Objective To explore the significance of parathyroid micro vascular anatomy in thyroid lobectomy with capsular technique. Methods The pertinent literatures in recent thirty years were screened with key words “parathyroid micro vascular anatomy, capsular technique, and protection”and reviewed. Results There were many types of number, origin, and length of parathyroid vascular, and specific measurements should be taken in thyroid lobectomy with capsular technique. Conclusion Fully awareness of parathyroid micro vascular anatomy will benefit to ensure preservation of their function during thyroid lobectomy with capsular technique.
With the implementation of “Good Clinical Practice”, the quality of clinical trials in China has increased constantly and more importance has been attached to the protection of the subject. Large scale clinical trials are primarily featured by long-term intervention, large sample size, many participant organizations, different levels of investigators, long test periods, and lots of adverse events. Consequently, the protection of subject is full of extensive complexities and difficulties and currently there is little experience to refer to. Hence, the article introduces the subject’s protection method adopted in the National Program Subject of Secondary Prevention Clinical Trial about Effect of Qi Shen Tonifying Qi on Myocardial Infarction (MISPS-TCM).
Objective To report the progression of breviscapine’s protective effect to hepatic ischemia-reperfusion injury. Methods Pertinent literatures and journal articles published in recent years were reviewed, and the progression of breviscapine protecting hepatic ischemia-reperfusion injury in the experimental and clinical research were analyzed and summarized. Results The role of breviscapine is considerable extensive. It can protect hepatic ischemia-reperfusion injury by anti-oxyradical and anti-lipid peroxidation, inhibiting mitochondrial damage, intracellular calcium overload, intra-thromboxane and apoptosis, improving microcirculation, and so on. Conclusion Breviscapine plays a protective role in hepatic ischemia-reperfusion injury, and it will be of great value to application and research.
ObjectiveTo discuss whether central lymph node dissection (CLND) should be performed for papillary thyroid cancer (PTC) patients. MethodsThe related domestic and foreign literatures were retrieved, the necessity of CLND and the risk of recurrent laryngeal nerve (RLN) injury in CLND were reviewed, and the application value of intraoperative nerve monitoring (IONM) in CLND were analyzed. Results① CLND can reduce the recurrence rate of PTC, improve postoperative survival rate, ease the difficulty of reoperation, and help to clarify tumor stage. ② CLND can increase the risk of RLN injury. ③ Application of INOM can decrease the risk of RLN injury. ConclusionsThe application of IONM during CLND effectively decrease the risk of RLN injury for surgeons, especially low seniority surgeons, and improve the survival quality and the prognosis. This combination will promote the implementation of routine CLND therapeutic strategy in thyroid cancer patients.
ObjectiveTo explore the feasibility to protect great auricular nerve during surgery for first branchial cleft anomaly in children.MethodsThe data of 17 cases of first branchial cleft cyst, sinus or fistula admitted to West China Hospital of Sichuan University from September 2005 to May 2015 were reviewed. The extent of lesion was demonstrated by enhanced CT before operation. Later surgery further proved that the middle ear and mastoid process was not involved in any of the cases. During surgery, some branches of great auricular nerve were dissected and spared. The cyst, sinus or fistula was obliterated completely while the whole or part of the parotid gland was preserved. The patients were followed-up from half a year to three years with mean time of 2 years and 1 month.ResultsNo recurrence or facial paralysis were found in all patients after surgery. Twelve patients had no paresthesia in the area innervated by great auricular nerve, five patients experienced numbness of ear lobe or inferior 1/3 of the auricle.ConclusionIt is feasible to protect some branches of great auricular nerve during the operation for first branchial cleft anomaly, which does not hinder the thoroughness of the resection for the lesion, and helps to improve the children’s quality of life.
