To introduce a way to correct mildly and moderately inverted ni pple. Methods From May 2004 to January 2007, 16 patients (aged 18-38 years) with original bilateral inverted nipple were corrected with the double stitching of U-shape. Sixteen cases included 11 mildly inverted nipples and 5 moderately inverted nipples. Six of them received massage and vacuum aspiration, and the results were not satisfactory. After the pull ing out inverted ni pples, four microincisions about 0.1 cm on the border of areola and the basilar part of nipple were designed in each horizontal and perpendicular directions, then double stitching was performed to increase the supporting and sustaining tissue under the nipple and close the way nipple invert. Results Most cases exhibited excellent aesthetic projection and incision healed by first intention. Followup examinations were performed at 6 months to 2 years and revealed no evidence of recurring inversion, ni pple tilting or lactation disturbance. No surgical compl ications such as infection, nipple and areola of breast necrosis, permanent numbness. The scars in the local site were l imited and not conspicuous. Conclusion Double stitching of U-shape is easy and simple with few compl ications. It is an ideal method to correct mild and moderate inverted nipple.
Objective To discuss the mechanisms and clinical effect of musculus extensor hallucis longus shifting in correcting hallux valgus (HV) deformity. Methods From April 2004 to December 2006,25 cases of HV (38 feet) were treated by musculus extensor hallucis longus shifing. There were 2 men and 23 women, aging from 22-60 years (mean 46.3 years).HV angle was 21.45° (mean 31.30°), intermetatarsal(IM) angle was 7-21° (mean 12.52°). The HV were corrected by cutting osteophyma of the first metatarsal bone, cutting transverse head of adductor pollicis, transferring musculus extensor hallucis longus and reconstructing its insertion. Results The patients were followed up 6-14 months after operation. HV angle and IM angle were 7.30°±2.62° and 6.50°±2.46° respectively, showing significantdifferences when compared with before operation (Plt;0.05). According to the American Orthopaedic Foot amp; Ankle Society (AOFAS) score system, the foot function was excellent in 25 feet, good in 7 feet and poor in 6 feet,and the excellent and good rate was 84.2%. Hallux varus occurred in 2 feet after 2 months of operation, metatarsophalangeal joint limitation of motion in 2feet after 3 months of operation, no HV recurred. ConclusionThe HV deforemity can be corrected by shifting the musculus extensor hallucis longus and reconstructing its insertion. It makes stress of metatarsophalangeal joint balance and prevent recurrance of HV deformity.

Results Drugs and cells modified by genecould be encapsulated in different materials and be implanted in vivo avoiding a host immune system rejection. It act as a continuous source of desired medicine for enhancement of bone healing, the treatment of bone tumor and bone infection, and the regeneration of bone and cartilage. Conclusion Microencapsulation can be used asa carrier for drugs and cells modified gene to treat related disease in orthopedics.
Day surgery is an important diagnosis and treatment modality to promote hierarchical diagnosis and treatment and form a “surgery in hospital, recovery in community” diagnosis and treatment mode. At the same time, the promotion and implementation of hierarchical diagnosis and treatment provides institutional support and promotion for the perfection of day surgery management. Based on the experience from the Day Surgery Centre of General Hospital of Eastern Theater Command, this paper investigates the role of “gate-keepers” and recovery guidance in community and secondary hospitals in the perfection of day surgery in tertiary hospitals. Taking orthopedic day surgery as an example, which accounts for the largest proportion in day surgery in General Hospital of Eastern Theater Command, this paper aims to explore a feasible model of hierarchical diagnosis and treatment of day surgery, which specifies the responsibilities and works of hospitals at all levels.
Objective To compare the clinical effects of traditional manual and robot-assisted implantation of cannulated screws in the treatment of femoral neck fracture. Methods The medical records of patients with femoral neck fracture in Department of Orthopaedics, People’s Hospital of Deyang City were analyzed retrospectively. The patients were divided into two groups, including the traditional manual implantation group from January to December 2018 and the robot-assisted implantation group from May 2019 to May 2020. The clinical therapeutic efficacy of the two groups were compared. Harris hip function score was used to evaluate hip function. Results A total of 85 patients were included. All patients had closed fractures. There were 45 cases in the traditional manual implantation group and 40 cases in the robot-assisted implantation group. There was no significant difference between the two groups in preoperative waiting time, operation time, hospitalization time, fracture healing time, complications within one year after operation, or Harris hip function score one year after operation (P>0.05). The placement nail time [(11.1±2.0) vs. (23.8±2.3) min; t=27.142, P<0.001], frequency of guide pin insertion [(4.7±1.2) vs. (11.4±1.7) times; t=20.640, P<0.001], frequency of intraoperative fluoroscopy [(10.8±1.7) vs. (21.0±1.8) times; t=26.990, P<0.001] and intraoperative blood loss [(8.1±2.0) vs. (12.0±1.7) mL; t=9.711, P<0.001] in the robot-assisted implantation group were less than those in the traditional manual implantation group. No wound infection or neurovascular injury was found in the two groups. Conclusion Robot-assisted implantation of cannulated screws in the treatment of femoral neck fracture has the advantages of less fluoroscopy, fewer guide pin insertion, less blood loss, more accurate screw placement than the traditional manual implantation of cannulated screws.
ObjectiveBased on the clinical data of patients with foot and ankle deformities in the QIN Sihe Orthopaedic Surgery Database, to analyze the characteristics and treatment strategies of foot and ankle deformities, and provide a basis for clinical decision-making. Methods A total of 22 062 patients with foot and ankle deformities who received orthopedic surgery between May 25, 1978 and December 31, 2020 were searched in the QIN Sihe Orthopedic Surgery Database. The gender, age at operation, regional distribution, etiology, type of deformity, operation method, postoperative fixation method, and other information were collected. Results Among the 22 062 patients, there were 13 046 males (59.13%) and 9 016 females (40.87%); the age at operation ranged from 1 to 77 years, with a median of 17 years, and 20 026 cases (90.77%) were aged 5 to 40 years. The patients came from 32 provinces, municipalities, and autonomous regions across the China and 5 countries including India and the United States, et al. The etiology and diseases type covered 154 kinds (of which sequelae of poliomyelitis, cerebral palsy, spina bifida and tethered spinal cord, congenital equinovarus foot, post-traumatic foot and ankle deformity, and Charcot-Marie-Tooth disease accounted for the highest proportion). The types of deformities included varus foot, equinus foot, valgus foot, talipes calcaneus, equinocavus, high arched foot, claw toe, and flail foot. Surgical methods included tendon lengthening, soft tissue release, tendon transposition, osteotomy orthopedics, and ankle arthrodesis. The 36 620 operations were performed, including 11 561 cases of hip, knee, and lower leg operations to correct the foot and ankle deformities. Postoperative fixation methods included Ilizarov external fixator in 2 709 cases (12.28%), combined external fixator in 3 966 cases (17.98%), and plaster or brace fixation in 15 387 cases (69.74%). ConclusionMale patients with foot and ankle deformities account for a large proportion, and the population distribution is mainly adolescents, with a wide distribution of regions, causes and diseases, and talipes equinovarus and varus foot are the main types of deformities. Foot and ankle deformities are often combined with deformities of other parts of the lower limb, which requires a holistic treatment concept. The application of foot soft tissue and bone surgery combined with Ilizarov external fixator and combined external fixators provides a guarantee for the correction of complex foot and ankle deformities.
