  • Surgical treatment of massive subretinal hemorrhage and vitreous hemorrhage associated with age-related macular degeneration

    Objective To investigate the method and the effects of the surgical treatment of massive subretinal hemorrhage and vitreous hemorrhage associated with age-related macular degeneration. Methods A retrospective study of 14 consecutive patients underwent a complete pars plana vitrectomy. Retinotomy was carried out for removing subretinal hemorrhage by using balanced salt solution. Complete air-fluid exchange and gas or silicone oil tamponade were performed in all patients. The follow-up period was within 3~7 months. Results Atrophy of eyeball in 2 eyes (14.3%) postoperatively. Improvement of corrected final visual acuity and anatomical retinal reattachment were achieved in 12 (85.7%) of the 14 eyes postoperatively. Seven days after operation, muddy-sand-hemorrhage in anterior chamber occurred in 4(28.6%)of the eyes and paracenteses of anterior chamber were performed for these eyes. Conclusion Surgical intervention should be applied in the eyes with the massive subretinal hemorrhage associated with age-related macular degeneration in order to avoid the affected eyes becoming atrophic due to the subsequent complication of vitreous hemorrhage, anterior chamber muddy sand hemorrhage,ghost cell-glaucoma or retinal detachment.   • Vitreous surgery in conjunction with laser for high myopic retinal detachment with macular hole

    Purpose To investigate the therapeutic effect and complications of vitreous surgery in conjunction with laser treatment for high myopic retinal detachment with macular hole. Methods Thirtyfive consecutive cases(38 eyes) of high myopic retinal detachment with macular hole in our institute from January 1994 to August 1997 were analysed retrospectively.Among them,11 cases(12 eyes)were treated with vitreous operation and 24 cases (26 eyes)with vitreous operation combined with argon laser photocoagulation of the macular holes.All of the eyes operated on were followed up for more than 6 months,and with an average period of 21.7 months. Results In the 12 nonlaser treated eyes the macular hole recured in 5 eyes (41.7%),and the visual acuity was 20/200 or more in 6 eye(50.0%).In the 26 laser treated eyes,the macular hole recurred in 2 eyes(7.7%),new retinal tear appearred in 1 eye, and the visual acuity was 20/200 or more in 13 eyes(50.0%).The probability of recurrence of the macular hole between nonlaser and laser treated eyes was 0.024,and of retinal probability redetachment between them was 0.0487.There was no significant difference in between pre and postoperative visaul acuity. Conclusion Vitreous surgery in conjunction with laser photocoagulation for high myopic retinal detachment with macular hole might be helpful in improving the operative successful rate. (Chin J Ocul Fundus Dis,1998,14:199-201)

  • 视网膜切开与切除手术临床应用初步报告

    Absract PURPOSE:TO inquire into usage of retinotmy and retinecomy in complicaed retinal detachment and subretinal operation. METHODS:To analysing retrospectively 28 cases of compicated retinal detachment and subretinal hemrrhage and novascular membrane receiving retinotomy and retinectomy from Jan.1994 to Apr.1995. RESULTS:Ten from 11cases receiving internal dranage,datached retinas reattached.Seven out of 8 subretinal membranes removed by retinotomy were successful,and 7out of 9 cases of complicated retinal detachment were releived by retinectomy of the stiff margins of retinal holes. CONCLUSIONS:Retinotomy and retinectmy were beneficial to improving th curative effect of complicated retinal detachment and subretinal opereation. (Chin J Ocul Fundus Dis,1996,12:3-6)

  • EVALUATION OF VITREOUS SURGERY FOR PROLIFERATIVE VITREORETINOPATHY GRADE D3

    In this paper,the results of vitreous surgery in 25 cases of PVR D3 were analyzed.Pars plana vitrectomy and membrane peeling were performed in all eyes.The follow-up is more than 3 months.The anatomic successful rate was 68%.The authors suggest that vitrectomy combined with membrane peeling and intraocular tamponade was key for the treatment of PVR D3.Silicone oil tamponade may improve the patients'prognosis.The authors suggest that the etiologic factors may include the operative irritation,giant tear,trauma and vitreous hemorrhage.The time and the complications of the vitreous surgery were discussed.   • THE CHARACTERISITICS OF PLASMA-SPRAYED COATINGS OF HYDROXY APATITE IN VITRO

    In order to investigate the stability of Hydroxyapatite (HA) coated material, the plasma-sprayed coatings of HA were divided into four groups: 1. Keeping in water vapour at 125 degrees C, with a pressure of 0.15 MPa for 6 hr; 2. Heating at 650 degrees C in air for half an hr; 3. Keeping in water vapour at 490 degrees C, with a pressure of 0.01 MPa for 2 hr; and 4. The control. The XRD, FTIR analysis and the dissolution test were carried out. The results showed: 1. The degree of crystalization in XRD analysis was 3. gt; 1. gt; 2. gt; 4.; 2. The ampitude of OH- peak in FTIR analysis was 3. gt; 1. gt; 2. gt; 4. and 3. The dissolution rate in tris-HCl buffer was 3. lt; 1. lt; 2. lt; 4. The conclusions were 1. The treating with water vapour could decrease the transformation temperature which was needed to convert the amorphous phase into cystalline phase; 2. Water vapour treatment could accelerate the transformation of Z-TCP, TCPM into crystalline HA; 3. Water vapour treatment could promote the structural integrity of plasma--sprayed coated of and HA and 4. Water vapour treatment could lower the dissolution rate of HA coated in Tris-HCl buffer.
