  • Effects of Fluid Resuscitation on Homeostasis and Acute Physiology and Chronic Health EvaluationsⅡof Patients with Septic Shock

    Objective To evaluate the effects and the clinical significances of liquid resuscitation on blood gas analysis, acid-base balance, electrolytes, acute physiology and chronic health evaluationsⅡ(APACHEⅡ) score of patients with septic shock, and then to analyze the relations between serum chlorine (Cl-) level and APACHEⅡscore and the volume of liquid resuscitation. Methods According to the target of resuscitation (centre venous pressure 8-12mm Hg and mean arterial pressure≥65mm Hg), 21 patients with septic shock received enough fluid for resuscitation during 24h . The results of blood gas analysis, acid-base balance, electrolytes, and APACHE Ⅱ score were compared between pre-resuscitation and 24h post-resuscitation by self-controlled prospective study. The relationships of the level of serum Cl- and APACHEⅡ score with the volume of liquid used in resuscitation were analyzed . Results The mean resus-citation duration was (18.09±4.57) h, and the volume of liquid during 24 h resuscitation was 5 320-11 028mL with mean volume of (7 775±1 735) mL in 21 patients with septic shock. Serum sodium (Na+, mmol/L) and Cl-(mmol/L)levels of post-resuscitation were significant higher than those of pre-resuscitation (Na+:138.71±5.67 versus 135.62±7.23, P=0.024;Cl-:109.10±4.90 versus 101.67±8.59, P=0.000). Compared with the levels of pre-resuscitation, the blood pH value, hematocrit (Hct,%), anion gap (AG, mmol/L), lactic acid (mmol/L), and APACHE Ⅱscore significantly decreased (pH:7.31±0.05 versus 7.37±0.06, P=0.000;Hct:28.48±2.56 versus 32.76±9.19, P=0.049;AG:8.33±3.45 versus 14.17±8.83, P=0.004;lactic acid:1.66±0.89 versus 2.96±1.23, P=0.001;APACHEⅡ:10.90±3.73 versus 17.24±4.06, P=0.000) after 24h resuscitation. The correlation analysis showed that the level of serum Cl- was positively correlated with the volume of liquid used in resuscitation (r=0.717,P<0.01). However, there was no correlation between APACHEⅡscore and the volume of liquid used in resuscitation (P>0.05). Conclusions The target of liquid resuscitation in patients with septic shock should be cautiously determined, including control of the volume of crystal liquid for resuscitation, in order to avoid acid-base imbalance or hyperchloraemia. At the same time, the change in internal environment should be monitored. An optimistic fluid resuscitation to decrease APACHE Ⅱ score in patients with septic shock is unrelated to the volume of liquid resuscitation.

  • Clinical Study of Insulin Resistance for Patients after Selective Operation in Department of General Surgery

    Objective To investigate the risk factors for insulin resistance (IR) after selective operation in the department of general surgery. Methods Two hundred and sixty-three patients including 122 males and 141 females after selective operation between March 2009 and October 2009 in The First Affiliated Hospital of Xi’an Jiaotong University were studied. Sex, age, histories of smoking and drinking, hypertensive disease, history of operation, height, weight, waist circumference, anesthesia method, operation duration, operation method, and volumes of transfusion and liquid injection during operation were recorded. The fasting blood glucose (BG) and fasting plasma insulin (INS) were tested for selectively operative patients on day 1 before and after surgery. Insulin resistance index (HOMA-IR) and the index of insulin secretion (HOMA-β) were calculated with homeostasis model assessment (HOMA). Logarithms of HOMA-IR (lnHOMA-IR) was taken because that HOMA-IR was not normal distribution. Results The levels of fasting BG, fasting plasma INS, and lnHOMA-IR on day 1 after operation were higher than those on day 1 before operation (Plt;0.001). IR was correlated with patients’ sex (P=0.002), the history of smoking (P=0.033), waist circumference (P=0.000), operation method (P=0.007), and the volume of liquid injection during operation (P=0.001). A significant elevation of the change of lnHOMA-IR level was found between abdominal and nonabdominal surgery (Plt;0.001). Conclusions IR occurs in selectively operative patients in the department of general surgery. It is helpful for depressing IR to control the intensity of surgery.

  • ROLE OF STEM CELL NICHES IN MAINTAINING CARDIAC STEM CELLS HOMEOSTASIS

    ObjectiveTo review the role of stem cell niches in maintaining cardiac stem cells homeostasis, and to foresee its prospects. MethodsThe literature on cardiac stem cells niches was extensively reviewed. The roles of stem cell niches components, extracellular matrix, and secretory factors in maintaining cardiac stem cell homeostasis were analysed and reviewed. ResultsLots of experiments reveal that stem cell niches are able to delay the aging of cardiac stem cells, protect from external damage, keep stem properties, and improve the purity and quantity. However, the mechanism is not fully understood. ConclusionThe stem cell niches have a very bright application prospect in homeostasis, purification, and amplification for the cardiac stem cells, and it needs further study.

  • Study on homeostasis and circadian rhythm of attention performance of different chronotypes in sleep deprivation

    Difference of chronotypes makes influence to cognitive performance of individuals in routine duties. In this paper, 55 subjects with different chronotypes were subjected to continuous sleep deprivation for 30 h by using the constant routine protocol, during which core body temperature was measured continuously, and subjective sleepiness self-rating and the performance of selective attention were measured hourly. The results showed that the phase difference of core body temperature has no significant difference, yet the amplitude and term difference among the three chronotypes are significant. There was an advance in phase between subjective sleepiness self-rating and core body temperature, and the self-rating sleepiness of evening type came the latest, and the self-rating sleepiness of morning type dissipated the fastest. The response time of selective attention showed a 2 h phase delay with subjective sleepiness self-rating. And the analysis of core body temperature showed that the later the chronotype was, the greater the phase delay was. The correct rate of selective attention of different chronotypes were inconsistent with delay of subjective sleepiness self-rating and core body temperature. We provide reference for industry, aviation, military, medical and other fields to make a more scientific scheduling/ shifting based on cognitive performance characteristics of different chronotypes.
