  • GROWTH OF COMPOUND LAYER TISSURE ENGINEERED ORAL MUCOSA AND ITS CLINICAL APPLICATION IN HETERO-TRANSPLANTATION

    Objective To investigate the growth of the tissue engineered mucosa after its heterotransplantation. Methods The epithelial cells and fibroblasts were isolated from a postoperative tissue of the 3month patient with labialcleft. The epithelial cells and fibroblasts were separately seeded on the polylactic/glycolic acid copolymer membrane, and then they were exposed to the air-liquid interface. Seven volunteer patients, whose traumatic beds were repaired with the tissue engineered oral mucosa. The biopsy tissue from one of the seven patients was observed under light icroscope 18 and 30 days after transplantation, respectively. Results The tissue engineered oral mucosa having 5-6 layers anticytoeratin staining positively cells in the epithelial layer and 3-7 layers anti-Cytoeratin staining negatively cells in the subepithelial layer grew well after the he terotransplantation. No differ ence could be found between the transplanting and normal areas. At 18 days, the epithelial layer and lamina propria grew well and the fibroblasts were found; at 30 days, collagen was obviously observed. The structures in both the transplanting and the normal areas were similar. Conclusion The tissue engineered oral mucosa can grow well after the heterotransplantation.

  • PRELIMINARY CLINICAL STUDY OF ANCHORING CERVICAL INTERVERTEBRAL FUSION CAGE

    Objective To study the cl inical appl ication of anchoring cervical intervertebral fusion cage (ACIFC) in anterior cervical discectomy with fusion. Methods From November 2006 to June 2007, 21 cases of degenerative cervical disease were treated with anterior cervical discectomy, bone graft by ACIFC and anchoring stators, and 28 ACIFCs were implanted. There were 12 males and 9 females aged 25-68 years old (average 47.9 years old). The course of disease rangedfrom 3 days to15 years (median 2.3 years). There were 7 patients with single-segment cervical spondylosis, 3 with two-segment cervical spondylosis, 2 with single-segment lower cervical spine instabil ity, 4 with single-segment cervical spondylosis and lower cervical spine instabil ity, and 5 with cervical disc herniation. Postoperatively, X-ray films were taken regularly to detect the fusion of bone graft and the intervertebral height of fused segment was measured. The symptoms, signs and cervical functions of patient before operation, shortly after operation and during the follow-up period were evaluated by “40 score” system. And the occurrence of postoperative axial symptom (AS) was assessed with the standard set by Zeng Yan et al. Results All incisions healed by first intention. AS occurred in 1 case 48 hours after operation and was improved from poor to good after symptomatic treatment. No other kind of compl ication was identified or reported during intra-operative and postoperative period. All the cases were followed up for 16-24 months (average 20.5 months), and fusion was reached in all the intervertebral discs. Evaluated by “40 score” system, the average score for the cervical spinal cord function before operation, shortly after operation and during the final follow-up period was 26.2, 30.6, and 35.5 points, respectively, indicating there were significant differences between different time points (P lt; 0.05). During the follow-up period of above 1 year, the average improvement rate was 67.4%. The average intervertebral height before operation, shortly after operation and during the last follow-up period were 1.9, 4.4 and 4.3 mm, respectively, showing there were significant differences between the preoperation and the immediate postoperative and last follow-up periods (P lt; 0.05). No degeneration of adjacent segment was observed during the follow-up period. Conclusion Using ACIFC in bone graft fusion and internal fixation for degenerative cervical disease is convenientand fast, has wide range of indications with satisfying cl inical effect, and can achieve obvious therapeutic effect in restoring and maintaining cervical intervertebral height.

