  • Adult Diameter Artificial Vascular for Right Pulmonary Artery Originated from Ascending Aorta

    ObjectiveTo summarize our experience of surgical treatment for right pulmonary artery originated from the ascending aorta by using adult diameter artificial vascular and study the operative indication, design, method, and therapeutic efficacy. MethodsWe retrospectively analyzed clinical data of 11 patients with right pulmonary artery originated from ascending aorta in The Second Affiliated Hospital of Harbin Medical University from May 2008 through December 2013, who were treated by using adult diameter artificial vascular. The patients ranged from 4 months to 25 months old, weighted 4-15 kg. Among of them, 4 patients had persistent truncus arteriosus and 7 had aortopulmonary septal defect. All patients were complicated with moderate pulmonary hypertension. All the patients underwent one stage surgical repair under extracorporeal circulation and cardiac arrest. During the surgery, end to side anastomosis was done between the right pulmonary artery and 16-18 mm diameter artificial blood vessels. And artificial blood vessel was connected to the main pulmonary artery or right ventricle outflow tract incision from the aorta above. ResultsThe average operation time was 179-325 (224±68) min. The average cardiopulmonary bypass (CPB) time was 81-208 (117±54) min. The average aortic clamping time was 29-63(42±21) min. The mean residence time in ICU was 71-197 (109±42) hours. The average assisted mechanical ventilator time was 59-191 (91±26) hours. The average length of stay in hospital was 21-39 (28±11) days. Low cardiac output syndromes caused by pulmonary arterial hypertension occurred in 5 patients including 2 deaths and 3 patients with good recovery by reducing the pulmonary arterial pressure and peritoneal dialysis. The result of postoperative cardiac color ultrasound examination of 9 survival patients showed vascular prosthesis, no distortion, no stenosis of the anastomosis, deformity correction satisfaction. Nine patients were followed up for 3-60 months. The results of echocardiography showed no anastomosis and artificial vascular stenosis, and the pulmonary arterial pressure decreased significantly. ConclusionThe right pulmonary artery originated from the ascending aorta in children should be operated as soon as possible. Compared the adult diameter artificial vascular treatment for one stage repair of right pulmonary artery from the ascending aorta with other operation methods, both short-term and long-term effects are good. Postoperative low cardiac output syndrome is a common complication.

  • Surgical Treatment for Mesh Infection after Prosthetic Patch Repair of Inguinal Hernia

    Objective To discuss the surgical treatment and experience of mesh infection after prosthetic patch repair of inguinal hernia. Methods The clinical data of 67 cases of mesh infection after prosthetic patch repair who were treated in Chao-Yang Hospital from Jan. 2011 to Jun. 2012 were retrospectively analyzed. Results All patients were treated with surgical operation successfully, including removing the infected mesh and surrounding tissues, primary suture, and a placement of wound drainage, without replacement of a new patch substitute. The hospital stay of the patients was 10-25days with an average of 16days. Of the 67 patients, 51 patients got primary healed and the other 16 patients healed delayed after local dressing change due to the superficial infection following stitch removal. Sixty-six patients were followed-up for 6-24 months (average of 20 months) after operation without recurrence and complication, including seroma, wound infection, intestinal fistula, and postoperative pain. Conclusions The treatment of mesh infection after inguinal hernia repair is very complicated, but the primary suture repair and a placement of wound drainage after removing infected mesh with complete debridement is a effective therapy for it.

  • Clinical Analysis and Recognition on Surgical Treatment of Remnant Gastric Cancer

    Objective To analyze surgical treatment and clinicopathologic features of remnant gastric cancer,and to recognize the strategies of treatment. Methods The clinical data of 26 cases patients with remnant gastric cancer diagnosed by endoscopy and pathological examination and underwent surgical treatment were retrospectively analyzed in our hospital between January 2004 and March 2011.In this study,14 cases of remnant gastric cancer from benign disease (RGCB) and 12 cases of remnant gastric cancer after stomach cancer operation (RGCC) were included.The clinical findings,Helicobacter pylori (HP) infection,surgical methods, histopathological features,and prognosis were analyzed.Results The patients developed a carcinoma in the gastric remnant about 15-44 years after operation for benign disease (median 26.3 years) and about 1-10.5 years after gastric cancer operation (median 4 years),and there was significant differences of the two groups(P<0.05).HP infection of the gastric remnant was found in 73.1%(19/26) patients,and infection rate of patients was 71.4%(10/14) in RGCB and 75.0%(9/12) in RGCC, but there was no statistical different in two groups (P>0.05).All 26 patients underwent surgical procedure,and the rate of radical resection was 46.2% (12/26), which was 57.1% (8/14) in RGCB and 33.3% (4/12) in RGCC respectively,there was no statistical different (P>0.05).Among the 12 cases underwent radical resection, the highest lymphatic metastasis rate was in No.3 group (83.3%, 10/12),which came in second in lymph node of mesojejunum or splenic hilum (33.3%, 4/12).Postoperative pathological staging was as follows:stageⅠin 3 cases, stageⅡ in 2 cases, stage Ⅲ in 14 cases and stage Ⅳ in 7 cases. In 18 patients underwent tumor excision,the metastasis of lymph node occurred in 13 cases (72.2%),and the pancreas,transverse colon,or spleen were invaded in 13 cases according to histopathological results. Meanwhile,peritoneal metastasis were founded in 8 cases patients,and the peritoneal metastasis rate of patients with RGCB (14.3%;2/14) was significantly lower than that with RGCC (50.0%, 6/12), P<0.05.The overall one-year survival rate and three-year survival rate was 54.5% and 38.5%,respectively, and the survival time was 2-61 months (median 12 months).Survival analysis indicated that pathological stage and radical resection were significant prognostic factors for patients with remnant gastric cancer(P<0.01),and radical resection was an independent prognostic factor (P<0.05),while age,gender,disease of first operation, degree of differentiation and HP infection were not (P>0.05). Conclusions Early detection and standard radical resection are the key factor to improve the prognosis of patients with remnant gastric cancer and laparoscopic exploration may minimize unnecessary injures of surgery.Because of the different clinical characteristics,strategy of treatment for RGCC and RGCB shall be discriminatory.

