  • Pulmonary actinomycosis: three cases report and review of the literature

    ObjectiveTo improve the knowledge of pulmonary actinomycosis.MethodsThree cases of pulmonary actinomycosis in this hospital and 65 cases reported in China were analyzed retrospectively.ResultsAmong the 68 patients 49 were male and 19 were female aged 6 to 77 years old. The most common clinical manifestations were cough, sputum and fever. Inflammatory indicators was slightly elevated. The most common site was on the right upper lung. The typical CT manifestations were the low-density liquefaction necrotic zone in the center of the mass with vacuoles of different sizes, namely, "air-space consolidation". Positron emission computed tomography showed a mild metabolic increase in lesions. The 68 patients were confirmed by surgery, CT guided percutaneous lung puncture or bronchoscopic biopsy. The average time of the diagnosis was 10 months while the longest time was 6.4 years. The rate of first diagnosis was 5.9%. Forty-one cases were treated with antibiotics alone and 12 cases were treated with simple operation, the rest were treated by antibiotics combined with surgical treatment. The cure rate was 88.7%. Although active treatment was conducted 3 patients in this hospital were not cured.ConclusionsThe clinical features of pulmonary actinomycosis are atypical and the misdiagnosis rate is high. When pulmonary actinomycosis is suspected, it should be fully communicated with the microbiologist to ensure the cultivation in anaerobic environment and extension of the incubation cycle. Tissue culture and pathological biopsy should be actively performed. Treatment depends on antibiotics or surgery with good prognosis, but for some cases the prognosis is not optimistic.

  • In vitro experimental study on the mechanical properties of biodegradable polymer stents

    In vitro experimental test for mechanical properties of a vascular stent is a main method to evaluate its effectiveness and safety, which is of great significance to the clinical applications. In this study, a comparative study of planar, V-groove and radial compression methods for the radial support property test were performed, and the effects of compression rate and circumferential position on the test results were conducted. Based on the three-point bending method, the influences of compression rate and circumferential position on flexibility were also explored. And then a best test proposal was selected to evaluate the radial support property and flexibility of the three self-designed stents and the comparative biodegradable vascular stent (BVS) (BVS1.1, Abbott Vascular, USA) with different outside diameters of 1.4 mm, 1.7 mm and 2.4 mm. The results show that the developing trends of the compression load with the compression displacement measured by the three radial support property test methods are the same, but normalized radial force values are quite different. The planar compression method is more suitable for comparing the radial support properties of stents with different diameters and structures. Compression rate has no obvious effect on the testing results of both the radial support property and flexibility. Compression circumferential position has a great impact on testing radial support property with the planar or V-groove compression methods and testing flexibility with three-point bending method. The radial support properties of all the three self-designed stents are improved at a certain degree compared to that of the BVS stent. The study has better guide significance and reference value for testing mechanical properties of vascular stents.

  • The cognition of busyness and main busy scenes in intensive care unit nursing care: a qualitative study

    Objective To explore the nurses’ cognition of busyness in intensive care unit (ICU), summarize the main busy scenes, and provide strategies for solving problems of busyness. Methods Nurses in three ICU departments of Shanghai Oriental Hospital were selected by purpose sampling method from September 2020 to January 2021. Face-to-face semi-structured in-depth interviews were conducted with nurses. The interview data were analyzed and thematically refined using the method of Colaizzi data analysis. Results A total of 10 nurses were interviewed, including 8 general nurses and 2 head nurses, all of whom were women. The cognition of busyness covered three elements: explosively increased workload, time pressure, and overwhelming information from multiple sources. Busy scenes included four themes: large amount of patients, critical conditions of patients, unstable conditions of patients, and frequent service transfer among different medical divisions. Conclusions According to the three elements of nurses’ cognition of busyness and scenes of it, nursing managers can put forward corresponding solutions. This can retain or attract more nurses to work in ICU and provide better services for patients.

  • Recombinant human serum albumin promotes differentiation of human pluripotent stem cells into cardiomyocytes in vitro