  • Expression of Fascin-1 Protein in Colorectal Adenocarcinoma and Relation with Its Clinicopathologic Characteristics

     Objective To investigate the expression of Fascin-1 protein in colorectal adenocarcinoma, and the relationship with its clinicopathologic features.  Methods The expressions of Fascin-1 protein in colorectal adenocarcinoma tissues of 60 cases, colorectal adenoma tissues of 30 cases and normal mucosa tissues (4 cm distance to neoplasm) of 30 cases were detected by Microwave-EliVisionTM immunohistochemistry method, and the relationship between the expression of Fascin-1 protein in colorectal adenocarcinoma tissues and its clinicopathologic characteristics was analyzed.  Results The expression of Fascin-1 protein was located in cytoplasm. The positive expression rates of Facsin-1 protein were 3.3% (1/30), 30.0% (9/30) and 53.3% (32/60) in normal mucosa tissues, colorectal adenoma tissues and colorectal adenocarcinoma tissues, respectively. The expression of Fascin-1 was gradually increased in these three tissues, and there was statistical difference among the three tissues (Plt;0.05). The expessions of Fascin-1 protein in patients with serous membrane invasion, lymph node metastasis and TNM Ⅲ+Ⅳ were higher than those of non-serous membrane invasion, non-lymph node metastasis and TNM Ⅰ+Ⅱ (Plt;0.05), but there was no significant difference among different differentiation degrees (Pgt;0.05).  Conclusion The high expression of Fascin-1 protein is correlated to high invasion ability and lymph node metastasis, which can play as a sensitive index in predicting the invasion and metastasis of colorectal adenocarcinoma.

  • Combination therapy of ionizing radiation and lipofectin-mediated cytosine deaminase/5-flurocytosine system for pancreatic cancer cell

    ObjectiveTo investigate the synergistic antitumor effects of ionizing radiation and the cytosine deaminase (CD)/5-flurocytosine (5-FC) system therapy in human pancreatic cancer cell.MethodsThe expression vector containing CD was transfected into the human pancreatic cancer cell line PC3. The clones were picked out after G418 selection. The CD gene integration and expression were confirmed by the RT-PCR. The cytotoxicity to the cells with or without CD and (or) ionizing radiation under the treatment with 5-FC was measured by the MTT assay. The clonogenic assay was used to investigate the radiosensitizing effect of 5-FC on the PC3 cells transfected or untransfected with CD gene.ResultsThe CD gene was stably expressed in the PC3 cells transfected with CD gene. The cytotoxic effect of 5-FC was superior on the PC3 cells transfected than that of untransfected with CD gene (P<0.05) and which were enhanced in combination with the ionizing radiation (P<0.05). The CD/5-FC enhanced the radiosensitivity of PC3 cells transfected with CD gene (P<0.05). The change in the radiosensitivity was quantified by calculating the sensitization enhancement ratio (SER) at the clinically relevant dose of 2 Gy. The SER was 1.5 in the PC3 cells transfected with CD gene by giving ionizing radiation of 2 Gy.ConclusionsCD/5-FC system is a potenial radiosensitizer in PC3 cells transfected with CD gene. Ionizing radiation and CD/5-FC system is more effective for killing effect of PC3 cells than ionizing radiation or CD/5-FC system alone.

