  • Effects of Hand Assistant Laparoscopic Splenectomy Plus Pericardial Devascularization on Systemic Stress Responses

    Objective To investigate the effects of hand assistant laporoscopic splenectomy plus pericardial devascularization on systemic stress responses. Methods Forty patients with cirrhotic portal hypertension were selected, 20 cases of which were underwent hand assistant laparoscopic splenectomy plus pericardial devascularization (LAP group), and the other 20 were underwent open splenectomy plus pericardial devascularization (OP group). The levels of blood glucose (BG), insulin (Ins), triiodothyronine (T3), tetraiodothyronine (T4), corticosteroid (CS) and other related clinical data were measured before operation and on day 1-3 after operation, which were compared between two groups. Results There was no statistical significance between two groups on those levels before operation. On day 1 after operation, BG and CS level in both two groups were higher than those before operation (P<0.05), but they were recovered on day 2 after operation in LAP group (Pgt;0.05), and on day 3 after operation in OP group (Pgt;0.05). BG and CS level in OP group were markedly higher than those in LAP group on day 2 after operation (P<0.05). On day 1 after operation, Ins, T3 and T4 level of two groups were lower than those before operation (P<0.05), but they were recovered on day 2 after operation in LAP group (Pgt;0.05) and on day 3 after operation in OP group (Pgt;0.05). Ins, T3 and T4 level in OP group were lower than those in LAP group on day 2 after operation (P<0.05). There was no significant difference in operation time between two groups (Pgt;0.05). But laparoscopic surgery had more advantages than conventional open surgery such as reducing bleeding quantity in operation, shortening recovery time of bowel and urinary bladder function and the length of stay. Conclusion Compared with laparotomy, the laparoscope not only imposes less impact on physical stress system, but also makes recovery after operation more quickly.

  • Inhibition Effects of Recombination Human Growth Hormone and TPN on Stress Response of Postoperative Gastrointestinal Carcinoma Patients

    【Abstract】ObjectiveTo explore the mechanisms of anabolism intensified by recombination human growth hormone (GH) on the basis of total parenteral nutrition (TPN) during postoperative in gastrointestinal carcinoma patients. MethodsNinety-four gastrointestinal carcinoma patients undergone operation were randomly divided into TPN group and TPN+GH group. The levels of TNF-α, IL-1, IL-6 and CRP were detected in the first, third, seventh postoperative day. ResultsThe levels of TNF-α, IL-1, IL-6 and CRP were significantly lower in TPN+GH group than those in the TPN group at the first, third, seventh postoperative day (P<0.01). The levels of TNF-α, IL-1, IL-6 and CRP were significantly higher at the indicated time of postoperative days than the pre-operative days in the two groups (P<0.01). ConclusionBy inhibiting TNF-α, IL-1, IL-6 and CRP production in gastrointestinal carcinoma patients undergone operation and blocking high catabolism induced by inflammatory cytokines, GH promotes the synthesis of anabolism.

  • Effects of Laparoscopic Resection on Systemic Stress Responses in Colorectal Cancer Patients

    ObjectiveTo investigate the systemic stress responses after laparoscopic resection in colorectal cancer patients.MethodsSixty patients were randomized into the laparoscopic resection group (30 cases) and open resection group (30 cases) from October 2001 to September 2002 in our hospital.Analgesic dosages, recovery time of intestinal peristalsis, postoperative temperatures, C reactive protein(CRP), IL6 and white blood cell (WBC) counts were recorded after operation. ResultsThe changes of postoperative temperatures and WBC counts showed no significant difference between two groups.But in the laparoscopic resection group, the recovery time of intestinal peristalsis, postoperative analgesic dosages, CRP and IL6 significantly decreased (P<0.01,P<0.05).ConclusionCompared with traditional open resection, laparoscopic resection of colorectal cancer can diminish the systemic stress responses and decrease the injury to patients.

