  • Clinical Analysis of 100 Consecutive Completely Thoracoscopic Lobectomy

    Objective To investigate the safety and efficacy of completely thoracoscopic lobectomy and the indications of this procedure. Methods Between Sep. 2006 and Jun. 2008, 100 consecutive patients(46 men,54 women, median age60.1±12.5 years,range from 18 to 82 years) underwent completely thoracoscopic lobectomy. All candidates were either peripheral pulmonary nodules suspected of lung cancer (85 pts.) or benign lesions (15 pts.) localized within single lobe who needed to receive lobectomy. The lobectomy was completed through three tiny incisions in the intercostal space. Anatomic lobectomies were carried out in all cases and systemic lymph node dissection was performed in malignancies. This group consisted of lobectomies of right upper lobe (n=25), right middle lobe (n=14), right lower lobe (n=22), left upper lobe (n=18), and left lower lobe (n=21). Results All procedures were successfully completed except for 3 conversions to thoracotomy. Postoperative diagnosis were primary lung cancer (n=81), lymphoma (n=1), metastasis of clear cell carcinoma from kidney (n=1), and, benign lesions (n=17). Five patients had mild complications in which two had atelectasis, one needed temperately echanical ventilation, one had pneumonia and one had chylothorax. All were treated conservatively without reoperation. No operative mortality or serious complications occurred in this group. The operative duration was 186.4±52.9min (range from 60 to 300 minutes). The blood loss was 233.9±275.9ml(range from 50 to 750ml), and only one case needed blood transfusion. Chest drainage time was 7.1±3.0 days. Postoperative hospital stay was 9.5±3.2 days. Followedup time was for 1 to 27 months, metastasis happened in two patients with primary lung cancer 15 and 3 months separately after operation. Conclusion The completely thoracoscopic lobectomy is a safe and feasible surgical procedure with minimal invasiveness. The advocated indications include selected peripheral typed early stage lung cancer and benign pulmonary lesions which need lobectomy.

  • The Value of Endobronchial Ultrasoundguided Transbronchial Needle Aspiration in the Evaluation of Thoracic Diseases

    Abstract: Objective To introduce the new procedure of endobronchial ultrasoundguided transbronchial needle aspiration (EBUSTBNA) for staging lung cancer and diagnosing thoracic diseases, in order to determine its value in the evaluation of thoracic diseases. Methods We retrospectively reviewed the data of all patients examined with EBUSTBNA our institution between September 2009 and May 2010. Among the patients, there were 75 males and 31 females with an average age of 62.3 years old. Based on their primary indication, we divided all the 106 patients into three categories. (1) There were 76 patients with known or bly suspected lung cancer. Enlarged mediastinal lymph nodes on radiographic examination of the chest (≥1.0 cm) were detected in all the patients. (2) There were 22 patients with enlarged mediastinal lymph nodes or mediastinal masses of unknown origin. (3) There were 8 patients with pulmonary mass located close to the central airways. Results (1) 76 patients underwent EBUSTBNA for known or bly suspected lung cancer. Among them, 58 patients were confirmed to have mediastinal lymph nodes metastasis on EBUSTBNA. Sixteen in the 18 patients with negative EBUSTBNA underwent thoracoscopy or thoracotomy for pulmonary resection and mediastinal lymph node dissection. Postoperative pathology confirmed that 12 patients did not have metastatic nodes, 2 patients had metastatic nodes and 2 other patients had benign lesions within the lung. The diagnostic sensitivity, specificity and accuracy of EBUSTBNA for the mediastinal staging of lung cancer were 96.66%(58/60), 100.00%(12/12) and 97.22%(70/72), respectively. (2) 22 patients underwent EBUSTBNA for the evaluation of mediastinal adenopathy or mass in the absence of any identifiable pulmonary lesion. Among them, 7 had malignancy, 13 had benign diseases on EBUSTBNA and the sensitivity of EBUSTBNA in distinguishing malignant mediastinal diseases was 87.50% (7/8). (3) 8 patients with pulmonary mass located close to the central airways were accessed by EBUSTBNA. Definite diagnosis was achieved in 7 patients, and lung cancer was detected in 6 patients. The sensitivity and the diagnostic accuracy of EBUSTBNA for the diagnosis of unknown pulmonary mass was 85.71%(6/7) and 87.50%(7/8), respectively. All the procedures were uneventful, and there were no complications. Conclusion EBUSTBNA is a highly effective and safe procedure. We believe that EBUSTBNA should be used routinely in the diagnosis and staging of thoracic diseases.

