  • Surgical Treatment of Tetralogy of Fallot with Anomalous Coronary Artery

Abstract: Objective To summarize our surgical experience of tetralogy of Fallot(TOF) with anomalous coronary artery(ACA), explore diagnostic method of ACA, and evaluate surgical strategy choices and clinical outcomes of right ventricular outflow tract(RVOT) reconstruction. Methods From January 2004 to January 2010, 29 patients with TOF and ACA underwent total correction in Wuhan Asia Heart Hospital. There were 18 male patients and 11 female patients with their median age of 7 years (5 months to 33 years)and median body weight of 18 (5 to 51) kg at operation. Their preoperative arterial oxygen saturation was 65%-91%. One patient underwent RVOT enlargement and repair of ventricular septal defect via right atrial approach. Three patients underwent RVOT enlargement, repair of ventricular septal defect and main pulmonary artery enlargement using autologous pericardium patch via right atrial approach. Three patients received pulmonary artery translocation (REV) technique. Five patients received double outlet technique. Eleven patients underwent RVOT enlargement via incisions above, below or beside coronary arteries (single patch or two patch technique). Six patients underwent RVOT reconstruction using trans-annular patch after coronary artery bypass grafting. Results The median cardiopulmonary bypass time was 78 (65-102) min, median aortic crossclamp time was 50(40-82) min, and median operation time was 150 (126-178) min. There was no early death or severe coronary artery injury. Two patients underwent reexploration because of postoperative bleeding. Two patients had low cardiac output and were both cured with inotropic support. The median follow-up period was 51 (21-83)months and there was no late death during follow-up. All the patients were in New York Heart Association (NYHA) classⅠduring follow-up, their left ventricular ejection fraction was normal, there was no sign of myocardial ischemia in electrocardiogram, and their arterial oxygen saturation was 96%-99%.Mean early RVOT gradient (△P) was 19 (8-38) mm Hg, and the RVOT gradient (△P) did not increase during follow-up. Conclusion Preoperative diagnosis of ACA in TOF patients can be made by 64-slice multislice compute tomography (64-MSCT). Proper surgical strategy for RVOT reconstruction should be chosen according to the distribution of coronary arteries to achieve satisfactory surgical outcomes.

  • Treatment Progress of Chronic Postoperative Pulmonary Regurgitation after Surgical Repair of Tetralogy of Fallot

Abstract: With the evolvement of surgical, anesthetic, perfusion, and perioperative management techniques, the mortality of routine corrective operation of tetralogy of Fallot(TOF) has been less than 2%-3%, while more and more attention has been paid to patient long-term prognosis. Chronic postoperative pulmonary regurgitation (PR) is one of the important prognostic factors which are puzzling cardiac surgeons. Subsequent right ventricle dilation, right ventricular dysfunction and fatal arrhythmias as chronic PR complications have important impact on the postoperative quality of life and long-term survival. Current treatment choice for PR includes pulmonary valve replacement(PVR), intervention, and hybrid procedures. PVR is the main surgical method for chronic PR which can significantly improve patient cardiac function and clinical symptoms, and prolong long-term survival. Intervention does not require thoracotomy once again, and can thus reduce surgical trauma and risks. Intervention is also helpful to improve right ventricular pressure conditions and PR degree. This article focuses on the pathophysiological changes of chronic PR after surgical repair of TOF, surgical indications for PVR, clinical treatment progress and early prevention of PR.

  • Right Ventricular Outflow Tract Reconstruction with Valved Bovine Jugular Vein Patch in Patients with Complex Congenital Heart Disease

