  • The Clinical Analysis of 128 Cases with Operation for the Treatment of Gluteal Muscle Contracture

    Objective To investigate the operative effects of gluteal muscle contracture. Methods A total of 128 cases of gluteal muscle contracture treated with operative methods from February 2001 to May 2009 were reviewed. Results All the patients received operation. Most patients were satisfied with the treatment via function exercise in the early stage. No severe complication was found. The patients were followed up for 6 to 84 months, at the average of 36 months. According to Huang Yaotians criteria, 67.9% (87/128) of the patients were excellent; 27.3% (35/128) were good; 3.9% (5/128) were fair; and 0.8% (1/128) were poor in effectiveness. Conclusion Operation and function exercise after operation have advantages of minitrauma and good function recovery, and it is a satisfactory method to treat the gluteal muscle contracture.

  • Practice of Evidence-based Treatment Strategy for Cervical Spinal Cord Injury

    ObjectiveTo explore the practice of the evidence-based treatment strategy for cervical spinal cord injury. MethodsOne patient with cervical spinal cord injury was admitted to our hospital on January 3, 2013. We obtained medical evidences by searching databases and regulated the best treatment after evaluating the patient's comprehensive conditions. And then, the whole treatment strategy was fully implemented. Finally, the consequent results were evaluated. ResultsThe evidence-based medicine showed that the therapeutic targets were to save the residual function, prevent complications, and promote the recovery of neural function. Based on the real-time conditions of patient, we developed and practiced the evidence-based comprehensive rehabilitation programs, including absolute rest in bed, high-dose steroids, neurotrophic drugs, Chinese medicine rehabilitation and prevention of complications. After a follow-up of half a year, the patient obtained a good curative effect. The patient was saved from paralyzing. Moreover, the patient restored the capacity of standing, walking and a certain level of self-care ability. ConclusionFor the cervical spinal cord injury, treatment decision based on evidence-based medicine is more scientific, and it can ensure maximum benefit for the patients. Therefore, it is worthy of popularizing.

  • Efficacy and safety of mechanical thrombectomy in cerebral venous sinus thrombosis: a literature review

    ObjectivesTo review the efficacy and safety of mechanical thrombectomy in patients with cerebral venous sinus thrombosis (CVST).MethodsWe searched The Cochrane Library, PubMed, EBSCO, Web of Science, CBM, CNKI and VIP databases to collect studies on mechanical thrombectomy in CVST patients from inception to April, 2018. Two reviewers independently screened literature, extracted the data and qualitative analysis of the included studies.ResultsA total of 33 studies including 552 patients were included for data analysis. Specifically, 157 (30%) patients had a focal neurological deficit, 145 (28%) patients had a pretreatment intracerebral hemorrhage or infarct, and 152 (29%) patients were stuporous or comatose. Wire was the most commonly used device. Overall, 483 (88%) patients had good outcome, while 35 (6%) patients deceased. Moreover, 333 (65%) patients had complete recanalization, 148 (29%) patients had partial recanalization, and 14 (2.5%) patients had worsen or new intracranial hemorrhage.Conclusions The current evidence suggests that mechanical thrombectomy is reasonably safe in the majority of cases. Due to limited quality and quantity of included studies, more high quality studies are required to verify above studies.

  • Considerations and recommendations for Chinese medicine case report guideline

    Chinese medicine case report, a special method, records the experience of practitioners and guides students to inherit and develop Chinese medicine. It can transfer a large amount of medical and humanistic information and contribute to the development of Chinese medicine. It is a literary style that has been widely accepted and helps to document and disseminate the culture of Chinese medicine. With the advent of big data and information, more Chinese medicine case reports have been published. However, many have insufficient quality to properly guide and apply in the clinical practice, which might be caused by little guidance of Chinese medicine case report standards published. This paper summarized the case report standards, synthesized and appraised the feasibility and problems specific for improving the quality of Chinese medicine case reports, and proposed suggestions and guidance for developing the standardization of Chinese medicine medical case reports.

  • The adverse reactions of piperacillin: a systematic review

    ObjectiveTo systematically review the adverse drug reactions (ADRs) caused by piperacillin. MethodsPubMed, EMbase, Web of Science, CNKI, WanFang Data and VIP databases were electronically searched to collect case reports on ADRs caused by piperacillin from inception to December 31, 2021. Two reviewers independently screened literature, extracted data and then a qualitative systematic review was performed. ResultsA total of 168 case reports involving 170 patients were included. The male to female ratio was 1.06∶1, and the median age was 54 year old. The statistical results showed that ADRs caused by piperacillin mostly involved the whole body (n=55), followed by the blood system (n=48), skin and soft tissue system (n=39) and nervous system (n=18). The most frequently reported cases were anaphylactic shock (n=29), followed by drug fever (n=19), rash (n=17) and thrombocytopenia (n=15). The most serious ADRs were anaphylactic shock (n=29) and bullous epidermal necrolysis (n=6). ConclusionCurrent evidence shows that ADRs caused by piperacillin mostly involved the whole body and the most serious ADRs are toxic epidermal necrolysis and anaphylactic shock.

  • Permanent neonatal diabetes mellitus case reports: a systematic review

    ObjectiveTo systematically review the clinical and genetic features of permanent neonatal diabetes mellitus (PNDM) case reports. MethodsThe PubMed, Embase, Scopus, SinoMed, Web of Science, CINAHL, Medrxiv, VIP, CNKI and WanFang Data databases were electronically searched to collect PNDM case reports from inception to June 2023. Two reviewers independently screened literature, extracted data and assessed the reporting quality of the included studies. Descriptive analysis was performed. ResultsA total of 105 case reports were finally included. Typical clinical manifestations of PNDM were early onset of persistent hyperglycemia, developmental delay and low birth weight. The results of genetic testing showed that mutations in the KCNJ11, INS, EIF2AK3, GCK, ABCC8, PTF1A, GATA6, IER3IP1, SLC19A2, NEUROG3, PDX1, and 6q24 genes were closely associated with the development of PNDM. In addition, there may be different clinical manifestations and prognosis of PNDM in different genotypes. ConclusionThis study reveales the clinical characteristics and genetic pattern of PNDM, and provides a direction for further research on the mechanism of PNDM.