  • The diagnostic value of GERDQ questionnaire for GERD: a meta-analysis

    ObjectivesTo systematically review the diagnostic value of GerdQ questionnaire for diagnosing the gastro-oesophageal reflux disease (GERD).MethodsPubMed, Web of Science, EBMR, CNKI, CBM, VIP and WanFang Data databases were searched to collect studies on the diagnostic value the GerdQ questionnaire in diagnosing the GERD from inception to January 1st 2018. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then meta-analysis was performed by using Meta-Disc 1.4 software. We used the Stata 12.0 software to assess the publication bias with funnel plots.ResultsA total of 20 studies were enrolled, including 7 978 patients. Among them, 4 848 patients were confirmed with GERD. The results of meta-analysis showed that: a) The pooled sensitivity, specificity, +LR, −LR, and DOR were 0.79 (95%CI 0.78 to 0.81), 0.66 (95%CI 0.65 to 0.68), 2.28 (95%CI 1.77 to 2.94), 0.37 (95%CI 0.27 to 0.52) and 6.34 (95%CI 3.59, 11.19), SROC(AUC) was 0.789 3, and Q* was 0.726 6. b) When the cut-off was 7, the diagnostic accuracy of GerdQ questionnaire for the GERD was the highest, and that of 9 was the second. c) The diagnostic accuracy of GerdQ questionnaire was higher when it was used in China.ConclusionsGerdQ questionnaire has a moderate accuracy for the diagnosis of GERD, which can be a useful complementary tool for diagnosing GERD, and can be popularized in clinical settings. Due to limitation of quantity and quality of included studies, the above conclusions requires verification by more high quality studies.

  • Evaluation and treatment of osteochondral injury of knee joint

    ObjectiveTo investigate evaluation and treatment of osteochondral injury of knee joint and its effectiveness.MethodsBetween January 2010 and January 2016, 17 patients with osteochondral injury of knee joint were admitted. There were 2 males and 15 females, with an average age of 19.3 years (range, 15-33 years). The causes of injury included the sprain in 14 cases and knee hyper-extension and varus due to violence in 3 cases. The osteochondral injury located at patella in 8 cases, lateral femoral condyle in 4 cases, medial femoral condyle in 1 case, and tibial plateau in 4 cases. There were 15 cases of fresh fractures and 2 cases of old fractures. The Lysholm score of the knee joint was 31.6±2.3. After open reduction of osteochondral fractures of 14 cases, the absorbable rods (9 cases), absorbable cartilage nail (3 cases), or absorbable sutures (2 cases) were selected for fixation. The osteochondral fractures at the medial tibial plateau margin (non-weight-bearing area) in 3 cases were removed.ResultsThe incision fat liquefaction occurred in 1 case after operation and healed after debridement. The other incisions had primary healing. All 17 patients were followed up 6 months to 2 years (mean, 13 months). Thirteen of 14 patients with internal fixation had good fractures healing without traumatic arthritis; 1 case of patella osteochondral fracture did not heal. Three patients with non-weight-bearing osteochondral removal had no narrowing of the medial joint space and traumatic arthritis during the follow-up. The Lysholm score of knee joint at 1 year after operation was 91.3±1.1, which significantly improved when compared with preoperative score (t=7.136, P=0.001).ConclusionFor the osteochondral injury of the knee joint, the osteochondral block with full-layer cancellous bone can be treated with open reduction and internal fixation; while osteochondral block with punctate cancellous bone can be directly remove.

