  • Research progress of clinical application of laparoscopic Heller myotomy and peroral endoscopic myotomy in treatment of achalasia of cardia

    Objective To compare effectiveness between laparoscopic Heller myotomy and peroral endoscopic myotomy (POEM) in treatment of achalasia of cardia (AC) in order to provide a basis for clinical choose. Method The literatures about the treatment of AC by laparoscopic Heller myotomy or POEM were retrieved from CNKI, Embase, PubMed databases, etc., and then the contents about curative effect and complications were summarized. Results The treatment models of AC included surgical treatment such as laparoscopic Heller myotomy and endoscopy such as POEM, but there was still lack of comparing data in these two treatment models, its selection remained controversial. There was a better short-term curative effect and slighter complications for POEM as compared with the laparoscopic Heller myotomy from the trend of published literatures. However, it’s long-term effects for these two treatment models were not clarified. Conclusions Both laparoscopic Heller myotomy and POEM are medicable for AC. POEM as a new treatment of AC shows some advantages of minimal invasion and exact efficacy, but it needs to be followed-up for a long-term. Treatment model for AC is chosen on basis of typing under endoscope, physical fitness, anatomy of easophagus, previous history, tolerance of surgery and other factors.

  • Etiology and Clinical Characteristics of 482 Children with Hand-Foot-Mouth Disease

    Objective To investigate the etiological and clinical characteristics of the outbreak of hand-foot-mouth disease (HFMD) in Chengdu in 2008. Methods A retrospective analysis was conducted to investigate 482 children with hand-foot-mouth disease in Chengdu from January to December 2008 in terms of epidemiology and clinical characteristics. Results Among the 482 children, 76.14% of the cases were infants, and male children numbered more than female ones. The proportion in the urban area was higher than that in the rural area. May to July was the peak epidemic period. Skin rash and fever were major clinical manifestations. Three cases were diagnosed as viral encephalitis. Conclusion The incidence of hand-foot-mouth disease is related to age and season. Effective prevention measures and isolated treatment are critical for prevention and therapy.

  • Research progress of drug-eluting beads in arterial chemoembolization for hepatocellular carcinoma

    ObjectiveTo summarize the research progress of four mainstream drug-eluting beads (DEB) in transcathete arterial chemoembolization (TACE) for hepatocellular carcinoma in recent years.MethodThrough retrieving relevant literatures at home and abroad, the physicochemical properties, pharmacokinetics, effectiveness, and safety of four mainstream DEBs were reviewed.ResultsThe current mainstream DEBs on the market had the characteristics of high drug loading rate and sustained release of drugs, which could reduce the adverse reactions after the embolization to a certain extent. The safety and tolerance of patients receiving DEB-TACE were slightly higher than those receiving traditional TACE (c-TACE). There were no serious complications after the DEB-TACE, but the short-term and long-term efficacies between the two methods were not significantly different.ConclusionsAt present, most studies believe that the safety and tolerance of DEB are slightly higher than that of c-TACE, but whether the former is more effective than the latter is still controversial at home and abroad, and more high-quality multicenter randomized controlled trials are needed to confirm it.
