  • Correlation between Immunohistochemistry and Pathology for Lung Cancer Lymphatic Metastasis

    Objective To analyze and screen the risk factors of both immunohistochemistry and pathology for lung cancer lymphatic metastasis, and to build a mathematical model for preliminary evaluation. Methods By conducting retrospective studies, the information of lung cancer patients in the General Hospital of Air Force from 2009 to 2011 were collected. Both single and multiple unconditional logistic regression analyses were applied to screen total 27 possible factors for lymphatic metastasis. After the factors with statistical significance were selected, the relevant mathematical model was built and then evaluated by means of receiver operating characteristic (ROC) analysis. Results A total of 216 patients were included. The single analyses on 27 possible factors showed significant differences in the following 10 factors: pathological grade (P=0.00), age (P=0.00), tumor types (P=0.01), nm23 (P=0.00), GSTII (P=0.01), TTF1 (P=0.01), MRP (P=0.01), CK14 (P=0.02), CD56 (P=0.02), and EGFR (P=0.03). The multiple factors unconditional logistic regression analyses on those 10 risk factors screened 4 relevant factors as follows: pathological grade (OR=2.34), age (OR=1.02), nm23 (OR=1.66), and EGFR (OR=1.47). Then a mathematical diagnostic model was established based on those 4 identified risk factors, and the result of ROC analysis showed it could improve the diagnostic sensitivity and specificity compared with the single factor mathematical diagnostic model. Conclusion Pathological grade, age, nm23, and EGFR are related with lung cancer lymphatic metastasis, and all of them are the risk factors which have higher adjuvant diagnostic value for lung cancer lymphatic metastasis.

  • Analysis of Women’s Quality of Life and Risk Factors Related to Diminished Ovarian Reserve

    Objective To explore the status of women’s quality of life and analyze risk factors related diminished ovarian reserve (DOR). Methods We applied the method of convenient sampling to investigate 61 patients with DOR (DOR group) and 60 women with normal ovarian reserve function (control group) who visited a reproductive centre of a Triple-A hospital from February to May 2013, using a questionnaire which included basic information table, Menopausal Quality of Life Scale (MENQOL) and SF-36 Health Survey for quality of life, and Kupperman Index Scale (KI) for the degree of clinical symptoms. Then, statistical analysis was performed using SPSS 17.0 software. Results Compared with the control group (43.60±16.82), the level of women’s quality of life in the DOR group (73.42±24.15) was significantly lower (Plt;0.01), while the degree of clinical symptoms was significantly higher (Plt;0.01). There were positive associations between quality of life and degree of clinical symptoms. The risk factors related to DOR were various such as age, weight, quality of sleep, menstrual disorders, number of abortion, age of the first pregnancy, gynecologic surgery, economic income, education, family relationship, unsatisfactory sexual life, and psychological factors. Conclusion In order to improve women’s quality of life, effective treatment should be conducted to relieve clinical symptoms of women with DOR. Good life style, harmonious family relationship, healthy mind and avoiding the risk factors, which could effectively help the prevention and treatment of the disease.