  • Surgical Treatment for Mesh Infection after Prosthetic Patch Repair of Ventral Hernia

    ObjectiveTo summarize the method and experience in surgical treatment for mesh infection after prosthetic patch repair of ventral hernia. MethodsThe clinical data of 16 patients with mesh infection after ventral hernia repair accepted surgical treatment in our department from June 2007 to May 2010 were analyzed retrospectively. There were 10 males and 6 females, the age range from 24 to 73 years with an average 45.2 years. The patients with mesh infection included 11 cases of infection after incisional hernia repair, 4 cases of infection after abdominal wall defects repair caused by abdominal wall tumor resection, 1 mesh infection combine with urinary fistula caused by parastomal hernia of ileal neobladder repaired by using prosthetic patch. Clinical manifestation included mesh exposion, abscess, chronic sinus, and enterocutaneous fistula. All patients accepted local treatment of change dressing by primary operative surgeon, but the wounds didn’t heal about 3 to 24 months. Then the patients performed radical removal of infected mesh and abdominal wall reconstruction. ResultsAll patients accepted affected mesh removal successfully. Five patients performed abdominal wall reconstruction by using components separation technique. Four cases accepted abdominal wall repair by using polypropylene mesh. Five patients performed abdominal wall repair by using human acelluar dermal matrix. One case accepted change dressing and vacuum aspiration on the infected wound surface without reconstruction. And one case closed the wound immediately after infected mesh removal. The postoperative hospitalization time was 9 to 25 d (average 14 d). Thirteen patients recovered with primary wound healing. The other 3 cases recovered with second healing by local change dressing. All patients were followed up from 6 to 34 months (average 22 months), no abdominal wall hernia recurrence occurred. ConclusionsIt is very difficult to deal with mesh infection after prosthetic patch repair of abdominal wall hernia or defect. The surgical treatment should be done according to specific condition of each individual so as to acquire satisfied results.

  • DIAGNOSIS AND TREATMENT OF CONGENITAL CHOLEDOCHAL CYSTS IN CHILDREN

    Objective To investigate the diagnosis and treatment of congenital choledochal cysts(CCC) in children. MethodsThe manifestation, auxiliary examination, mode of operation and effect of 42 patients with CCC in children from Jan., 1980 to June, 1999 were analyzed retrospectively.Results The patients with the triad of jaundice, an abdominal mass, and pain was 38.1% among the 42 patients. B-ultrasonic diagnosis was made with a correct diagnostic rate of 95.2%. The effective rate of internal drainage was significantly lower than that of resection of the cyst(χ2=19.36, P<0.001) while the reoperation rate and incidence of carcinoma of internal drainage were higher than those of resection of the cyst(χ2=11.59, P<0.001 and χ2=4.97, P<0.05). Conclusion B-ultrasonic diagnosis is recommended as the first examination method. Internal drainage should be abandoned. Resection of the cyst with Roux-Y hepaticojejunostomy is recommended as the treatment of choice on extrahepatic cholangiectasis. Liver transplantation is a reasonable choice to treat the diffuse intrahepatic cholangiectasis.

  • THE DIAGNOSIS AND SURGICAL TREATMENT OF MULTIPLE PRIMARY COLORECTAL CARCINOMA

    【Abstract】Objective To analyze the clinical features of multiple primary colorectal carcinoma(MPCC). Methods Data in 21 patients with MPCC during the past 10 years in our hospital were analyzed retrospectively. Results The incidence of synchronous and metachronous carcinoma was 1.1% and 1.2% respectively. The sites and pathologic stages of tumors showed no significant difference compared with single colorectal carcinoma. 47.6% of the cases accompanied with colorectal adenoma. 77.8% of the MPCC could be found during operation. Patients with carcinoma involved rectum had relatively poor survival. Conclusion The full-course colonoscopy, careful intraoperative exploration and regular postoperative colonoscopic follow-up are essential in improving the diagnosis and prognosis of patients with MPCC.

