  • Interventional Ultrasound Combined with Endoscopic Technique Sequentially Minimally Invasive Treatment for Aged and High Risk Symptomatic Pancreatic Pseudocyst

Objective To summarize the experience of sequentially minimally invasive treatment for aged and high risk symptomatic pancreatic pseudocyst under ultrasound guided percutaneous catheter drainage combined with endoscopic technique. Methods The clinical data of 30 patients with aged and high risk pancreatic pseudocyst treated from January 2009 to January 2012 in this hospital were analyzed retrospectively. Results Percutaneous tubes were successfully placed in 30 patients, cystic liquid was sufficiently drained, and the compression symptom of cyst was relieved immediately. After stable disease, 12 patients with communicating cysts were diagnosed by using endoscopic retrograde cholangiopancreatography. Pancreatic duct stents were inserted in 12 patients with communicating cysts for 60-90d (with an average 70d) after endoscopic sphincterectomy and endoscopic pancreatic sphincterectomy. Cysts disappeared in the other 18 cases receiving external drainage with external drainage tubes, the drainage time was 15-90 d with an average 30d. Neither recurrence nor complications were found in all the cases during 12-21 months (with an average 18 months) follow-up. Conclusion Interventional ultrasound combined with endoscopic technique sequentially minimally invasive treatment for aged and high risk symptomatic pancreatic pseudocyst has superiorities in little trauma, fewer complications, and exact effect.

  • Clinical Application of Fast Track Surgery in Treatment for Cholecystolithiasis Combined with Calculus of Common Bile Duct by Combination of Laparoscope and Duodenoscope

Objective To evaluate the use of fast track surgery (FTS) in the treatment for cholecystolithiasis combined with calculus of common bile duct (CBD) by combination of laparoscope and duodenoscope. Methods One hundred and twenty patients with cholecystolithiasis combined with calculus of CBD underwent laparoscopic cholecyst-ectomy (LC) and endoscopic retrograde cholangiopancreatography (ERCP) were divided into FTS group (n=55) and conventional group (n=65),which were accepted the perioperative therapy of FTS or conventional therapy, respectively. After operation, the incision pain, nausea and vomiting, infusion time, loss of body weight,out-of-bed time, dieting time, postoperative hospitalization, hospital costs, and complications were compared in two groups. Results Compared with the conventional group, the postoperative infusion time, dieting time,out-of-bed time, and postoperative hospitali-zation were shorter, the incidence rates of pulmonary infection, and urinary systems infection, pancreatitis, nausea and vomiting, and incision pain were lower, the loss of body weight was lower in the FTS group (P<0.05),but the differences of WBC and serum amylase at 24 h after operation were not significant between the FTS group and conventional group(P>0.05). Conclusion The FTS is safe, economic, and effective in the treatment for cholecystolithiasis combined with calculus of CBD by combination of laparoscope and duodenoscope.

  • The Compare and Analysis of Biliary Tract Stent Placing for Malignant Obstruction of Biliary Tract by Percutaneous Transhepatic Cholangiography or Endoscopic Retrograde Cholangiography

ObjectiveTo compare the therapeutic efficacy of biliary tract stent placing for malignant obstruction of biliary tract by percutaneous transhepatic cholangiography (PTC) or endoscopic retrograde cholangiography (ERC). MethodsPTC approach: choosing the expansion intrahepatic bile duct which had a large angle traveling with common bile duct at the ultrasound-guided, we performed bile duct puncture and inserted the drainage pipe into it, then stent was placed with 68 cases (2 cases among the total were failure of ERC approach) after a week drainage. ERC approach: inserting drainage tube into the common bile duct by duodenal endoscopic retrogradely, the angiography showed obstruction site, the guide wire inserted through the obstruction site, then stent was placed along the guide wire with 53 cases. ResultsThe achievement ratio of stent placing by PTC was 100%(68/68), and which by ERC was 96.2%(51/53). The complications (bleeding, bile leakage) didn’t happen in two groups. 1-18 months (average 12.4 months) of follow-up, the died cases of PTC group and ERC group were 7 and 5 cases within 6 months, respectively; the survive cases of which were 17 and 9 cases after 18 months of treatment, respectively. ConclusionsThe biliary tract stent placing is a safe and effective method to the malignant obstruction of biliary tract patients who can not drainage tube be treated by operation. It can relieve biliary obstruction efficiency, and can increase live time and life quality for patients. We can choose the stent placing method by ERC for cases whose obstruction site is at the inferior of common bile duct or duodenal ampulla, and the cases whose obstruction site is at the above of hepatic porta should be chosen by PTC.
