  • Edaravone Combined with Shuxuening for Cerebral Infarction:A Systematic Review

    ObjectiveTo evaluate the effects and safety of edaravone combined with shuxuening for cerebral infarction. MethodsThe Cochrane Library, PubMed, EMbase, CBM, CNKI and VIP databases were searched from their establishments up to March 31, 2013. We used the method recommended by the Cochrane collaboration to perform a meta-analysis of randomized controlled trails (RCTs) with RevMan 5.0 software. ResultsSeventeen studies were included. The results of Meta-analysis demonstrated that edaravone combined with shuxuening were more efficient in reducing the score of neural function deficient scale and higher in the total effective rate (P<0.05), while there was no difference in the incidence of adverse reactions (P>0.05). ConclusionThe study suggests that edaravone combined with shuxuening is effective for cerebral infarction, but it also needs further studies to provide more sufficient evidence.

  • Survey on the Antidepressant Use in In-patients with Depression

    ObjectiveTo investigate the use of traditional and new-type antidepressants in the in-patients with depressive disorder. MethodsThe antidepressant use of 165 in-patients with depressive disorders from March 1st to April 30th, 2013 were investigated. ResultsThe new-type antidepressants became the best drug in clinic, and single drug use was carried out mostly. The most used drug was atypical antipsychotics. ConclusionThe use of antidepressant drugs in the in-patients with depressive disorder in our hospital is regular and reasonable.

  • Vacuum Sealing Drainage for Early Deep Infection after Posterior Spinal Internal Fixation

    ObjectiveTo analyze the curative effect of vacuum sealing drainage for early deep infection after posterior spinal internal fixation. MethodsFrom March 2009 to March 2012, 9 patients with early deep infection after posterior spinal internal fixation, including 5 males and 4 females aged between 21 and 64 years, averaging at 44.6, underwent debridement and vacuum sealing drainage (VSD). Original fixtures in all the patients were not taken out. ResultsThe patients were treated by VSD for once to three times with an average of 2.1 times, and VSD continued for 3 to 7 days every time. Two patients underwent one time of VSD, 4 underwent twice, and 3 underwent three times. In these patients, 6 achieved wound healing after VSD, one changed to ordinary dressing and wound suturing two weeks later due to hemorrhage of VSD, one changed to ordinary dressing and wound suturing wound three weeks later due to impeded drainage, one accepted skin-grafting after three times of VSD. All the patients were followed up for 6 to 38 months (18 months on average) and all of them were cured. There was no obvious back pain or signs of incision infection. The X-ray films showed that there were no bone destruction and the white blood cell count, erythrocyte sedimentation rate, and C-reactive protein significantly decreased to normal after surgery. ConclusionOne-stage debridement and vacuum sealing drainage therapy is an effective method for treating early deep infection after posterior spinal internal fixation.

  • Research Progress of Micro-arc Oxidation Surface Treatment of Magnesium Alloy

    An oxide ceramic coating can be formed on the surface of magnesium alloy by micro-arc oxidation so that the corrosion resistance of the magnesium alloy can be enhanced. In this paper, a general overview of the surface treatment of micro-arc oxidation on the surface of magnesium alloy is presented, the related research on the treatment of several kinds of magnesium alloys is introduced in detail, and a brief introduction of biological activity of magnesium alloy due to micro-arc oxidation is given. Finally, the technical advantages and existing problems are summarized.

  • Effect of NEP1-40 Gene Modified Neural Stem Cell Transplantation on the Behavior Recovery of Rats after Spinal Cord Injury

    ObjectiveTo investigate the behavioral recovery of spinal cord injury (SCI) rats that received transplantation of NEP1-40 gene-modified neural stem cells. MethodsNeural stem cells (NSCs) were derived from the cortex tissue of rat embryo at the age of 18 days and identified by Nestin immunofluorescence. The lentiviruses were transduced to NSCs to construct NEP1-40 gene modified NSCs. Spinal cords of 30 Sprague-Dawley rats were hemisected at the nineth thoracic vertebrae level. The rats were randomly assigned to three groups. Cell culture medium, NSCs and NEP1-40 gene-modified NSCs were transplanted into the lesion site of rats of SCI group, NSCs group and NEP1-40-NSCs group respectively 7 days after injury. Additional 10 rats served as blank control group (sham group), which only received laminectomy. Following transplantation, behavior tests including Basso, Beattie, Bresnahan (BBB) Locomotor Rating Scale and grid test were utilized to evaluate spinal cord functional recovery. ResultsBehavior tests 8 weeks after cells transplantation showed that the rats in SCI group got worst results, the BBB scores improved and the grid drop times reduced significantly in NSCs transplantation group (P<0.01) and behavioral test outcomes were best in the NEP1-40 gene-modified NSCs group (P<0.01). ConclusionNEP1-40 gene modification can significantly improve the behavioral recovery of SCI rats that received transplantation of pure neural stem cells. It can provide a new idea and reliable experimental base for the study of NSCs transplantation for spinal cord injury.

