  • Clinical Management of Pulmonary Pleomorphic Carcinoma:Report of 7 Cases and Literature Review

    Abstract: Objective To investigate clinical characteristics, surgical strategy and prognosis of pulmonary pleomorphic carcinoma, and improve the diagnostic and therapeutic level of pulmonary pleomorphic carcinoma. Methods We retrospectively analyzed clinical data of 7 patients with pulmonary pleomorphic carcinoma who underwent surgical resection from January 2006 to August 2011 in West China Hospital of Sichuan University. There were 5 male patients and 2 female patients with the male/female ratio of 2.5︰1.0 and the mean age of 58.85 (43-69) years old. We also conducted a literature review through PubMed using pulmonary pleomorphic carcinoma and surgery as the key words, and 8 patients with integral clinical data from 2005 to 2011 were identified. There were 7 male patients and 1 female patient with the male/female ratio of 7︰1 and mean age of 70.25 (51-79) years old. All the patients underwent surgical resection and systemic lymph node dissection. Results The mean age of this group was 64.93 (43-79) years old. Among the 15 patients, there were 12 males and 3 females with the male/female ratio of 4︰1. The main symptoms were cough, blood in phlegm, hemoptysis and chest pain. Pathology diagnosis confirmed pleomorphic carcinoma in all the patients. Among the 7 patients of our hospital, there were 3 patients with spindle cell with squamous cell carcinoma, 2 patients with spindle cell with adenocarcinoma, and 2 patients with spindle cell with large cell carcinoma and adenocarcinoma. During follow-up, 3 patients died with the longest survival time of 49 months, and the other 4 patients were still alive. Among the 8 patients in the literature review, there were 4 patients with spindle cell with squamous cell carcinoma, 1 patient with spindle cell with adenocarcinoma, 1 patient with spindle cell with large cell carcinoma and squamous cell carcinoma, and 2 patients with spindle cell with adenocarcinoma and squamous cell carcinoma. During follow-up, 5 patients died with the longest survival time of 22 months, and the other 3 patients were still alive. Conclusion Pulmonary pleomorphic carcinoma is extremely rare and surgical resection is an effective treatment strategy for it.

  • Advance in microRNAs and epidermal growth factor receptor mutation on metastasis of non-small cell lung cancer

    MicroRNA (miRNA) is a noncoding RNA and protein involved in regulating gene expression in the transcription level. Epidermal growth factor receptor (EGFR) is a protein tyrosine kinase receptor and its mutations have been confirmed in non-small cell lung cancer (NSCLC) by a large number of studies in recent years. EGFR tyrosine kinase inhibitor (EGFR-TKI) is widely used for treatment of NSCLC patients with EGFR mutation. In recent years, miRNA is more and more important in tumor metastasis. The role of EGFR mutations in NSCLC has become a hot spot as well. New researches report that the relationship between miRNA and EGFR mutations plays an important role in NSCLC metastasis. Therefore, we write this review to discuss the mechanisms of miRNA and EGFR mutations in metastasis of NSCLC.

  • Research progress of fraction of exhaled nitric oxide in common pulmonary diseases

    Detection of the fraction of exhaled nitric oxide (FeNO) is a safe, simple and easy method to assess airway inflammation noninvasively. Thus, FeNO detection has been paid more attention to diagnosis and guide treatment of pulmonary diseases. The common feature of pneumonia, asthma, chronic obstructive pulmonary disease and chronic cough is the existence of varying degrees of airway inflammation. In this review, FeNO production and its potential pathologic and physiologic role in various pulmonary diseases were discussed.

  • Right visual double lumen endotracheal tube versus common right double lumen endotracheal tube lung isolation technique: A randomized controlled study

    ObjectiveTo compare the clinical efficacy between right visual double lumen tube (VDLT) intubation and right common double lumen tube (DLT) intubation in lung isolation technique. MethodsA total of 57 patients undergoing thoracoscopic surgery with right DLT lung isolation technique in the First People's Hospital of Chenzhou City and West China Hospital from June 2020 to June 2021 were randomly divided into two groups: a DLT group (n=29, 16 males and 13 females, with a mean age of 54.3±13.2 years) and a VDLT group (n=28, 18 males and 10 females, with a mean age of 55.1±13.7 years) at 1 : 1 with random number table generated by the computer. The clinical data of the two groups were compared. ResultsCompared with the DLT group, the catheter positioning time in the VDLT group was statistically shorter (74.9±47.5 s vs. 151.6±88.9 s, P<0.001), the right upper lung occlusion rate (21.4% vs. 51.7%) and the intraoperative re-adjustment catheterization rate (14.3% vs. 48.3%) were lower (P<0.05). The quality of lung collapses immediately after thoracotomy (67.9% vs. 24.1%) and 20 minutes after thoracotomy (100.0% vs. 75.9%) were improved (P<0.05). There was no significant difference in the rate of fiberoptic bronchoscope assistance for positioning, or the incidence of pharynx pain and hoarseness between the two groups (P>0.05). ConclusionCompared with common DLT, VDLT is more efficient, accurate and intuitive in the location of right bronchial intubation.

