  • Related ED Risk Factors and Clinic Therapy Evaluation in 120 Type 2 Male Diabetic Mellitus

    摘要:目的: 总结2型糖尿病患者阴茎勃起功能障碍(ED)的相关危险因素及临床治疗评价。 方法 :分析收集两家医院120例住院及门诊2型糖尿病男性患者的性功能状况、年龄、糖尿病病程、血压、糖化血红蛋白、血脂、吸烟与否等指标,以及接受真空负压吸引联合万艾可治疗的79名患者追踪3个月的疗效评分。 结果 :男性2型糖尿患者中ED的患病率为658%,多因素回归分析显示患者年龄、糖尿病病程、糖化血红蛋白与ED的发生独立相关,而血压、血脂等因素〖WTBX〗P >005,无显著性差异。接受真空负压吸引联合万艾可治疗的79例ED患者其有效率达到759%。 结论 :男性2型糖尿病患者中ED是常见的合并症,患病率随年龄、糖尿病病程、糖化血红蛋白的增加而增加。真空负压吸引联合万艾可治疗糖尿病合并ED疗效可靠。Abstract: Objective: Make a conclusion on related ED risk factors and clinic therapy evaluation in 120 Type 2 male diabetic mellitus. Methods : Analyze and collect 120 cases in two hospitals regarding sexual function,age,diabetes course,blood pressure,HbA1c,bolldfat, smokingcondition,etc;follow up 79 cases for 3months and evaluate therapy as a result of vacuum subpressure absorption associated with viagra (sildenafil) therapy. Results : The ED rate in Type 2 male diabetic is 658%;the multifactorial regression analysis shows that occurrence of ED is respectively related with age,diabetes course,HbA1c;there isn’t obvious discrdpancy in blood pressure,blood fat,etc(P >005); the effective rate of vacuum subpressure absorption associated with Viagra (sildenafil) therapy in 79 cases is759%. Conclusion : It’s common complications to get ED among the Type 2 male Diabetics and the case rate goes up with the increase of age,diabetes course, HbA1c;it’s reliable to take therapy of vacuum subpressure absorption associated with Viagra.

  • The Perioperative Nursing Care of TURP

    摘要:目的:探讨良性前列腺增生经尿道前列腺电切术围手术期的护理经验。方法:回顾性分析96例良性前列腺增生患者临床资料。结果:96例患者手术顺利,围手术期经周密的护理,疗效满意,无明显并发症。结论:周密的手术期护理对经尿道前列腺电切术治疗老年良性前列腺增生十分重要。Abstract: Objective: To investigate the perioperative nursing care of transurethral prostatic resection (TURP). Methods: The data of 96 TURP cases were analyzed retrospectively. Results: All the operations were performed successfully, and there were no obvious complications among the patients with precise nursing care. Conclusion: It is very important for precise nursing care to the patients who underwent TURP.

  • Surgical Treatment of the Intrahepatic Lithiasis Combined with High Hepatic Duct Strictures

    Objective To discuss the effective surgical treatment of intrahepatic lithiasis combined with high hepatic duct strictures. MethodsTwo hundreds and sixteen cases of intrahepatic lithiasis and high hepatic duct strictures treated in this hospital from January 1993 to October 2002 were analysed retrospectively.ResultsOne hundred and eightythree cases underwent different selective operation by selected time; 33 cases complicated with acute obstructive suppurative cholangitis underwent emergency were performed single biliary drainage, in which 30 cases were reoperated. The operative procedure were: hepatic lobectomy,high cholangiotomy and plastic repair,exposure of hepatic duct of the 2nd and the 3rd order,and plastic repair with own patch and choledochojejunostomy.Two hundreds and six cases were cured,the curative rate was 95.4%; 8 cases improved (3.7%), and 2 cases died (0.9%).Conclusion The best effective surgical treatment of intrahpatic lithiasis is hepatic lobectomy. Exposure of hepatic duct of the 2nd and the 3rd order is a satisfactory to release the hepatic duct strictures and to clear the intrahepatic lithiasis. For patients with normal extrahepatic bile duct and Oddi’s function, plastic repair of bile duct with own patch is possible to keep the normal form and function. Cholangioscopy may play an important role in the treatment of intrahepatic tract lithiasis during operation.