  • CLINICAL APPLICATION OF MICRO TRANSVERSE FLAP PEDICLED WITH SUPERFICIAL PALMAR BRANCH OF RADIAL ARTERY FROM PALMAR WRIST TO REPAIR SKIN DEFECT OF FINGER

    Objective To investigate the clinical application of micro transverse flap pedicled with superficial palmar branch of radial artery from palmar wrist to repair skin defect of finger. Methods Twenty-six cases (26 fingers) with skin defect of finger were repaired with micro transverse flap pedicled with superficial palmar branch of radial artery from palmar wrist between December 2011 and February 2013. There were 20 males and 6 females with the average age of 31.5 years (range, 20-56 years). The causes of injury included cutting injury in 20 cases and crushing injury in 6 cases. The time from injury to admission was 1-5 hours with an average of 2 hours. Injured fingers included thumb in 6 cases, index finger in 6 cases, middle finger in 6 cases, ring finger in 4 cases, and little finger in 4 cases; the locations were the dorsal side of the finger in 6 cases, lateral side in 6 cases, and the volar of the finger tip in 14 cases; there were 4 cases on the proximal phalangeal skin, 8 cases on the middle phalangeal skin, and 14 cases on the distal phalangeal skin. The defect area ranged from 2.0 cm × 1.5 cm to 4.0 cm × 2.0 cm, and the flap area ranged from 2.5 cm × 2.0 cm to 4.5 cm × 2.5 cm. All the donor sites were directly sutured. Results The flaps of 25 cases survived well after operation, and wound healed by first intention; 1 case had partial necrosis and the wound had a scar healing by changing dressing. All cases were followed up 6-12 months (mean, 10 months). The color and appearance of the flaps were satisfactory with tender texture. The two-point discrimination of the flap was 5-8 mm (mean, 6.8 mm). The donor sites healed primarily without scar contracture and limitation of wrist flexion or extension. Conclusion The micro transverse flap pedicled with superficial palmar branch of radial artery from palmar wrist is a good option to repair skin defect of finger. It has the advantages of hidden donor site, the same operative field, great comparability of flap and finger skin, and it can be used as a vascularized tendon or nerve graft.

  • DIAGNOSE AND TREATMENT OF INTRA-ARTICULAR FRACTURE OF FIFTH METACARPALE BASE WITH CARPOMETACARPAL JOINT DISLOCATION

    Objective To investigate the diagnose and treatment of intra-articular fracture of the 5th metacarpale base with carpometacarpal joint dislocation (reverse Bennett fracture). Methods Between January 2008 and March 2012, 26 cases of reverse Bennett fracture were treated. There were 20 males and 6 females, aged 19-48 years (mean, 26 years). The injury causes included boxing injury in 19 cases, falling injury in 3 cases, heavy pound injury in 3 cases, and crushing injury in 1 case. According to Lundeen classification, there were 8 cases of type A, 9 cases of type B, 3 cases of type C, and 6 cases of type D. The time from injury to operation ranged 1-8 days with an average of 4 days. All patients underwent open reduction and internal fixation with Kirschner wires, screws, or plates by L-shaped dorsal incision. Results Primary healing was obtained in all incisions; no infection, hematoma, and necrosis occurred after operation. All patients were followed up with an average time of 12.5 months (range, 8-24 months). X-ray films showed that all fractures healed after 6-8 weeks (mean, 6.5 weeks); no delayed union or nonunion and no the 5th carpometacarpal joint dislocation were observed. Two cases had mild osteoarthritis. According to the upper extremity functional evaluation standard by Hand Surgery Branch of Chinese Medical Association, the results were excellent in 22 cases, good in 3 cases, and fair in 1 case, with an excellent and good rate of 96.2%. Conclusion For patients with reverse Bennett fracture, good results can be obtained if early diagnose is done and appropriate internal fixation is selected.