    Objective To investigate whether recombinant human serum albumin (rHSA) can replace traditional B27 as a basic medium for differentiation of human pluripotent stem cells (hPSCs) into cardiomyocytes. Methods hPSCs were seeded at a cell density of 1.2×104/cm2; until up to 75% confluency hPSCs were induced by differentiation medium containing various concentration of rHSA (0, 50, 100, 200 g/L). Light microscope and fluorescence microscope recorded the whole process of stem cells differentiating into myocardium. Flow cytometry was used to detect the cardiac differentiation efficiency at different concentrations of rHSA. Immunofluorescence staining was used to detect the cardiac specific protein α-actinin and troponin T (cTnT) and electron microscope to observe the ultrastructure of human pluripotent stem cell-derived cardiomyocytes (hPSC-CM) and beating rates of hPSC-CMs response to drugs. Results A large number of spontaneous beating cardiomyocytes were observed 9 days after induction and differentition. The percentage of colonies showing beating cardiomyocytes was 60.4% at the concentration of 200 g/L of rice derived-rHSA. Beating cardiomyocytes were α-actinin and cTnT positive. Ultrastructural analysis showed scattered sarcomeres and mitochondrial. hPSC-CMs were dose-dependent on isopropyl adrenaline and verapamil. Conclusion Using such simple media to differentiate hPSCs into functional cardiomyocytes is cost-effective and highly efficient, and can be used in the clinical research.

  • Value of CT arterial enhancement fraction based on histogram analysis in severity of liver cirrhosis

    ObjectiveTo investigate the diagnostic performance of parameters of arterial enhancement fraction (AEF) based on enhanced CT with histogram analysis in the severity of liver cirrhosis.MethodsThe patients with liver cirrhosis clinically confirmed and met the inclusion criteria were included from January 2016 to December 2018 in the First Affiliated Hospital of Chengdu Medical College, then them were divided into grade A, B, and C according to the Child-Pugh score. Meanwhile, the patients without liver disease were selected as the control group. All patients underwent the upper abdomen enhanced CT scan with three-phase and the biochemical examination of liver function. The parameters of AEF histogram were obtained by using the CT Kinetics software, and the aspartic aminotransferase and platelet ratio index (APRI) was calculated. The differences of parameters of AEF histogram and APRI among these patients with liver cirrhosis and without liver disease were analyzed. The diagnostic performance was evaluated by using the area under curve (AUC) of receivers operating characteristic curve.ResultsEighty-five patients with liver cirrhosis were included in this study, including 25, 41, and 19 patients with grade A, B, and C of Child-Pugh score, respectively, and there were 20 patients in the control group. The consistencies in measuring the parameters of AEF histogram twice for the same observer and between the two observers were good (intraclass correlation coefficient was 0.938 and 0.907, respectively). The mean, median, and kurtosis of AEF histogram and the APRI among the grade A, B, C of Child-Pugh score, and control group had significant differences (all P<0.001) and these indexes were positively correlated with the severity of liver cirrhosis (rs=0.811, P<0.001; rs=0.827, P<0.001; rs=0.731, P<0.001; rs=0.711, P<0.001). The AUC of the mean, median, kurtosis, and APRI in diagnosing grade A of liver cirrhosis was 0.829, 0.841, 0.747, and 0.718, respectively; which in diagnosing grade B of liver cirrhosis was 0.847, 0.734, 0.704, and 0.736, respectively; in diagnosing grade C of liver cirrhosis was 0.646, 0.825, 0.782, and 0.853, respectively.ConclusionThe mean and median of AEF histogram parameters based on enhanced CT with three-phase and serological APRI are useful in diagnosis of grage A, B, and C of liver cirrhosis, respectively.

  • Diagnostic study of machine learning model based on combinatorial optimization to predict postoperative infectious complications of gastric cancer

    Objective To explore the application of combined optimized machine learning algorithm for predicting the risk model of postoperative infectious complications of gastric cancer and to compare the accuracy with other algorithms, so as to find reliable biomarkers for early diagnosis of postoperative infection of gastric cancer. Methods The clinical data of 420 patients with gastric cancer at the Third Affiliated Hospital of Anhui Medical University from May 2018 to April 2023 were retrospectively analyzed and the patients were randomly divided into training set and validation set. Univariate analysis was used to determine the risk factors of postoperative infectious complications. Six conventional machine learning models are constructed using the training set: linear regression, random forest, SVM, BP, LGBM, XGBoost, and MGA-XGBoost model. The validation set was used to evaluate the seven models through evaluation indicators such as ACC, precision, ROC and AUC. Results Postoperative infectious complications were significantly correlated with age, operation time, diabetes, extent of resection, combined resection, stage, preoperative albumin, perioperative blood transfusion, preoperative PNI, LCR and LMR. Among the seven machine learning models, the MGA-XGBoost model performed best. Among the seven machine learning models, the MGA-XGBoost model performed best, with AUC of 0.936, ACC of 0.889, recall of 0.6, F1-score of 0.682, and precision of 0.79 on the validation set. Diabetes had the greatest influence on the internal structure of the model. Conclusion This study proves that the MGA-XGBoost model incorporating comprehensive inflammation indicators can predict postoperative infectious complications in patients with gastric cancer.