  • Clinical Significance of Para Left Gastric Artery Lymph Node Resection in Radical Operation of Esophageal Cancer

    Objective To investigate the related factors affecting the metastases to left gastric artery lymph nodes in patients of esophageal cancer, and evaluate the clinical significance of resection of left gastric artery. Methods One hundred and eighty-six patients with esophageal cancer undergone esophagectomy and esophagogastrostomy were involved in these case-control study. The left gastric artery, lymph nodes and fat tissue around it were removed in these patients. Patients were divided into 2 groups according to the occurrence of metastases to para left gastric artery lymph nodes. Chi-square test, rank sum test, t-test and the logistic regression were adopted to analyze the correlations between these related factors and the metastases to para left gastric artery lymph nodes. Results Thirty-three patients had para left gastric artery lymph nodes metastases (17. 74%). Related factors that affect the metastases to para left gastric artery lymph nodes were showed by monovariate analysis as follows: TNM staging of tumor, the metastases to paraesophageal nodes, paracardial nodes and subcarinal nodes (P〈0.001, P=0.025, 0.047,0.038). Multivariate analysis showed that location of tumor was the only independent factor that influences the metastases of para left gastric artery lymph nodes(P= 0. 002). Skip metastasis was a distinct feature of esophageal cancer, with a frequency of 78.79%(26/33). Conclusions This study suggests that the major correlative factor of para left gastric artery lymph node metastasis is location of tumor. Resection of left gastric artery as a routine procedure in radical operation of esophageal cancer should be considered.

    ObjectiveTo summarize the research progress of functional surgery in upper and middle gastric cancer.Method" functional gastric surgery” " pylorus-preserving gastrectomy” and " proximal gastrectomy” were used as search terms to retrieve the literatures, and various surgical methods and their application status were reviewed.ResultsFunctional gastric surgery can effectively improve the postoperative quality of life of patients with early gastric cancer in the upper and middle stomach.ConclusionFunctional gastric surgery is a feasible surgical method for early gastric cancer.

  • Arthroscopic treatment of scaphoid fracture nonunion with bone graft and Kirschner wire combined with screw fixation

    ObjectiveTo explore the effectiveness of arthroscopic treatment of scaphoid fracture nonunion with bone graft and Kirschner wire combined with screw fixation. MethodsThe clinical data of 14 patients with scaphoid fracture nonunion who met the selection criteria between February 2021 and September 2022 were retrospectively analyzed. There were 13 males and 1 female with an average age of 32 years ranging from 17 to 54 years. The time from injury to operation ranged from 6 to 15 months, with an average of 9.6 months. According to the Slade-Geissler classification of scaphoid fracture nonunion, there were 3 cases of grade Ⅲ, 8 cases of grade Ⅳ, and 3 cases of grade Ⅴ. The preoperative visual analogue scale (VAS) score was 5.9±1.0, and the modified Mayo wrist score was 53.2±9.1. There were 2 cases of scaphoid nonunion advanced collapse, both of which were stage Ⅰ. All patients were treated with arthroscopic bone graft and Kirschner wire combined with screw fixation, and the fracture healing was observed by X-ray film monthly after operation, and the effectiveness was evaluated by VAS score and modified Mayo wrist score before and after operation. ResultsAll patients were followed up 6-14 months, with an average of 8.4 months. All fractures healed in 4-8 months, with an average of 6.3 months. The postoperative pain symptoms and wrist function of the patients significantly improved when compared with those before operation, and the VAS score at last follow-up was 2.4±1.3, and the modified Mayo wrist score was 87.1±6.7, which were significantly different from those before operation (t=12.851, P<0.001; t=−14.410, P<0.001). According to the modified Mayo wrist evaluation, 9 cases were excellent, 3 cases were good, and 2 cases were fair. Conclusion Arthroscopic bone graft and Kirschner wire combined with screw fixation is an effective surgical method for the treatment of scaphoid fracture nonunion.

