Although extracorporeal membrane oxygenation (ECMO) has been in existence since the 1970s as a means of supporting respiratory or cardiac function, early application of this technology was plagued by high complication rates. Peripheral veno-arterial extracorporeal membrane oxygenation (V-A ECMO) causes higher left ventricular end-diastolic pressure, pulmonary edema, left ventricular distention, ventricular arrhythmia, low coronary perfusion, myocardial ischemia, substantial thrombus formation within left ventricule cavity and even multiple organ dysfunction. Mechanical left ventricular decompression is required to treat these related complications. In this article, we reviewed the problems associated with left ventricular decompression supported by peripheral V-A ECMO in patients with cardiogenic shock.
The cardiac conduction system (CCS) is a set of specialized myocardial pathways that spontaneously generate and conduct impulses transmitting throughout the heart, and causing the coordinated contractions of all parts of the heart. A comprehensive understanding of the anatomical characteristics of the CCS in the heart is the basis of studying cardiac electrophysiology and treating conduction-related diseases. It is also the key of avoiding damage to the CCS during open heart surgery. How to identify and locate the CCS has always been a hot topic in researches. Here, we review the histological imaging methods of the CCS and the specific molecular markers, as well as the exploration for localization and visualization of the CCS. We especially put emphasis on the clinical application prospects and the future development directions of non-destructive imaging technology and real-time localization methods of the CCS that have emerged in recent years.
ObjectiveTo investigate the reliability and safety of the technique of percutaneous left ventricular transapical access guided by cardiac three dimensional CT angiography (3D-CTA) combined with echocardiography applied in structural heart defects.MethodsThe clinical data of 9 patients (7 males and 2 females with a median age of 50 years ranging from 43 to 64 years) with paravalvular leaks closed by percutaneous left ventricular transapical access in West China Hospital, from April 2015 to August 2018, were retrospectively analyzed. We applied preoperative cardiac 3D-CTA to define the puncture site and trace, which was established by combining with real-time guidance of transesophageal echocardiography (TEE/3D-TEE), and an occluder was deployed at the apical access point for hemostasis with real-time guidance of transthoracic echocardiography (TTE).ResultsThe puncture needles were successfully introduced into the left ventricular cavity at one time in all patients without injury of lung tissue, coronary artery or papillary muscle. There was no occluder displacement or apex bleeding. One patient developed pleural effusion caused by intercostal artery injury.ConclusionThat cardiac 3D-CTA is used to define puncture sites and trace with advantages of simplicity and repeatability. A safe access and secure exit of left ventricle can be achieved by combining with real-time guidance of echocardiography. There are acceptable technology-related complications.
Objective To analyze the metabolic characteristics of myocardial infarction (MI) using metabolomics to better understand its pathogenesis and to explore new therapeutic directions for MI. Methods Serum metabolites in ten acute MI mice and five sham-control mice were analyzed by UHPLC-QqQ/MS, and SPSS was used for statistical analysis. MetaboAnalyst 5.0 was used to analyze the metabolic pathways of the differential metabolites and build a metabolic network. Results One hundred and twenty-nine metabolites were detected by UHPLC-QqQ/MS. Significant serum metabolite differences were found between MI mice and normal controls. Fifty out of 129 metabolites in serum were associated with MI. In addition, the most important metabolic pathways were D-glutamate metabolism, alanine, aspartate and glutamate metabolism, glycine, serine and threonine metabolism, glyoxylate and dicarboxylate acid metabolism. ConclusionMetabolites in serine-related metabolic pathways reduce in serum in MI. We propose a new therapeutic direction for myocardial protection in MI.
