  • Comparison of the value of bedside lung ultrasound and lung stretch index in guiding optimal positive end-expiratory pressure during lung recruitment in patients with acute respiratory distress syndrome

    Objective To investigate the guiding value of bedside lung ultrasound and lung stretch index for optimal positive end-expiratory pressure (PEEP) in lung recruitment of patients with acute respiratory distress syndrome (ARDS). Methods From February 2020 to October 2023, 90 patients with ARDS requiring invasive mechanical ventilation were selected from the Department of Critical Care Medicine, the Second Affiliated Hospital of Zhengzhou University. According to the setting method of PEEP after lung recruitment, they were randomly divided into an ultrasound group (45 cases) and a stretch group (45 cases). Both groups were treated with PEEP incremental method for lung recruitment, and the ultrasound group was treated with bedside ultrasound-guided method to set PEEP after lung recruitment. PEEP was set by lung stretch index method in the stretch group. The dynamic changes of oxygenation index (PaO2/FiO2), dynamic compliance (Cdyn), mean airway pressure and peak airway pressure were monitored before lung recruitment and 15 min, 1 h, 6 h and 24 h after lung recruitment. Heart rate, mean arterial pressure and central venous pressure were monitored before and 24 h after lung recruitment in the two groups. The optimal PEEP value and the corresponding volume at the end of recruitment were explored. The mechanical ventilation time, ICU hospitalization time, incidence of barotrauma, incidence of extrapulmonary organ failure, and 28-day mortality were recorded as well. Results After lung recruitment, the oxygenation index, Cdyn, mean airway pressure, and peak airway pressure in the ultrasound group were higher than those in the stretch group at 15 min, 1 h, 6 h, and 24 h after recruitment (all P<0.05). There was no significant difference in heart rate, mean arterial pressure or central venous pressure between the two groups at 24 h after lung recruitment (all P>0.05). After lung recruitment, the optimal PEEP value and the corresponding volume at the end of recruitment in the ultrasound group were higher than those in the distraction group (both P<0.05). The mechanical ventilation time and ICU stay in the ultrasound group were shorter than those in the stretch group (both P<0.05). There was no significant difference in the incidence of barotrauma, extrapulmonary organ failure rate or 28-day mortality between the two groups (all P>0.05). Conclusions Both bedside lung ultrasound-guided PEEP and lung stretch index-guided PEEP can improve oxygenation and respiratory compliance, and have no adverse effects on hemodynamics. Bedside lung ultrasound-guided PEEP can make the alveoli fully expand, which is more conducive to improving patients’ oxygenation and respiratory compliance, and the guiding value is higher than the lung stretch index.

  • Methodological quality evaluation on clinical prediction models of traditional Chinese medicine: a systematic review

    Objective To systematically review the methodological quality of research on clinical prediction models of traditional Chinese medicine. Methods The PubMed, Embase, Web of Science, CNKI, WanFang Data, VIP and SinoMed databases were electronically searched to collect literature related to the research on clinical prediction models of traditional Chinese medicine from inception to March 31, 2023. Two reviewers independently screened literature, extracted data and assessed the risk of bias of the included studies based on prediction model risk of bias assessment tool (PROBAST). Results A total of 113 studies on clinical prediction models of traditional Chinese medicine (79 diagnostic model studies and 34 prognostic model studies) were included. Among them, 111 (98.2%) studies were rated at high risk of bias, while 1 (0.9%) study was rated at low risk of bias and risk of bias of 1 (0.9%) study was unclear. The analysis domain was rated with the highest proportion of high risk of bias, followed by the participants domain. Due to the widespread lack of reporting of specific study information, risk of bias of a large number of studies was unclear in both predictors and outcome domain. Conclusion Most existing researches on clinical prediction models of traditional Chinese medicine show poor methodological quality and are at high risk of bias. Factors contributing to risk of bias include non-prospective data source, outcome definitions that include predictors, inadequate modeling sample size, inappropriate feature selection, inaccurate performance evaluation, and incorrect internal validation methods. Comprehensive methodological improvements on design, conduct, evaluation, and validation of modeling, as well as reporting of all key information of the models are urgently needed for future modeling studies, aiming to facilitate their translational application in medical practice.

  • Clinical analysis of surgical correction for congenital vascular ring in children in a single center

    ObjectiveTo investigate the best anatomical classification, surgical timing, procedure and clinical outcomes of congenital vascular ring.MethodsThe clinical data of 58 patients who underwent congenital vascular ring surgery in Pediatric Surgery Center, Fuwai Hospital between 2014 and 2019 were retrospectively analyzed. There were 32 (55.2%) males and 26 (44.8%) females with a median age of 16.5 (2-73) months. Preoperative symptoms, imaging examinations, anatomical classifications, surgical procedures and postoperative recovery were assessed.ResultsThere were 20 (34.5%) patients of double aortic arch, 22 (37.9%) patients of right aortic arch with left arterial duct or ligament, 15 (25.9%) patients of left aortic arch with aberrant right subclavian artery, and 1 (1.7%) patient of circumflex aorta with cervical aorta arch. The median ventilator supporting time was 6.0 (0-648) h, and the median hospital stay time was 14.5 (7-104) d. One patient with coarctation of aorta died of severe pulmonary infection during perioperative period, and the others survived without symptoms and reoperation after discharge. The median follow-up time was 7.0 (1-62) months.ConclusionFor children with unexplained dyspnea and dysphagia, or with right aortic arch, preoperative imaging examinations such as computed tomography or magnetic resonance imaging are required to confirm the diagnosis of vascular ring. Surgical correction of congenital vascular ring is safe and reliable, and can effectively relieve symptoms. The mortality rate and reoperation rate are low, and the follow-up results are satisfactory.

  • Conversion to thoracotomy during minimally invasive esophagectomy: Retrospective analysis in a single center

    Objective To explore the causes of conversion to thoracotomy in patients with minimally invasive esophagectomy (MIE) in a surgical team, and to obtain a deeper understanding of the timing of conversion in MIE. Methods The clinical data of patients who underwent MIE between September 9, 2011 and February 12, 2022 by a single surgical team in the Department of Thoracic Surgery of the Fourth Hospital of Hebei Medical University were retrospectively analyzed. The main influencing factors and perioperative mortality of patients who converted to thoracotomy in this group were analyzed. Results In the cohort of 791 consecutive patients with MIE, there were 520 males and 271 females, including 29 patients of multiple esophageal cancer, 156 patients of upper thoracic cancer, 524 patients of middle thoracic cancer, and 82 patients of lower thoracic cancer. And 46 patients were converted to thoracotomy for different causes. The main causes for thoracotomy were advanced stage tumor (26 patients), anesthesia-related factors (5 patients), extensive thoracic adhesions (6 patients), and accidental injury of important structures (8 patients). There was a statistical difference in the distribution of tumor locations between patients who converted to thoracotomy and the MIE patients (P<0.05). The proportion of multiple and upper thoracic cancer in patients who converted to thoracotomy was higher than that in the MIE patients, while the proportion of lower thoracic cancer was lower than that in the MIE patients. The perioperative mortality of the thoracotomy patients was not significantly different from that of the MIE patients (P=1.000). Conclusion In MIE, advanced-stage tumor, anesthesia-related factors, extensive thoracic adhesions, and accidental injury of important structures are the main causes of conversion to thoracotomy. The rate varies at different tumor locations. Intraoperative conversion to thoracotomy does not affect the perioperative mortality of MIE.
