  • Risk Factors for Shortterm Adverse Events in Infants Receiving Open Heart Surgery

    Abstract: Objective To identify the risk factors for shortterm adverse events in infants with congenital heart diseases receiving open heart surgical correction with cardiopulmonary bypass (CPB), in order to improve the outcome by adopting appropriate treatment measures. Methods We retrospectively analyzed the clinical data of 98 consecutive children with congenital heart diseases who underwent surgical correction with CPB in Beijing Fu Wai Hospital from November 2009 to December 2009. The patients were divided into two groups according to the postoperative complications. Among the patients without complications(n=40): there were 24 males and 16 females with an age of 7.60±0.40 months and a weight of 7.80±0.30 kg. In the patients with complications (n=58): there were 42 males and 16 females with an age of 6.20±0.40 months and a weight of 6.70±0.20 kg. In both groups, perioperative data were recorded, including preoperative fast blood glucose, creatinine, time of aortic crossclamp, modified or zerobalanced ultrafiltration, postoperative glucose level, concentration of lactate, notrope score and complications. Risk stratification was performed by Risk Adjusted Classification for Congenital Heart Surgery (RACHS-1). Univariate analysis and logistic regression analysis were used to identify the risk factors for shortterm adverse events. Results One patient(1.02%) died of circulatory failure during the perioperative period. Thirtyseven patients [CM(159mm]were supported by at least 2 vasoactive drugs for more than 48hours,29 by mechanical ventilation for more than 24 hours, 5 needed reintubation, 1 experienced tracheotomy, 31 suffered from noscomial infection, 4 had wound infection, 3 developed renal failure, and 1 developed hepatic dysfunction. By logistic regression analysis, age (OR=0.750, P=0.012), percutaneous oxygen saturation (OR=0.840,P=0.005), aortic crossclamp time (OR=1.040, P=0.008), postoperative glucose level (patients with a mean glucose level lower or equal to 8.33 mmol/L had a probability of developing adverse outcomes five times higher; OR=5.051, P=0.011) were found to be the risk factors for shortterm adverse outcomes. Conclusion Age, percutaneous oxygen saturation and aortic crossclamp time are associated with the shortterm adverse outcome of infants undergoing congenital heart disease correction with CPB. The present results do not support perioperative hyperglycemia as a risk factor for adverse outcome.

  • MINIPLATE INTERNAL FIXATION AND AUTOGENOUS ILIAC BONE GRAFT IN SURGICAL TREATMENT OF OLD METATARSAL FRACTURES

    Objective To summarize the experience of treating old metatarsal fractures with surgery methods of miniplate internal fixation and autogenous il iac bone. Methods Between May 2009 and July 2010, 7 patients with old metatarsal fractures were treated surgically, including 5 multi-metatarsal fractures and 2 single metatarsal fractures. There were 5 males and 2 females aged from 25 to 43 years (mean, 33 years). The time from fracture to operation was 4-12 weeks. The X-rayfilms showed that a small amount of callus formed at both broken ends with shortening, angulation, or rotation displacement. The surgical treatments included open reduction, internal fixation by miniplate, and autogenous il iac bone graft (1.5-2.5 cm3). The external plaster fixation was used in all patients for 4 to 6 weeks postoperatively (mean, 5 weeks). Results All incisions healed by first intention. The 7 patients were followed up 8-18 months (mean, 13.5 months). The cl inical fracture heal ing time was 6 to 12 weeks postoperatively (mean, 8.4 weeks). No pain of planta pedis occurred while standing and walking. The American Orthopaedic Foot and Ankle Society (AOFAS) mesopedes and propodium score was 75-96 (mean, 86.4). Conclusion It has the advantages of rel iable internal fixation, high fracture heal ing rate, less compl ications to treat old metatarsal fractures with surgery methods of miniplate internal fixation and autogenous il iac bone graft, so it is an effective treatment method.

  • Pulse Oximetry Screening for Congenital Heart Disease in Asymptomatic Children:A Systematic Review and Meta-Analysis

    Objective To investigate the accuracy of pulse oximetry screening for congenital heart disease (CHD) in asymptomatic children. Methods We electronically searched VIP database,CNKI database,CBMdisc,Wanfandatabase,PubMed,Excerpta Medica Database (EMBASE) and the Cochrane Library from the time of database establishment to February 2013 to identify literatures regarding pulse oximetry screening for CHD in asymptomatic children. Data extraction was performed by two researchers independently. Quality of the included literatures was evaluated with qualityassessment for diagnostic accuracy studies (QUADAS). Meta-analysis was performed using RevMan 5.1.1. ResultsTwelve studies were included in this study with a total of 137 582 newborns. Fourteen QUADAS criteria were fulfilled by all the 12 studies. Among the 12 studies,only the studies with positive test results received verification using a gold standardtest,and the execution of the gold standard test was described in sufficient detail only in 1 study. There were 10 studies in which the gold standard test results were interpreted with knowledge of the diagnostic test results. Meta-analysis showed that pooled sensitivity and specificity of pulse oximetry test for the diagnosis of CHD were 22% with 95% CI (19%,25%) and 99% (99%,99%) respectively. Pooled positive likelihood ratio (LR+) and negative likelihood ratio (LR-) were 157.30 with 95% CI (11.80,2 096.95) and 0.61 with 95% CI (0.46,0.82) respectively. Pooled diagnostic odds ratio was 398.25 (34.5,4 596.81). The area under summary receiver operating characteristic (SROC) curve was 0.809,and Q index was 0.744. Conclusion Pulse oximetry is a lowly sensitive and highly specific diagnostic method for children with CHD,and is helpful for early diagnosis of CHD.