  • Preoperative Oral Carbohydrate Administration Can Ameliorate Perioperative Stress and Metabolism for Patients with Surgical Thoracic Oncology

    Abstract: Objective To investigate the effect of preoperative oral carbohydrate (CHO) administration on perioperative risks of patients with surgical thoracic oncology,and provide evidence for establishing new scientific preoperative fasting strategy.Methods?In this prospective study, from July to September 2010,32 out of 65 enrolled patients with surgical thoracic oncology in Department 1 of Thoracic Surgery,Cancer Hospital of Peking University, were randomly allocated to preoperative experiment group (fasting overnight and oral 12.5% dextrose 400 ml administration 2 h before anesthesia induction) or control group (fasting overnight and water deprivation from midnight). Clinical data were collected including subjective evaluation of thirst and hunger measured by visual analogue scale (VAS), blood glucose level(BGL),serum insulin level, homeostasis model assessment insulin resistance(HOMA-IR),postoperative length of hospital stay (LOS) and complications.Results?Sixteen patients were enrolled in each group. VAS scores of thirst and hunger of the preoperative experiment group at 1 h before anesthesia induction were significantly lower than those of the control group(24 vs. 49,24 vs. 62 ,P=0.000). BGL(8.59±0.43 mmol/L vs. 5.59±0.43 mmol/L, P=0.000), serum insulin level (24.33±1.80 mIU/ ml vs. 16.28±1.80 mIU/ml, P=0.004)and HOMA-IR(9.23±0.77 vs. 4.03±0.77,P=0.000)of the preoperative experiment group before anesthesia induction were significantly higher than those of the control group,and these three variables of the preoperative experiment group returned to baseline level soon after surgery. There was no statistical difference in postoperative LOS and complication rate between the two groups (P>0.05).Conclusion?Preoperative oral CHO treatment is safe for non-diabetic patients with surgical thoracic oncology, can alleviate their subjective discomfort,decrease insulin resistance, and ameliorate their perioperative stress and metabolism.

  • RESEARCH PROGRESS OF STRESS FRACTURE OF LUMBAR PEDICLE

    Objective To review the research progress of stress fracture of the lumbar pedicle. Methods The literature about the stress fracture of the lumbar pedicle was reviewed extensively and summarized. Results There are two types of stress fracture: fatigue and structure insufficient. Stress fracture of lumbar pedicle occurred mainly in the crowd with repetitive and large activities of spine, contralateral spondylolysis, or previous surgery of lumbar vertebra. The main stresses causing stress fracture of the lumbar pedicle are shear stress and twisting stress, followed by sudden hyperflexion or hyperextension of the spine. Stress fracture of lumbar pedicle was easily missed by conventional X-ray examination, usually XCT, MRI, or bone scan was needed to confirm the diagnosis. It is divided into 4 types or 4 periods according to MRI findings: stress reaction, incomplete fracture, complete fracture, and pseudarthrosis. For patients with incomplete, complete, and juvenile stress fractures of the lumbar pedicle without nerve root irritation, the majority of claims preferred conservative treatment and the healing rate of fracture was high; for patients with bilateral pseudarthrosis and with nerve root irritation as well as patients who failed to the conservative treatment, surgical management was advocated and the operation result is good. Conclusion Stress fracture of the lumbar pedicle as one of the causes of low back pain is extremely rare, and is easily missed clinically. Surgery or conservative management should be selected based on type of fracture and specific condition of the patient, the treatment results are satisfactory.

  • CELLULAR THERAPY OF STRESS URINARY INCONTINENCE

    【Abstract】 Objective To review the progress and cl inical appl ication of cellular therapy for stress urinaryincontinence (SUI). Methods The l iterature about cellular therapy of SUI was extensively reviewed. Results Becauseof having no or poor regeneration capacity, the cl inical application of chondrocytes and myoblasts were l imited. Based on the rapid progress in stem cell biology, an increasing number of animal experiments and cl inical trials about cellular therapy of SUI have been reported with encouraging results. All these show that cellular therapy has great potential in cl inical application. Stem cells are considered as ideal seeded-cells for treatment of SUI. Conclusion Cellular therapy, especially stem cells, provides a novel approach for treatment of SUI, but the mechanism needs further study.

