  • Surgical Treatment of Ascending Aortic Root Aneurysms

    Abstract:Objective To summarize the experiences in surgical treatment of ascending aortic root aneurysms. Methods One hundred and one patients (age ranging from 14 to 72 years, mean 42.7 years) with ascending aortic root aneurysms were diagnosed as having Marfan syndrome (58 cases), annuloaortic ectasia (34 cases), bicuspid aortic valve (5 cases), aorto arteritis (4 cases) combined with aortic valve incompetence (96 cases) and aortic valve stenosis (4 cases) , preoperatively with type A dissection (26 cases) and acute left heart failure (5 cases). Our operations consisted of 4 Wheat procedures, 13 Cabrol or modified Cabrol procedures, 1 David and 83 Bentall procedures. Concomitant operations included hemi-arch replacement or descending aorta intraluminal stent grafting (16 cases), total arch replacement or descending aorta intraluminal stent grafting (4 cases), mitral valve replacement or anuloplasty (14 cases) and coronary artery bypass grafting (CABG, 8 cases). Results The overall mortality was 6.9%(7/101), and decreased to 3. 6%(3/83) after the year 2000. Main postoperative complications were low cardiac output (10 cases), respiratory insufficiency (9 cases), and renal inadequacy (9 cases). Follow-up was completed in 94 patients. During the period of follow-up, one patient died and 5 patients with Marfan syndrome suffered with type B dissection. Conclusion Bentall procedure is the method of choice for ascending aortic root aneurysms. Preoperative left heart function and surgical techniques give the crucial impact on the outcome of surgery.

  • Changes of Th17 Cells inMice with Respiratory Syncytial Virus Infection

    Objective To observe the immune responses of T helper cells 17 ( Th17) to respiratory syncytial virus ( RSV) infection induced lung inflammation in mice, and explore its roles on the host immune responses to RSV.Methods Female BALB/ c mice aged 3 to 5 weeks were randomly divided into a RSV group ( n=18) and a control group ( n = 12) . The mice were intranasally administrated by a 107.5 50% tissue culture infective dose ( TCID50) of RSV in 0.1 mL of culture medium. Sterile medium ( 0.1 mL/ mouse) was used as control. After infected on 1st , 4th, 8th day, the mice were sacrificed, and specimens from the lungs and lymph nodes were collected. The lung sections were stained by hematoxylin-eosin to observe the changes of lung inflammation after RSV infection. IL-17A, IL-17F and IL-23p19 mRNA expressions in the lung tissue were determined by real-time PCR. The frequencies of Th17 subsets in hilar lymph node were analyzed by flow cytometry. Results On 4th day after RSV infection, a typical lung interstitial inflammation was observed. However, this inflammation was alleviated on 8th day after RSV infection. The viral load in the lung tissue on 4th day after RSV infection were 9.208 ±0.548, which was the highest among all RSV subgroups ( P lt;0.001) . IL-23p19 and IL-17A cytokine expressions in the lung tissue were significantly increased on 4th day and 8th day after RSV infection compared with control groups ( P lt;0.01) , and the peak was on 4th day. However, IL-17F mRNA expression in the lung tissue on different day after RSV infection had no significant difference compared with the control group ( P gt;0.05) . The frequencies of Th17 subsets in hilar lymph node on 4th day and 8th day after RSV infection were ( 0.37 ±0.043) % and ( 0.853 ±0.048) % respectively, which were higher than those in control groups ( P lt;0.05) . The frequencies of Th17 on 8th day after RSV infection were significantly higher than that on 4th day after RSV infection ( P lt; 0.01) . Conclusions The expression of IL-17A in the lung tissue is increased and the level of Th17 cells in hilar lymph nodes is also elevated in the lung infected by RSV, which indicates that Th17 cells might be involved in host antiviral immune.

