  • RESEARCH PROGRESS IN REPAIR AND RECONSTRUCTION OF ISOLATED TRAUMATIC RADIAL HEAD DISLOCATION WITH ANNULAR LIGAMENT INJURY IN CHILDREN

    Objective To review the research progress in the repair and reconstruction of isolated traumatic radial head dislocation with annular l igament injury in children. Methods In recent years, the related l iterature concering isolated traumatic radial head dislocation with annular l igament injury in children was reviewed. Results For isolated traumatic radial head dislocation with annular l igament injury in children, the surgery should be chosen as the main treatment, includingopen reduction and annular l igament reconstruction surgery. Triceps aponeurosis is usually used as reconstruction materials of annular l igament, mainly because the position of taking material of annular l igament is at the operative incision with less surgery trauma and short operative time; aponeurosis is tough and thick with rigid fixation and low risk of re-dislocation. Artificial materials are paid attention to increasingly because they are easy to get, have rigid fixation, and can avoid operative injury caused by taking material of annular l igament. Conclusion Currently active annular l igament reconstruction surgery should be taken; triceps aponeurosis is widely adopted as reconstruction materials of annular l igament and artificial materials have come to be a new research trend.

  • Leaflet extension in reoperation after tricuspid valve repair in children

    Objective To summarize our experience on leaflet extension in reoperation after tricuspid valve repair in children at age≤15 years and to explore the application indicators and skills of this technique. Methods We retrospectively analyzed the clinical data of 23 children who underwent reoperation after tricuspid valve repair in Xinhua Hospital between January 2006 and October 2015. There were 15 males and 8 females with a mean age of 8.7 years, ranging from 5 to 15 years. The leaflet was extended by artificial pericardium patch. After surgery, warfarin anticoagulation therapy was done, and international normalized ratio was maintained 2.0 to 3.0. Results The average cardiopulmonary bypass time was 87-132 (98.5±35.7) minutes, and average aortic cross-clamping time was 56-97 (68.40±23.78) minutes. One patient died in hospital. There were 3 patients with complications including respiratory failure in 1 patient, acute renal failure in 1 patient, and right heart insufficiency in 1 patient. All the children cured and were followed up for 5 months to 10 years, with a mean follow-up of 3.5 years. One patient died during the follow-up. Six patients suffered mild to moderate tricuspid regurgitation and tricuspid valve function of the rest patients was good. No other redo-valve surgery or complications correlated to anticoagulation occurred. Conclusion Leaflet extension in reoperation after tricuspid valve repair in children is useful with optimistic middle to long term efficacy and needs intensive care therapy during the perioperative period.

  • Efficacy comparison between robot-assisted and conventional mitral valve surgery: A systematic review and meta-analysis

    ObjectiveThrough comparing the therapeutic efficacy of robot-assisted surgery (RS) and conventional surgery (CS) for mitral valve disease by meta-analysis to guide the choice of clinical operation.MethodsDatabases including The Cochrane Library, PubMed, EMbase, China National Knowledge Infrastructure (CNKI), China Biology Medicine disc (CBMdisc) and Wanfang Database were searched by computer from inception to June 2020. The literature of efficacy comparison between RS and CS was collected. Two reviewers independently screened the literature according to inclusion and exclusion criteria, extracted the data, and evaluated the quality of the literature. Meta-analysis was performed using RevMan 5.4 software.ResultsWe identified 11 studies of RS versus CS with 4 330 patients. Among them, 2 212 patients underwent RS and 2 118 underwent CS. Meta-analysis demonstrated that compared with the CS, RS had longer cross-clamp time (MD=25.00, 95%CI 15.04 to 34.95, P<0.000 01), cardiopulmonary bypass time (MD=44.11, 95%CI 29.26 to 58.96, P<0.000 01) and operation time (MD=46.40, 95%CI 31.55 to 61.26, P<0.000 01). However, ICU stay (MD=–22.13, 95%CI –31.88 to –12.38, P<0.000 01) and hospital stay (MD=–1.81, 95%CI –2.69 to –0.92, P<0.000 01) were significantly shorter in the RS group; and the incidences of blood transfusion (OR=0.38, 95%CI 0.16 to 0.89, P=0.03) and complications (OR=0.73, 95%CI 0.57 to 0.94, P=0.01) were significantly lower in the RS group.ConclusionAlthough RS has a longer operation time than CS, it has less damage, less bleeding, faster recovery and better curative efficacy.

