Objective To evaluate the clinical application value of ocular fundus photochromy and optical coherence tomography (OCT) in epimacular membrane of traumatic eyes following vitreorential surgery. Methods Ocular fundus photochromy and OCT were used to observe the morphological changes of macula in 139 consecutive patients with unilateral ocular trauma after vitreoretinal surgery between July 2003 and September 2004. Results At the 1st, 2nd, 3rd, 6th, and 12th month postoperatively, epimacular membranes were found in 5, 8, 17, 19, 20 patients, respectively by using photochromy; and in 7, 10, 20, 26, 27 patients, respectively by using OCT. There was no significant difference in epimacular membrane between photochromy and OCT(chi;2=0.173,Pgt;0.05). The results of OCT showed that epimacular membranes completely adhered to the macular region in 8 patients(29.63%), and retinal neurosensory layer swelled in 12 patients(44.44%)with the fovea thickness of (290.18plusmn;288.05) mu;m. Conclusion Ocular fundus photochromy and OCT are helpful in observing and discovering the epimacular membrane after virtreoretinal surgery of the patients with traumatic retinal detachment during the early and long-term follow-up period; and OCT can provide important information of the structue and position of these epimacular membranes hence to conduct to appropriate treatment. Objective To study the factors affecting the prognosis of vitrectomy in mechanically injured eyes. Methods One hundred and thirty-Seven eyes undergone vitrectomy were in cluded.Recorded to EPIINFO data base were visual acuity (VA) immediately after injury,type and location of injury,the interval from injury to surgical intervention,retinal detachment,basic surgical maneuvers,proliferative vitreo-retinopathy (PVR),stage of trauma,complications,final VA,and final retinal status.Data were analyzed by SAS. Results We got anatomic success in 107 eyes (75.9%).Final VA of 74 eyes (54.1%) were better than 0.02.Multiple-variable analysis showed that the factors significantly affecting prognosis of vitrectomied eyes of mechanical injury were retinal detachment (RD) before surgery,delay of surgical intervention,complexity of surgical maneuvers,complications after surgery. Conclusions RD before surgery,delayed surgical intervention,complexity of surgery and post vitrectomy complications are the most important factors contributing to the poor prognosis of VA and failure of surgery. PURPOSE: Determining the efficacy of vitrectomy in explosive injuries of eye globes and assessing the curcept concept of enucleation for severe traumatized eyes. METIIODS: Clinical records were reviewed on 36 consecutive patients(44 eyes)with severe explosive eyeball injuries. RESULTS:The injuries were caused by explosion of detonator (10 eases), fire-crackers(7 cases) ,explosive and guns(19 cases). Ten eyes(22.7%)were ruptured. Fourty eyes(90. 9%)underwent vitrectomy for posterior segment injuries including vitreous hemorrhage,intraocular foreign bodies, endophthalmitis, and retinal detachment more than 2 weeks after trauma and primary wound repair. Postoperative visual acuity improved in 25 eye(62.5%) ,was stable in 11 eyes(27.5%) ,and decreased in 4 eyes(10%). Final vision was 0. 02 or better (up to 0.7)in 20 eyes(47.6%). No more enucleation was performed except two ruptured eyes (4.5% ) removed in primary clinical units. CONCLUSION :The results suggest that primary wound repair with microsurgery and secondary vitrectomy may reconstruct the eyeball and restore visual functions.at least partially.in the majority of eyes,even though the explosive ocular injuries often induce severe damages and eyeball rupture. It is.thus,recommended that primary enueleation of traumatized eyes should not be performed with an occasional exception. (Chin J Ocul Fundus Dis,1996,12: 169-171)
The pathogensis of choroidal vascular changes in traumatic rtinopethy remains uncertain.We performed scanning electron micrmcopie (SEM) observation of methyl methalerylare vascular corrosion casts in a rabbi model with severe retinal contusion. Areas of filling defects in corrosion casts of the choriocapillaries, correspending to the areas of impact retinal lesions were noted in the traumatized eyes one to 28 days after trauma.No neovascularization was found in the eyes 56 days after trauma. The results confirm that obstruction and disappearance of involved choriocapillaries are the main changes of choroidal vasculatrue in severe blunt tram. The changes may be associated with continuous necrosis, of the photoreceptors 4 weeks after injury. (Chin J Ocul Fundus Dis,1993,9:5-7)
