  • Clinical Analysis of Peutz-Jeghers Syndrome and Its Canceration

    Objective To analyse the clinical features of Peutz-Jeghers syndrome and its canceration, and to summarize the management and the follow-up strategy for patients with Peutz-Jeghers syndrome. Methods Clinical data of 30 patients with Peutz-Jeghers syndrome dating from October 1985 to September 2005 in West China Hospital of Sichuan University were analyzed retrospectively. Results Fourteen (46.67%) definite family histories of Peutz-Jeghers syndrome were found. Pigmentation of skin and mucosa, abdominalgia and hematochezia were major clinical manifestations of the syndrome. There were 18 patients (60.00%) complicated with intussusception and acute intestinal obstructions, 16 patients (53.33%) with gastrointestinal bleeding, and 6 patients (20.00%) developed malignancy. The frequent types of malignancy were carcinoma of small intestine (3 cases), colon carcinoma (2 cases) and gastric carcinoma (1 case) in order. The mean age of the canceration-diagnosed patients was 32 years old. The type of pathohistology of all the malignancy was poorly differentiated mucus adenocarcinoma. High-frequency endoscopic electroresection, orthdox polypectomy and enterectomy were the major means of treatment. Conclusion Patients with Peutz-Jeghers syndrome are at high risk of canceration at relatively early ages and usually the differentiation of the tumor is poor. Endoscopy should be performed regularly and the high-frequency electroresection is an effective therapy in disposing intestinal polyp. Screening can also improve the efficacy of Peutz-Jeghers syndrome.

  • CONGENITAL CYSTIC DISESE OF THE LIVER (REPORT OF 30 CASES)

    The authors analysed the medical records of 30 patients with congenital cystic disease of the liver treated in this hospital and with a review of the article some problems of diagnosis and treatment are discussed. B-altrasonic scaner (B-US), computerized tomographic scanning and magnetic resonance imaging appeared to be most helpful in diagnosing and treating this disease. After comparing different treatments, such as aspiration .alcohol sclerotherapy, fenetration, cyst resection and partial hepatectomy, the arthors state the best results could be achieved by alcohol sclerotherapy under B-US guidence (6 cases) or fenestration (15 cases) with no postoperative complication. Malignant change was found in one patient of this group.

  • Does Diabetes and Long-acting Insulin Glargine Increase the Risk of Malignancies: An Evidence-based Treatment for a Diabetic Patient Accompanied with Pancreatic Cancer

    Objective Through studying a diabetic patient accompanied with pancreatic cancer by means of evidence-based clinical practice, to find out the relationship between diabetes mellitus and cancer and whether the long-acting insulin glargine increases the risk of cancer or not, which is regarded as a disputable hot issue at present. Methods Such databases as The Cochrane Library (Issue 3, 2010), OVID-EBM Reviews (1991 to Sept. 2010), MEDLINE (1950 to Sept. 2010) and CNKI (2000 to Sept. 2010) were retrieved to collect high quality clinical evidence, and the best therapy was formulated in accordance with the willingness of patients themselves. Results Eight randomized controlled trials (RCTs), four meta-analyses and one RCT meta-analysis were included. The evidence indicated that: a) Diabetes mellitus was kind of related to the occurrence of malignancies; b) There was no evidence at present showing the relationship between long-acting insulin glargine and cancer; c) Strictly controlling of blood sugar did not increase the risk of tumorigenesis, but hyperglycemia causing cancer was proofless; and d) Whether the diabetic patient with cancer should stop taking long-acting insulin glargine or not should require suggestions from specialists rather than patients themselves. Conclusion No evidence at present shows that tumorigenesis is related to diabetes mellitus, long-acting insulin glargine and strict controlling of blood sugar. It is necessary to require more evidence to decide whether the therapy should be adjusted or not for the diabetic patient with cancer who is in the process of glargine therapy.

  • Etiology of pleural effusions

    Objective To investigate the etiology of pleural effusions. Methods All adult patients with pleural effusions of unknown etiology admitted to this hospital between January 2011 and December 2013 were investigated. The etiological data of these patients with pleural effusion were retrospectively reviewed. Results During the 3-year period, 1 541 patients eventually were included in this study. The most frequent cause of pleural effusions was bacterial infection (38.7%), followed by malignancy (23.7%), congestive heart failure (13.1%), and tuberculosis (10.7%). The etiology of 120 patients (7.8%) remained uncertain. Conclusions The most frequent cause of pleural effusion is bacterial infection, followed by malignancy, cardiac failure, and tuberculosis. These four etiologies account for over 85 percent of all pleural effusions.