Objective To study feasibility of retrograde infusion of hyperpolarization-activated cyclic nucleotide-gated channel protein 4 (HCN4) and connexin fluorescent dye (Alexa Fluor 633)-labeled antibodies through the aorta to image the cardiac conduction system (CCS) in rat hearts. The optimal dosage, infusion time and photochemical stability of fluorescent dyes were also studied. Methods Ex vivo rat heart anterograde perfusion models were established in 33 male SPFSD rats, and the primary and secondary antibody solutions were injected sequentially. The atrioventricular junction was observed and the fluorescence intensity of the area was recorded when the perfusion reaches the scheduled time. We set five dose gradients (3 rats per gradient), 5 perfusion time gradients (three rats per gradient) and 10 LED continuous illumination time gradients in 3 rats under specific dose and perfusion time, the fluorescence intensities of the region were observed and recorded. Standard immunofluorescence stained paraffin sections and frozen sections were prepared for histological comparison. Results A HCN4 red fluorescence signal aggregation region was observed in the atrioventricular junction, which was identified as the AVN structure based on HCN4/Cx43 semi-quantitative fluorescence intensity analysis and histological comparison. With increasing antibody perfusion time, both AVN and background fluorescence intensity showed no statistically significant difference. The ratio of AVN to background fluorescence intensity also increased with the increasing antibody perfusion time. When the illumination time of AVN was prolonged, the fluorescence intensity of both AVN and background showed a downward trend but no statistically significant difference. Conclusion The anterograde perfusion with fluorescent dye (Alexa Fluor 633)-labeled antibody can successfully image the AVN of the CCS in ex vivo rat hearts under stereoscopic fluorescence microscopy. Increasing the antibody dose results in different AVN imaging effects. The imaging effect of AVN improves with an increase antibody perfusion time. Even after long-term (8 h) exposure to light, Alexa Fluor 633 can still maintain a certain level of AVN image.
目的 观察和评价采用偏心型封堵器导管介入治疗干下型室间隔缺损的近期疗效和安全性。 方法 2011年8月-12月,6例经无主动脉瓣脱垂的干下型室间隔缺损(直径≤7 mm)患者(年龄>3岁)在杂交手术室接受介入治疗。造影评估后,建立动静脉轨道,在保留导丝的情况下置入合适型号的国产偏心型室间隔缺损封堵器,并于术后定期随访复查。 结果 6例患者缺损直径4~7 mm(平均5.3 mm),其中5例成功地接受了导管介入封堵治疗,置入封堵器直径5~9 mm(平均6.4 mm)。仅1例因封堵器置入后出现主动脉瓣受压影响关闭,即改由外科微创经胸封堵成功。所有患者在随访期内,无栓塞、残余分流、瓣膜功能障碍、房室传导阻滞、死亡等并发症。 结论 无主动脉瓣脱垂的干下型室间隔缺损患者接受导管介入封堵治疗是安全、可行的,且短期随访结果良好。
Objective To compare the effect and degree of satisfaction of different analgesic interventions for tubes of pericardium and mediastinum removal after cardiac surgery. Methods From December 2017 to June 2018, 94 patients undergoing open heart cardiac surgery with tubes of pericardium and mediastinum were allocated randomly into three groups including a group A (32 patients), a group B (35 patients), and a group C (27 patients). In the group A, intravenous Dezocine was given as analgesics. In the group B, intravenous Dezocine plus oral ibuprofen were given. In the group C, interventions of group B plus specific nursing guidance were given. With the help of visual analogue scale (VAS), scores of pain during and after the tubes removal were collected. The degree of satisfaction and other adverse effects were also recorded. Results The VAS scores and fading time of pain in the group B were significantly lower than those of the group A (P<0.017). The score of sleeping was better in the group B as well. The VAS scores and fading time of pain in the group C were also significantly lower than those of the group A, and the scores of mood, activity as well as degree of satisfaction were all higher than those in the group A (P<0.017). The scores of activity and degree of satisfaction in the group C were higher compared with the group B (P<0.017). Conclusion Combination of intravenous Dezocine and oral ibuprofen seems to be more effective than each individual. Professional and specific nursing guidance could increase the degree of satisfaction for chest tube removal after cardiac surgery.
