  • Predictive value of preoperative pulmonary function test indexes for postoperative pneumonia in patients undergoing upper gastrointestinal surgery

    ObjectiveTo analyze the relationship between preoperative pulmonary function indexes and postoperative pneumonia (POP) in patients undergoing upper gastrointestinal surgery.MethodsThe clinical data of 303 patients who underwent lung function examination and upper gastrointestinal surgery in West China Hospital, Sichuan University from September 2020 to January 2021 were prospectively collected and analyzed. There were 217 males and 86 females, with an average age of 61.61±10.42 years. Pulmonary function was evaluated from four aspects including ventilatory function, pulmonary volume, diffusion function and airway resistance. Relevant pulmonary function indicators were displayed as the percentage of actual measured value to predicted value (%pred). The outcome index was pneumonia within 30 days after the surgery. Logistic regression was used to analyze the relationship between preoperative pulmonary function indicators and POP.ResultsA total of 196 patients with gastric cancer and 107 patients with esophageal cancer were included, and the incidence of POP in patients undergoing upper gastrointestinal surgery was 26.7% (81/303). Patients with preoperative low peak expiratory flow (PEF%pred) had a 3.094 times higher risk of developing POP than those with normal PEF%pred [OR=3.094, 95%CI (1.362, 7.032), P=0.007]. The incidence of POP had no correlation with the other preoperative indicators.ConclusionPreoperative PEF%pred may be an important indicator for predicting the occurrence of POP in patients undergoing upper gastrointestinal surgery.

  • Research progress of patient intelligent monitoring and risk warning

    ObjectiveTo summarize the research status of intelligent patient monitoring and risk warning, and provide reference and enlightenment for promoting the construction of intelligent monitoring and management platform of clinical patient risk.MethodThe literatures about patient monitoring and risk warning at home and abroad in recent years were reviewed.ResultsAt present, the research at home and abroad mainly focused on the retrospective construction of the prediction model of severe complications of inpatients by using the electronic medical record database. The clinical decision support system based on real-time vital signs and dynamic electronic medical record data was still in the early development stage, and there was no mature product with high market share.ConclusionsThe construction process of structured electronic medical record system should be further strengthened, and the fully integrated clinical decision support system and artificial intelligence self-learning system should be the key research and development direction in the future, so as to promote the deep integration of big data and artificial intelligence technology with clinical scenes.

  • Effectiveness and cost analysis of enhanced recovery after surgery in patients underwent radical resection of hepatocellular carcinoma

    ObjectiveTo analyze the clinical effects and hospitalization costs of the enhanced recovery after surgery (ERAS) mode for patients underwent radical resection of hepatocellular carcinoma (HCC). MethodThe clinicopathologic data of adult patients who underwent radical resection of HCC in the West China Hospital of Sichuan University from January 2017 to December 2019 were collected retrospectively. ResultsA total of 1 082 patients were collected according to inclusion and exclusion criteria in this study, including 469 patients in the ERAS mode group and 613 patients in the traditional mode group. There were no significant differences in baseline data such as gender, age, body mass index, preoperative complications, operation type, and medical group between the two groups (P>0.05). Compared with the traditional mode group, the total hospitalization cost, medical service fee, examination fee, bed fee, and cooling and heating fee in the ERAS mode group were lower (P<0.05), the average total hospitalization time was shortened by about 1.2 d (P<0.01), the preoperative hospitalization time was shortened by about 0.3 d (P=0.03), and the postoperative hospitalization time was shortened by about 0.8 d (P<0.01) in the EARS mode group. There were no significant differences in the incidences of specific complications and total complications between the two groups (P>0.05). ConclusionERAS for patients with HCC might play a role in improving rehabilitation efficiency and reducing hospitalization costs.

  • Current situation analysis and thinking of nursing discipline construction and development in China

    Nursing science is an important subject for maintaining and promoting the physical and mental health of humans. In recent years, with the rapid development of global life health science and technology, the nursing discipline has faced new opportunities and challenges under the guidance of the national Double First-Class discipline construction plan. This article briefly reviews the development process and present situation of the nursing discipline, and puts forward the outlook for nursing discipline construction. These suggestions include constructing a nursing ideological education system based on Chinese culture, enhancing the nursing discipline system, optimising nursing talent allocation, and establishing nursing science and technology support platforms, in order to provide reference for promoting the development of the nursing discipline.

