With significantly increasing proportion of high-risk patients undergoing cardiovascular surgery, a quantitative risk stratification system of perioperative patients is needed for cardiovascular surgeons. European system for cardiac operative risk evaluation (EuroSCORE) is a widely-used risk prediction model for adult patients undergoing cardiovascular surgery in the world. Research data from Chinese Cardiac Surgery Registry show that the performance of EuroSCORE in the prediction of postoperative risk of Chinese cardiovascular surgical patients is not satisfactory. Thus, the first Chinese coronary operative risk evaluation model (Sino system for coronary operative risk evaluation,SinoSCORE) is established with latest cardiovascular surgery data by Collaboration Association of Cardiovascular Surgeon in China, and has been widely used in clinical practice. This review focuses on the application and prospect of EuroSCORE and SinoSCORE for the prediction of mortality after cardiovascular surgery in adults.
Objective To verify the predictive value of Sino System for Coronary Operative Risk Evaluation(SinoSCORE) on quality of life in patients undergoing coronary artery bypass graft (CABG) surgery. Method A total of 234 patients in Peking University People’s Hospital undergoing CABG between November 2008 and September 2010 hadcompleted the preoperative and 6-month postoperative Short Form-36 (SF-36). There were 172 (73.5%) male patients and63 (26.5%) female patients. The average age was 63.0±10.1 years. According to the SinoSCORE, 234 patients were dividedinto three groups:low risk (SinoSCORE score less than 1 point,n=67), medium risk (SinoSCORE score 2-5 points,n=77) and high risk (SinoSCORE score more than 6 points,n=90) group. Clinical information of the 234 patients was collected, andthe score values of all patients were calculated according to the SinoSCORE model. Statistic methods were performed toevaluate the relationship between quality of life and SinoSCORE. Results The postoperative quality of life have improvedsignificantly, but the improvement of quality of life have no significantly different between groups. There was statisticallysignificant correlation between quality of life and SinoSCORE (P<0.05, r value at-0.150 to 0.255).Linear regression analysis showed that SinoSCORE was significantly collected with quality of life in multiple subgroups (P<0.05, r 2<0.1) .Conclusion SinoSCORE have statistically correlated with quality of life, and have certain but limited predictive value on quality of life in CABG patients.
Abstract: Objective To evaluate prediction validation of Sino System for Coronary Operative Risk Evaluation (SinoSCORE) on in-hospital mortality in adult heart surgery patients in West China Hospital.?Methods?We included clinical records of 2 088 consecutive adult patients undergoing heart surgery in West China Hospital from January 2010 to May 2012, who were also included in Chinese Adult Cardiac Surgical Registry.We compared the difference of preoperative risk factors for the patients between Chinese Adult Cardiac Surgical Registry and West China Hospital. SinoSCORE was used to predict in-hospital mortality of each patient and to evaluate the discrimination and calibration of SinoSCORE for the patients.?Results?Among the 2 088 patients in West China Hospital, there were 168 patients (8.05%) undergoing coronary artery bypass grafting (CABG), 1 884 patients (90.23%) undergoing heart valve surgery, and 36 patients (1.72%) undergoing other surgical procedures. There was statistical difference in the risk factors including hyperlipemia, stroke, cardiovascular surgery history, and kidney disease between the two units.The observed in-hospital mortality was 2.25% (47/2 088). The predicted in-hospital mortality calculated by SinoSCORE was 2.35% (49/2 088) with 95% confidence interval 2.18 to 2.47. SinoSCORE was able to predict in-hospital mortality of the patients with good discrimination (Hosmer Lemeshow test: χ2=3.164, P=0.582) and calibration (area under the receiver operating characteristic curve of 0.751 with 95% confidence interval 0.719 to 0.924). Conclusion SinoSCORE is an accurate predictor in predicting in-hospital mortality in adult heart surgery patients who are mainly from southwest China
