  • Study of Effects of Different Levels of Estradiol on Inhibitory of Tamoxifen on Human Mammary Cancer Cells(ER+) in Vitro

    【Abstract】 Objective To study the effects of different levels estradiol on inhibitory of tamoxifen on human mammary cancer cells(ER+) in vitro. Methods Estrogen receptor (ER) positive MCF7 human breast cancer cell line was treated by the same level of tamoxifen and different levels of estradiol in vitro. Cell proliferation was evaluated by MTT assay. Results E2 at concentrations between 1 × 10-12 mol/L to 1× 10-7 mol / L significantly stimulated the growth of MCF 7 . TAM (10 μmol/ L) inhabited the growth of MCF7 significantly. E2 at different levels may influence inhibitory of tamoxifen on MCF7 cell lines. E2 (1×10-8 mol/L) makes inhibitory of tamoxifen on MCF7 cell lines valueless.Conclusion E2 is the risk factor of breast cancer, and the concentration around breast cancer cells may influence the effects of TAM.

  • Comparison of Laparoscopic Cholecystectomy between the Highland and Non-highland Area

    【摘要】 目的 探讨高原地区腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)患者的特点,以便更好地进行围手术期处理。 方法 对2009年2月-2010年5月收治的长期生活在西藏高原地区的患者(高原组)367例和非高原地区患者(非高原地区组)167例的一般资料、术前诊断、合并症情况进行回顾性分析,两组患者性别、年龄及病程比较,差异无统计学意义(Pgt;0.05),有可比性。两组患者诊断均以胆囊结石为主,其次为胆囊息肉,诊断构成比较,差异无统计学意义(Pgt;0.05);两组患者合并症比较,高原组患者高血压、冠心病、血红蛋白增多症及窦性心动过缓的发生率高于非高原地区组(Plt;0.05);肺部疾病、肝硬化、糖尿病及脑梗死的发生率两组患者比较差异无统计学意义(Pgt;0.05)。两组患者均采用常规LC进行治疗,对两组患者术后临床结果、并发症等进行统计学分析。 结果 高原组患者手术中转开腹率(7.1%)高于非高原地区组(2.4%)患者(Plt;0.05);高原组患者较非高原地区组患者住院时间长、手术时间长、术中出血量多(Plt;0.05);术后并发症比较差异无统计学意义(Pgt;0.05)。 结论 高原地区LC患者宜及时中转开腹,其围手术期处理得当将有助于减少术后并发症的发生。【Abstract】 Objective To explore the characteristics of patients undergoing laparoscopic cholecystectomy in highland area, in order to carry out better perioperative management. Methods We collected and analyzed the general information, preoperative diagnosis and complications of 367 patients living in highland area and 167 patients living in inland between February 2009 and May 2010. There was no significant difference between the two groups in sex, age and course of disease (Pgt;0.05). Cholecystolithiasis was the main disease followed by gallbladder polyps, and there was no difference between them in the kind of diseases (Pgt;0.05). The incidence of hypertension, coronary heart disease, hereditary persistence of fetal hemoglobin and sinus bradycardia was higher in patients in highland area than that in patients in non-highland area (Plt;0.05). There was no significant difference in the incidence of lung disease, liver cirrhosis, diabetes mellitus and cerebral infarction between the two groups (Pgt;0.05). Conventional laparoscopic cholecystectomy was conducted in both two groups. Comparative analysis of treatment outcome and postoperative complications was done. Results The rate of conversion from laparoscopic surgery to laparotomy in Tibetan patients (7.1%) was higher than that in patients in non-highland area (2.4%) (Plt;0.05). Hospitalization time, operation time and blood loss in Tibetan patients were significantly higher than those in patients in non-highland area (Plt;0.05), but there was no significant difference in postoperative complications between the two groups of patients (Pgt;0.05). Conclusions Laparoscopic cholecystectomy for patients in highlardarea should be converted to laparotomy when necessary. Appropriate perioperative management is helpful in reducing the incidence of postoperative complications.

  • Study on The Effect of Estradiol on Proliferation of Diverse Mammary Primary Cells in Vitro

    Objective To understand the effect of estradiol in different concentrations on proliferation of diverse mammary primary cells in vitro. Methods The primary cells of cancer tissue, the adjacent tissue to tumors and normal mammary tissue from patiens with breast cancer were obtained using collagenase digesting method. All the tissue samples were cultivated in vitro, and were given estradiol in different concentrations. The effect of estradiol on the proliferation of those primary cells was measured by MTT. Results Estradiol remarkedly promoted the proliferation of primary cells of cancer tissue and peritumor tissue in vitro, whose ER expression were positive. Whereas, the promotion effect of estradiol on the proliferation of normal mammary primary cells was relatively weak, and there was no correlation between the promotion effect with the expression of ER in cancer tissue. Conclusion The risks of occurrence and relapse of breast cancer would increase significantly when the concentration of estradiol is no less than 103 pmol/L in vivo.

