ObjectiveTo systematically review the efficacy and safety of acupuncture on postpartum depression (PPD).MethodsPubMed, EMbase, The Cochrane Library, Web of Science, CNKI, WanFang Data and VIP databases were electronically searched to collect randomized controlled trials (RCTs) on the efficacy and safety of acupuncture in treatment of PPD from inception to February 2021. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies. Meta-analysis was then performed by using Stata16.0 software.ResultsA total of 13 RCTs involving 899 patients were included. The results of meta-analysis showed that there was no significant difference between hand acupuncture and fluoxetine hydrochloride in HAMD score (MD=0.45, 95%CI −0.52 to 1.41, P=0.36), clinical effective rate (RR=0.93, 95%CI 0.70 to 1.23, P=0.59), and clinical cure rate (RR=0.88, 95%CI 0.44 to 1.76, P=0.73). However, hand acupuncture was superior in safety to fluoxetine hydrochloride (RR=0.04, 95%CI 0.01 to 0.28, P<0.05). There was no significant difference in clinical effective rate (RR=1.08, 95%CI 0.87 to 1.36, P=0.49) and cure rate (RR=1.31, 95%CI 0.84 to 2.04, P=0.24) between both groups.ConclusionsThe current evidence shows that there are no differences between hand acupuncture and non-acupuncture in reducing HAMD score, improving the clinical effective rate and clinical cure rate. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusions.
Objectives To investigate calcium supplement in pregnancy for prevention of preeclampsia and relevant outcomes. Methods The Cochrane Library, PubMed, EMbase, CBM, WanFang Data, VIP and CNKI databases were searched online to collect randomized controlled trials (RCTs) of calcium supplement in pregnancy for prevention of preeclampsia and relevant outcomes from inception to July 2018. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Meta-analysis was then performed by RevMan 5.3 software. Results A total of 33 RCTs involving 29 234 subjects were included. The results of meta-analysis showed that: compared with control group, the calcium supplement group was associated with lower preeclampsia (RR=0.48, 95%CI 0.38 to 0.62, P<0.000 01) and gestational hypertension (RR=0.65, 95%CI 0.55 to 0.77,P<0.000 01) incidence. The incidence of premature delivery, intrauterine growth retardation and severe preeclampsia in calcium supplement group was lower than that in placebo group, and the neonatal weight in calcium was higher than that in placebo group. However, there was no significant difference in the pregnancy cycle between the two groups. Conclusions Current evidence shows that calcium supplement is associated with lower risk of preeclampsia and gestational hypertension. Due to limited quality and quantity of the included studies, more high quality studies are required to verify the above conclusion.
Evidence mapping is a new type of comprehensive evidence research method that systematically collects, evaluates, and synthesizes existing evidence to clarify research status and gaps, thereby promoting scientific research and decision-making. After nearly 20 years of development, the methodology of evidence mapping has been continuously improved, and has gained wide attention and recognition from the international community. China has paid much attention to evidence mapping at an early stage, but there are relatively few theoretical and practical researches, and the concept definition is inconsistent. This paper introduces the methodology of evidence mapping production and reporting for researchers to use for reference, with a view to further promoting the research and development of evidence mapping in China.
ObjectiveTo systematically review the efficacy and safety of different tyrosine kinase inhibitors (TKIs) in the treatment of chronic myelocytic leukemia (CML).MethodsPubMed, EMbase, The Cochrane Library, CBM, WanFang Data and CNKI databases were electronically searched to collect randomized controlled trials (RCTs) of nilotinib, dasatinib, flumatinib and imatinib for CML from inception to August, 2020. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies; network meta-analysis was then performed using Stata 15.0 software and R 3.4.0 software.ResultsA total of 8 RCTs involving 2 775 patients were included. Compared with other TKIs, flumatinib had higher 3-month early molecular response and 1-year progression free survival, and the incidence of serious side effects was relatively low. Major molecular response and complete cytogenetic response were significantly superior to imatinib, and had the same or similar effects to other second-generation TKIs.ConclusionsCurrent evidence shows that flumartinib in the treatment of CML is obviously superior to imatinib, has the same or similar effect with other second generation TKIs. Due to limited quality and quantity of the included studies, more high quality studies are needed to verify above conclusions.
