  • Key Points of Nursing Care for Abdominal Enhanced CT Examination

    ObjectiveTo discuss the key points of nursing care for enhanced abdominal CT examination. MethodsA retrospective analysis of the clinical data of 1 500 patients receiving abdominal enhanced CT scan between March 2013 and March 2014 was carried out. Then, we summarized the nursing experiences before, during and after scanning, and various complications and accidents occurring during the examination. ResultsAll the patients completed the examination safely. Ten patients had mild adverse reactions, and the symptoms relieved or disappeared after active treatment; no medium or severe adverse reactions occurred. Two patients had extravasation of contrast medium, and the allergic symptoms gradually improved after the dressing of 50% wet magnesium sulfate. Five patients had poor image quality, including 2 with contrast agent extravasation, and 3 with respiratory motion artifacts. ConclusionGood nursing work is an important guarantee for the success of abdominal enhanced CT examination.

  • Influence of Low-tidal Volume and Positive End Expiratory Pressure Protective Ventilation on Cardiac Output Volume in Elderly Patients under General Anesthesia

    Objective To study the influence of low-tidal volume and positive end expiratory pressure (PEEP) protective ventilation on cardiac output volume in elderly patients under general anesthesia. Methods From August 2012 to July 2014, 60 elderly patients undergoing selective surgery were divided into three groups with 20 patients in each. Group A was treated with conventional ventilation: tidal volume at 8 mL/kg, PEEP at 0 cm H2O (1 cm H2O=0.098 kPa); group B was treated with a tidal volume of 6 mL/kg and a PEEP of 5 cm H2O; group C was treated with a tidal volume of 6 mL/kg and a PEEP of 8 cm H2O. We then observed and analyzed the blood pressure, heart rate, cardiac output, arterial blood gas and airway mean pressure before induction of anesthesia (T0), 15 minutes of mechanical ventilation after the induction of anesthesia (T1), 60 minutes after anesthesia induction (T2), and 15 minutes after tracheal extubation (T3). Results In all the three groups, the mean arterial pressure and cardiac output were stable. In group B and C, central venous pressure increased significantly, the mean airway pressure and lung compliance increased, and the arterial oxygen branch pressure also increased significantly (P < 0.05). Conclusion Low-tidal volume combined with 5-cm H2O or 8-cm H2O positive end expiratory pressure lung-protective ventilation had a small influence on the cardiac output of elderly patients under anesthesia, which can be safely used.

  • Disease Spectrum and Cost Constitution of Patients Aged over 65 Years with Respiratory Diseases in A Tertiary Hospital from 2010 to 2014

    ObjectiveTo investigate the disease spectrum and cost constitution of patients aged over 65 years with respiratory diseases in Sichuan Provincial People's Hospital from 2010 to 2014. MethodsThe inpatients' clinical data and costs were collected from hospital information system. The diseases were classified and the cost constitution were analyzed. The data were analyzed using SPSS 18.0 software. ResultsThe total number of the inpatients aged over 65 years was 17600 from 2010 to 2014, with more males (64.0%) than females (36.0%). The patients aged 76.9 years on average and were mainly in the 65-89 age group which accounted for 94.4%. The patients were mainly distributed in geriatric department, respiratory department and emergency department, accounting for 78.2% of the total. The top three respiratory diseases were pneumonia, chronic obstructive pulmonary disease (COPD) and pulmonary tumor accounting for 86.5% of the total with more males than females. There was an upward trend in the proportion of pulmonary tumor, bronchiectasis and pulmonary tuberculosis, and a downward trend in the proportion of asthma, COPD and pneumothorax. The average length of hospital stay decreased from 17.6 days to 16.0 days since 2010, and the average cost per capita increased from ¥20162.6 yuan to ¥30015.1 yuan since 2010. The proportion of drug cost to inpatients hospitalization cost decreased from 57.2% to 48.8% since 2010. ConclusionsPneumonia, COPD and pulmonary tumor are main respiratory diseases of inpatients over 65 years in Sichuan Provincial People's Hospital from 2010 to 2014. The male inpatients are more than female inpatients. The incidence of pulmonary tumor and pulmonary tuberculosis rises, and of asthma and COPD goes down. The incidence of lung tumor is increasing in younger age groups. The average length of hospital stay and the proportion of drug cost have declined in recent five years, while the average cost per capita has increased.

  • Serum Immunoglobulin G4 Level in 32 Patients with Active-phase Pemphigus Vulgaris

    ObjectiveTo learn the serum immunoglobulin G4 (IgG4) level in patients with pemphigus vulgaris (PV) in the active phase by detecting the IgG4 concentration in the patients with PV. MethodsWe collected the serum samples from patients with active-phase PV (including acute-onset and chronic active phases) between April and December 2014. The serum IgG4 level was detected by immuno-scatter turbidmetry, and the difference of serum IgG4 level between PV patients and normal subjects was assessed by Student's t-test. ResultsThirty-two patients with pemphigus vulgaris in the active phase were enrolled for this study, among whom 22 (including 8 males and 14 females) had normal serum IgG4 level, 8 (including 3 males and 5 females) higher and 2 (including 1 male and 1 female) lower. Compared with normal subjects, the average level of serum IgG4 in PV patients was not significantly different (P>0.05). The serum IgG4 level of acute-onset and chronic active PV patients was (1.015±0.825) g/L and (1.058±1.133) g/L, respectively, with no significant difference (P>0.05). ConclusionThe serum IgG4 level of patients with pemphigus vulgaris in the active phase can be normal, higher or lower. And there is no obvious difference in the serum IgG4 level between acute-onset and chronic active PV patients.