  • PRELIMINARY CLINICAL STUDY ON ARTIFICIAL CERVICAL DISC REPLACEMENT BY Mobi-C PROSTHESIS

    Objective To study the cl inical appl ication of Mobi-C prosthesis in treatment of anterior cervical discectomy and artificial disc replacement (ADR). Methods Between January 2009 and June 2009, 20 cases of degenerative cervical disease were treated with anterior discectomy and ADR by Mobi-C prosthesis, including 13 cases of cervical disc herniation and 7 cases of cervical spondylotic radiculopathy, and 25 Mobi-C prosthesis were implanted. There were 8 males and 12 females, aged 29-54 years (mean, 45.2 years). The disease duration was from 4 days to 5 years (mean, 1.2 years). Affected segments of process included C3, 4 in 1 case, C4, 5 in 2 cases, C5, 6 in 7 cases, C6, 7 in 5 cases, C4, 5 and C5, 6 in 2 cases, and C5, 6 and C6, 7 in 3 cases. Radiographs were taken regularly, and cervical range of motion (ROM) on segments of disc replacements were measured. The functions of cervical spinal cord were evaluated by “40 score” system (COA) preoperatively, immediately postoperatively, and at follow-up. The qual ity of l ife was evaluated by neck disabil ity index (NDI) and visual analogue scale (VAS) score. Results All incisions healed by first intention. No perioperative compl ication was found. All cases were followed up 16.5 months on average (range, 14-18 months). There was no significant difference in cervical ROM of operatied segment between preoperation and follow-up duration (t=0.808,P=0.440). No heterotopic ossification was found at follow-up. COA score at last follow-up (38.20 ± 1.14) was significantly higher than preoperative one (32.10 ± 2.96) , (t=9.278,P=0.000) , and the improvement rate at last follow-up was 77.2% ± 5.4%. VAS score at last follow-up (3.20 ± 1.23) had significant difference when compared with preoperative one (5.10 ± 1.29), (t=10.585,P=0.000). NDI score at last follow-up (29.40 ± 4.55) had significant difference when compared with preoperative one (39.20 ± 3.80), (t=16.039, P=0.000). Conclusion A satisfactory short-term curative effect can be obtained by using Mobi-C prosthesis in treatment of anterior cervical discectomy and ADR.

  • The Expressions of Galectin-3, Fascin-1, and β-catenin Protein in Colorectal Adenocarcinoma and Their Relations to Clinicopathologic Characteristics

    Objective To explore the expressions of Galectin-3, Fascin-1, and β-catenin protein in colorectal adenocarcinoma and the relations to clinicopathologic characteristics. Methods The expressions of Galectin-3, Fascin-1, and β-catenin protein were detected in 60 cases of colorectal adenocarcinoma, 30 cases of adenoma, and 30 cases of normal mucosa by microwave-EliVisionTM immunohistochemistry method, and analyzed the expressions of them and the relations to clinicopathologic characteristics. Results The expression rate of Galectin-3, Fascin-1, and β-catenin protein in CRC was 68.3% (41/60), 53.3% (32/60), and 81.7% (49/60) respectively, which was 46.7% (14/30), 30.0% (9/30), and 43.3% (13/30) respectively in adenoma, and 20.0% (6/30), 3.3% (1/30), and 13.3% (4/30) respectively in normal mucosa, the differences had statistical significance (P<0.05). The expressions of Galectin-3, Fascin-1, and β-catenin protein had statistically significant correlation with the TNM stage, invasive degree, and lymph node metastasis of colorectal adenocarcinoma (P<0.05, P<0.01). The expressions of Galectin-3 and β-catenin protein had statistically significant correlation with the different differentiation degree of colorectal adenocarcinoma (P<0.05), but the expression of Fascin-1 protein was not related to differentiation degree of colorectal adenocarcinoma (P>0.05).The expressions of Galectin-3, Fascin-1, and β-catenin protein had not statistically significant correlation with the patient’s age and gender, and tumour size (P>0.05).There were positive correlations between the Galectin-3 and Fascin-1 or β-catenin (r=0.728,P<0.01;r=0.696,P<0.01), and there was positive correlation between β-catenin and Fascin-1 (r=0.507,P<0.01). Conclusions The high expressions of Galectin-3, Fascin-1, and β-catenin protein in colorectal adenocarcinoma tissues are some extent correlated to the high invasive ability and lymph node metastasis, which could be used for the indexes to predict the invasion and metastasis in colorectal carcinoma potentially.

