  • Application of Painless Pediatric Bronchoscopy in Treatment of Pulmonary Complications of Postoperative Patients with Congenital Heart Diseases

    Abstract: Objective To evaluate clinical outcomes of painless flexible fiberoptic bronchoscopy in the treatment for pulmonary complications in postoperative pediatric patients with congenital heart diseases. Methods We retrospectively analyzed clinical data of 58 patients who received fiberoptic bronchoscopy for pulmonary complications after surgical repair for atrial septal defect, ventricular septal defect, tetralogy of Fallot, double outlet right ventricle or transposition of the great arteries in First Affiliated Hospital of Harbin Medical University From August 2009 to February 2012. There were 26 male patients and 32 female patients with their age ranging from 20 days to 2 years. Olympus xp-60 fiberoptic bronchoscopy was used for removal of airway secretions and bronchial alveolar lavage under anesthesia with propofol, fentanyl and lidocaine. Pulse oximetry, respiratory sound and chest X-ray changes were observed. Results The examination time of painless fiberoptic bronchoscopy was 10-25 minutes in the 58 patients. After removal of airway secretions by fiberoptic bronchoscopy, their respiratory status improved significantly, pulse oxygen saturation increased by 5%-12%, and their pulmonary crackles were significantly reduced. In 29 patients with atelectasis, their pulmonary lobes demonstrated significant reexpansion in chest X-ray reviews on the next day after fiberoptic bronchoscopy, and 8 patients received another fiberoptic bronchoscopy treatment on the next day to achieve complete pulmonary reexpansion. One patient with severe pulmonary hypertension (PH) stayed in intensive care unit (ICU) for 6 days, 3 patients with transposition of the great arteries stayed in ICU for 5 days, 3 patients with double outlet of right ventricle and moderately high PH stayed in ICU for 4 days, and all the other patients were discharged from ICU within 48 hours after admission. There was no severe complication related to fiberoptic bronchoscopy, except 2 patients with minor nasal mucosa bleeding who were cured with hemostatic drugs and local compression. Conclusion Painless flexible fiberoptic bronchoscopy can significantly enhance clinical outcomes and shorten ICU stay for postoperative pediatric patients with congenital heart diseases.

  • Rational and Design of Chinese Hypertension Intervention Efficacy Study (CHIEF): A Multi-center, Randomized Controlled Trial of a Combination of Antihypertensive, Lipid Modification and Lifestyle Interventions in Hypertensive Patients (protocol)

    Background and Aim Morbidity due to hypertension morbidity is increasing in the Chinese population, and the blood pressure control rate is low. The aim of this trial is to optimize the hypertension treatment plan, promote blood pressure goals and reduce cardio-cerebrovascular events. Objective and Methods Patients are eligible for inclusion if they are diagnosed with essential hypertension, aged 50-79 years with at least one cardiovascular risk factor and sign the consent forms. This project is a multi-centre, randomized, controlled and blind-endpoint trial. 12 000 patients will be randomly assigned to low-dose Amlodipine and Telmisartan or Amlodipine and diuretics. Patients whose blood cholesterol is between 4.0-6.1mmol/L will also be randomized into small dose of statin-based regimen or standard management regimen; and patients will be randomly assigned to intensive lifestyle intervention group or standard intervention group according to the community area where they live. The primary study outcome is a composite of non-fatal stroke/myocardial infarction and cardiovascular death. Patients will be followed-up for 4 years. Expected Results The cumulative primary outcome is estimated to be 500 cases, and the hypertension control rate is expected to reach 70%. Cardiovascular events are expected to be reduced by 20% in the optimized treatment group and poor lifestyle will be markedly improved. A total of 150 clinical centres in China will join in the study, which will be completed at the end of 2011.

  • Comparison of Graft Patency between On-pump and Off-pump Coronary Artery Bypass Grafting

    Objective To compare the graft patency rates of conventional on-pump coronary artery bypass grafting (CCABG) and off-pump coronary artery bypass grafting (OPCAB) at 2 weeks,3 months and 1 year after surgery. Methods Clinical data of 200 patients who underwent coronary artery bypass grafting (CABG) in the First Affiliated Hospital of Harbin Medical University from May 2010 to November 2011 were retrospectively analyzed. All the 200 patients were divided into two groups according to different surgical procedures they received:CCABG group,61 patients including 32 male patients and 29 female patients with their age of 59.8±4.7 years;and OPCAB group,139 patients including 72 male patients and 67 female patients with their age of 59.6±8.9 years. Graft patency of all the patients was evaluated by 256-slice multislice computed tomography angiography (256-MSCTA) at 2 weeks,3 months and 1 year after CABG and compared between two groups. Results At 2 weeks,3 months and 1 year after CABG,the graft patency rates of left internal mammary artery (LIMA) grafting were not statistically different between CCABG group and OPCAB group,as well as those of great saphenous vein (GSV) grafting (P>0.05). The 1 year graft patency rates of LIMA grafting of CCABG group and OPCAB group were 92.31% and 91.94% respectively,and 1 year graft patency rates of GSV grafting of CCABG group and OPCAB group were 91.35% and 90.00% respectively. To compare the graft patency rates of different distal anastomotic locations,grafting to the left anterior descending (LAD) coronary artery had significantly higher patency rate than grafting to the right coronary artery (RCA,P<0.05). The 1 year graft patency rates of LAD grafting and RCA grafting were 97.78% and 85.90% respectively. But there was no statistical difference in graft patency rates at different respective distal anastomotic locations between OPCAB group and CCABG group (P>0.05). To compare early clinical outcomes of two groups,postoperative mechanical ventilation time,length of ICU stay,24-hour mediastinal drainage,24-hour blood transfusion,length of hospital stay,and hospitalization cost of OPCAB group were significantly shorter or lower than those of CCABG group (P<0.05). Conclusion Short-term graft patency rates of CCABG and OPCAB are quite similar. Patency rate of grafting to LAD is higher than that of grafting to RCA. OPCAB can produce better perioperative clinical outcomes than CCABG.
