  • Experimental study of bone morphogenetic protein-4 in promoting recovery of small intestinal mucosal barrier during recovery period of intestine ischemia-reperfusion injury

    Objective To investigate the mechanism of bone morphogenetic protein-4 (BMP4) in promoting the recovery of small intestinal mucosal barrier function during the recovery period of small intestine ischemia-reperfusion (I/R) injury. Methods Twenty-eight C57BL/6J male mice aged 6–8 weeks were randomly selected and assigned to small intestine I/R group (n=24) and sham operation (SO) group (n=4) by random number table method. Small intestine I/R injury models of 24 mice were established, then 4 mice were randomly selected at 6, 12, 24 and 48 h after I/R established modeling and killed to observe the morphological changes of small intestinal mucosa and detect the expression of BMP4 mRNA in the jejunal epithelial cells, the other 8 mice were allocated for the experimental observation at the recovery period of small intestine I/R injury (24 h after I/R was selected as the observation time point of recovery period of small intestine I/R injury according to the pre-experimental results). Twelve mice were randomly divided into I/R-24 h-BMP4 group (recombinant human BMP4 protein was injected intraperitoneally), I/R-24 h-NS (normal saline) group (NS was injected intraperitoneally), and I/R-24 h-blank group (did nothing), 4 mice in each group. Then the small intestinal transmembrane electrical impedance (TER) was measured by Ussing chamber. The expressions of BMP4 protein and tight junction proteins (occludin and ZO-1), Notch signaling pathway proteins (Notch1 and Jagged1), and Smad6 protein were detected by Western blot. Results At 24 h after I/R injury, the injuries of villous epithelium, edema, and a small part of villi were alleviated. The BMP4 mRNA expressions at 6, 12, 24 and 48 h after I/R injury in the small intestinal epithelial cells were increased as compared with the SO group. Compared with the I/R-24 h-NS group and the I/R-24 h-blank group, the TER was increased, and the expression levels of occludin, ZO-1, p-Smad6, Notch1, Jagged1 were increased in the I/R-24 h-BMP4 group. Conclusion From the preliminary results of this study, during recovery period of small intestine I/R injury, the expression of BMP4 in small intestinal epithelial cells is increased, permeability of jejunal mucosal barrier is increased, which might promote the recovery of small intestinal mucosal barrier function by activating the Notch signaling pathway (Notch1 and Jagged1), Smad classic signaling pathway, and promoting the increase of tight junction protein expression (occludin and ZO-1).

  • A survey on the current situation of writing and publishing papers in international journals for postgraduate students of traditional Chinese medicine

    Objective To survey the current difficulties and guidance expectations of postgraduate students of traditional Chinese medicine (TCM) in writing and publishing international journal papers, in order to optimize the teaching mode of international journal articles for TCM postgraduates. Methods By means of a questionnaire survey, TCM postgraduate students in China were selected as the survey objects from four aspects: basic information, current situation of thesis publication and writing, paper writing difficulties and current situation and expectation of course offering. SPSS 26.0 software was used for data analysis. Results A total of 2 874 valid questionnaires were collected. A total of 1 365 postgraduate students had plans to write international journal papers; 324 postgraduate students had published international journal papers as the first author. The degree of difficulty in writing was generally rated high by postgraduate students, and lack of writing skills and time were the main reasons for the low motivation of thesis writing. A total of 2 547 graduate students felt that guidance on writing papers for international journals was urgently needed, with literature search methods and clinical research methods and experimental design as the most needed guidance, and reviewers and supervisors as the most desired guides. Conclusion The number of TCM postgraduates who have plans to write international journal papers is small, the proportion of publications is low, and the demand for international journal paper guidance courses is high. It is suggested that standardized international journal paper guidance courses be offered to improve the system of cultivating scientific research ability.

