ObjectiveTo investigate the clinical nursing path of health education in patients with gout. MethodsA total of 220 patients with gout treated in People's Hospital of Jiangyou from September 2010 to November 2011 were randomly divided into observation group and control group with 110 patients in each.After routine health education for the control group and clinical nursing path health education for the observation group,we compared patients'compliance with doctors,gout-related indicators and health education satisfaction degree between the two groups of patients. ResultsThere was no statistically significant difference in compliance with doctors between the two groups before health education (P>0.05).After health education,the score of five single items on compliance with doctors and the total score for patients in the observation group were all significantly improved (P<0.05).For patients in the control group,the scores of each item and the total score were also significantly improved (P<0.05) except the item of regular drug-taking and regular health examination (P>0.05).After health education,all scores and the total score of the observation group were higher than those of the control group (P<0.05). ConclusionFor health education for gout patients,clinical nursing path is better than routine health education in scores of compliance with doctors,improvements of symptoms,and health education satisfaction degree of the patients.
ObjectiveTo investigate the effect of 5% dextrose intravenous infusion for phlebitis caused by amiodarone. MethodsA total of 136 patients treated in our hospital between June 2011 and March 2012 were randomly assigned into control group with 63 patients and intervention group with 73 patients. The control group was administrated 600 mg amiodarone with 15 drops/minute intravenous injection once daily, for 3 days, while for the intervention group patients, we added 5% dextrose solution infusion in the same channel at the same time. ResultsPhlebitis occurred in 22 patients (34.9%) in the control group, while the intervention group had only 11 (15.0%) (χ2=7.252,P=0.007). ConclusionInfusion of 5% dextrose solution can significantly reduce the occurrence of phlebitis caused by amiodarone.
ObjectivesTo analyze the theoretical and practical research and effect evaluation of integrated care at home and abroad, so as to provide evidence for the development of integrated care in China.MethodsPubMed, CNKI and WanFang Data databases and government documents, research reports were searched to collect studies on integrated care, and then literature review was then performed.ResultsForeign integrated care was dominated at government level. The integration involved numerous departments such as hospital and insurance institutions. The degree of integration was high and funds were sufficient. The theoretical framework was studied from the macro, meso and micro perspectives, and people-centered, value-based integration was proposed. However, in China the integrated care was primarily guided by the government nationally. Local government was responsible for specific integration practices. The degree of integration was low and funds were insufficient. The theoretical research mostly focused on cooperation, interest mechanisms and so on. At home and abroad, researchers focused on the evaluation of health service and quality. Foreign countries paid more attention to medical costs, while in China, due to the imbalance of interest mechanisms, researchers paid more attention to medical expenses and performance evaluation.ConclusionThere is no unified theoretical framework and method for integrated medical care. Researchers and policy makers should combine the successful experience and characteristics at home and abroad, consider the complex health policy context of the country to implement integrated care.
With the establishment and development of regional healthcare big data platforms, regional healthcare big data is playing an increasingly important role in health policy program evaluations. Regional healthcare big data is usually structured hierarchically. Traditional statistical models have limitations in analyzing hierarchical data, and multilevel models are powerful statistical analysis tools for processing hierarchical data. This method has frequently been used by healthcare researchers overseas, however, it lacks application in China. This paper aimed to introduce the multilevel model and several common application scenarios in medicine policy evaluations. We expected to provide a methodological framework for medicine policy evaluation using regional healthcare big data or hierarchical data.
