  • Effect of Recombinant Human Growth Hormone on Intestinal Bacteria and Endotoxemia Translocation in Experimental Obstructive Jaundice

    Objective To investigate the effect of recombinant human growth hormone (rhGH) on intestinal bacteria and endotoxin translocation in experimental obstructive jaundice. MethodsObstructive jaundice rat models were made and divided into three groups: sham operation (SO) group, obstructive jaundice (OJ) group and obstructive with rhGH (OG) group. The number in each group was 20. The mice in rhGH group underwent subcutaneous injection each day of Saizen, with the dose of 0.75 u/kg, while SO group and OJ group received nitric sodium injection. All these maitained for 2 weeks, then the animals were killed and the endotoxin were determined by limulus test, and bacterial cultures of ascites, blood, mesenchymal lymph node, kidney, spleen and liver were made, and the height of villi and the thickness of intestinal walls were examined.ResultsThe value of endotoxin in OJ group was (0.77±0.03) u/ml, higher than that in OG group and SO group, while it was (0.40±0.02) u/ml and (0.33±0.03) u/ml (Plt;0.01). The bacteria translocation rate in OJ group was 58.8%, much higher than that in OG group, which was 10.0% (Plt;0.01). There was no difference between OG group and SO group (Pgt;0.05). Villi height in OJ group was (183.39±11.09) μm, and thickness was (255.62±16.58) μm. While in OG group was (237.52±13.65) μm, and (320.81±14.34) μm (Plt;0.01) respectively.Conclusion rhGH has significant effect on protecting the injuried mucosa barrier in obstructive jaundice, and can decrease endotoxemia and bacteria translocation.

  • Effect of Ghrelin on Insulin Secretion and Expression of Glut-2 Protein in Isolated Pancreas of rat

    Objective To explore the effect of ghrelin on insulin secretion and expression of glucose transporter protein-2 (Glut-2) in isolated pancreas of rats. Methods Twenty five Wistar rats were randomly devided into normal control group (NC group), high concentration of glucose group (HCG group), high concentration of glucose with high concentration of ghrelin group (10-8mol/L, HCG+HCGh group), medium concentration of ghrelin group(10-9mol/L, HCG+MCGh group), and low concentration of ghrelin group (10-10mol/L, HCG+LCGh group) with 5 rats in each group. The rat isolated pancreas perfusion models were established firstly, then from the distal end of abdominal aortas, the models were perfused with low concentration of glucose (5.5mmol/L), high concentration of glucose (33.3mmol/L) or high concentration of glucose added with different concentrations of ghrelin. Levels of insulin outflowed from portal vein were tested by ELISA method, expression levels of Glut-2 protein were tested by immunohistochemical method,and ultrastructure changes of islet β cell were observed under the transmission electron microscope. Results There were no significant difference on levels of fasting blood glucose (FBG), fasting insulins (FINS), homeostasis model of assess-ment for insulin resistence index (HOMA-IR), and homeostasis model of assessment for pancreatic β cell function (HOMA-β),(P>0.05). There were no significant difference on insulin levels of effluent from portal vein of 5 groups (P>0.05) when isolated pancreas perfused with 5.5mmol/L glucose, while had 2 secretion peaks in 3min and 10-12min after 33.3 mmol/L glucose perfusion, where HCG+HCGh group at the top. The mean density value of Glut-2 protein in NC group was higher than that of other 4 groups (P<0.05). The results of transmission electron microscopy showed that apoptosis was lighter in NC group than that of other 4 groups, and apoptosis of HCG+HCGh group was lighter than that of HCG+MCGh group and HCG+LCGh group. Conclusions In isolated pancreas of rats, ghrelin promotes high concentration of glucose-stimulated insulin secretion, decreases expression of Glut-2 protein, and protects the islet β cell.