  • Bibliometrics on Electronic Health Records of Web of Science

    Objective To investigate the current status and development of electronic health records (EHR) at home and abroad to grasp the development trends of EHR, so as to point out the direction of the development and relevant research on EHR. Methods Based on the Web of Science citation database and the principle of bibliometrics, we analyzed the retrieved literature in aspects of publication date, language, country/region, institution, author, etc. Results A total of 1 262 eligible studies were identified. The number of articles on EHR increased rapidly from only 2 in 1995 to 218 in 2012. In terms of country/region, the United States ranked the top in all countries (763 articles, accounting for 60.46%). In terms of institution, Harvard University ranked the top (135 articles, accounting for 10.70%). In terms of journal, the Journal of the American Medical Informatics Association ranked the top (106 articles, accounting for 8.40%). In terms of authors, David W. Bates ranked the top (45 articles, accounting for 3.57%). In terms of subject type, health care sciences services and medical informatics were mainly focused on. Conclusion The research on EHR has become a global hot spot and relevant bibliometrics will contribute to the timely and correctly grasp the whole picture of its development trends and main research direction.

  • Treatment for resectable cervical esophageal carcinoma: A surgical perspective

    Definitive chemoradiotherapy (dCRT) is the general recommendation for the treatment of cervical esophageal cancer for organ preservation. However, the long-term survival of dCRT is not satisfactory. Surgical resection alone is not superior to dCRT in the treatment of cervical esophageal cancer. Surgical resection is often combined with laryngectomy, which will affect the quality of life. Recent evidence suggests that neoadjuvant therapy combined with surgery improves the long-term survival of cervical esophageal cancer. On the other hand, the development of technologies such as laryngeal preservation surgery and minimally invasive esophagectomy has reduced the risk of operation and improved the quality of life. This article will review the new progress in the comprehensive treatment of cervical esophageal cancer from the perspective of surgery.

  • Based on the optimization application of RBRVS in performance appraisal

    Objective To further optimize the performance appraisal system based on resource based relative value scale (RBRVS) by evaluating the surgical operation ability of surgeons. Methods From January 1, 2019 to December 31, 2019, all patients in diagnosis related groups(DRG)group named laparoscopic cholecystotomy without common bile duct exploration and code 51.2300 in International Classification of Diseases ICD-9-CM-3 were retrospectively collected. The key indicators related to surgical operation ability of surgeons were selected through literature review and expert interviews, and the evaluation model of surgical operation ability of surgeons was established by principal component analysis. Results The RBRVS coefficients of each operation were further modified by the constructed evaluation model of surgeons' surgical ability. After adjustment, the RBRVS coefficients of different surgeons were different. Conclusion The evaluation model of surgical operation ability of surgeons is operable, which can reflect the difference of different doctors' level and ability in the treatment of the same medical service item, and promote the refined development of hospital performance management.

  • Study of Relationship Between Cholecystectomy and Helicobacter Pylori Infection

    ObjectiveTo study the relationship between cholecystectomy and Helicobacter pylori (Hp) infection. MethodsOne hundred and eleven patients with cholecystolithiasis were chosen as the investigation group, while 577 patients with upper digestive tract symptoms without cholecystolithiasis as the control group. All the patients took the 13C breath test to determine whether they were infected by Hp. All the patients with Hp infection continued eradical therapy for Hp infection for one course after cholecystectomy and were followed up on outpatient basis. ResultsThe infection rate in the investigation group was 45.9%, while 27.4% in the control group. During the 3 to 6 months of followup for the patients undergoing eradical therapy for Hp infection, we found no patient complaining of epigastric pain, malaise, belching and nausea. ConclusionThe infection rate of Hp in patients with cholecystolithiasis is high, Hp may be one of the factors causing “postcholecystectomy syndrome”. Eradical therapy for Hp after cholecystectomy will help improve the effects of operation.