  • Experimental Study on Differentiation of Human Cord Blood Stem Cells into Hepatocytes

    Objective To investigate the feasibility of differentiating human umbilical cord blood stem cells into hepatocytes. Methods Thirty-six BALB/c nude mice were randomly divided into experimental group and control group(18 in each of the group), and experimental group was again randomly divided into group A, B and C (six in each of the group). The mice in experimental group and control group were exposed to 350 cGy radiation produced by 60Co. After 3 h, karyocytes at different concentrations in the fresh human umbilical cord blood were injected into the mice in experimental group A, B, C via their tail veins, and the equal volume of normal sodium (NS) was also injected into control group via tail veins. After one month, carbon tetrachloride (CCl4) was injected into experimental group A, B and control group via abdominal cavity, and the equal volume of normal sodium was injected into experimental group C. After two months, immunohistochemistry and reverse transcriptase polymerase chain reaction (RT-PCR) were used to detect the expressions of human cytokeratin-18 (CK18), cytokeratin-19 (CK19) and albumin (ALB) in liver tissues of all mice. Results The expressions of CK18, CK19 and ALB in injured liver tissues were all positive, and the expressions of experimental group B were higher than those of experimental group A (P<0.05), but the expressions of CK18, CK19 and ALB in the liver tissues of control group and experimental group C, whose were not injured with CCl4, were all negative. Conclusion Human umbilical cord blood-derived stem cells can differentiate into hepatocytes and express ALB under special microenvironment after liver injured by CCl4 , and the expression level of ALB maybe directly related to the number of human umbilical cord blood stem cells.

  • Surgical Treatment of Metastatic Hepatic Cancer (Report of 208 Cases )

    【Abstract】ObjectiveTo explore the effect of surgical treatment of metastatic hepatic cancer. MethodsTwo hundred and eight patients with metastatic hepatic cancer received surgical treatment in our department during the past seven years, and their information were analyzed retrospectively in this paper. The ages of these patients ranged from 19 years to 82 years, and 133 of them were male, 75 of them were female. Two of them were complicated with hepatocirrhosis, and 5 with polycystic liver. The metastatic cancer originated from gastrointestinal tract in 121 cases (58.2%), and from other parts of the body in 87 cases (41.8%). One hundred and sixteen of the patients received resection treatment (resection group), and 92 of them received nonresecton treatment (nonresection group). The survival rates of the two groups were compared through Chi square test.ResultsThe 1, 3 and 5year survival rates for all patients were 56.3%,23.1% and 13.0%, respectively. The 1, 3 and 5year survival rates were 74.1%,39.7% and 23.3% in the resection group respectively and 33.7%, 2.2% and 0 in the nonresection group, respectively. Resection group had a higher survival rate than that of the nonresection group (P<0.05). The main postoperative complications include pulmonary infection (6 cases), subphrenic infection (2 cases), incisional infection (4 cases).ConclusionSurgical resection is an effective treatment method for the patients with metastatic hepatic cancer. Resection should be conducted as long as they could withstand the surgery.

  • Effects of thyroid globulin antibody and thyroid peroxidase antibody statuses on central lymph nodes in patients with differentiated thyroid cancer: a multicenter clinical study

    ObjectiveTo investigate the effects of thyroid globulin antibody (TgAb) and thyroid peroxidase antibody (TPOAb) statuses on central lymph node (CLN) metastasis in patients with differentiated thyroid cancer (DTC).MethodsA retrospective analysis was performed on 526 patients with DTC confirmed by pathology from nine participating institutions, who underwent the bilateral thyroidectomy plus bilateral CLN dissection. The clinicopathologic characteristics of different TGAb and TPOAb statuses of patients with DTC were compared, and whether the TGAb and TPOAb statuses were the independent risk factors of CLN metastasis in DTC patients or not was analyzed.ResultsAll of 526 patients with DTC were included in this study, 152 were males and 374 were females. The age was (44±11) years old. There were 63 cases of TgAb+TPOAb+, 60 cases of TgAb+TPOAb-, 30 cases of TgAb-TPOAb+, and 373 cases of TgAb-TPOAb-. It was found that there was a significant difference in the gender among the four different antibody statuses of patients, that was, women with abnormal antibodies were more common (P<0.001), not found that there were related to the tumor size, blood vessel invasion, nerve invasion, CLN metastasis, tumor multifocality, and bilateral tumor or not (P>0.050). In this study, there were 389 cases of CLN with metastasis and 137 cases of CLN without metastasis. The results of multivariate analysis found that the age and gender of the patients were the independent risk factors (P<0.001), but didn’t find the TgAb and TPOAb Statuses and other factors were related to the CLN metastasis (P>0.050).ConclusionsStatuses of TGAb and TPOAb aren’t obviously associated with CLN metastasis in patients with DTC, which is inconsistent with other studies. It needs to be further researched after expanding existing sample size and determining new predictive factors.