Objective To evaluate the effectiveness of Medicated γ-IUD compared with other IUDs for contraception. Methods CBMdisc (1986 -2005), VIP (1989 -2005) , CNKI (1994-2005) , The Cochrane Library (Issue 4, 2005 ) and 10 related journals were searched for randomized controlled trials (R.CTs) and quasi-randomized controlled trials (quasi-RCTs) on the comparison between γ-IUD and other IUDs. The quality of included trials was critically appraised. RevMan 4.2.8 software was used for statistical analysis. Results Ten published studies with 8 381 participants were included. The results of meta-analysis were expressed with Peto OR ( cumulative pregnancy rate, cumulative expulsion rate, cumulative rate of removing for medical reasons) or OIL ( cumulative continuation rate ) and 95% CI. Compared with MSSR-165, Medicated γ-25,200 resulted in lower pregnancy rate at 1, 2, 3 years of follow-up [ with the OR (95% CI) 0. 31 (0.15,0.64) , 0. 31 (0. 17,0.54) , 0. 31 (0.19,0. 51) , respectively], lower cumulative expulsion rate in5 year follow-up [0.26 (0.17,0.40), 0.28 (0.20,0.41), 0.30 (0.21,0.42), 0.29 (0.19,0.44), 0.32 (0.21, 0. 48 ), respectively], lower cumulative rate of removing for medical reasons.after 4 year follow-up [0.57 (0.36,0. 92 ) ], and higher rate of cunmlative continuation in 5 years of follow-up 13.35 ( 2.29,4.89 ) , 2.76 ( 2.06,3, 69 ) , 2.41 (1.89,3.08), 2.22 (1.69,2.91), 1.99 (1.55,2.55), respectively]. Compared with TCu220, Medicated γ-25,200 resulted in lower cumulative pregnancy rate after follow-up 2, 3 years [ 0.46 (0.25 ,0.87 ) ; 0.48 (0.27,0. 85 ) ], lower cumulative expulsion rate after 1 year follow-up [ 0. 58 (0.35,0.97) ] , lower cumulative rate of being removed for medical reasons in5 year follow-up [0.37 (0.23,0.59), 0.38 (0.26,0.56), 0.45 (0.32,0.64), 0.47 (0.30,0.73), 0.58 (0.39,0.87), respectively ] and higher rate of cumulative continuation in 5 years of follow-up [ 2. 15 ( 1.61,2.87 ), 2.02 ( 1.56,2.63), 1.72 ( 1.34,2.20), 1.44 ( 1.08,1.91 ), 1, 39 ( 1.07,1.80), respectively]. Compared with Uterus-Cu, Medicated γ-25,200 resulted in higher rate of cumulative continuation after follow-up 1 year [ 0.51 (0.34, 0.77) ]. Compared with MLCu375, Medicated γ-25,200 resulted in lower cumulative rate of being removed for medical reasons and higher rate of cumulative continuation after follow-up for 3 years [ 0. 22 (0.09,0.52) , 2.84 ( 1.64,4. 94 )]. Compared with TCu380A, Medicated γ-25, 200 resulted in lower cumulative rate of being removed for medical reasons and higher rate of cumulative continuation in follow-up for 2 years [ 0.36 ( 0.18,0.73 ), 0.29 ( 0. 12,0.70 ) ; 3.06 ( 1.72,5.44), 3.61 (1.75,7.47) ]. Conclusions Available evidence shows that Medicated γ-IUD has better or equal effectiveness when compared with other IUDs. However, more RCTs with high quality and longer follow up period are needed to confirm the conclusion.