  • Risk Factors of Secondary Tricuspid Regurgitation: A Meta-Analysis

    Objective To analyze and explore the risk factors of secondary tricuspid regurgitation (TR) after left-sided valve surgery (left cardiac valve replacement or valvuloplasty) using meta-analysis, so as to provide evidence for clinical diagnosis and treatment of secondary TR. Methods We electronically searched databases including PubMed, MEDLINE, CBM, CNKI, VIP, for literature on the risk factors of secondary TR after left-sided valve surgery from 1995 to 2012. According to the inclusion and exclusion criteria, we screened literature, extracted data, and assessed methodological quality. Then, meta-analysis was performed using RevMan 5.0 software. Results A total of 6 case-control studies were included, involving 437 patients and 2 102 controls. The results of meta-analysis showed that, the risk factors of progressive exacerbation of secondary TR after left-sided valve surgery included preoperative atrial fibrillation (OR=3.90, 95%CI 3.00 to 5.07; adjusted OR=3.04, 95%CI 2.21 to 4.16), age (MD=5.36, 95%CI 3.49 to 7.23), huge left atrium (OR=5.17, 95%CI 3.12 to 8.57; adjusted OR=1.91, 95%CI 1.49 to 2.44) or left atrium diameter (MD=4.85, 95%CI 3.18 to 6.53), degradation of left heart function (OR=2.97, 95%CI 1.73 to 5.08), rheumatic pathological change (OR=3.06, 95%CI 1.66 to 4.68), preoperative TR no less than 2+ (OR=3.52, 95%CI 1.26 to 9.89), and mitral valve replacement (MVR) (OR=2.35, 95%CI 1.68 to 3.30). Sex (OR=1.54, 95%CI 0.94 to 2.52) and preoperative pulmonary arterial hypertension (OR=1.28, 95%CI 0.77 to 2.12) were not associated with secondary TR after left-sided valve surgery. Conclusion The risk factors of progressive exacerbation of secondary TR after left-sided valve surgery include preoperative atrial fibrillation, age, huge left atrium or left atrium diameter, degradation of left heart function, rheumatic pathological change, preoperative TR no less than 2+, and MVR. Understanding these risk factors helps us to improve the long-time effectiveness of preventing and treating TR after left-sided valve surgery.

  • Risk Factors on Lung Cancer: A Meta-Analysis

    Objective To study the risk factors of lung cancer and provide scientific evidence for preventing and managing such disease. Methods?The database of MEDLINE, CNKI, and CBM were searched and literature domestically and internationally from January 1997 to January 2007 was collected. The RevMan 4.2 software was used for meta-analysis. Results A total of 40 studies involving 16 559 cases and 25 119 controls were included. The pooled OR values and population attributable risk percentage (PARP) for smoking, female passive smoking from husband, female passive smoking from colleague, chronic bronchitis, emphysema, pulmonary tuberculosis, family history of cancer, and family history of lung cancer were 5.75 (69.16%), 1.32 (14.52%), 1.21 (5.87%), 1.68 (7.45%), 2.70 (10.18%), 1.58 (1.91%), 1.24 (8.92%), and 1.59 (5.33%), respectively. Conclusion Risk factors related to the incidence of lung cancer are smoking, female passive smoking from husband and colleague, chronic bronchitis, emphysema, pulmonary tuberculosis, family history of cancer, family history of lung cancer and so on. Besides, the results of PARP indicate that smoking is the most important factor, followed by female passive smoking from husband, emphysema, family history of cancer sequentially, which suggest that environmental and genetic factors play important roles in the development of lung cancer.

  • Risk Factors for Hypertension in China during Different Times: A Meta-analysis

    Objective To find out what risk factors are related to hypertension in China, and whether there are differences in studies from different years. Methods We retrieved all of the hypertension-related case-control studies which were published in CNKI and PubMed since 1990 and used meta-analysis to calculate the pooled OR and 95%CI. Then all the studies were divided into two groups according to whether they were published before 2000 or after, and results of different group were compared. Results Fourteen studies were included in which the relationship was assessed between hypertension and risk factors such as drinking, smoking, being overweight, and other relevant risk factors. The results of meta-analysis showed that hypertension had a relationship with being overweight, family history of hypertension, and high salt intake. The pooled OR and 95%CI were 3.26 (2.87, 3.70), 4.79 (2.81, 8.14), and 2.33 (1.55, 3.51), respectively. And the same results were obtained in subgroups analysis. The effect of smoking, irritable personality, and literacy between case group and control group showed no significant difference. Their pooled OR values (95%CI) of smoking, irritable personality, and literacy were 1.20 (0.84, 1.72), 2.73 (0.76, 9.82), and 1.22 (0.51, 2.95), respectively, and the same results were obtained in subgroups analysis. Although the effect of drinking between case group and control group showed a difference, its OR value (95%CI) was 1.61 (1.06, 2.45), and the subgroup analysis showed drinking was not related to hypertension. Conclusion Being overweighed, family history of hypertension, and high salt intake have relationship with hypertension, and these conclusions do not differ depending on the time of the studies conducted.