  • Experimental Research of Renal Aquaporin-3 Expression in Obstructive Jaundice

    ObjectiveTo explore the value of Aquaporin-3 (AQP-3) on the detection of early renal function damage by investigating the expressions of renal AQP-3 mRNA and protein of rats with obstructive jaundice (OJ). MethodsForty mature male Wistar rats were divided into two groups randomly: experimental group (n=20) in which the model of OJ rats was established, and control group (n=20, sham operation group). The levels of serum total bilirubin (TBIL), direct bilirubin (DBIL), creatinine (Cr), and blood urea nitrogen (BUN) were detected by fullautomatic biochemical analyzer on 7 d and 14 d after operation. The expressions of renal AQP-3 mRNA and protein of rats were detected by RT-PCR and Western blotting, respectively. ResultsThe levels of serum TBIL and DBIL were significantly higher on 14 d than those on 7 d after operation in experimental group (P=0.000), which were significantly higher than those at corresponding time point in control group (P=0.000), while the difference within control group was not significant (P=0.154). Thus, the OJ models of rats were established successfully. The difference of serum Cr levels of rats between inter-and intragroup were not significant (Pgt;0.05). Serum BUN level on 14 d after operation in experimental group was significantly higher than those on 7 d after operation in experimental group and on 14 d after operation in control group (P=0.001), although serum Cr levels were not different between 7 d and 14 d after operation in control group (P=0.288). The expressions of AQP-3 protein of rats on 7 d and 14 d after operation in experimental group were significantly lower than those at corresponding time point in control group (P=0.033, P=0.000), meanwhile on 14 d after operation in experimental group was significantly lower than those on 7 d after operation in experimental group (P=0.000). The expressions of AQP-3 mRNA of rats on 7 d and 14 d after operation in experimental group were significantly higher than those at corresponding time point in control group (P=0.000), but the difference at different time point in two groups was not significant (P=0.139, P=0.059). ConclusionsThe changes of renal AQP-3 protein and mRNA expressions are prior to the changes of serum Cr and BUN levels of rats suffered from OJ complicated renal function damage, which are promised to improve the early diagnosis rate of renal function damage in rats with OJ.

  • Investigation on Perioperative Renal Aquaporin 2 Expression in Experimental Obstructive Jaundice

    Objective To investigate the changes of renal medulla aquaporin 2 expression and morphological changes of epithelia of collecting tube after bile duct recanalizaiton operation. Methods Thirty rats were divided into two groups randomly. Common bile duct ligation was performed on 20 experimental rats with silicon tubes 2 mm in extre-diameter, and sham operation on the other 10 rats. Seven days later, bile duct recanalizaiton was performed on obstructive jaundice group and sham operation on contrast group. Experimental rats were divided into two subgroups randomly. Half of them were killed immediately and the others would be killed 24 hours later. Serum of each rat was collected to detect hepatic function and renal function. Renal medulla was fixed for microscopic examination and was kept in the -80 ℃ refrigerator for aquaporin 2 expression measurement by Western blot technique. Results All of the animals accomplished the experiment smoothly. Golden ascites were found in the rats of obstructive jaundice group. Twenty-four hours after recanalization, serum bilirubin levels decreased 〔(45.95±8.39) μmol/L〕, P<0.01, and there was no significant change in blood urine and creatine level. Compared with sham operation group (21 966.20±1 544.70), expression of aquaporin 2 decreased significantly after common bile duct ligation in obstructive jaundice group (15 665.30±1 181.85), P<0.01. After recanalizaion, the expression of aquaporin 2 in obstructive jaundice group increased (19 490.80±4 239.32), P<0.01. Conclusion Common bile duct obstruction would lead to epithelium injury of renal collecting tube, and down regulate the aquaporin 2 expression.

  • Comparative Study and False Negative Cases Analysis of Preoperative Ultrasonography, 99Tcm-Sestamibi Scinti-graphy, and CT in Primary Hyperparathyroidism