  • The progress of evaluation indexes for enhanced recovery after surgery model athome and abroad

    Objective To analyze the progress of evaluation indexes for enhanced recovery after surgery (ERAS) model at home and abroad, and to propose suggestions for constructing systematic evaluation model of ERAS. Methods Atfirst checked the Chinese and English databases, including Medline, Embase, Sciencedirect, ACP Journal Club, BioMed Central, the Cochrane Central Register of Controlled Trials (CENTRAL), Science Citation Index Expanded, Cochrane Library, NHS Economic Evaluation Database, CNKI, VIP, and Wanfang databases (retrieval time was from January 1997 to December 2017), and then filtered the literatures, excluded duplicate documents, a total of 1 020 English literatures and 786 Chinese literatures were enrolled eventually to make an review. Results The literatures showed that, at present, there was no comprehensive and systematic evaluation index system about ERAS at home and abroad. The existing evaluation indexes mainly included the following deficiencies: localization and fragmentation of evaluation indicators, lack of evaluation indicators of ERAS organizational framework and process management, as well as lack of standardized operational definition of evaluation indicators. Conclusions The evaluation indexes of ERAS at home and abroad do not constitute a systematic evaluation index system according to scientific principles, which will restrict the standardization of accelerated surgical rehabilitation in our country. To establish a multidimensional and comprehensive quality evaluation index system based on multi-evaluation of hospital, patient, social, and medical management institutions, which covers ERAS organizational structure, process management, and clinical outcomes, is a necessary condition for the development of ERAS model.

  • Preoperative intestinal dysbacteriosis in colorectal cancer patients and its associated risk factors

    ObjectiveTo investigate the prevalence of preoperative intestinal dysbacteriosis in colorectal cancer patients and its risk factors.MethodsFrom March 2016 to December 2017, patients who received colorectal cancer surgery in the Department of Gastrointestinal Surgery in West China Hospital were prospectively recruited in the study. Fresh fecal samples were collected on the day of admission for gram-stainsmears. The diet, preexisted diseases, medication, and treatment before admission were investigated by a self-designed semi-structured questionnaire.ResultsSum to 257 patients were recruited, 123 patients (47.9%) of them had normal intestinal bacterial, 48 patients (18.7%) were in preoperative dysbacteriosis grade Ⅰ, 61 patients (23.7%) were in gradeⅡ, 25 patients (9.7%) were in grade Ⅲ. Orderly logistic regression showed that patients who received antibiotics for 30 days or more per year had a higher risk of preoperative dysbacteriosis than those who did not receive antibiotics (OR=3.38, P=0.025). Patients with BMI≥24.0 kg/m2 had a lower risk of dysbacteriosis than patients with BMI<18.5 kg/m2 (BMI 24.0~27.9 kg/m2, OR=0.36, P=0.030; BMI≥28.0 kg/m2, OR=0.23, P=0.032).ConclusionMore than half of colorectal cancer patients have intestinal dysbacteriosis prior to surgery, which may be associated with exposure to antibiotics before admission and low BMI.

    ObjectiveTo explore gut microbiome influences on anastomotic healing following gastrointestinal surgery and its mechanism.MethodThe relevant literatures about gut microbiome and its impact on healing of gastrointestinal anastomosis and their mechanisms were reviewed.ResultsSeveral symbiotic intestinal microbiota such as the Enterococcus faecalis, Pseudomonas aeruginosa, Serratia marcescens, etc. could transform into the pathogenic bacteria with high toxic phenotype in an inflammatory environment in the body, and dissolve the extracellular matrix by degrading collagen or activating matrix metalloproteinase 9, resulting in the anastomotic leak.ConclusionIn general, exploring of effect of intestinal microbiome on healing process of anastomotic stoma is just beginning, conditions and mechanisms for transformation of bacteria from symbiotic to pathogenic still need to be explored.

    During the coronavirus disease 2019 (COVID-19) epidemics, universities take responsibility for the health of their students and epidemic control. Our urgent recommendation focuses on four key questions of emergency management in universities following rigorous evidence-based approaches and provides timely suggestions to university managers, academic faculties and student affairs managers. We recommend universities during the COVID-19 epidemics should: 1) suspend offline lectures; 2) provide proper health education on the disease; 3) encourage face masks, however oppose using N95 masks on the campus; 4) encourage hand hygiene and provide sanitizing products on the campus.