ObjectiveTo analyze the clinical characteristics and evaluate the effect and safety of anti-vascular endothelial growth factor (VEGF) therapy in retinopathy of prematurity (ROP) in Sichuan province. MethodsA retrospective study. From January 2013 to January 2022, 156 patients (306 eyes) with ROP who received intravitreal anti-VEGF therapy for the first time in the Department of Ophthalmology, West China Hospital of Sichuan University were selected. According to the type of anti-VEGF drugs, the children were divided into intravitreal injection of ranibizumab (IVR) group and intravitreal injection of conbercept (IVC) group; IVC group was divided into hospital group and referral group according to the different paths of patients. After treatment, the patients were followed up until the disease degenerated (vascular degeneration or complete retinal vascularization) or were hospitalized again for at least 6 months. If the disease recurred or progressed, the patients were re-admitted to the hospital and received anti-VEGF drug treatment, laser treatment or surgical treatment according to the severity of the disease. Clinical data of these children was collected, including general clinical characteristics: gender, gestational age at birth (GA), birth weight (BW), history of oxygen inhalation; pathological condition: ROP stage, zone, whether there were plus lesions; treatment: treatment time, postmenstrual gestational age at the time of the first anti-VEGF drug treatment; prognosis: re-treat or not, time of re-treatment, mode of re-treatment; adverse events: corneal edema, lens opacity, endophthalmitis, retinal injury, and treatment-related systemic adverse reactions. The measurement data between groups were compared by t test, and the count data were compared by χ2 test or rank sum test. ResultsOf the 306 eyes of 156 children with ROP, 74 were male (47.44%, 74/156) and 82 were female (52.56%, 82/156). Each included child had a history of oxygen inhalation at birth. The GA was (28.43±2.19) (23.86-36.57) weeks, BW was (1 129±335) (510-2 600) g, and the postmenstrual gestational age was (39.80±3.04) (31.71-49.71) weeks at the time of the first anti-VEGF drug treatment. All patients were diagnosed as type 1 ROP, including 26 eyes (8.50%, 26/306) of aggressive ROP (A-ROP), 39 eyes (12.74%, 39/306) of zone Ⅰ lesions, and 241 eyes (78.76%, 241/306) of zone Ⅱ lesions. The children were treated with intravitreal injection of anti-VEGF drugs within 72 hours after diagnosis. Among them, 134 eyes (43.79%, 134/306) of 68 patients were treated with IVR, and 172 eyes (56.21%, 172/306) of 88 patients were treated with IVC. In IVC group, 67 eyes of 34 patients (38.95%, 67/172) were in the hospital group and 105 eyes of 54 patients (61.05%, 105/172) were in the referral group. 279 eyes (91.18%, 279/306) were improved after one treatment, 15 eyes (4.90%, 15/306) were improved after two treatments, and 12 eyes (3.92%, 12/306) were improved after three treatments. The one-time cure rate of IVR group was lower than that of IVC group, but the difference was not statistically significant (χ2=1.665, P=0.197). In different ROP categories, IVC showed better therapeutic effect in A-ROP, and its one-time cure rate was higher than that in IVR group, with statistically significant difference (χ2=7.797, P<0.05). In the hospital group of IVC group, the GA, BW and the postmenstrual gestational age at first time of anti-VEGF drug treatment were lower than those in the referral group, and the difference was statistically significant (t=-2.485, -2.940, -3.796; P<0.05). The one-time cure rate of the hospital group and the referral group were 94.94%, 92.38%, respectively. The one-time cure rate of the hospital group was slightly higher than that of the referral group, but the difference was not statistically significant (χ2=0.171, P=0.679). In this study, there were no ocular and systemic adverse reactions related to drug or intravitreal injection in children after treatment. ConclusionsCompared with the characteristics of ROP in developed countries, the GA, BW and postmenstrual gestational age of the children in Sichuan province are higher. Both IVR and IVC can treat ROP safely and effectively. There is no significant difference between the two drugs in the overall one-time cure effect of ROP, but IVC performed better in the treatment of A-ROP in this study.
Objective To evaluate the application effect of epilepsy prevention and management project in rural areas of Neijiang City. MethodsAccording to the requirements of the "Rural Epilepsy Prevention and Management Project Work Manual" (2021 edition) of the National Epilepsy Prevention and Management Project Office, the follow-up data of patients in rural areas of Neijiang City from the beginning of the epilepsy prevention and management project to December 2023 were collected, and the patient treatment effect of the project was evaluated. Results The total effective rate was more than 70% after 3 months of treatment, As the treatment time is extended, the total efficiency is upward. Adverse drug reactions occurred mainly in the first 3 months of treatment.The patients dropped out of the group because they did not achieve the expected effect, poor treatment compliance and could not tolerate serious adverse reactions. Conclusion The prevention and treatment of epilepsy in rural areas of our city has a good effect of promotion.The follow-up management of patients in the first 3 months of treatment should be strengthened to ensure the continuity and standardization of treatment.