  • Influence of Preoperative Atrial Fibrillation on Midterm and Longterm Outcomes of Patients after Mitral Valve Replacement

    Abstract: Objective To determine the influence of preoperative atrial fibrillation (AF) on midterm and longterm clinical outcomes of patients after mitral valve replacement (MVR). Methods We retrospectively analyzed clinical data of 1 029 patients who underwent MVR with or without tricuspid valve repair in Changhai Hospital, Second Military Medical University, from January 2000 to December 2005. According to the exclusion criteria, 621 patients were selected and divided into two groups depending on presence of preoperative AF. Those 395 patients with preoperative AF belonged to the AF group, including 134 males and 261 females with their average age of 51.1±11.5 years. Those 226 patients with preoperative sinus rhythm (SR) were in the SR group, including 82 males and 144 females with their average age of 48.2±14.1 years. Early postoperative outcomes, midterm and longterm mortality and morbidity of the two groups were compared. Results During 10 years of follow-up, there was no statistical difference in early postoperative mortality and morbidity between the two groups, but the incidence of late thromboembolism was significantly higher in AF group than that in SR group [0.9‰ (31 patients/33 984 patient-months) vs. 0.4‰ (9 patients/21 151 patient-months), χ2=4.26, P=0.039]. Ten-year survival rate in patients in AF group was significantly lower than that in SR group (83.2% vs. 92.7%, χ2=10.26, P=0.002). Multivariate analysis identified preoperative AF [HR=2.878, 95% CI (1.166,4.129)], low left ventricular ejection fraction [HR=0.948, 95% CI (0.917,0.981)] , and old age [HR=1.073, 95% CI (1.038,1.109)] as independent risk factors for late mortality after MVR. Apart from its influence on patient survival rate and incidence of thromboembolism, preoperative AF also had an adverse effect on left ventricular function, right ventricular function and tricuspid regurgitation. Conclusion AF is an independent risk factor for poor prognosis after MVR. Prognosis after MVR might be improved if surgery could be performed early when patients have predictive signs of AF such as multiple premature atrial contractions or left atrium enlargement.

  • Clinical analysis of redo aortic root replacement after cardiac surgery

    ObjectiveTo analyze the etiologies, operation techniques and outcomes of redo aortic root replacement after cardiac surgery.MethodsBetween December 2013 and December 2019, 30 patients who had at least one previous cardiac operation received aortic root replacement in our hospital, including 20 males and 10 females with an average age of 50.4±12.7 years. The mean time interval between this operation and the previous one was 8.0±8.5 years. The principal indication for surgery was aortic sinus dilatation and ascending aortic aneurysm in 14 patients (47%), acute aortic dissection in 5 patients (17%), pseudoaneurysm in 3 patients (10%), prosthetic valve endocarditis in 4 patients (13%), prosthetic leakage in 4 patients (13%). Bentall procedure was used in all 30 patients, with concomitant mitral valve plasticity or replacement in 5 patients, tricuspid valve plasticity in 6 patients, coronary artery bypass grafting in 3 patients, and total aortic arch replacement and elephant trunk procedure in 2 patients.ResultsThe mean cardiopulmonary bypass time was 96-296 (161.3±43.0) min, and the mean aortic occlusion time was 48-117 (85.7±20.4) min. There were 5 in-hospital deaths with an overall in-hospital mortality of 17%. The causes of deaths were low cardiac output syndrome in 2 patients and septic shock in 3 patients. The follow-up time was 3-75 (33.5±21.1) months. Three patients died during the follow-up, 1 patient died of septic shock and 2 died of cerebral hemorrhage.ConclusionRedo aortic root replacement is difficult to deal with, and the risk is high. Preoperative evaluation is required, appropriate surgical approach, adequate myocardial protection, and a complete surgical plan are essential to ensure the success of the operation.

  • Transcatheter tricuspid valve replacement via vena jugularis interna: A case report

    We reported a 54-year-old female patient with severe tricuspid regurgitation, who received mechanic valve in the mitral position 15 years ago. The patient’s Society of Thoracic Surgeons score was 8.27%, and was intolerant to open heart surgery, so she was recommended for transcatheter tricuspid valve replacement via right vena jugularis interna. The procedures were guided by echocardiography and X ray fluoroscopy on November 13, 2021, the prosthesis was implanted successfully, and the patient was recoved without any adverse events. After 1 month follow-up, her general condition was apparently improved.