ObjectiveTo explore the current situation, progress and hot spots of hospital-based health technology assessment (HB-HTA) at home and abroad. MethodsThe China National Knowledge Infrastructure (CNKI) and Web of Science databases were electronically searched to collect publications on HB-HTA. The bibliographic item co-occurrence mining system and VOSviewer software were used to extract and visually analyze HB-HTA literature. ResultsA total of 6 016 publications were included. Research on HB-HTA started late in China, however, it gradually increased in recent years. The Chinese research topics mainly focused on the application of hospital health technology evaluation in the management of medical equipment and consumables, and the English research topics mainly focused on the evaluation of the effectiveness, safety and cost of hospital health technology, especially information technology. ConclusionIn recent years, increasing attention has been given to HB-HTA. Related research topics have mainly focused on hospital technology access, management, and hospital health decision management.
In order to promote the harmonization of decision-making processes in public health and the harmonization of decision-making criteria, McMaster University in Canada and the Estonian Health Insurance Fund jointly published an article on an evidence ecosystem for health decision-making in 2022 with the support of the World Health Organization. Based on the proposed health decision-making evidence ecosystem model of the article, this paper interpreted the processes and elements in the model, aiming to provide reference for evidence-based public health decision making in China.
ObjectiveTo systematically review the efficacy and safety of major ozonated autohemotherapy in the treatment of ischemic stroke.MethodsPubMed, EMbase, The Cochrane Library, Web of Science, CBM, WanFang Data, VIP and CNKI were electronically searched to collect randomized controlled trials (RCTs) of the efficacy and safety of major ozonated autohemotherapy in the treatment of ischemic stroke from inception to July 1st, 2020. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies. Meta-analysis was then performed by using RevMan 5.3 software.ResultsA total of 25 RCTs involving 3 681 patients were included. The results of meta-analysis showed that the major ozonated autohemotherapy combined with conventional therapy in the treatment of patients with ischemic stroke in terms of total effective rate (RR=1.20, 95%CI 1.15 to 1.25, P<0.001), national institutes of health stroke scale (MD=−3.15, 95%CI −4.72 to −1.59, P<0.001), total cholesterol (MD=−1.00, 95%CI −1.48 to −0.53, P<0.001), triglyceride (MD=−0.74, 95%CI −1.04 to −0.43, P<0.001), low-density lipoprotein cholesterol (MD=−0.65, 95%CI −1.22 to −0.09, P=0.02), and activity of daily living (MD=11.97, 95%CI 4.48 to 19.47, P=0.002) were superior to the conventional treatment group. There was no significant difference between the two groups in high-density lipoprotein cholesterol (MD=0.25, 95%CI −0.46 to 0.96, P=0.49) and the incidence of adverse effects (OR=3.15, 95%CI 0.93 to 10.63, P=0.06).ConclusionsThe major ozonated autohemotherapy can significantly improve the prognosis of patients with ischemic stroke while not affecting the adverse effects. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusions.
ObjectiveTo analyze the research status and hot spots of health technology assessment (HTA). MethodsDatabases including China National Knowledge Infrastructure (CNKI) and PubMed were searched to collect published studies on HTA. The software of BICOMS-2 and VOSviewer 1.6.10 was used to draw the network diagram and to conduct cluster analysis of keywords. ResultsA total of 1170 studies in Chinese and 8584 studies in English were included. The number of HTA-related publications increased annually, among which countries with the most publications were the United States, the United Kingdom, and Canada. Fudan University had the largest number of studies published in Chinese and York University had the largest number of studies published in English. The research topics mainly focused on hospital HTA, HTA decision transformation research, pharmacoeconomic evaluation, etc. The most frequent diseases were cancer and depression. Emerging keywords were real-word evidence, randomized controlled trials, cost-benefit analysis, etc. ConclusionHTA-related studies increase annually. Many HTA-related studies are performed in developed countries, such as the United States and the United Kingdom. The relevant research topics involve hospital HTA, HTA decision-making transformation research, and pharmacoeconomic evaluation. Current research focus is on the evaluation of health economics in the field of cancer based on the evidence of real-world research and randomized controlled trials.
Lumbar disc herniation (LDH) is one of the most important causes of back and leg pain, which seriously affects the quality of life of patients. As the first-line treatment for LDH, non-operative treatment can relieve 80% to 90% of symptoms among the patients with LDH. This guideline followed Guidelines for the Formulation/Revision of Clinical Treatment Guidelines in China (2022 edition) and WHO handbook for guideline development (2014 edition) to set up guideline working group. This guideline identified fourteen clinical questions through the literature review and clinical experts’ consensus. We drafted the recommendations after systematically searching and evaluating the evidence; delphi method was adopted for expert consensus on the preliminary recommendations, finally, 19 recommendations were made to guide non-operative treatments for LDH. This guideline can provide guidance for the clinical practice of Chinese and western orthopedics practitioners.