  • Establishment and Characterization of Human Gastric Cancer Cell Line SGC-7901/HCPT Xenograft in Nude Mice

    Objective To establish a xenograft model of hydroxycamptothecine (HCPT)-resistant human gastric cancer cell line (SGC-7901/HCPT) in nude mice and study its biological characteristics. Methods The SGC-7901 and SGC-7901/ HCPT cells were cultured in vitro. The cell suspension was injected subcutaneously into the nude mice. When the subcutaneous carcinoma was 1.0 cm in diameter, it was cut off and divided into pieces of 0.1-0.2 cm in diameter. Then the small pieces of tumor were re-transplanted subcutaneously into the second generation nude mice until the fourth generation. The morphological feature, ultramicro-structure, and growth characteristics of the fourth generation transplanted tumor were examined. The drug resistance was measured by methyl thiazolyl tetrazolium (MTT) assay. Results The transplanted tumor in nude mice was round or oval, and many blood vessels were on its surface. Under the light microscope, the sizes of SGC-7901 transplanted tumor cells were similar, and sizes of cell nuclei were also similar; Meanwhile, the morphous of SGC-7901/HCPT transplanted tumor cells were irregular and in disorder, and the size of the cell nuclei was different from each other. Under the electron microscope, the mitochondria and endoplasmic reticulum of SGC-7901 transplanted tumor cells were nearly normal and no swelling in cell nuclei; Meanwhile the cell nuclei of SGC-7901/HCPT transplanted tumor cells were lightly swelled, a the mitochondria and endoplasmic reticulum were obviously swelled. By MTT assay, compared with SGC-7901 transplanted tumor cells, the resistance index of SGC-7901/HCPT transplanted tumor cells was 9.02±0.78 in HCPT, and resistance index to Adriamycin, Mitomycin C, 5-fluorouracil, and Etoposide was 1.24±0.09, 1.31±0.17, 0.96±0.12, and 1.07±0.16, respectively. Conclusions A transplanted tumor model of SGC-7901/HCPT in nude mice is established successfully, and showing stable drug resistance to HCPT and no cross-resistance to other chemotherapeutics, which can be used for further experiments.

  • Change of Th17 cells, Treg cells and their related cytokines in patients with postoperative atrial fibrillation after coronary artery bypass graft

    Objective To investigate the influence of T helpers 17 (Th17) cells, regulatory T (Treg) cells and their related cytokines on postoperative atrial fibrillation (POAF) after coronary artery bypass graft (CABG). Methods A total of 132 consecutive patients undergoing CABG between May 2013 and July 2016 were recruited. There were 82 males and 50 females with the age ranging from 39-76 years. Venous blood samples were collected within 2 hours after surgery. The expression of Th17 cells, Treg cells and their related cytokines in the peripheral blood was determined. Results POAF occurred in 35 patients (a POAF group) and 97 patients did not develop POAF (a No POAF group). Compared to the No POAF group, the proportion of Th17 cells and Th17/Treg ratio in the peripheral blood significantly increased in the POAF group (P>0.05) while proportion of Treg cells remained no significant change (P>0.05). The expression of Th17-related cytokines (IL-6, IL-8 and IL-17) all obviously increased in the POAF group (P>0.05). However, no significant difference was found in the expression of Treg-related cytokines (IL-10 and TGF-β) between the two groups (P>0.05). Conclusion Th17/Treg is unbanlanced in POAF patients and regulation of this imbalance may decrease the occurrence of POAF.

