  • EXPRESSION OF TRANSFORMING GROWTH FACTOR β1 AND INSULIN-LIKE GROWTH FACTORⅠIN THE REGENERATED BONES AFTER LOW FREQUENCY MICROMOVEMENT

    Objective To investigate the expression of transforminggrowth factor β1(TGF-β1) and insulin-like growth factorⅠ(IGF-Ⅰ)in new bone after low frequency micromovement. Methods Fifteen female sheep from Shandong province were involved in the study and their bilateral tibias transversely osteotomized in the middle shafts with a defect of 2 mm.The hind limbs were fixed with unilateral external fixators connected to a controlled micromovement device. Ten days after osteotomy, one hind limb of each sheep randomlywas selected to perform micromovement at an amplitude of 0.25 mm and a frequency of 1 Hertz, 30 min a day for 4 weeks ( micromovement group). The other hindlimb served as the control group. Five sheep were sacrificed at 3,4 and 6 weeks after osteotomy, respectively, and specimens were harvested for detecting the expression of TGF-β1 and IGF-Ⅰby immunohistochemistry and RT-PCR. Results Immunohistochemistry: In the third postoperative week in the micromovement group, the expression of TGF-β1 was detected in different areas of new chondrocytes at the margin of callus, mainly in proliferating area, and IGF-Ⅰexpressed in osteoblasts at the margin of endochondral ossification area, calcified and mature chondrocytes and osteocytes. There was seldom expression ofIGF-Ⅰ and little expression of TGF-β1 in the corresponding area in the control one. In the 4th postoperative week in the micromovement group, theexpression of TGF-β1 diminished gradually with the mature of new bone and be located in extracellular matrix and osteoblasts around ossified areas; The expression ofIGF-Ⅰ reached the peak and be located mainly in osteoblasts of new bone surface, maturing osteocytes and calcifing osteoid. But there was little expression of them in the control group. In the sixth postoperative week in the micromovement group, there was a little expression of IGF-Ⅰ expression but little expression of TGF-β1; there was nearly no expression of them in the control group. In the micromovement group, the absorbance values of TGF-β1 at 3 and 4 weeksand of IGF-Ⅰat 3, 4 and 6 weeks were significantlyhigher than those in control group(P<0.05). RTPCR: In the third and fourth postoperative weeks in the micromovement group, there was higher expression of mRNA of TGF-β1 and TGF-I than those in control group; in the sixth postoperative week, the expression diminished gradually, but was higher than that in control group. The absorbance values of TGF-β1 at 3 and 4 weeks and IGF-Ⅰat 3, 4 and 6weeks were significantly higher than those of control group(P<0.05). Conclusion Low frequency and controlled micromovement in the early stage of the fracture healing can promote the expression of TGF-β1 and IGF-Ⅰ.They worked together to regulate the process of the endochondral ossification, while in the late stage the differentiation of osteocytes and mineralization of osteoid were regulated mainly by IGF-Ⅰ, which played an important role in regulating the cell biological behavior during micromovement.

  • Evaluation of Acute Appendicitis in Children with Dual-Source Computed Tomography

    ObjectiveTo evaluate the manifestations and diagnostic value of pediatric acute appendicitis with dual-source CT (DSCT). MethodsRetrospectively analysis of CT features of 97cases of surgically and pathologically confirmed pediatric acute appendicitis in our hospital were performed. ResultsAmong 97 patients, 7 cases were diagnosed acute simple appendicitis, 20 cases with acute suppurative appendicitis, perforated and gangrenous appendicitis in 58 cases, and appendiceal abscess in 12 cases. According to the location of appendix confirmed by CT, 28 cases of appendicitis could not be clearly manifested, the cohort of the remaining 69 cases were composed of 20 cases (29.0%) with appendix located in pelvic, 15 cases (21.7%) with appendix in front of ileum, 11 cases (15.9%) with appendix behind ileum, 12 cases (17.4%) with appendix behind cecum, 3 cases (4.3%) with appendix below cecum, 1 case (1.5%) with appendix outside of cecum, and 7 cases (10.2%) with appendix located in other positions. CT and three-dimensional reconstruction findings were as followed:swelling enlarged appendix, appendicoliths, periappendiceal fat fuzzy, peritoneal thickening, ileocecal thickening, mesenteric lymphadenopathy, periappendiceal mass, and abdominal or pelvic fluid. The diagnostic rate of acute simple appendicitis with CT was 85.7% (6/7), acute suppurative appendicitis was 80.0% (16/20), perforated and gangrenous appendicitis was 100% (58/58), appendiceal abscess was also 100% (12/12), the overall diagnostic yield was 94.8% (92/97). ConclusionDSCT can well demonstrate the anatomical location of appendix and pathological changes of surrounding tissues, and has higher diagnostic accuracy, provide powerful information for surgeons.

  • Application of 3D Digital Lung Software in Preoperative Planning of Patients with Mul-tiple Pulmonary Nodules and Poor Pulmonary Function

    ObjectiveTo assess the feasibility of 3D digital lung software used in preoperative planning of patients with multiple pulmonary nodules and poor pulmonary function. MethodsFive patients with multiple pulmonary nodules in the left lung, meanwhile with a history of single lung lobectomy in the right lung were included in our hospital between June and December 2015. There were 4 males and 1 female at an average age of 50.4±2.6 years. A 320-slice volumetric CT scanner was used to the CT angiography (CTA) of the pulmonary artery. The data of CT images were imported into the 3D digital lung software that was researched and developed by Xiamen QiangBen Science and Technology Company. The 3D reconstruction of digital virtual lung was completed by this software based on those data. At the same time the soft-ware completed the automatic segmentation of the lung based on the pulmonary artery system and the 3D reconstruction of the pulmonary nodules. The 3D digital lung software calculated the volume proportion of the intended removal (segm-ental lesions) to the whole lung, estimated the effect of surgery on forced expired volume in one second (FEV1), and the patient's tolerance ability to surgery. After the preoperative planning, the patients received multiple pulmonary segmental/subsegmental resection under the general anesthesia by video-assisted thoracoscopic surgery (VATS). ResultsThe 3d reconstruction of the pulmonary arteries reached 5 levels in 5 patients. And the software automatically identified out the lung segment/subsegment to show the lung nodules of lung segment/subsegment. The preselection lung volume of 5 patients accounted for 14.00%-27.00% of total lung volume. The software estimated FEV1 as 1.16-1.46 L which can tolerate the operation. The 5 patients were successfully performed surgery of multiple pulmonary segmental/subsegmental resection under the general anesthesia by VATS. The software located lung nodules from the resection of pulmonary segments during operation immediately. Then we sent them to the rapid pathological examination for diagnosis. After operation, the patients recovered well, and had no respiratory insufficiency. Hospitalization day was 4 days. ConclusionThe 3D digital pulmonary software can not only automatically identify the pulmonary segments, precisely position the pulmonary nodule, show the relationship among the target pulmonary segments artery, vein, bronchus and the surroun-ding artery, vein, and bronchus, but also calculate the volume of the pulmonary segments, estimate the impact of the pulmonary segmentectomy on the FEV1. It is useful for precise evaluation of the tolerant capacity of multiple pulmonary nodules in patients with unstaged multiple pulmonary segments.