  • Development of integrated online and offline internet hospitals and its application status and progress in thoracic surgery

    With the continuous advancement of internet technology and the widespread internet literacy, the concept of integrated online and offline internet hospitals is gradually being embraced and applied both domestically and internationally. This model's relevance in thoracic surgery lies in its ability to improve efficiency and provide comprehensive, diverse, and personalized medical services to address complex and serious medical conditions. However, the implementation of internet hospitals also presents certain challenges such as hardware conditions and diagnostic risks. Through further research and localization in the future, the integrated online and offline internet hospital model is expected to develop and improve, contributing to better medical services for patients in thoracic surgery clinical practice.

  • Analysis of prognosis factors of postoperative cardiac complications in esophageal cancer patients with comorbid coronary artery disease

    Objective To investigate the prognosis factors of postoperative cardiac complications in esophageal cancer patients co‐morbidated with coronary artery disease. Methods Clinical data of the patients with esophageal cancer surgery and concomitant coronary heart disease admitted to the esophageal surgery department of our hospital from December 2019 to June 2023 were collected. They were divided into two groups based on the occurrence of postoperative cardiac complications. Using postoperative cardiac complications as the dependent variable, we established a multivariate logistic regression analysis model to explore the relevant influencing. ResultsA total of 223 patients were collected, including 148 (70.71%) males and 75 (20.29%) females with an average of 71.78±6.31 years, ranging from 53 to 88 years. There were 71 (31.84%) patients experiencing at least one cardiac complication, including 2 patients of acute coronary syndrome, 13 patients of cardiac insufficiency, and 59 patients of new postoperative arrhythmias. The results of univariate analysis showed that age, systemic immune-inflammation index, pulmonary infection, respiratory failure requiring invasive ventilation, ARDS, acute delirium, additional drainage for pleural effusion, and acute renal insufficiency were risk factors for postoperative cardiac complications (P<0.05). The results of the multivariate logistic regression model showed that age (OR=1.062, 95%CI 1.008-1.120, P=0.024) and ARDS (OR=7.690, 95% CI 3.498-16.903, P<0.001) were independent risk factors for postoperative new cardiac complications in esophageal cancer patients with concomitant coronary heart disease. Conclusion Strengthening perioperative management of esophageal cancer, from strict preoperative evaluation to the treatment of postoperative complications, with particular attention to indicators such as age and ARDS, is crucial for improving the prognosis quality of patients with coronary heart disease complicated with esophageal cancer after surgery.

  • The opinion of nurse on enhanced recovery after surgery in thoracic surgery: A questionnaire survey

    Objective To analyze the nurses' current view and perceptions of enhanced recovery after surgery (ERAS) by a questionnaire and to promote the clinical application of ERAS. Methods We conducted a questionnaire study for nurses who attended the First West China Forum on Chest ERAS in Chengdu during September 26-27, 2016 and 259 questionnaires were collected for descriptive analysis. Results (1) The application status of ERAS: There were 13.5% responders whose hospital took a wait-an-see attitude, while the others' hospital took different actions for ERAS; 85.7% of nurses believed that ERAS in all surgeries should be used; 58.7% of nurses believed that the concept of ERAS was more in theory than in the practice; 40.2% of nurses thought that all patients were suitable for the application of ERAS; (2) 81.9% of nurses believed that the evaluation criteria of ERAS should be a combination of the average hospital stay, patients’ comprehensive feelings and social satisfaction; (3) 70.7% of nurses thought that the combination of subjects integration, surgery orientation and surgeon-nurse teamwork was the best model of ERAS; 44.8% of nurses thought the hospital administration was the best way to promote ERAS applications; (4) 69.1% of responders believed that immature plan, no consensus and norms and insecurity for doctors were the reasons for poor compliance of ERAS; 79.5% of nurses thought that the ERAS meeting should include the publicity of norms and consensus, analysis and implementation of projects and the status and progress of ERAS. Conclusion ERAS concept has been recognized by most nurses. Multidisciplinary collaboration and hospital promotion is the best way to achieve clinical applications.