  • SURGICAL TREATMENT AND PROGNOSIS OF CONGENITAL INTESTINAL ATRESIA(REPORT OF 40 CASES)

    【Abstract】Objective To investigate the operative methods and the factors affecting the prognosis of congenital intestinal atresia. Methods The clinic data of 40 cases of congenital intestinal atresia was reviewed, including duodenal atresia (6), jejunal atresia (12), ileum atresia (20) and colonic atresia (2). The types of atrasia were septal type (10),blindend type (26), and multisegmental type (4). All had been diagnosed before operation, 4 patients refused the treatment and 26 cases accepted the operation. Results Twenty-six cases of the 36 which accepted operation were cured and survived,the rate of postoperative survival were 72%,postoperative follow-up of 20 patients for 1-21 years had well-pleasing curative effect.Ten cases died postoperatively. Conclusion Operation is the only treatment of this disease,the mode of operation should be selected depending on the site and the type of atresia. The curative effect and the prognosis of this disease are affected by multiple factors.

  • LONG-TERM RESULTS OF SURGICAL TREATMENT OF CHRONIC PANCREATITIS

    To evaluate the present status of treatment of chronic pancreatitis, 116 consecutive patients with chronic pancreatitis during the last decade (1986~1996) have been surveyed retrospectively. The clinical date has been analized statisticaly. Etiology: biliogenic 56 cases (48.3%), alcoholic 17 (14.6%), idiopathic 34 (29.3%) and other 9 cases (7.8%). Better result was achieved in surgical treatment group (81 cases) than in conservative group (35 cases), pain free: 65.5% vs 33.3%. The accumulated five-year survival rate was 56.3%, 92.2% and 78.1% in alcoholic, billiogenic and idiopathic type of chronic pancreatitis respectively. Morbidity and mortality of diarrhea and diabetes mellitus increased at followup. The authors conclude that the chronic pancreatitis patients associated with obstruction of biliopancreatic duct should undergo operation early and will ameliorate abdominal pain.

  • THE IMPORTANCE OF SURGICAL TREATMENT OF MASSIVE UPPER GASTROINTESTINAL HEMORRHAGE CAUSED BY ACUTE GASTRIC MUCOSAL LESIONS

    From 1984 to 1994, 196 patients with massive upper gastrointestinal hemorrhage (hemorrhagic gastritis 137 cases, gastric ulcer 59 cases) caused by acute gastric mucosal lesions were treated in our hospital. As soon as the diagnosis was established, the stress factors in relation to acute gastric mucosal lesions and the factors damaging gastric mucosal barrier should he dispeled and hypovolemia should he corrected. In this group, the operative mortality were as follow: stress ulcer 6.3%, hemorrhagic gastritis 33.3%. According to this result, we consider that in cases of hemorrhagic gastritis the surgical operation must be considered with great care, but for stress ulcer with massive bleeding energetic surgical operation should be taken.

  • Clinical Characteristics and Surgical Treatment of Thoracic Hemangioma

    Abstract: Objective To investigate the clinicopathological characteristics, improve the accuracy of clinical diagnosis, and reduce postoperative complications of thoracic hemangioma. Methods Clinical records of 9 patients with thoracic hemangioma who underwent surgical resection in West China Hospital of Sichuan University from January 2006 to August 2012 were retrospectively analyzed. There were 2 males and 7 females with their average age of 47.9±19.3 (18-71)years. Six patients underwent thoracotomy, 1 patient underwent complete video-assisted thoracoscopic surgery (VATS), 1 patient underwent video-assisted mini-thoracotomy, and 1 patient underwent staged operations which were performed by neurological surgeons and thoracic surgeons separately. Imaging characteristics, intraoperative gross tumor features, and pathological characteristics of resected hemangioma specimens were analyzed. Clinical outcomes of different surgical strategies for the treatment of hemangioma were compared. Results Nine patients with thoracic hemangioma were included in this study. Imaging studies showed the tumor as a round-shaped mass in 66.7% (6/9) of these patients and a lobulated mass in the remaining 33.3% (3/9) patients. Magnetic resonance imaging (MRI) assessment was helpful for preoperative diagnosis of hemangioma, with T2 weighted imaging tumor enhancement as the main MRI feature. Thoracic operation time was 106.3±60.1 (60-192) min, and intraoperative blood loss was 91.1±43.7 (30-150) ml. All the patients were followed up for 3-6 years except 1 patient who was followed up for 1 month. None of the patients had hemangioma-related death, recurrence or metastasis during follow-up. Conclusions Thoracic hemangiomas are usually benign entities and often locate in the mediastinum. Surgical strategies should be determined by the size and location of the tumor as well as the surgeon’s technique level. VATS has the advantages of being minimal invasive, causing less intraoperative blood loss and shorter length of hospital stay for the treatment of hemangioma. Resection of a dumbbell-type hemangioma may need cooperation between neurological and thoracic surgeons.