  • Analysis of the Characteristics of Outpatients in Neurological Department in Different Periods

    ObjectiveTo analyze the characteristics of outpatients in Neurological Department in different periods and to provide guidance on medical decision-making for Neurological Department. MethodOutpatients treated between August and September 1994 (group 1994) and between August and September 2012 (group 2012) in the Department of Neurology of a general hospital were included in our study. Group 1994 had 1 000 consecutive patients; while group 2012 had 18 995 patients excluding those repeat visitors, patients waiting to be treated (including dizziness), patients misdiagnosed to have severe mental diseases (such as schizophrenia), and patients with physical diseases. Then we compared the demographic and disease distribution of patients in the two groups, and performed statistical analysis. ResultsCompared with the year 1994, the year 2012 had more female, less young and more elderly patients (P<0.05). The constituent ratio of neurosis, cerebrovascular disease, internal medicine diseases with nerve damage, peripheral nerve disease, brain post-traumatic syndrome, intracranial space-occupying lesions reduced significantly while headache, epilepsy, extrapyramidal disease (such as Parkinson's disease) increased significantly (P<0.01) with no obvious change of constituent ratio of muscle disease, neuropathic muscular dystrophy, spinal cord, brain atrophy and dementia diseases. ConclusionsThere are significant changes in characteristics of neurology clinical patients between 1994 and 2012. Revelation of these differences can provide evidence for the optimization of outpatient resources allocation and the prevention policy.

  • Effects of Resveratrol on Oxidative Damage in Central Nervous Systerm of Rats with Obstructive Jaundice

    ObjectiveTo investigate the effects of different levels of resveratrol on oxidative stress injury in central nervous system of rats with obstructive jaundice and its protective effect and mechanism of oxidative stress injury. MethodsThirty two female SD rats of 6 weeks old were used as experimental object. The animals were randomly divided into four groups, 8 rats in each group. Sham operation group (SO group), the common bile duct were seperated without ligation; while the models of obstructive jaundice of obstructive jaundice group (OJ group), obstructive jaundice+low dose resvera-trol (L-Res)treatment group (OJ+L-Res group), and obstructive jaundice+high dose resveratrol (H-Res) treatment group (OJ+H-Res group) were established by operation. After the operation, the rats in OJ+L-Res group and OJ+H-Res group were treated with different doses of resveratrol, the rats in SO group and OJ group were given the same dose of normal saline. On the 14th day after operation, blood were tested for total bilirubin (TBIL), direct bilirubin (DBIL), ALT, and AST. And cerebral cortex specimen were collected, then malondialdehyde (MDA), total superoxidedismutase (T-SOD) activity, and HO-1 protein expression in the rats brain of the four groups were measured. ResultsThe levels rise of TBIL and DBIL after modeling suggested that obstructive jaundice model were estabilshed successfully, but there was no significant difference among the OJ group, OJ+L-Res group, and OJ+H-Res group. In the OJ group, OJ+L-Res group, and OJ+H-Res group, the levels of ALT, AST, and MDA were increased while levels of T-SOD and HO-1 protein expression were decreased when compared with SO group(P < 0.05). Among the OJ group, OJ+L-Res group, and OJ+H-Res group, levels of ALT, AST and MDA were lower in the treatment groups than in the OJ group(P < 0.05), while levels of T-SOD and HO-1 protein expression which reflects the oxidative stress were higher in the treatment group(P < 0.05). Different doses of resveratrol had different effects on T-SOD and HO-1 protein expression with statisticl significance (P < 0.05). ConclusionsResveratrol have little effect on TBIL and DBIL of obstrctive jaundice rats, but it can protect the liver function, and it has antioxidant properties of decreasing MDA and incresing SOD and HO-1 protein expression levels in the cerebral cortex cells of obstructive jaundiced rats.

  • RESEARCH PROGRESS OF IN VIVO NUCLEIC ACID DELIVERY WITH POLY (ETHYLENIMINE)

    ObjectiveTo review the current status and advances of in vivo nucleic acid del ivery mediated by poly(ethylenimine) (PEI). MethodsThe related home and abroad literature about nucleic acid del ivery with applications in cancer treatments and tissue engineering was extensively reviewed and analyzed. ResultsA variety of in vivo study on the potential of PEI-mediated nucleic acid del ivery has been carried out and made certain effects in the animal model, ranging from cancer treatments to rectification of physiological defects (eg. cranial defect and corneal epithelium defect). Tail vein injection is the most commonly adopted route of administration in vivo, followed by pulmonary administration and intralesional injection. However, transfection and expression in vivo have some shortcomings, such as low transfection efficiency and short expression time, so there are some limitation in the cl inical application. ConclusionPEI mediated nucleic acid del ivery provides a good method for cancer treatments and rectification of physiological defects. For future research, not only should more in vivo animal testing be done, but the procedures of experimentation also need to be standardized.