    Objective To evaluate the diagnostic significance and to analyze reasons of false negative cases forpreoperative ultrasonography, 99Tcm-sestamibi scintigraphy (MIBI scintigraphy), and CT in primary hyperparathyroidism(PHPT). Methods Clinical data of 69 patients with PHPT, who underwent operation in Affiliated Shengjing Hospital of China Medical University between Jan. 2003 and Aug. 2012 were retrospectively analyzed. Results There were 76 parathyroid lesions in 69 PHPT patients proved by operation and pathology, including 58 cases of parathyroid adenoma with 60 lesions, 7 cases of parathyroid hyperplasia with 11 lesions, and 4 cases of parathyroid carcinoma with 5 lesions. The sensitivity of ultrasonography, CT, and 99Tcm-MIBI scintigraphy were 81.94% (59/72), 61.76% (21/34), and 69.57% (16/23), the accuracy of 3 kinds of tests were 78.67% (59/75), 61.76% (21/34), and 66.67% (16/24), the positive predictive value were 95.16% (59/62), 100% (21/21), and 94.12% (16/17) respectively. There was significant differ-ence only between ultrasonography and CT in sensitivity (P=0.03), no other significant difference was found (P>0.05).Conclusions Ultrasonography is complementary to 99Tcm-MIBI scintigraphy, but CT has little significance in diagnosis of PHPT. Both of ultrasonography and 99Tcm-MIBI scintigraphy should be used before operation routinely to localize parathyroid lesions.

  • Comparison of Therapeutic Effects of Laparoscopic Versus Open Operation in Treatment for Adult Congenital Cholangiectasis

    Objective To explore the feasibility and safety of laparoscopic cyst resection and Roux-en-Y hepatojejunostomy in treatment for adult congenital cholangiectasis and to compare the therapeutic effects of laparoscopic procedure with conventional open procedure. Methods The clinical data of 33 adult patients with congenital cholangiectasis from May 2008 to September 2011 in the department of general surgery of Shengjing Hospital of China Medical University were retrospectively analyzed. Fourteen patients received laparoscopic procedure (laparoscopic group),whereas the other 19 patients received conventional open procedure (conventional group). Results All the operations were carried out successfully through laparoscopic procedure. The mean time of operation in the laparoscopic group was significantly longer than that in the conventional group (195min versus 130min,P<0.01). The average intraoperative blood loss in the laparoscopic group was significantly less than that in the conventional group (80ml versus 270ml,P<0.01). In contrast,the mean time of bowel peristalsis recovery and postoperative hospital stay in the laparoscopic group were significantly shorter than those in the conventional group (time of bowel motion recovery:76h versus 104 h,P<0.01;hospital stay:6.1 d versus 9.6 d,P<0.01). There were no differences in the early complications between two groups (P>0.05). Conclusions Totally laparoscopic treatment for congenital cholangiectasis in adult is feasible and safe. It is worth to be generally applied because of its minimal invasion and fast postoperative recovery.

  • Effect of Urokinase on Severity Acute Pancreatitis with Renal Injury in Rat

    Objective To explore the effect of renal microcirculation following severity acute pancreatitis (SAP) on renal injury and to explore the protection effect of urokinase on them. Methods A total of 192 Wistar rats were randomized divided into normal control group, SAP group, and urokinase group, then rats of 3 groups were sub-divided into 2, 6, 12, and 24 hours group, each group enrolled 16 rats. Of the 16 rats in each subgroup, 8 rats underwent blood flow of renal test, other 8 rats were sacrificed to get blood samples and to perform histopathological examination. The rat models of SAP were established by retrograde injecting with 5% sodium taurocholate into the cholangiopancreatic duct. Radioactive biomicrosphere technique was used to measure the blood flow of renal, levels of plasma thromboxane B2(TXB2) and 6-keto-prostaglandin F1α (6-Keto-PGF1α) were tested by the TXB2 kit and 6-Keto-PGF1α kit, and histopa-thological changes of renal tissues were observed by using HE staining. Results Compared with normal control group at the same time point, the blood flow of renal were lower (P<0.05), activity ratio of TXB2 to 6-Keto-PGF1α were higher(P<0.01), and the histopathological injury were worse (P<0.01) in rats of SAP group and urokinase group. Compared with SAP group, the blood flow of renal at 2, 6, and 12 hours in urokinase group were higher (P<0.01), the activity ratios of TXB2 to 6-Keto-PGF1α were lower (P<0.01), and the histopathological injury were lighter (P<0.05) in all the 4 time points of urokinase group. Conclusions The renal microcirculation dysfunction and increase of activity ratio of TXB2 to 6-Keto-PGF1α may play an important role in renal injury following SAP in early stage. Urokinase can protect the renal from such injuries.