  • In vitro Cytotoxicity to LoVo Cells of Amygdalin Following Specific Activation by Anti-CEA McAb-β-glucosidase Conjugate

    【Abstract】ObjectiveTo study the specific cytotoxicity of amygdalin(a new prodrug) to LoVo cells in antibody directed enzyme prodrug therapy (ADEPT). MethodsThe specific activation of amygdalin by anti-CEA McAb-β-glucosidase conjugate and the cytotoxicity of amygdalin to LoVo cells were assessed throughTrypan blue exclusion. ResultsThe cytotoxicity of amygdalin itself to LoVo cells was low. However, when amygdalin was combinated with anti-CEA McAb-β-glucosidase conjugate, its cytotoxic effect was enhanced by nearly 40 times, and that effect was specific to to LoVo cells expressing CEA . When LoVo cells and MCF-7 cells were co-cultured in various ratios, the cytotoxic effect of amygdalin combinated with anti-CEA McAb-β-glucosidase conjugate was measured. The survival rate of cultured cells decreased while the percentage of LoVo cells increased, suggesting the cytotoxic effect to be specific to LoVo cells. ConclusionThe toxicity of amygdalin is low,and it can be activated by anti-CEA McAb-β-glucosidase conjugate effectively to kill targetd cells specifically which may be a new way for colorectal tumor therapy.

  • Differential expression of BKCa channels in atrial fibroblasts in patients with sinus rhythm and atrial fibrillation

    Objective Through analyzing BKCa channel expression in atrial fibroblasts in patients with sinus rhythm and atrial fibrillation (AF), to explore the mechanism of myocardial fibrosis and provide new therapeutic strategies for the treatment and reversal of AF structure reconstruction. Methods We selected 10 patients of rheumatic heart valvular disease who underwent valve replacement surgery. They were 5 patients with sinus rhythm (a sinus rhythm group, 2 males and 3 females with an average age of 49.1±8.3 years) and 5 with AF (an AF group, 3 males and 2 females with an average age of 50.3±5.8 years). About 100 mg tissue was obtained from the right auricula dextra, and the atrial fibroblasts were cultured by tissue block adherence method, and the expression of BKCa channel genes and proteins in cultured fibroblasts was detected by quantitative real-time polymerase chain reaction (qRT-PCR) and western blotting methods. Results (1) The general data of 10 patients between the AF group and the sinus rhythm group were compared. There was no significant difference between the two groups in age (t=1.21, P=0.67) and sex (t=2.56, P=0.75). There was statistical difference in the left atrial diameter and the right atrium diameter between the two groups (t=19.45, P=0.01; t=23.52, P=0.06); (2) the mRNA expression of BKCa subunit was detected by qRT-PCR method, and there was no significant difference in the mRNA expression of BKCa α and BKCa β1 between the two groups (t=3.14, P=0.79; t=2.88, P=0.69); (3) the expression of BKCa protein was detected by western blotting method, and there was no significant difference in the protein expression of BKCa α and BKCa β1 between the two groups (t=0.55, P=0.31; t=0.73, P=0.46). Conclusion BKCa pathway may not be involved in the pathogenesis and maintenance of AF, but it may play an important role in the process of myocardial fibrosis.

  • Validation of multivariate selection method in clinical prediction models: based on MIMIC database

    ObjectiveTo verify the influence of different variable selection methods on the performance of clinical prediction models. MethodsThree sample sets were extracted from the MIMIC database (acute myocardial infarction group, sepsis group, and cerebral hemorrhage group) using the direct entry of COX regression, step by step forward, step by step backward, LASSO, and ridge regression, based on random forest. These existing six methods of variable importance algorithm, and the optimal variable set of different selected methods were used to construct the model. Through the C index, the area under the ROC curve (AUC value) and the calibration curve, and the results within and between groups were compared. ResultsThe variables and numbers selected by the six variable selection methods were different, however, whether it was within or between groups did not reflect which method had the advantage of significantly improving the performance of the model. ConclusionsPrior to using the variable selection method to establish a clinical prediction model, we should first clarify the research purpose and determine the type of data. Combining medical knowledge to select a method that can meet the data type and simultaneously achieve the research purpose.

  • Use of patient-sharing techniques to study healthcare provider networks: a scoping review

    ObjectiveTo provide a scoping review of the healthcare provider patient-sharing network. MethodsPubMed, EMbase, Scopus, ProQuest, Web of Science Core Collection, ScienceDirect, SAGE, Wiley Online Library, Google Scholar, CNKI and WanFang Data databases were electronically searched to collect studies on patient-sharing network of healthcare providers from inception to July 31, 2021. Two reviewers independently screened literature, extracted data and then Arksey and O 'Malley's scoping review method was used to analyze the study. ResultsA total of 110 studies were included. In which, 70.0% were published in 2016 and later, 78.2% were carried out in the United States, 96.4% used secondary data, and 45.5% adopted social network analysis methods such as exponential random graph model. In terms of network characteristics, 43.6% of the studies adopted the theoretical framework of social network theory, and the network node type was mainly 1-mode, accounting for 87.3%. When constructing the physician patient-sharing networks, 64.5% of the studies had a threshold of 1 patient. We also synthesized existing studies on patient-sharing networks of healthcare providers in the light of factors of networks and related outcomes. ConclusionThe studies of healthcare provider patient-sharing network have potentials to improve clinical practice and health policies. Further studies should consider adopting longitudinal design to validate evidence of study, expanding the scope of study subjects except physicians and enriching the evidence of the relationship between network and health-related outcomes.