  • Expressions of Galectin-3 and CD105 Protein in Colorectal Cancer and Their Significances

    ObjectiveTo explore the expressions of galectin-3 protein and CD105 protein in colorectal cancer and the relationship with clinicopathologic features. MethodsThe expressions of galectin-3 protein and CD105 protein 〔microvessel density (MVD)〕 were detected in 60 cases of colorectal cancer tissues, 30 cases of adenoma tissues, and 30 cases of normal mucosa tissues (at least 4 cm far from carcinoma) by MicrowaveEliVisionTM immunohistochemistry, and the relationship with clinicopathologic features was analyzed. ResultsThe expressions of galectin3 protein and MVD in normal mucosa tissues, adenoma tissues, and cancer tissues gradually increased (Plt;0.05). The expression of galectin-3 protein and MVD in colorectal cancer tissues were correlated to TNM stage, invasive depth, and lymph node metastasis (Plt;0.05, Plt;0.01), and the expression of glectin-3 protein was also correlated to differentiated degree (Plt;0.05). The expression of galectin-3 protein in colorectal cancer tissues was positively correlated to MVD (r=0.420, Plt;0.01). ConclusionsThe high expressions of galectin-3 protein and CD105 protein are correlated to the high invasion ability and lymph node metastasis, which may be potential sensitive index to predict the invasion and metastasis of colorectal cancer.

  • Selection of Appropriate Graft Directed by Venous Drainage Types of Median Hepatic Lobe in Adult-to-Adult Living Donor Liver Transplantation

    Objective To discuss venous drainage types of median hepatic lobe and their guiding significances on the selection of grafts. Methods Between April 2005 and March 2009, 109 potential living donors underwent 3-dimensional reconstruction of computed tomography (CT) and the volume of graft was determined in the center of organ transplantation of Ruijin Hospital. The venous drainage types of median hepatic lobe of each donor were analyzed by the computer-based liver operation-planning system in detail to assign middle hepatic vein (MHV) types according to Marcos classification and venous types of Ⅳb segment according to Nakamura classification. Results The branching pattern of MHV was divided into 3 types: Type Ⅰ and Ⅱwere relatively more accounting for 44.0% (48/109), 37.6% (41/109), and type Ⅲ was fewest 〔18.3% (20/109)〕. There were no significant differences in volume of whole liver, volume of left liver or left liver/total liver volume ratio among various types of MHV of the donor (Pgt;0.05). Ⅳb vein was also divided into 3 types: The most common was type Ⅰ, accounting for 72.4% (79/109); Type Ⅱ 〔12.8% (14/109)〕, type Ⅲ 〔14.7% (16/109)〕 were relatively fewer. At last, 37 donors provided right liver, for Marcos Ⅰ, Ⅱ, and Ⅲ type of donors, donors remained with MHV was 12/17, 8/11, and 5/9; for Nakamura Ⅰ, Ⅱ, and Ⅲ type of donors, those number were 16/26, 4/6, and 5/5. Conclusion In adult-to-adult living donor liver transplantation, there may be great significances in accordance with Marcos and Nakamura typing results to harvest right lobe liver graft with or without MHV.

  • Association between acute kidney injury and clinical outcomes in non-surgical patients receiving intensive cardiac care