  • Analysis of Risk Factors of Recurrent Inguinal Hernia by Cox Proportional Hazard Regression Model

    ObjectiveTo retrospectively analyze the risk factors of recurrent groin hernia. MethodsData came from all hernia repairs recorded in our hospital between 1991 and 2001. A total of 1 082 cases of groin hernia was analyzed through Cox proportional hazard model to determine the relative risks between recurrence and mode of admission, types of groin hernia, repair methods (traditional/tensionfree hernioplasty), and postoperative complications.ResultsFrom 1991 to 2001,1 082 groin hernia operations were performed in our hospital, 88.9% for primary hernia and 11.2% for recurrent hernia. As compared with traditional hernia repair methods, Lichtenstein’s tensionfree hernioplasty technique had less relative risk for recurrent hernia repair than that for primary hernia repair. Postoperative complications and indirect hernia were linked to an increased relative risk for recurrence. ConclusionThere are many factors affecting the recurrence of inguinal hernia, but the use of tensionfree hernia repair may decrease rerecurrence of recurrent hernia.

  • Bacterial analysis of community acquired lower respiratory tract infection in hospitalized patients with underlying chronic respiratory tract diseases

    Objective To explore the distribution of bacteria among community acquired lower respiratory tract infection (LRTI) inpatients with underlying chronic respiratory tract diseases.Methods The clinical data,sputum culture and drug susceptibility results of 212 community acquired LRTI patients who were hospitalized during the period 2001-2005 were retrospectively analyzed.All patients had various underlying chronic respiratory tract diseases.Results A total of 229 strains of pathogens were detected,with the majority being gram negative bacteria.In pathogens of acute exacerbation of chronic obstructive pulmonary disease,gram negative bacteria occupied 73.9%.And Pseudomonas aeruginosa and Klebsiella pneumoniae were the most common pathogens,with each occupying 18.2% and 13.6% respectively.Gram positive bacteria occupied 23.8%,mainly Staphylococcus aureus (10.2%) and Streptococcus pneumoniae (9.1%).In patients with bronchiectasis exacerbated by bacterial infection,86.2% were caused by gram negative bacteria,the top three being,in descending order,Pseudomonas aeruginosa (27.5%),Haemophilus parainfluenzae (13.7%),and Haemophilus influenzae (11.8%).Bronchiectasis was the major risk factor of getting Pseudomonas aeruginosa infection (OR=5.590,95%CI 2.792~11.192).The risk factors of getting Acinetobacter baumanii infection were antacid usage within 1 month (OR=9.652,95%CI 2.792~11.192) and hypoalbuminemia (OR=2.679,95%CI 1.108~6.476).For enterobacters infections,including Klebsiella pneumoniae,Enterobacter cloacae and Escherichia coli,the risk factors were antibiotic usage within 1 month (OR=4.236,95%CI 1.982~9.057),having renal diseases (OR=4.305,95%CI 1.090~17.008) and diabetes mellitus (OR=2.836,95%CI 1.339~6.009).Conclusions Gram negative bacteria were the main pathogens of community acquired LRTI in hospitalized patients with underlying chronic respiratory tract diseases.The pathogens were influenced by underlying diseases,severity of diseases and drug usage history of patients.

