  • FEASIBILITY AND SHORT-TERM EFFECTIVENESS ANALYSIS OF MODIFIED RADICAL RESECTION AND RECONSTRUCTION SURGERY FOR MALIGNANT PROXIMAL HUMERUS TUMOR

    Objective To explore the feasibility and short-term effectiveness of the modified radical resection and reconstruction in the treatment of malignant proximal humerus tumor. Methods The relevant anatomic data from 30 normal adult shoulder joint MRI were measured to analyze the feasibility of modified radical resection and reconstruction surgery in the treatment of malignant proximal humerus tumor. Five patients with malignant proximal humerus tumor were treated by using the modified radical resection and reconstruction surgery between March 2012 and January 2016. There were 1 male and 4 females, aged from 9 to 69 years (median, 46 years). There were 4 cases of osteosarcoma (Enneking IIA in 2 cases and Enneking IIB in 2 cases) and 1 case of metastatic carcinoma (moderately differentiated adenocarcinoma). The disease duration was 7 to 12 months (mean, 9 months). Recurrence of tumor was observed after operation, and the shoulder function was assessed according to Enneking skeletal muscle tumor function scoring system. Results Radiographic results showed that modified radical resection and reconstruction surgery was feasible, which was in allowable range of the maximum longitudinal diameter ( < 29.8 mm) and depth ( < 4 mm). The operation was successfully completed in all 5 cases, and pathological examination suggested that purposes of radical resection had achieved. All patients were followed up 3 to 49 months (mean, 15.6 months). One patient had local recurrence at 12 months after operation, and a shoulder joint amputation was performed; the other 4 patients had good prosthesis survival. At last follow-up, the function of the shoulder joint was obviously recovered when compared with preoperative function; Enneking's skeletal muscle tumor function score was 25.8 points (range, 24 to 27 points). Conclusion Modified radical resection and reconstruction surgery is feasible for the treatment of proximal humerus tumor, and it can maintain a good early shoulder function.

  • Analysis of Influence Factors of Gallstone

    ObjectiveTo explore the influence factors of gallstone. MethodsClinical data of 511 patients who were admitted to our hospital from Apr. 2015 to Apr. 2016 were retrospectively analyzed. ResultsOf 511 patients, there were 274 patients with gallstone (gallstone group) and 237 patients without gallstone (control group). Univariate analysis results showed that, no significant difference of age, gender, and diabetes was found between gallstone group and control group (P > 0.050), but the levels of serum total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL-C), and fasting blood glucose (FBG) were significantly higher in patients of gallstone group (P < 0.050), but the level of serum high density lipoprotein (HDL-C) was significantly lower in patients of gallstone group (P=0.001). Logistic regression results showed that four factors including diabetes〔OR=4.491; 95% CI is (2.021, 9.976); P < 0.001〕, the serum TC〔OR=2.548; 95% CI is (1.944, 3.338); P < 0.001〕, HDL-C〔OR=0.115; 95% CI is (0.056, 0.237); P < 0.001〕, and FBG〔OR=1.277; 95% CI is (1.086, 1.502); P=0.003〕 entered the final regression model after controlling confounding factors. The results showed that patients who combined with diabetes, high levels of serum TC and FBG, and low level of serum HDL-C, had higher ratio of galls-tone. ConclusionDiabetes, high-level of serum TC and FBG, low-level of serum HDL-C were risk factors of gallstone.

  • Study on the Effects of Artificial Pricing System in Wound Care Clinic

    ObjectiveTo analyze the application of artificial pricing system in wound care clinic. MethodsWe retrospectively analyzed the pricing and charging by five nurses with artificial pricing system between January and May 2013 in the wound care clinic of a grade-A level-three hospital in Beijing. Among the 1012 times of pricing, 448 were out-of-pocket cases, 372 were insurance cases and 192 were military cases. ResultsGenerally, the average fee of the three types of cases had no significant difference (P>0.05). The military case was a little higher than the other two kinds of cases, and the cost of insurance cases was the lowest of all. There were no significant difference between the average price of out-of-pocket cases and military cases, and the general average price, but there was some differences in the aspect of insurance pricing. In the insurance cases, two nurses' pricing was significantly lower and one significantly higher than the general average (P<0.05). ConclusionThe artificial pricing system is unstable and uncertain, and it has high risk on patients'safety in wound care clinic.

  • PHOSPHORYLATABLE SHORT PEPTIDE CONJUGATED CHITOSAN MEDIATED GENE THERAPY FOR REPAIR OF ARTICULAR CARTILAGE DEFECT IN RABBITS