    Objective To explore the clinical characteristics, in-hospital outcomes, and short-term survival of patients with acute kidney injury (AKI) in a large non-surgical cardiac intensive care unit (ICCU) in China. Methods Patients who had been admitted to the ICCU of the Department of Cardiology, West China Hospital of Sichuan University between June 2016 and May 2017 were retrospectively included. The diagnosis and staging of AKI were based on the Kidney Disease: Improving Global Outcomes criteria. The in-hospital outcomes were the composite of all-cause death or discharge against medical advice under extremely critical conditions. Patients without in-hospital composite outcomes were followed up to determine whether all-cause death occurred during the study period. The association of AKI with in-hospital composite outcomes or short-term survival was analyzed. Normally distributed quantitative data were expressed as mean±standard deviation, and non-normally distributed quantitative data were expressed as median (lower quartile, upper quartile). Results This study included 2083 patients, with an average age of (65.5±14.6) years old, and 681 (32.7%) were women. The prevalence rate of AKI was 15.0% (312/2083) (stage 1: 6.9%; stage 2: 4.9%; stage 3: 3.2%; respectively). Compared with patients without AKI, patients with AKI were older [(68.9±14.3) vs. (64.9±14.6) years old, P<0.001], had a higher Charles Comorbidity Index [4.0 (3.0, 6.0) vs. 2.0 (1.0, 3.0), P<0.001] and a greater Oxford Acute Illness Severity Score [32.0 (24.0, 41.2) vs. 21.0 (16.0, 26.0), P<0.001]. The incidence of in-hospital composite endpoint events was 8.4% (174/2083). Multiple logistic regression analysis showed that as the AKI stage increased, the risk of in-hospital composite endpoint events was higher [AKI stage 1 vs. no AKI: odds ratio (OR)=1.13, 95% confidence interval (CI) (0.57, 2.24); AKI stage 2 vs. no AKI: OR=2.21, 95%CI (1.08, 4.51); AKI stage 3 vs. no AKI: OR=10.88, 95%CI (4.50, 26.34); P for trend<0.001]. The patients without in-hospital composite endpoint events were followed up for a median time of 13.5 (10.7, 16.6) months, and the all-cause mortality rate was 5.5% (105/1909). Multiple Cox regression analysis showed that AKI was independently associated with all-cause death [hazard ratio=2.27, 95%CI (1.40, 3.69), P<0.001]. Conclusions AKI is common in the large ICCU in China and is more likely to occur in older patients who have more significant chronic illness complexity and acute illness severity. Moreover, AKI is independently associated with the in-hospital composite endpoint events and short-term survival.

  • Minimally Invasive Liver Resection:from Laparoscopic to Robotic

    Objective?To approach feasibility, safety, and the application range of pure laparoscopic resection (PLR), hand-assisted laparoscopic resection (HALR), and robotic liver resection (RLR) in the minimally invasive liver resection (MILR). Methods?The clinical data of 128 patients underwent MILR in the Surgical Department of the Shanghai Ruijin Hospital from September 2004 to January 2012 were analyzed retrospectively. According to the different methods, the patients were divided into PLR group, HALR group, and RLR group. The intraoperative findings and postoperative recovery of patients in three groups were compared.?Results?There were 82 cases in PLR group, 3 cases of which were transferred to open surgery;the mean operating time was (145.4±54.4) minutes (range:40-290 minutes);the mean blood loss was (249.3±255.7) ml (range:30-1 500 ml);abdominal infection was found in 3 cases and biliary fistula in 5 cases after operation, but all recovered after conservative treatment;the mean length of hospital stay was (7.1±3.8) days (range:2-34 days). There were 35 cases in HALR group, 3 cases of which were transferred to open surgery;the mean operating time was (182.7±59.2) minutes (range:60-300 minutes);the mean blood loss was (754.3±785.2) ml (range:50-3 000 ml);abdominal infection was found in 1 case, biliary fistula in 2 cases, and operative incision infection in 2 cases after operation, but all recovered after conservative treatment;the mean length of hospital stay was (15.4±3.7) days (range:12-30 days). There were 11 cases in RLR group, 2 cases of which were transferred to open surgery; the mean operating time was (129.5±33.5) minutes (range:120-200 minutes); the mean blood loss was (424.5±657.5) ml (range:50-5 000 ml); abdominal infection was found in 1 case and biliary fistula in 1 case after operation, but all recovered after conservative treatment; the mean length of hospital stay was (6.4±1.6) days (range:5-9 days). The operating time (P=0.001) and length of hospital stay (P=0.000) of the RLR group were shortest and the blood loss (P=0.000) of the PLR group was least among three groups. Conclusions?Minimally invasive resection is a safe and feasible. Different surgical procedures should be chosen according to different cases. The robotic liver resection provides new development for treatment of liver tumor.