  • Invasive aspergillosis in nonneutropenic patients:risk factors,clinical features and outcome

    Objective To investigate the risk factors,clinical features and outcome of invasive aspergillosis(IA) in nonneutropenic patients.Methods Fifty-four patients with IA at the First Affiliated Hospital of Zhengzhou University from Jan 2001 to Dec 2006 were analyzed retrospectively.According to the definitions of EORTC/MSG,proven diagnosis was made in 9 cases,probable diagnosis in 30 cases and possible diagnosis in 15 cases.Results In the neutropenic group(n=24),hematological malignancies were the major underlying conditions(n=20).In nonneutropenic group(n=30),the main underlying condition was steroid-treated COPD(n=11).Fever,dyspnea,cough,chest pain and haemoptysis were commonly symptoms.Thoracic computed tomography showed that segmental consolidation occurred more frequently in neutropenic patients,whereas diffuse nodules more frequently in nonneutropenic patients.Nodules or consolidation with evidence of cavity lesion had a higher sensitivity than the halo sign or air crescent sign in both groups.The total mortality of IA was 72.2%.The mortality of nonneutropenic group was higher than that of neutropenic group(83.3% vs 58.3%,P=0.042).Multivariate analysis showed that secondary central nerves system IA and delayed diagnosis were associated with poor outcome of IA.Conclusion There were high incidence and mortality of IA among nonneutropenic patients,especially those with COPD treated with long-term corticosteroids.

  • Risk factors and therapy stra tegies of acute pulm onary edem a in critically ill patients after the 2008 W enchuan earthquake

    Objective To analyze risk factors and therapy strategies of critically ill patients with acute pulmonary edema(APE)after the 2008 Wenchuan earthquake.Methods Data including the level of hemoglobin,mean arterial pressure(MAP),central venous pressure(CVP),serum albumin as well as complications and liquid balance 1 week,3 days,1 day before onset of APE was collected an d an alyzed retrospectively.Resets Among 142 patients during two months after May 12 earthquake APE was detected in 17 cases for 25 times.The hemoglobin Was (86.04 ±16.31)s/L,MAP was(99.40±17.38)mm Hg,CVP Was (13.64 ±4.09)mm Hg and serum albumin was(27.80±8.10)g/L.Acute renal failure,severe infection,cardiovascular disease and extremity lost were more common in APE patients.Theliquid net intake Was (1 725.05±4 624.84)mL for one week,(1 574.70±2 857.13)mL for 3 days and (368.56±1 589.89)mL for 1 day before the onset of APE.The liquid intake Was significantly higher in young group.APE Was alleviated promptly after integrated therapy in all cases.Conclusions Traumapatients with ARF or extremity lost are prone to be compromised with APE.Severe infection and overburden of liquid may be other predisposing factors.

  • Nosocomial Non-fermented Bacterial Infection in Lower Respiratory Tract and Risk Factors for Multidrug Resistant Bacterial Infection

    Objective To investigate nosocomial non-fermented bacterial infection in lower respiratory tract and the risk factors for multi-drug resistant bacterial infection. Methods 229 patients with nosocomial nonfermented bacterial infection in lower respiratory tract from January to December in 2007 in Xiangya Hospital were analyzed retrospectively. The distribution and drug sensitivity of pathogens were recorded. Of those 229 patients,183 cases were infected by non-fermented multi-drug resistant bacteria( MDRB) . The risk factors for non-fermented MDRB infection in lower respiratory tract were analyzed by multi-factor logistic multiple regression analysis.Results The top four non-fermented bacteria isolated were Pseudomonas aeruginosa( 47.6%) , Acinetobacter baumannii( 36. 3% ) , Acinetobacter spp( 8. 6% ) , and Stenotrophomonas maltophilia( 5. 1%) . Higher isolatated rate was found in neurosurgery ( 25. 7% ) and central ICU( 22. 9% ) . The isolated non-fermented bacteria except Stenotrophomonas maltophilia were resistant to all antibiotics except cefoperazone-sulbactam and meropenem. ICU stay( P lt; 0. 001) , tracheotomy or tracheal intubation( P = 0. 001) , and previous use of carbapenemantibiotics( P =0. 032) were independent risk factors for non-fermented MDRB infection. Conclusion Non-fermented bacillus were important pathogens of nosocomial infection in lower respiratory tract with high rates of antibiotic resistance. It is important to prevent non-fermented MDRB infection by strict limitation on the indication of ICU stay,tracheotomy and use of carbapenem.
