortic root squeezing in high frequency by hand can significantly increase the automatic heart re-beating rate, shorten the CPB supporting time after heart re-beating, and improve postoperative clinical outcomes.
Objective To evaluate systematically the effectiveness and safety of procalcitonin ( PCT) -guided therapy in comparison with standard therapy in patients with suspected or confirmed severe bacterial infections in intensive care unit ( ICU) . Methods Five randomized controlled trials ( 927 patients) were included for statistical analysis by the cochrane collaboration′s RevMan5. 0 software. Results PCT-guided therapy was associated with a significant reduction in duration of antibiotic therapy [ MD =- 2. 01, 95% CI ( - 2. 37, - 1. 64) , P lt;0. 00001] , but the mortality [ OR =1. 11, 95% CI ( 0. 83, 1. 49) ,P =0. 47] and length of ICU stay[ MD = 0. 49, 95% CI( - 1. 44, 2. 42) , P = 0. 62] were not significantly different. Conclusions An algorithmbased on serial PCT measurements would allow a more judicious use of antibiotics than currently traditional treatment of patients with severe infections in ICU. It can reduce the use of antibiotics and appears to be safe.
ObjectiveTo explore the effectiveness and operation of repairing multiple soft tissue defects in hands with lateral arm lobulated flaps. MethodBetween October 2013 and September 2015, 13 cases of multiple soft tissue defects in the hand with tendon or bone exposure were treated with lateral arm lobulated flaps. All patients were males with average age of 28 years (range, 23-45 years). Defects were caused by penetrating injury in 7 cases, traffic accident injury in 3 cases, and hot-crush injury in 3 cases. Six patients had skin defect of the left 2 fingers after opening finger amputation, and 7 patients had skin defect of the palm and the back after hand injury. The size of skin defects ranged from 6 cm×5 cm to 9 cm×6 cm. All patients underwent emergency debridement and two-stage repair; the duration from injury to operation was 5-9 days (mean, 7 days). The size of flap was 6 cm×5 cm-9 cm×6 cm. ResultsAll flaps survived completely, with no vascular crisis. Primary healing was obtained at donor and recipient sites, and the grafted skin survived. All cases were followed up 3-24 months (mean, 12 months). The appearance and texture of the flaps were similar to those of adjacent skin. Bulky flap was observed in 4 cases, and second stage operation was performed to make the flap thinner at 3 months after operation. The sensation of flap reached S3-S4. ConclusionsThe lateral arm lobulated flap based on the radial collateral artery has constant vascular anatomy, easy-to-harvest, and large rotation angle. It is an effective procedure to repair small and medium size skin defects of the hand with satisfied texture and sensory recovery.
Objective To investigate the surgical treatment of adult secondary atrial septal defect with paroxysmal atrial fibrillation. Methods We retrospectively analyzed the clinical data of 84 patients with adult secondary atrial septal underwent surgical treatment in Beijing Anzhen Hospital from January 2002 through May 2013. There were 31 males and 53 females at mean age of 56.3 (24-72) years. Eleven patients were treated by cardiopulmonary bypass operation with 6 patients by radiofrequency catheter ablation simultaneously. Seventy three patients were implanted with septal defect closure with 52 patients by radiofrequency ablation simultaneously. Rhythm of the heart was detected at the immediate postoperative period, 7 days, 3 months and 6 months after the operation. Results No preoperative death, thoraecotomy and no arrhythmic complications in all the patients no matter with or without RF occurred. In the patients with cardiopulmonary bypass operation, the time of aortic occlusion was 35.21±12.32 min, the cardiopulmonary bypass time was 45.67±16.93 min, and the ICU observation time was 11.87±12.36 hours. In the patients with septal defect closure implanted, ICU observation time was 10.93+13.67 hours. The incidences of atrial fibrillation in the patients with radiofrequency ablation on postoperative 7 days,3 months and 6 months were lower than those in the patients without radiofrequency ablation (P<0.05). Conclusion For adult secondary atrial septal defect patients with paroxysmal atrial fibrillation, no matter cardiopulmonary bypass operation or septal defect closure implanted, radiofrequency ablation at the same time is safety and can reduce incidence of recurrence of atrial fibrillation.
