  • Exploration and Practice of Personnel Framework in Multi-Disciplinary Treatment for Colorectal Cancer

Objective To explore the personnel framework strategy of multi-disciplinary treatment (MDT) for colorectal cancer. Methods Combined the characteristics of large public hospital, with recognized treatment pathway in international MDT for colorectal cancer and management of medical human resource, colorectal cancer MDT project team summarized a system of multi-disciplinary treatment-working for colorectal cancer of West China Hospital (MDT-CRC-WCH) by own management mechanism and subject feature. Results Based on the main four principles——whole, match, voluntary and interactive, MDT-CRC-WCH constructed reciprocation “concentric circle” with the team of directors, coordinators, colorectal surgeons, related professors, nursing and other assistants. Conclusion With construction of multi-layers, MDT for colorectal cancer is completing MDT personnel framework and suitable layers relation. And how to construct prolong active and innovative system of MDT human resource management needed more research.

  • Tumor Data Interacted System Design Based on Grid Platform

In order to satisfy demands of massive and heterogeneous tumor clinical data processing and the multi-center collaborative diagnosis and treatment for tumor diseases, a Tumor Data Interacted System (TDIS) was established based on grid platform, so that an implementing virtualization platform of tumor diagnosis service was realized, sharing tumor information in real time and carrying on standardized management. The system adopts Globus Toolkit 4.0 tools to build the open grid service framework and encapsulats data resources based on Web Services Resource Framework (WSRF). The system uses the middleware technology to provide unified access interface for heterogeneous data interaction, which could optimize interactive process with virtualized service to query and call tumor information resources flexibly. For massive amounts of heterogeneous tumor data, the federated stored and multiple authorized mode is selected as security services mechanism, real-time monitoring and balancing load. The system can cooperatively manage multi-center heterogeneous tumor data to realize the tumor patient data query, sharing and analysis, and compare and match resources in typical clinical database or clinical information database in other service node, thus it can assist doctors in consulting similar case and making up multidisciplinary treatment plan for tumors. Consequently, the system can improve efficiency of diagnosis and treatment for tumor, and promote the development of collaborative tumor diagnosis model.

  • The framework of evidence-based continued quality improvement

Objective This study was to develop a framework of evidence-based continuous quality improvement and provide a framework for nursing professionals to promote evidence transfer and clinical nursing quality improvement. Methods Guided by the principles of PDCA, evidence-based nursing and work process, a framework of evidence-based quality continuous improvement was established through a citation analysis of evidence implementation related projects and literatures. Results The framework of evidence-based continuous quality improvement consisted of four stages and twelve steps, including evidence searching, practice audit, evidence implementation and outcome evaluation, and then it would turn to next cycle. Conclusion The framework of evidence-based continuous quality improvement provides nursing professionals a concept and method of promoting evidence into practice and promoting clinical nursing quality improvement.

  • The framework of the innovation of clinical practice: from PARIHS to i-PARIHS

Evidence-based nursing practice has received wide attention by scholars both domestically and abroad since it was proposed, and evidence utilization forms the key link of evidence-based practice activities. Promoting action on research implementation in health services framework (PARIHS) has been widely accepted and applied in the innovation of clinical practice. The research team has improved it to i-PARIHS based on years of continuous testing and improvement, as well as feedback from users. This article aims to compare PARIHS and i-PARIHS, and elaborates on basic connotations and core elements of i-PARIHS, so as to provide basis for the innovation of clinical nursing practice in China.

  • Theories, models and frameworks of implementation science

Implementation science is a new field that aims to scale up and sustain the application of evidence-based practice. It primarily studies the dissemination, adoption, evaluation, and improvement, etc. of the implementation process of evidence-based practice. In recent years, tens of implementation theories, frameworks and models have been developed. We systematically collected 41 of them and classified them into several categories. The first is the classical theories which are extended from the traditional social science and used as the predecessors of the implementation science. The second category is the implementation science frameworks/models that are derived from the first category, and can be divided into four groups: 1) the implementation models that explain the law of a certain aspect in the implementation; 2) the implementation determinants frameworks that summarize and classify the implementation determinants; 3) the process models that discuss how research translates into practice; 4) the evaluation models that evaluate the implementation and outcome. The implementation science theories can provide implementation researchers with valuable guidance for development of implementation strategies and evaluation of implementation outcomes.