  • Status quo of enhanced recovery after surgery in different regional thoracic surgery in the mainland of China

    Objective To analyze the current status and difficulties of ERAS applicated in thoracic surgery in different regional hospitals. Methods A total of 773 valid questionnaires were collected during the First West China Forum on Chest ERAS and analyzed by geographical distribution. The content of the questionnaire was divided into two parts, including the respondents’ institute and personal information, as well as 10 questions about ERAS. Results There were 83.57%, 83.82%, 89.58%, 93.75%, 94.74% and 92.86% of surgeons and nurses in Sichuan province, municipalities, the eastern, western, southern and northern respectively who believed that ERAS in all surgeries should be used. There were 61.84%, 60.29%, 65.97%, 81.25%, 73.68% and 75.00% of surgeons and nurses who thought that the concept of ERAS was more in the theory than in the practice; 77.99%, 80.88%, 74.31%, 78.13%, 83.33% and 69.64% of respondents agreed that average hospital stay, patients’ experience and social satisfaction should be the evaluation standard of ERAS practice while 58.50%, 63.24%, 54.86%, 62.50%, 70.18% and 58.93% of respondents believed that immature procedure, lack of consensus and specifications and insecurity for doctors were the reasons for poor compliance of ERAS; 63.23%, 67.65%, 59.72%, 68.75%, 72.81% and 67.86% of respondents thought that the best team of ERAS should be based on the combination of subject integration, surgery orientation and surgeon-nurse teamwork; 43.73%, 44.12%, 43.75%, 46.88%, 59.65% and 41.07% of respondents thought that multidisciplinary cooperation, multi-modality conducted and surgical programming should be the best way for ERAS implementation; 72.98%, 69.12%, 62.50%, 65.63%, 80.70% and 55.36% of responders agreed that the ERAS forum or conference should include the norms and consensus, analysis and implementation of projects and the status and progress of ERAS. Conclusion There are more consistent views on the clinical application of ERAS in all regions. The subject integration, surgeon-nurse teamwork and multidisciplinary, multi-modality cooperation are the best team and best way for ERAS program implementation.

  • Safety analysis of day surgery program of video-assisted thoracoscopic surgery for pulmonary nodule

    ObjectiveTo explore the safety of day surgery program of video-assisted thoracoscopy surgery in the treatment of pulmonary nodules.MethodsWe retrospectively analyzed the clinical features of the patients who received video-assisted thoracoscopy surgery between June and November 2019 in Day Surgery Center of West China Hospital, Sichuan University. The basic conditions, surgery duration, intraoperative blood loss, placement time of thoracic drainage, postoperative pain score, hospitalization expenses, and postoperative complications were observed. Postoperative telephone follow-up was conducted on the 2nd, 3rd, and 30th day after operation. The 1st day after operation means the next day after operation.ResultsA total of 29 patients were included with 5 males and 24 females. Surgeries were successfully performed on these 29 patients and they were all discharged as planned. The mean surgery duration was (78.14±16.37) min, the mean intraoperative blood loss was (38.15±23.04) mL, and the mean placement time of thoracic drainage was (577.45±233.70) min. Intraoperative open chest surgery and massive hemorrhage were not occurred. The Pain Numerical Rating Scale score at the 6th hour after surgery was 2.10±0.56, and the average hospitalization expense was (33 600±4 611) yuan. In the 29 patients, the postoperative complications included pneumothorax in 2, urinary retention in 1, tachycardia in 1, and persistent cough in 9. No recurrence of the listed complications was reported on the 30th day of telephone follow-up. No severe complications or postoperative death occurred.ConclusionDay surgery program of video-assisted thoracoscopy surgery is safe and effective in the treatment of pulmonary nodules.

  • Long-term efficacy and influencing factors of extended thymectomy for myasthenia gravis with thymic atrophy

    ObjectiveTo analyze the surgical efficacy and influencing factors of myasthenia gravis (MG) patients with thymic atrophy after thymectomy. MethodsThe clinical data of MG patients with thymic atrophy undergoing thymectomy between October 2014 and May 2018 in Daping Hospital of Army Medical University and Shijiazhuang People Hospital were retrospectively analyzed. ResultsA total of 71 patients were collected, including 40 males and 31 females with a mean age of 45.17±12.42 years. All patients received the surgery successfully. After the surgery, 20 (28.17%) patients were stable remission, 12 (16.90%) patients were minimal manifestation status,19 (26.76%) patients were improved, 5 (7.04%) patients showed no change, 3 (4.23%) patients were worsened, 10 (14.08%) patients were exacerbated and 2 (2.82%) patients were dead. Multivariate logistic regression analysis showed that the preoperative illness duration (OR=4.61, 95%CI 1.13-18.85, P=0.03), and postoperative pyridostigmine combined with immunosuppressive (OR=0.12, 95%CI 0.03-0.45, P=0.00) were independent risk factors for long-term efficacy of thymectomy for MG patients with thymic atrophy. ConclusionEarly surgery after diagnosis of MG and postoperative pyridostigmine combined with immunosuppressive treatment is beneficial to the prognosis of MG patients with thymic atrophy.