    Objective To review the possible mechanisms of the mammal ian target of rapamycin (mTOR) in theneuronal restoration process after nervous system injury. Methods The related l iterature on mTOR in the restoration ofnervous system injury was extensively reviewed and comprehensively analyzed. Results mTOR can integrate signals fromextracellular stress and then plays a critical role in the regulation of various cell biological processes, thus contributes to therestoration of nervous system injury. Conclusion Regulating the activity of mTOR signaling pathway in different aspects cancontribute to the restoration of nervous system injury via different mechanisms, especially in the stress-induced brain injury.mTOR may be a potential target for neuronal restoration mechanism after nervous system injury.

  • A FINITE ELEMENT ANALYSIS OF PELVIC RING RECONSTRUCTION WITH FIBULAR AUTOGRAFT FOLLOWING PERIACETABULAR TUMOR RESECTION

    Objective To establ ish sophisticated three-dimensional finite element model of reconstructing the whole pelvis and defects in pelvis caused by the resection of periacetabular tumor, and to research the stress distribution regularity ofthe pelvis reconstructed by the fibular transplantation through three different internal fixation techniques. Methods The CTdatasets including L3 to middle-femur, unilateral fibular and internal fixation system from 1 healthy 35-year-old male volunteer were collected to establ ish finite element models of reconstructing the pelvis after the resection of periacetabular tumors through 3 different internal fixation means, namely fibular with plates, pedicle-rods and sacral-il iac rods. Bilateral leg standing position was simulated, then vertical load of 500 N was imposed on the surface of L3, the stress distribution regularity of reconstructed pelvis, transplanted fibular and internal fixation system were evaluated. Results The finite element models of the pelvis reconstruction after resection of periacetabular tumors were establ ished. The stress concentration of transplanted fibular was extremely high in the vicinity of the host junction sites. For the three internal fixation systems, the connection between steel plate and screw or between titanium bar and screw inclined to have stress concentration; and when the titanium bar was adopted to reconstruct, the transplanted fibular and the healthy side of femoral neck had less stress concentration, while sacral-il iac rods had the most obvious stress concentration. Conclusion For the reconstruction pelvis, the three fibula transplantation and steel plate internal fixation are consistent with intact state of pelvis in terms of the stress distribution, which is a relatively good method for the treatment of bone defect after periacetabular tumor. The finite element model can be used as a tool for the pelvis biomechanics research.

  • BIOMECHANICAL EVALUATION OF TiNi SHAPE-MEMORY SAWTOOTH-ARM EMBRACING PLATE AND ITS EFFECT ON FRACTURE HEALING

    Objective  To investigate the detailed biomechanics of TiNi shape-memory sawtooth-arm embracing plate (TiNi SMA) by comparing with l imited-contact dynamic compression plate (LC-DCP) and static interlocking intramedullary nail (SIiN), so as to provide theoretical evidence for cl inical appl ication. Methods  Eight paired cadaveric femurs immersed in formaldehyde were harvested from eight specimens of adults. After making X-ray films and modeling midpiece transverse fracture, one side randomly was fixed by TiNi SMA (group A) and SIiN (group C) orderly, the other side was fixed by LC-DCP (group B). The axial compression, three-point bending (pressed from plate side and opposite side both of group A and group B, from inside of group C), and torsion were tested, and the stress shielding rate was compared.  Results At every classified axial compression load, the strains of group A were greater than those of group B and group C (P lt; 0.05), the displacements of group A were greater than those of group B and group C (P lt; 0.05) except 100 N. At every classified three-point bending moment, the displacement of group A were greater than those of group B and group C pressed both from two sides, but there was no difference when pressing from two sides under the same load of group A (P gt; 0.05). At every torsion moment, the torsion angels of group A were greater than those of group B (P lt; 0.05), but equal to those of group C (P gt; 0.05). At 600 N of axial compression load, the stress shielding rates of groups A, B, C were 48.30% ± 22.99%, 89.21% ± 8.97%, 95.00% ± 3.15%, respectively, group A was significantly less than group B and group C (P lt; 0.01).  Conclusion The anti-bending abil ity of TiNi SMA is weaker than LC-DCP and SIiN; the anti-torsion abil ity of TiNi SMA is weaker than SIiN, but TiNi SMA is a center-type internal fixation, the superior stress shielding rate and micromovement promote the stress stimulation of fracture, which makes it an ideal internal fixation device.