  • Efficacy of His-bundle pacing and right ventricular pacing: a meta-analysis

    ObjectivesTo systematically review the efficacy of His-bundle pacing (HBP) and right ventricular pacing (RVP).MethodsPubMed, The Cochrane Library, Web of Science, EMbase, CNKI, VIP and WanFang Data databases were electronically searched to collect randomized controlled trials (RCTs) and cohort studies on efficacy of HBP and RVP from inception to December, 2018. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies, then, meta-analysis was performed using RevMan 5.3 software.ResultsA total of 8 studies involving 1 130 patients were included. The results of meta-analysis showed that: HBP group was superior to RVP group in QRS duration (MD=–43.88, 95%CI –52.53 to –35.22, P<0.000 01), LVEF (MD=4.53, 95%CI 2.67 to 6.38, P<0.000 01), and NYHA (MD=–0.85, 95%CI –1.14 to –0.56, P<0.000 01). However, the operation time (MD=15.21, 95%CI 11.44 to 18.98, P<0.000 01) and fluoroscopy duration (MD=2.98, 95%CI 2.10 to 3.85, P<0.000 01) of HBP group were longer than that of RVP group.ConclusionsCurrent evidence shows that, compared with RVP, HBP is superior in maintaining of QRS duration, LVEF and NYHA; however, the operation time is longer. Due to limited quality and quantity of the included studies, more high quality studies are required to verify above conclusion.

  • Role of Vaspin in Roux-en-Y Gastric Bypass on Type 2 Diabetes Mellitus Rats

    Objective To study the therapeutic effect of Roux-en-Y gastric bypass (RYGB) on type 2 diabetes mellitus (T2DM) rats and explore the possible mechanism of vaspin in RYGB on T2DM. Methods Twenty SD rats with T2DM and 20 age- and sex-matched normal SD rats were randomly divided into 4 groups according to the random digits table:T2DM-RYGB group, T2DM-sham operation (SO) group,RYGB group,and SO group,10 rats in each group. Fasting plasma glucose (FPG) level,serum insulin (INS) level,vaspin level,and homeostasis model of insulin resistance (HOMA-IR) were determined before operation and on week 4,8 after operation,respectively.At the same time,the correlation between vaspin and the indicators (FPG,INS,or HOMA-IR) was analyzed.Results Compared the indicators after operation with before operation,the FPG level,INS level,vaspin level,and HOMA-IR were not significantly different between the T2DM-RYGB group and T2DM-SO group (P>0.05) or between the RYGB group and SO group (P>0.05),but the FPG level,INS level,vaspin level,and HOMA-IR in the T2DM-RYGB group and T2DM-SO group were significantly higher than those in the RYGB group (P<0.05) and SO group (P<0.05),respectively. On week 4 after operation,the FPG level,INS level,vaspin level,and HOMA-IR decreased in the T2DM-RYGB group,except for the FPG level,the other indexes had no significant differences as compared with the values before operation. On week 8 after operation,the FPG level,INS level,vaspin level,and HOMA-IR further decreased in the T2DM-RYGB group,there were significant differences of these indicators between before operation and on week 8 after operation. Compared the indicators after operation with before operation,the FPG level,INS level,vaspin level,and HOMA-IR were not statistically significant (P>0.05) in the T2DM-SO group,RYGB group,or SO group. The changes in serum vaspin level correlated positively with those in INS and HOMA-IR before operaion and on week 4,8 after operaion in the T2DM-RYGB group and T2DM SO group rats (P<0.05),respectively. Conclusions RYGB surgery has a therapeutic effect on T2DM rats,and serum vaspin level decreases and insulin resistance is improved after RYGB surgery,which may be one of the mechanisms of the treatment for T2DM.

