  • Targeted Surveillance of Nosocomial Infection in Neurosurgical Intensive Care Unit

    ObjectiveTo analyze targeted surveillance results of nosocomial infection in Neurosurgical Intensive Care Unit (ICU) and investigate the characteristics of nosocomial infection, in order to provide reference for constituting the intervention measures. MethodsWe monitored the incidence of nosocomial infection, the application and catheter-related infection of invasive operation, and the situation of multiple resistant bacteria screening and drug resistance characteristics of each patient who stayed more than two days in neurosurgical ICU during January to December 2013. ResultsThere were a total of 1 178 patients, and the total ICU stay was 4 144 days. The nosocomial infection rate was 4.92%, and the day incidence of nosocomial infection was 13.75‰. The nosocomial infection rate was significantly higher in January and between July and December compared with other months. Ventilator utilization rate was 9.75%; ventilator-associated pneumonia incidence density was 14.85 per 1 000 catheter-days; central line utilization rate was 28.40%; central line-associated bloodstream infection incidence density was 0.85 per 1 000 catheter-days; urinary catheter utilization rate was 97.90%; and the incidence density of catheter-associated urinary tract infection was 0.25 per 1 000 catheter-days. ConclusionThe nosocomial infection rate has an obvious seasonal characteristic in neurosurgical intensive care unit, so it is necessary to make sure that the hospital infection control full-time and part-time staff should be on alert, issue timely risk warning, and strengthen the risk management of hospital infection.

  • Targeted Surveillance and Risk Factors Analysis on Surgical Site Infection after Colorectal Resections

    ObjectiveTo investigate the risk factors for surgical site infection (SSI) in patients after colorectal surgery, in order to provide a basis for regulation and implementation of preventive measures against SSI. MethodsFrom February to December 2012, a targeted surveillance on surgical site infection of "colon resection" and "rectum resection" surgery patients in the Department of Gastrointestinal Surgery was carried out. We analyzed the monitoring data, and explored the occurrence of postoperative SSI. At the same time, by case-control study, both single and multiple regression logistic analyses were performed on the 12 variables such as hypertension, diabetes mellitus duration during operation, America Society of Anesthesiologists score, grade of incision and so on to analyze the risk factors for SSI. ResultsAmong the 535 patients who underwent colorectal resections, 44 had SSI with an infection rate of 8.22%. Multiple logistic regression analysis showed that the length of hospital stay[OR=1.070,95%CI(1.033,1.109), P<0.001]and emergency surgery[OR=6.320,95%CI(1.932,20.669),P=0.002] were independent risk factors for SSI after colorectal resections. ConclusionThere are many risk factors for SSI after colorectal surgery. Through the implementation of targeted surveillance, we can find the main risk factors, which provides a basis for the regulation and implementation of intervention measures against SSI.

  • The Targeted Surveillance of Newly-defined Ventilator-associated Pneumonia

    ObjectiveTo carry out targeted surveillance on ventilator-associated pneumonia (VAP) newly defined by the Centers for Disease Control and Prevention of the United States in 2013, and to understand its applicability and influence on the prognosis, and infection rate and risk factors of the disease. MethodsTargeted surveillance was carried out on all patients receiving mechanical ventilation in the general ICU of our hospital between January and December 2014. VAP infection rate was studied, and patients were divided into groups based on the development of the disease. SPSS 18.0 was used for statistical analysis of the prognostic indicators. ResultsA total of 885 patients received mechanical ventilation and were monitored, 31 of whom had VAP. The VAP case infection rate was 3.5% and its daily infection rate was 3.9‰. The results of multiple factors regression analysis showed that age (OR=1.025, P=0.025) and combining other types of hospital infection (OR=4.874, P<0.001) were independent risk factors for the development of VAP. VAP was the independent risk factor for both length of stay in the ICU and length of mechanical ventilation (P<0.001), but it was not the independent risk factor for mortality in the ICU (P=0.515). ConclusionThe applicability of the newly defined ventilator-associated pneumonia may be under restrictions in developing countries. It may influence the outcomes of patients by prolonging the length of stay in ICU and the length of mechanical ventilation.