ObjectiveTo explore the mechanism of volume-related mitral regurgitation (MR) from the anatomy of mitral valve.MethodsA total of 32 patients with ventricular septal defect (VSD) combined MR meeting inclusion criteria in West China Hospital from September 2018 to November 2019 were enrolled in this study. The direction relative to the cardiac axis: the deviation of the MR bundle along the left atrial wall was eccentric, otherwises it was central. There were 23 patients of VSD and eccentric MR (EMR, a VSD-EMR group), including 10 males and 13 females aged 21 (10, 56) months, and 9 patients of VSD and central MR (CMR, a VSD-CMR group), including 4 males and 5 females aged 26 (12, 87) months. Besides, 9 healthy children were enrolled in a control group, including 4 males and 5 females aged 49 (15, 72) months. All patients underwent transthoracic echocardiography (TTE) examination at 2 weeks before surgery and 6 months after surgery, respectively, The MR degree, end-diastolic volume (EDV), end-systolic volume (ESV), stroke volume (SV), ejection fraction (EF), antero-posterior diameter (AP), annulus circumference (AC), commissural diameter (CD) were assessed.ResultsBefore operation, EDV, ESV, SV, AP, AC and CD in the VSD-EMR and VSD-CMR groups were significantly larger or longer than those in the control group (P<0.05); after operation, EDV, ESV, SV, AP and CD decreased compared with those before operation (P<0.05), but there was no significant difference compared with the control group (P>0.05). Compared with the control group, AC was slightly decreased (P<0.05). There was no significant difference in EF between and within groups before and after operation (P>0.05). The improvement rate of MR was 78.9% (15/19) in the VSD-EMR group and 100.0% (9/9) in the VSD-CMR group.ConclusionAfter unloading of volume, the valve structure is back to normal except AC. The improvement rate of MR in the VSD-EMR group is lower than that in the VSD-CMR group, which may indicate that the mechanism of VSD-EMR is more complicated.
Objective To evaluate the effectiveness of mifepristone concomitant with misoprostol for medical abortion. Methods We searched the related original studies worldwide, and controlled prospective studies and systematic reviews based on randomized controlled trials (RCTs). Nine electronic databases were searched. Ten journals and reference lists of eligible studies were handsearched. Two reviewers independently screened the studies for eligibility, evaluated the quality, and extracted the data from eligible studies, with confirmation by cross checking. Any disputes were decided by a third person. Meta-analysis was conducted using statistical software RevMan 4.2. After heterogeneity test was done (α=0.05 ) , data without heterogeneity were pooled using a fixed effect model, and those with heterogeneity could be solved by sensitivity" analysis, subgroup analysis or random effect model. Results We found eight original trials (n = 3 348 ) that compared medical abortion with surgical abortion, nine trials (n =6 116) that investigating the effect of gestational ages on medical abortion, five trials (n = 1 934) on the use of mifepristone and two trials (n =2381 ) on intervals of administration of mifepristone and misoprostol were located. Quality of foreign studies was better than that of Chinese studies. Therate of complete abortion was higher in surgical abortion group than that in medical abortion group with odds ratio (OR) 0. 18 and 95% confidence interval (CI) 0. 11 to 0.27. The rate of incomplete abortion and abortion failure was higher in medical abortion group with OR 3.32, and 95% CI 1.79 to 6.17, OR 7.36, 95% CI 4. 17 to 12.98, respectively. The rate of complete abortion in the group with gestational age over 49 days was lower than that with gestational age under 49 days with OR 0.51 and 95% CI 0.43 to 0.61. The rate of incomplete abortion and abortion failure was higher in the group with gestational age over 49 days with OR 1.66, 95% CI 1.32 to 2.09 and OR 3.37, 95% CI 2.30 to 4. 94.There were no significant differences observed in the rates of complete abortion, incomplete abortion, abortion failed, time of expelling pregnant sac and time of menses recovery between the single and multi-dosage of mifepristone. Except for the rate of complete abortion, which was higher in 〈48h group, there was a comparable effectiveness for different intervals of mifepristone and ntisoprostol. Conclusions This review showes that it is important to improve the quality of Chinese original studies. Although the effectiveness is better in the surgical abortion group, the rate of complete abortion of medical abortion achieved is 91.6% (1 648/1 800). This is acceptable for clinicians and women who do not want to be pregnance. The rate of complete abortion is lower in the gestation over 49 days, which had a statistically difference, but little clinical significance. It is necessary to consider increasing the gestational age of medical abortion, especially for those women who have contradictions of surgical abortion or are afraid of operation. Effectiveness of single dosage of mifeprostone is similar to that ofmulti-dosage, but single dosage might be more convenient. This review suggests that shortening the interval ofmifepristone and misoprostol administration should be considered and the best and shortest interval time need to be identified with better evidence.
