  • Long term followup of the comparison between combined surgery and sequential surgery in treating proliferative diabetic retinopathy

    Objective To observe the therapeutic efficacy and postoperative complications of combined surgery and sequential surgery in treating proliferative diabetic retinopathy (PDR) and cataract. Methods The cilinical data of 59 patients (66 eyes) with PDR were retrospectively analyzed. The patients were divided into combinedsurgery group and sequential surgery group. Combined surgery was performed on patients with obvious lens opacity which was an obstacle to the ocular fundus surgery, while sequential surgery was performed on the patients with transparent lens or lens with light opacity on which ocular fundus surgery could be performed. Lens excision in cataractopoiesis could only be performed when the state of ocular fundus was stable and cataractopoiesis was the main cause of vision damage. A total of 28 patients (32 eyes) in combinedsurgery group underwent vitrectomy combined with phacoemulsification and IOL implantation; 31 patients (34 eyes ) in sequentialsurgery group underwent vitrectomy, lens excision and IOL implantation. Corrected visual acuity (BCVA) and complications were observed. The mean followup period was (25plusmn;8.5) months. The measurement data were analyzed with t test and enumeration data with 2 test. Results In combinedsurgery group, visual acuity improvement was achieved in 27 eyes (84.4%), remained unchanged in 2 eyes (6.3%)and decreased in 3 eyes (9.4%). In sequentialsurgery group, visual acuity improved in 26 eyes (76.5%), remained unchanged in 2 eyes(5.8%)and decreased in 6 eyes(17.7%.). However, anterior chamber fibrin exudation occurred in 4 eyes in the combinedsurgery group and no eyes in the sequentialsurgery group; compared with each other, the difference is statistically significant (chi;2=4.524,P=0.033). Conclusion Combined surgery and sequential surgery are all safe and effective on treating PDR and postoperative complications have no obvious correlation with the surgery procedure.

  • Effect of Fufang Xueshuantong Combined with Laser Photocoagulation versus Laser Photocoagulation Along for the Treatment of Diabetic Retinopathy: A Meta-analysis

    ObjectiveTo evaluate the effects of Fufang Xueshuantong combined with laser photocoagulation versus laser photocoagulation alone in the treatment of diabetic retinopathy (DR). MethodThe Web of Science, Medline, Embase, the Cochrane Library, China Biology Medicine disc, China National Knowledge Infrastructure, VIP and Wanfang databases were searched from their establishment until June 2015. We used the method recommended by the Cochrane Collaboration to perform a meta-analysis of randomized controlled trails. RevMan 5.2 software was used for the analysis. ResultsEight studies were included. The results of Meta-analysis demonstrated that Fufang Xueshuantong combined with laser photocoagulation was superior to laser photocoagulation alone in the effective rate of visual acuity[RR=1.19, 95%CI (1.12, 1.26), P<0.000 01], fundus[RR=1.34, 95%CI (1.18, 1.52), P<0.000 01], and elimination rate of macular edema[RR=1.29,95%CI(1.09,1.54),P=0.004]. ConclusionsFufang Xueshuantong combined with laser photocoagulation is effective for DR. However, due to the limited quantity and quality of the included studies, the results suggest that larger-scale trials are needed.

  • Comparison of the efficacy of multi-point or single-point mode of 577 nm laser in the treatment of diabetic retinopathy

    ObjectiveTo compare the therapeutic effects of 577 nm laser panretinal photocoagulation (PRP) between one time multi-point scanning mode and multiple time single-point mode in the treatment of eyes with non-proliferative diabetic retinopathy (NPDR). MethodsThis is a prospective controlled study from August 2013 to February 2014. A total of 29 patients (46 eyes) with clinically diagnosed severe NPDR were randomly divided into two groups including the treatment group (12 patients, 22 eyes) and the control group (17 patients, 224 eyes). The treatment group received one time PRP of multi-point scanning mode, and the control group received 3-4 times of PRP with single-point mode. In order to evaluate its efficacy, the best corrected visual acuity was measured before treatment, and 1 day, 1, 2, 6 and 12 months after treatment. The average threshold sensitivity, a/b-wave amplitude of flash ERG (F-ERG) in the 30°-60° visual field, and fundus fluorescein angiography (FFA) of the change were also compared between the 2 groups. The laser energy and the number of laser spots were compared, and the laser energy density was calculated. ResultsThe response rate was 86.4% and 79.2%, respectively in the treatment and control group, the difference was not statistically significant (χ2=0.414, P > 0.05). Compare to the pre-treatment measurement, the average threshold sensitivity, a/b-wave amplitude of F-ERG in the 30°-60° visual field were reduced at 1 day after treatment both in treatment and control group, the differences were statistically significant (P < 0.05). The average threshold sensitivity, a/b-wave amplitude of F-ERG were no difference between treatment and control group at 2m, 6m and 12m after treatment (P > 0.05). The average laser power, number of laser spots and energy density were (537.50±64.69) mW and (339.09±132.09) mW, (1934.32±426.38) points and (2061.42±375.49) points, (0.35±0.12) mW o ms/μm2 and (1.95±0.86) mW·ms/μm2 in the treatment group and the control group, respectively. The average laser power and energy density was statistically different between the 2 groups (P < 0.05), while the number of laser spots was no difference (P > 0.05). Conclusions577 nm multi-point scanning laser can complete the PRP at one time, and achieve the same therapeutic outcomes with the single-point mode which need several times to complete the PRP in the eyes with severe NPDR, and have lower energy density, and thus relative minor function damage.