  • LOCATION CHANGE OF ROTATION CENTER AFTER SINGLE SEGMENTAL CERVICAL DISC REPLACEMENT WITH ProDisc-C

    ObjectiveTo evaluate the effectiveness of the single segmental cervical disc replacement with ProDisc-C, and to explore the location change of the flexion/extension center of rotation (COR) of the target level as well as its clinical significance. MethodsBetween June 2010 and February 2012, 23 patients underwent single segmental cervical disc replacement with ProDisc-C, and the clinical data were retrospectively analyzed. Of 23 patients, 9 were male, and 14 were female with the age range from 27 to 65 years (mean, 45 years), and the disease duration ranged from 10 to 84 months (mean, 25 months). There were 15 patients with radiculopathy, 5 patients with myelopathy, and 3 patients with mixed cervical spondylosis. The involved segments were C4,5 in 5 cases, C5,6 in 14 cases, and C6,7 in 4 cases. Japanese Orthopaedic Association (JOA) score and neck disability index (NDI) were adopted to evaluate the effectiveness. Preoperative and Postoperative radiographic parameters, such as cervical overall range of motion (ROM), target segmental ROM, the adjacent segmental ROM, and intervertebral height were compared. Besides, the location changes of the COR of the target level were further analyzed by the alteration of its coordinates (COR-X,COR-Y), and the relationships between the location changes of the COR and the effectiveness or the radiographic results were analyzed. ResultsAll the operations were completed successfully; 1 case had hoarseness after operation, which disappeared at 3 months after operation. All cases were followed up 18.3 months on average (range, 6-36 months). There was no device migration, loosening, subsidence, or fracture at last follow-up. The JOA score increased significantly and the NDI score decreased significantly at last follow-up when compared with preoperative scores (P<0.05). No difference was found in the cervical overall ROM, target segmental ROM, the adjacent segmental ROM, and the COR-Y of the target level between pre-operation and last follow-up (P>0.05); while the intervertebral height and the COR-X increased significantly (P<0.05). The change of the COR-X had no obvious correlation with the postoperative JOA, NDI, and target segmental ROM (P>0.05). According to whether the difference of the COR-X between pre- and post-operation was less than the average value 1.86 mm or not, the patients were divided into 2 groups; significant difference was shown in the postoperative target segmental ROM between 2 groups (P<0.05), but no significant difference was found in the postoperative JOA、NDI, cervical overall ROM, adjacent segmental ROM, and the intervertebral height between 2 groups (P>0.05). ConclusionSingle segmental cervical disc replacement with ProDisc-C can obtain satisfactory outcomes. The cervical overall ROM, target segmental ROM, and adjacent segmental ROM can be effectively maintained, and the intervertebral height is increased. The location of the flexion/extension COR of the target segment shifts forward after insertion of the ProDisc-C prosthesis, and the postoperative target segmental ROM becomes smaller as the distance of the displacement of the COR becomes greater.

  • Study on Early Effects of Vectra-T Dynamic Plate for Cervical Spondylosis

    ObjectiveTo study the efficacy of anterior cervical Vectra-t dynamic plate and titanium mesh for cervical spondylosis. MethodsA retrospectively analysis involving 20 cases (44 sections) from June 2010 to October 2012 was performed in the department of orthopaedic in the West China Hospital of Sichuan University. All patients received the operation of anterior cervical vertebral resection decompression, titanium mesh and bone graft, and Vectra-T plate. The data of the JOA scores, cervical curvature, the conditions of vertebral body fusion after bone graft and the situation of titanium mesh subsidence were all collected. Statistical analysis were performed using SPSS 19.0 software. Resultsa) All the 20 cases received a 12-to 30-month follow-up. The results of statistical analysis showed that:the JOA scores were as follows:7.68±1.66 for preoperative, 10.91±1.97 for after surgery and 12.74±1.27 for the endpoint of follow-up, and significant differences were found between these scores (P < 0.05). b) There was no loose of screw or plate. The local cervical angle and regional cervical angle were improved after surgery. In the 44 segments of fusion, 43 segments fused within 6 months, one of the segment delayed to fuse 12 months after surgery. c) The subsidence of titanium mesh were 2.7±2.7 mm on average in 3 months after surgery, 3.6±1.0 mm on average in 6 months after surgery, 3.9±1.2 mm on average in 12 months after surgery and 4.0±1.2 mm on average in last follow-up. Titanium mesh subsidence mainly concentrated in the postoperative period of 6 months. After six months of titanium mesh subsidence was no longer apparent. ConclusionThe application of Vectra-T dynamic plate can effectively improve the curvature of the cerical sagittal position, prevent protrusion deformity, reduce the related complications after bone graft, effectively avoid the stress shelter in the static plate, andspromote the fusion between the cervical vertebral bodies. However, the long-term curative effects are still needed to be assessed by large sample randomized controlled trials with long-term follow-up.