    ObjectiveTo investigate the effect of phosphorylatable short peptide (pSP) conjugated chitosan (CS) (pSP-CS) mediated insul in-l ike growth factor 1 (IGF-1) gene and human interleukin 1 receptor antagonist (IL-1Ra) gene local transfection on the repair of articular cartilage defect. MethodsCo-expression plasmid pBudCE4.1-IL-1Ra+IGF-1, single gene expression plasmid pBudCE4.1-IL-1Ra and pBudCE4.1-IGF-1 were constructed and combined with pSP-CS to form pSP-CS/ pDNA complexes. Thirty 3-month-old healthy male New Zealand white rabbits, weighing 2.0-2.5 kg, double legs were randomly divided into 5 groups (n=12). Lateral femoral condyle articular surface was only exposed in sham-operated group (group A); full-thickness cartilage defects were created in the articular surface of the lateral femoral condyle of the knee in 4 intervention groups: pSP-CS/pBudCE4.1 (group B), pSP-CS/pBudCE4.1-IL-1Ra (group C), pSP-CS/pBudCE4.1-IGF-1(group D), and pSPCS/ pBudCE4.1-IL-1Ra+IGF-1 (group E). At 1 week after operation, intra-articular injection of pSP-CS/pDNA complexes was administrated 2 times a week for 7 weeks in each intervention group, the same volume normal sal ine in group A. The general condition of animal was observed after operation, and rabbits were sacrificed at 8 weeks. Knee joint synovial fluid was collected to measure the concentrations of the IL-1Ra and IGF-1 by ELISA; mRNA expressions of Aggrecan, matrix metalloproteinase 3 (MMP-3), and MMP inhibitor 1 (TIMP-1) were detected by real-time fluorescent quantitative PCR; the chondrogenic phenotype of nascent cells in the damage zone was identified by alcian blue-periodic acid/schiff (AB-PAS) histochemistry and Aggrecan immunohistochemistry staining. ResultsThirty experimental rabbits all survived to the end of experiment, without infection and death. Large amounts of exogenous proteins of IGF-1 and IL-1Ra were detected in the synovial fluid of 4 intervention groups. There were significant differences between groups D, E and group A in IGF-1 protein expression, and between goups C, E and group A in IL-1Ra protein expression (P < 0.05). Aggrecan and TIMP-1 mRNA expressions were significantly up-regulated in group E, simultaneously MMP-3 mRNA expression was significantly down-regulated when compared with groups C and D (P < 0.05). Varying degrees of cartilage repair appeared in groups C, D, and E, showing positive staining of AB-PAS and Aggrecan, and group E had better results than groups C and D (P < 0.05); inflammatory cell infiltration and fibrous tissue prol iferation were seen in the defect region of group B, without significant cartilage repairing. ConclusionpSP-CS is an ideal gene del ivery system for cartilage defect gene therapy; IL-1Ra and IGF-1 double gene transfection has better biologic effect on cartilage defect repair.

  • Differentiation and Handling of Homogeneity in Network Meta-analysis

    Compared with traditional head to head meta-analysis, network meta-analysis has more confounding factors and difficulties to handle. Due to the mutual transitivity of evidence in network meta-analysis, heterogeneity may be brought into indirect meta-analysis. Hence, effective differentiation and correct handling of heterogeneity are being current focus. In order to ensure the reliability of the results of network meta-analysis, the concept of homogeneity is proposed and a series of methods are developed for differentiation and handling of homogeneity. Based on the extension of Bucher methods, current methods for differentiation and handling of homogeneity has extended to ten quantitative measures (eg., node analysis method, hypothesis tests, and two-step method). However, because of the differences and the focus of fundamental methodological theories as well as the limitation of statistics power, no highly-effective method has been worked out. Therefore, the exploration of highly-effective, simple and high-resolved methods are still needed.

  • Field Effect of a Novel Plant Molluscicide “Luo-Wei” Against 50% Wettable Powder of Niclosamide Ethanolamine Salt: A Systematic Review

    ObjectiveTo evaluate the two means of field molluscicidal effect by two different snail control drugs--new plant snail control agent luo-wei (Tea-seed distilled saponins, TDS) and 50% wettable powder of niclosamide (WPN). MethodsSuch databases as CNKI, WanFang Data, VIP and CBM databases were searched for controlled trials about TDS vs. WPN for the molluscicidal effect from their establishment to April 2014. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data and assessed the methodological quality of included studies, and then meta-analysis was performed by using RevMan 5.2 software. ResultsA total of 9 studies were included. The results of meta-analysis showed that:Snails mortality rate of TDS on 7 d was lower (RR=0.96, 95%CI 0.93 to 1.00, P=0.04) than WPN and no significant difference of the snails mortality rate on 1 d or 3 d between the two groups was found by means of immersion. Meanwhile, by means of spraying, there were no significant differences on the snails mortality rate on 1 d, 3 d, 7 d, 15 d between the two groups; densities of living snails on 15 d wasn't, either. Based on GRADE system, the evidence was at level B or C. ConclusionCompared with WPN, TDS has reached the requirements of the natural source twin-screw agent field evaluation. The molluscicidal effect of TDS is better, with its large stock and low price, it is worthy to do further popularization.

  • Clinical Application of Hemodilution Autotransfusion in Liver Resection for Patients with Hepatic Echinococcosis

    ObjectiveTo evaluate the effect of dilute autotransfusion in liver resection for patients with hepatic hydatid disease. MethodThirty patients with hepatic hydatid disease met the inclusion criteria were included. The changes of intraoperative hemodynamics, internal environment, and each system functions at different time point were observed. Results①There were no significant differences among mean arterial pressure, central venous pressure, heart rate in all the patients at each time point (P > 0.05), while, the other indexes including the concentrations of hemoglobin (Hb), hematocrit (Hct) after blood collection were all greatly lower than those before blood collection (P < 0.05), Na+, K+, Cl-, pH, and base excess after blood collection were all greatly lower than those before blood collection (P < 0.05). But all the indexes above mentioned (except for ph and base excess) returned to normal.②Compared with these indicators before surgery, the blood concentrations of AST, ALT, cardiac troponin I, and creatinin were all greatly increased (P < 0.05), the blood concentrations of Hb, Hct, platelet were all decreased apparently after surgery 1 day (P < 0.05). The prothrombin time and activated partial thromboplastin time were all prolonged (P < 0.05). But after surgery one week, those indicators had rebounded to the normal range. ConclusionThe application of dilute autotransfusion in liver surgery for patients with hepatic hydatid disease is safe, feasible, and saving.