  • Causal associations between obstructive sleep apnea and cardiovascular diseases: a two-sample Mendelian randomized study

    ObjectiveTo analyze the causal relationship between obstructive sleep apnea (OSA) with its typical symptoms (daytime sleepiness and snoring) and cardiovascular diseases (hypertension, coronary heart disease, myocardial infarction, heart failure) by using Mendelian randomization. MethodsWe used the instrumental variables (IV) in the FINNGen database and the UK Biobank to perform two-sample Mendelian randomization (TSMR) analysis. The results of random-effects inverse variance weighting method (IVW) were the main results. MR-Egger method was used for pleiotropic analysis and sensitivity analysis was performed by the leave-one-out method to verify the reliability of the data. ResultsOSA could lead to hypertension (IVW β=0.043, 95%CI 0.012 to 0.074, P=0.006) and heart failure (IVW β=0.234, 95%CI 0.015 to 0.452, P=0.036). Daytime sleepiness also had a pathogenic effect on heart failure (IVW β=1.139, 95%CI 0.271 to 2.006, P=0.010). There was no causal association between OSA and CHD or MI, snoring and the four CVDs. There was no causal association between daytime sleepiness and hypertension, CHD or MI.ConclusionOSA and daytime sleepiness have pathogenic effects on hypertension and heart failure, with heart failure being the most affected.

  • Analysis of related factors affecting recovery of anal function after transanal total mesorectal excision

    ObjectiveTo evaluate the postoperative anal function of rectal cancer patients treated with transanal total mesorectal excision (taTME), and to analyze the influencing factors which resulted in low anterior resection syndrome (LARS) after taTME in this paper, so as to provide guidance for clinical practice.MethodsThe data about the patients with rectal cancer treated with taTME were collected at the Affiliated Nanchong Central Hospital of North Sichuan Medical College from December 2018 to December 2019, including the clinical data and follow-up data. Postoperative recovery condition of the patients’ anal function and the affecting factors caused the occurrence of severe LARS after taTME were analyzed. The patients’ anal function within 1, 6, and 12 months after taTME were evaluated, and the evaluation tools were LARS scale and Wexner scale. The follow-up period was up to December 30, 2020.ResultsA total of 67 patients were completed preoperative and postoperative follow-up at 1, 6, and 12 months. In terms of anal function, within 1 month after taTME was the worst period in which the anal function was the worst among all the points of time evaluated (1.49±0.33, 10.28±0.64, 6.42±0.60, and 3.73±0.61, respectively), and there was time trend during the follow-up period (F=66.101, P<0.001). In the first year after taTME, the patient’s anal function was in a state of continuous recovery, and the differences between the three points of time at 1, 6, and 12 months after taTME and preoperative anal function were statistically significant (P<0.010). The results of multivariate analysis indicated that the distance between the anastomotic stoma and the anal verge was independent risk factor affecting the postoperative anal function of the patients with taTME at 1 month and 6 months (P<0.010).ConclusionsWith time going, the postoperative anal function of the patients with taTME can be recovered to a certain extent. The distance between the anastomotic stoma and the anal verge was the independent factor affecting the postoperative function of the rectal cancer patients received taTME.

  • Panvasculopathy in diabetes: concept, status and challenges

    Diabetic panvasculopathy refers to a clinical syndrome in which a wide range of blood vessels (including heart, brain, kidney, eye, peripheral and other large vessels and microvessels) in diabetic patients have atherosclerosis as the common pathological feature, cardiovascular and cerebrovascular events as the main outcome, and active intervention of metabolic abnormalities is helpful to improve the prognosis. Early diagnosis, early prevention and active exploration of clear therapeutic targets are very important to prevent and delay the occurrence and development of diabetic panvasculopathy, but they are also challenges at present. The cardiorenal benefit of new hypoglycemic drugs is independent of blood glucose control, which has attracted increasing academic attention. The concept of panvasculopathy in diabetes plays an important role in improving the comprehensive management of the disease and multidisciplinary collaborative diagnosis and treatment, which is helpful to promote the vigorous development of the subspecialty of panvasculopathy and the continuous improvement of the comprehensive prevention and treatment level.