  • The Essentials and Categories of Healthcare Risk Research in China

    Objective Healthcare risk has become a popular topic in medical research field all over the world. With the increasing of healthcare safety adverse events and medical conflict cases every year, publications about this topic have been rapidly increased in China, but research on the essentials and categories of healthcare risk are still lacked. This paper aims to summarize the essentials and categories of healthcare risk research systematically through analyzing domestic studies in healthcare risk over the past decade, so as to provide decision-making evidences and policy suggenstions on healthcare risk management in China. Methods We searched the China Academic Journal Network Publishing Database of CNKI (1999-2010), in which those papers’ title containing the phrase of healthcare risk had been brought into our research, then we summerized the domestic concepts and contents in healthcare risk by essential analysis and frequency statistics. Results 255 papers were included, including 26 Class B papers (accounting for 10.20%), six Class C papers (accounting for 2.35%) and 223 Class D papers (accounting for 87.45%); but there were only 79 papers (accounting for 35.11% ) which had a clear definition of healthcare risk. From these papers, we found the essentials of healthcare risk included object, consequences, process and causes, and the research category of healthcare risk included prevention and treatment of risk, causes and effects of factors, risk sharing mechanisms, healthcare risk of special diseases, concept and characteristics, education and training, management method, risks of medical appliances, the species and object of healthcare risk, the category of healthcare risk, the research methods of healthcare risk etc. Conclusion At present, the study of healthcare risk focuses on the risk control and how to release risks from the results, besides, systematic studies are lacked, and the unified definition of healthcare risk has not been formed, Therefore, we recommend that systematic study of the healthcare risks should be strengthened, the definition of healthcare risk should be unified, and the early warning, monitoring, evaluating, and identifying methods should be positively developed, providing decision-making evidences to establish healthcare risk management system in China.

  • Prevalence and Risk Factor Analysis on Snoring in Adults Aged over 35 Years in Kelamayi City

    Objective To study the prevalence of snoring in adults aged over 35 yrs in Kelamayi city of Xinjiang province, and screen the snoring associated factors.Methods 2600 subjects( age≥35 yrs) were enrolled from a random sample of the population living in 5 streets, two districts in Kelamayi city. All subjects were required to answer questions about their snoring by himself or bed partner at home. The questionnaire included items concerning snoring, daytime sleepness ( Epworth sleep scale, ESS) , smoking and drinking habits, while height, weight, neck circumference, abdominal circumference, waistline, and hip circumference were meatured. The subjects were assigned to a high risk group and a low risk group according to the questionaire of scoring. Results 2590 subjects had completed the questionnaire and 2513 ( 97. 0% )were eligble for evaluation. The cases of ever snoring was 1312 ( 52. 2% ) , among which moderate to severe snoring accounted for 38. 9% . Before the age of 60 yrs, the risk of snoring increased with age, and higher in males than females( 61. 7% vs. 45. 1% ) . The prevalence of snoring increased with neck circumference( P lt;0. 05) . The body mass index ( BMI) [ ( 28. 2 ±3. 6) kg/m2 vs. ( 25. 1 ±3. 9) kg/m2 ] , neck circumference [ ( 37. 7 ±3. 6) cmvs. ( 35. 6 ±3. 6) cm] , abdominal circumference [ ( 96. 9 ±13. 6) cm vs. ( 88. 7 ±11. 1) cm] , waistline [ ( 92. 2 ±9. 8) cm vs. ( 84. 7 ±10. 2) cm] , hip circumference [ ( 102. 9 ±9. 6) cm vs.( 96. 4 ±9. 3) cm] , proportionlity of waistline to hip circumference ( 0. 90 ±0. 07 vs. 0. 88 ±0. 08) , systolic blood pressure [ ( 132. 0 ±17. 5) mm Hg vs. ( 125. 6 ±16. 8) mm Hg] , and diastolic blood pressure [ ( 83. 3 ±12. 8) mm Hg vs. ( 78. 3 ±12. 6) mm Hg] were significantly different bettween the high and the low risk groups ( P lt; 0. 01) . Logistic regression analysis revealed that age ( OR = 1. 519) , BMI ( OR =2. 549) , neck circumference (OR = 2. 473) , smoking (OR = 2. 765) , ESS(OR = 2. 575) , and postmenopause( OR=1. 806) were main risk factors for snoring( P lt; 0. 05) . Conclusions The prevalence of snoring in adults over 35 yrs is high in Kelamayi city. The high risk factors for snoring are age, BMI, neck circumference, smoking, ESS, and post-menopause.