  • DECOMPRESSION AND ANTERIOR TRANSPOSITION OF ULNAR NERVE WITH INFERIOR ULNARCOLLATERAL ARTERY FOR CUBITAL TUNNEL SYNDROME

    Objective To report the operation method and the cl inical effect of decompression and anterior transposition of the ulnar nerve with inferior ulnar collateral artery for cubital tunnel syndrome. Methods From September 2005 to May 2006, 25 cases of cubital tunnel syndrome were treated by the method of decompression and anterior transposition of the ulnar nerve with inferior ulnar collateral artery. There were 19 males and 6 females with an average of 60 years (20-72 years). The disease course was 2 months to 3 years (mean 6.7 months). The causes were ostesarthritis in 23 cases, cubital tunnel cyst in 1 case and ulnar nerve ol isthy in 1 case. According to Pasque grading system for cubital tunnel syndrome, 19 cases were graded as good and 6 cases were graded as poor. Electrophysiological examination showed the motor nerve conduction velocity of the ulnar nerve around the elbow joint was less than 42 m/s. Results All wounds healed by first intention and no operative compl ications and recurrences occurred. All patients were followed up for one year to two and half years (13.9 months on average). According to Pasque grading system for cubital tunnel syndrome, 15 cases were graded as excellent, 9 cases as good and 1 case as fair. The excellent and good rate was 96%, indicating a significant difference compared with the results before operation (P lt; 0.05). Electrophysiological examination showed the motor nerve conduction velocity of the ulnar nerve around the elbow joint was more than 42 m/s. Conclusion The method of decompression and anterior transposition of the ulnar nerve with inferior ulnar collateral artery is safe and effective for the treatment of cubital tunnel syndrome.

  • Effect of Mongolian medicine fumigation combined with sciatic nerve and rectal probe pelvic floor electrical stimulation on muscle spasticity of cervical spinal cord injury

    ObjectiveTo observe the effect of Mongolian medicine fumigation combined with sciatic nerve and rectal probe electrical stimulation on muscle spasticity of spinal cord injury.MethodsBetween January 2012 and January 2018, a total of 65 patients with muscle spasticity after spinal cord injury were randomly divided into two group: the observation group (32 cases) and the control group (33 cases). The patients in the observation group were treated with Mongolian medicine (Wu Wei Gan Lu-Decoction) fumigation combined with sciatic nerve and rectal probe electrical stimulation, while the patients in the control group were treated with medicine, physical therapy, and exercise therapy. Both two groups were treated for 8 weeks. The patients were scored with Ashworth Score, American Spinal Injury Association (ASIA) score, and Barthel Index before and after treatment.ResultsThe pre-treatment ASIA scores (light touch sensation, pain sensation, and muscle strength) and Barthel Index of the two groups were not statistically significant (P>0.05). The post-treatment ASIA scores and Barthel Index of both groups performed significantly better than the pre-treatment levels (P<0.05). The post-treatment ASIA muscle strength item was 58.55±10.83 in the observation group and 50.69±11.32 in the control group (P<0.05). The post-treatment Barthel Index was 74.22±11.53 in the observation group and 68.46±9.92 in the control group (P<0.05). The effective rate in the observation group was significantly better than that in the control group (84.4% vs. 60.6%, P<0.05). Conclusion Mongolian medicine fumigation combined with sciatic nerve and rectal probe electric stimulation could improve the muscle spasticity of spinal cord injury and patients’ ability of daily life effectively.