  • Biatrial Cox Maze Ⅳ cryoablation procedure combined with minimally invasive mitral valve surgery for concomitant atrial fibrillation

    Objective To evaluate the safety and efficacy of biatrial Cox Maze Ⅳ cryoablation for concomitant atrial fibrillation (AF) during minimally invasive valve surgery. Methods A total of 47 patients (26 males, 21 females, age of 42-69 years) with mitral valve disease and long-standing persistent AF received minimally invasive biatrial Cox Maze Ⅳ cryoablation procedure combined with mitral valve surgery through right minithoracotomy from January 2014 to September 2015. The etiology of mitral valve disease was rheumatic (n=31) and degenerative (n=16). AF duration ranged from 2 to 11 years. Diameter of the left atrium ranged from 43 to 60 mm. Concomitant biatrial Cox Maze Ⅳ cryoablation procedure was performed through right lateral minithoracotomy. Results All 47 patients successfully underwent this minimally invasive concomitant biatrial Cox Maze Ⅳ cryoablation procedure and valve surgery. No patient needed conversion to sternotomy during the surgery. The mean cardiopulmonary bypass time, aortic cross-clamp time and cryoablation time was 95-146 (120.3±12.3) min, 82-115 (93.3±7.7) min and 32-48 (38.6±4.5) min, respectively. There was no death perioperatively. The average postoperative length of hospital stay was 5-16 (7.9±1.9) d. At discharge, 44 patients (44/47, 93.6%) maintained sinus rhythm. At a mean follow-up of 6-26 (14.4±5.4) months, sinus rhythm was maintained in 41 patients (41/47, 87.2%). Cumulative maintenance rate of normal sinus rhythm without AF recurrence at one year postoperatively was 86.3%±5.8%. Conclusion Biatrial Cox Maze Ⅳ cryoablation procedure is safe, feasible and effective for AF during concomitant minimally invasive valve surgery.

  • Exploration of referral patterns for rehabilitation of patients with spinal cord injury in community hospitals

    Objective To explore the referral patterns for rehabilitation of patients with spinal cord injury (SCI) in community hospitals and establish green channel for SCI rehabilitation between community hospitals and large comprehensive hospitals. Methods Thirty SCI patients who were referred from the Center of Rehabilitation Medicine, West China Hospital to community hospitals between February 2013 and June 2014 were selected for this study. All the patients were assessed with American Spinal Injury Association (ASIA 2011), activities of daily living (ADL), and instrumental activities of daily living (IADL). Based on the assessment results, rehabilitation plan was made and the patients underwent community rehabilitation treatment. Results After rehabilitation treatment in community hospitals, the scores of ASIA, ADL and IADL in all the SCI patients were significantly higher than those at the time of referral (P<0.001). Conclusions SCI patients can accept rehabilitation treatment in community hospitals with good conditions. This practice can provide a new type of seamless referral pattern between large comprehensive hospitals and community hospitals for rehabilitation of SCI patients.

  • Progressive volume deflation followed by rate reduction in weaning intra-aortic balloon pump

    Objective To investigate the changes of hemodynamics in patients weaning intra-aortic balloon pump (IABP) by using progressive volume deflation followed by rate reduction. Methods We retrospectively analyzed the clinical data of 36 patients aged 68.9±4.7 years, 22 males and 14 females, who underwent progressive volume deflation followed by rate reduction for IABP weaning in Xinhua Hospital between September 2006 and January 2016. Progressive volume deflation followed by rate reduction was used to wean IABP and collect hemodynamics parameters of each time point. Results All the patients successfully weaned IABP. One patient got re-IABP assistant 36 hours after the first successful weaning. One early death and three patients (8%) with postoperative IABP-related complications were embolization of the toe artery. One was in ipsilateral limb, and two of contralateral limb. One patient with acute hepatic insufficiency and one patient with acute renal insufficiency cured after treatment. Conclusion Intra-aortic balloon pump weaning is successful by using volume deflation followed by rate reduction which allowed better hemodynamic parameters.
