  • Construction of an Anticoagulation Therapy Database of Chinese Patients after Heart Valve Replacement

Abstract: Objective To construct an Anticoagulation Therapy Database of Chinese Patients after Heart Valve Replacement in accordance with blood coagulation characteristics of Chinese patients, fill the gap of Chinese clinical research in valvular heart diseases, and provide a scientific and objective basic data and information exchange platform. Methods A national multicentre, prospective and cohort clinical research method was applied to establish an anticoagulation therapy database of Chinese patients after heart valve replacement, using the Internet as a platform. A case report form (CRF), which was in line with the actual situation of Chinese anticoagulation patients after heart valve surgery, was formulated through the discussion of experts from 36 cardiovascular surgery centers in China in the starting meeting of National Science amp; Technology Support Program during the Twelfth Five-year Plan Period. We planned to prospectively include patients receiving warfarin anticoagulation therapy and formal anticoagulation monitoring after heart valve replacement from January 1, 2011 to December 31, 2014. Database was constructed using warehousing technology, which allowed not only data monitoring, query and statistics, but also regular data backup and system updates. Results A network database entitled Anticoagulant Therapy Database of Chinese Patients after Heart Valve Replacement was constructed and linked with the homepage of Chinese Journal of Clinical Thoracic and Cardiovascular Surgery (http://www. zgxxwkzz. com), which constituted a national Internet information platform. From 1 January 2011 to 1 December 2012, 8 452 anticoagulation patients after heart valve replacement from 34 level-3A hospitals in China had been registered in the database. Further follow-up of these patients was being carried out in respective hospitals. Conclusion A large multi-center and open database and network information platform has been constructed. The database variables are in line with clinical characteristics of Chinese anticoagulation patients after heart valve replacement, which provide scientific and objective basic data and support for future clinical research and systemic analysis.

  • Epidemiological Characteristics of Malignant Tumor Deaths in the Registered Population of Zhuhai City, from 2004 to 2005

Objective To investigate the epidemiological characteristics of malignant tumor deaths in the registered population of Zhuhai city. Methods A retrospective survey of death from malignant tumor during 2004 and 2005 was performed in the registered population of Zhuhai city. This was based on the retrospective protocol of the third national survey of death cause. The mortality of main malignant tumor and cause of deaths were analyzed. Results During 2004 to 2005, the mean crude mortality and age-specific standardized mortality of malignant tumor were 90.3/100 000 and 97.2/100 000, respectively. Malignant tumor death was the most common cause of death in Zhuhai city, accounting for 21.8% of all deaths. The standardized mortalities in males and females were 119.7/100,000 and 71.6/100,000, respectively. The standardized rate in the male population was significantly higher than that of the female population (u=10.3, Plt;0.01). The mortality of malignant tumor tended to increase with age. The top five causes of malignant tumor deaths were lung cancer, liver cancer, colorectal cancer, nasopharyngeal cancer and gastric cancer. Their standardized mortalities were 20.9, 18.0, 8.7, 8.2, 8.1/100,000, respectively. Conclusion The mortality of mal ignant tumors has showed an increasing trend in Zhuhai city, with the main tumors being lung cancer, liver cancer, colorectal cancer, nasopharyngeal cancer and gastric cancer. Malignant tumor has become a common and frequently-occurring disease with a major impact on the life and health of people in Zhuhai city.

  • Cooperation and basic characteristics of systematic reviews/meta-analyses on diagnostic test registered in PROSPERO platform

ObjectivesTo analyze the cooperation and basic characteristics on systematic reviews/meta-analyses (SRs/MAs) in diagnostic test registered in PROSPERO platform, in order to provide reference for registering and preparing the SRs/MAs on diagnostic test.MethodsWe searched PROSPERO platform from inception to November 12th, 2017, to identify SRs/MAs on diagnostic test. Two reviewers independently screened records and extracted data by the inclusion and exclusion criteria. The forest figure was prepared by STATA 13.0 software. The figure of network of authors, organizations and countries were prepared by NetDraw software.ResultsA total of 240 SRs/MAs in diagnostic test were included. There was a serious imbalance in the quantity of the distribution of authors (1 050), organizations (360) and countries (34). These SRs/MAs in diagnostic test were related to 21 systematic diseases, with cancer ranking the first (52, 22%). Tissue biopsy was the most used gold standard, and imaging diagnosis was the most used diagnostic test. The most commonly retrieved English databases were PubMed, EMbase The Cochrane Library, while the Chinese databases were CNKI, WanFang Data and CBM. The search strategy were reported in 105 (43.8%) SRs/MAs on diagnostic test. The QUADAS-2 was used to assess the quality of included studies in 159 (66.3%) SRs/MAs in diagnostic test, and 105 (43.8%) SRs/MAs were supported by funding.ConclusionsThe absolute quantity of SRs/MAs in diagnostic test is still minimal. There are relationships among different authors, organizations and countries. The researcher should focus on the effective sizes and statistical methods in future.