  • Effect of core endurance isokinetic strength training on subacromial impingement syndrome rehabilitation

    ObjectiveTo observe the effectiveness of core endurance isokinetic strength training on subacromial impingement syndrome (SAIS) rehabilitation.MethodsSAIS patients were selected from the Rehabilitation Department, Second Affiliated Hospital of Inner Mongolia Medical University from January 2018 to June 2019. The patients were randomly divided into observed group and control group by random number table method. The patients in control group were treated by keritherapy, cold, standard physiotherapy (PT) and exercise. At the same time, the patients in observed group were treated by core endurance isokinetic strength training combined with keritherapy, cold and standard PT. Both two groups were treated for 8 weeks. The patients were scored with visual analogue scale (VAS), Shoulder Pain and Disability Index (SPADI), Short Form 36 Healthsurvey Questionnaire (SF-36), flexor peak torque (FPT) and extensor peak torque (EPT) in pre and post treatment.ResultsA total of 59 patients with SAIS were enrolled. No exfoliating cases. There were 31 cases in the observed group and 28 cases in the control group. Pre-treatment, there were no significant difference in FPT, EPF, VAS, SPADI and SF-36 between the two groups (P>0.05). Post-treatment, there were no significant difference in EPT and VAS between the two groups (P>0.05); FPT [(62.65±10.17) N·m], SPADI [(53.18±10.25) points] and SF-36 [(35.54±11.39) points] in the observed group were significantly improved compared with the control group [(56.28±9.55) N·m, (61.05±9.41) points, (42.65±10.74) points] (t=2.473, 3.061, 2.460; P=0.014, 0.003, 0.017); FPT in both groups, and EPT in the observed group were improved compared with that of the pre-treatment (P<0.05); the EPT of the control group was improved compared with that of the pre-treatment, but the difference was not statistically significant (P>0.05); VAS, SPADI and SF-36 in the two groups were improved compared with those of the pre-treatment (P<0.05).ConclusionsCore endurance isokinetic strength training could improve the core stability of subacromial impingement syndrome patients. Satisfactory musculoskeletal dynamic chain plays a vital role in subacromial impingement syndrome rehabilitation.

  • Application of radial-lateral forearm free perforator flap on repairing of soft tissue defects in finger

    ObjectiveTo investigate the effectiveness of radial-lateral forearm free perforator flap on repairing of soft tissue defects in the finger.MethodsBetween January 2017 and May 2018, 26 cases of finger skin defects were treated with radial-lateral forearm free perforator flap based on the radial branch of the posterior interosseus artery. There were 21 males and 5 females, with an average age of 26.6 years (range, 19-56 years). The cause of injury included the cutting injury in 16 cases and crush injury in 10 cases. The interval between injury and admission was 30 minutes to 4 hours (mean, 1.5 hours). The injury located at thumb in 6 cases, index finger in 8 cases, middle finger in 6 cases, ring finger in 3 cases, and little finger in 3 cases; and at the dorsum of finger in 6 cases, the lateral side in 6 cases, and the palm in 14 cases. All wounds were accompanied with the tendon and bone exposures, and phalangeal fractures occurred in 10 cases. The size of the defects ranged from 2.0 cm×1.0 cm to 4.0 cm×2.5 cm. And the size of the flap ranged from 2.5 cm×1.5 cm to 4.5 cm×3.0 cm. All wounds at donor sites were sutured directly.ResultsAll the 26 cases were followed up 4-12 months (mean, 7 months). The 24 flaps survived uneventfully after operation, and the wounds healed by first intention. Partial necrosis occurred at the distal part in 2 flaps, and secondary healing achieved after debridement and dressing. All incisions at donor sites healed by first intention. The appearance and texture of all flaps were satisfactory. The two-point discrimination of the flaps was 5-10 mm (mean, 8 mm) at 4 months after operation. Sensory of the flaps was grade S3. Only linear scar was noted at the donor site.ConclusionThe radial-lateral forearm free perforator flap in repairing of the soft tissues in finger can shorten the disease duration, reduce the damage of the donor site, and improve the patients’ quality of life.