Objective To evaluate early results of valved bovine jugular vein patch for reconstruction of the right ventricular outflow tract (RVOT).?Methods From May 2009 to March 2010, a total of 60 patients with complex congenital heart diseases underwent reconstruction of RVOT with valved bovine jugular vein patch in Wuhan Asia Heart Hospital. There were 42 males and 18 females with their mean age of 6.2±8.9 years (ranging from 5 months to 33 years) and mean body weight of 27.5±24.0 kg, and 34 patients were less than 1 year. Preoperative clinical diagnosis included tetralogy of Fallot (n=38) and double outlet of right ventricle with pulmonary stenosis (n=22). All the patients underwent one-stage surgical repair. Before operation, 4 patients underwent catheter intervention for their major aortopulmonary collaterals. The diameters of pulmonary arterial ring of all the patients were 2 standard deviation less than normal range, and trans-annular patch was chosen for RVOT reconstruction. All the patients were postoperatively followed up for 18 to 26 months (mean 21.2±4.6 months).?Results There was no in-hospital death. And no second surgical intervention was needed for conspicuous RVOT stenosis or pulmonary regurgitation. Three patients needed reintubation for lung edema after extubation as a result of major aortopulmonary collaterals. Four patients underwent reexploration for postoperative bleeding. And all the other patients were discharged uneventfully. Mean cardiopulmonary bypass time was 84.0±22.0 min, and mean aortic cross-clamping time was 42.0±12.0 min. Mean RVOT gradient right after surgery was 18.0±4.5 mm Hg, which was not statistically different from mean RVOT gradient of 19.2±5.4 mm Hg measured by transthoracic echocardiography at their last postoperative follow-up(P>0.05). The degree of pulmonary regurgitation right after surgery was trivial in 32 patients(1+), mild in 28 patients(2+), which were not statistically different from the degree of pulmonary regurgitation at their last postoperative follow-up: trivial in 28 patients (1+), mild in 27 patients(2+), and moderate in 5 patients(3+). Calcification was not observed on the valved bovine jugular vein patch and valve cusp, and the valve cusp motioned well. No thrombosis or endocarditis was observed on the valved bovine jugular vein.?Conclusions For patients with tetralogy of Fallot or double outlet of right ventricle (DORV) and pulmonary stenosis, valved bovine jugular vein patch is a good choice for trans-annular reconstruction of RVOT. There is no severe postoperative complication related to bovine jugular vein, the RVOT pressure gradient does not increase significantly, and anti-regurgitation result is satisfactory in short-term follow-up. Further follow-up is required to evaluate its long-term outcome.

  • Immediate and Intermediate Outcomes of Correction on Patients with Tetralogy of Fallot and Absent Pulmonary Valve

Objective To summarize the immediate and intermediate outcomes of surgical correction on patients with tetralogy of Fallot and absent pulmonary valve (TOF/PVAB). Methods From January 1996 to August 2009, 14 patients, including 5 males and 9 females, aged 3.4±3.4 years (0.2-11.0 years) with an average weight of 12.0±6.3 kg (4-26 kg), underwent complete surgical correction in Beijing Fu Wai Cardiovascular Hospital. The right ventricular outflow tract was reconstructed with valved conduit in 4 patients, and monocusp with transannular patch was used in 10 patients. Six patients underwent pulmonary artery wall reduction, and 2 patients underwent both pulmonary artery plication and wall reduction. Results There were 2 (14.3%) perioperative deaths. Both were low bodyweight infants. One died of low cardiac output and respiratory failure, and the other died of central nervous system complications. Ten patients were followed up for an average time of 8.3±4.3 years (0.6-13.0 years). All patients followed up survived. The echocardiogram found pulmonary valvular dysfunction in 4 patients. The patients’ cardiac function were classified as New York Heart Association(NYHA) Ⅰ to Ⅱ. There was no late death or reoperation. Conclusion The immediate and intermediate outcomes of surgical correction of TOF/PVAB are good, but the function of pulmonary valves and conduit should be followed-up closely.

  • Surgical Intervention for Tetralogy of Fallot in Early Infancy

Objective To summarize the experiences of surgical intervention for tetralogy of Fallot(TOF) in early infancy and to discuss the relevant issues about primary treatment procedures in the period. Methods We retrospectively analyzed the clinical operative information of 21 patients in their early infancy (less than 6 months) with TOF treated in Children’s Hospital of Shanghai from June 2008 to August 2010. There were 14 males and 7 females with a mean age of 4.86±1.15 months and a mean body weight of 6.84±1.33 kg. All patients were diagnosed by heart color Doppler ultrasound. Four patients underwent CT or magnetic resonance imaging(MRI) or right heart catheter arteriography examination. The McGoon ratio was 1.86±0.41 and the pulmonary artery index(PAI) was 142.54±59.46 mm2/m2. The ventricular septal defect (VSD) was closed with autologous pericardium using continuous sutures through right atrium (19 cases) or right ventricle (2 cases). Transannular repair was performed when pulmonary valve annulus was one standard deviation less than the normal Z value (18 cases). If the annulus diameter approached or reached the normal Z value, the valve annulus was preserved and pericardium was used to enlarge the right ventricular outflow tract(RVOT) and the main pulmonary artery (3 cases). Results There was one death due to heart failure on the 15th day after operation, one patient had acute laryngeal edema after removal of endotracheal intubation on the second day after operation, and received reintubation and assisted ventilation for three days. All the other patients recovered well. Eighteen patients were followed up for 9.89±6.47 months. Their heart functions were in modified Ross class I or II. Echocardiography during the followup showed that RVOT pressure was 21.20±12.27 mm Hg (8.10-45.14 mm Hg); pulmonary incompetence (PI) was mild in 10 cases, moderate in 5 cases, and no severe PI occurred. Two cases of residual VSD were spontaneously closed. Compared with the early postoperative period, RVOT pressure and PI levels were not significantly different (Pgt;0.05). Right heart function was good.onclusion Early complete repair of TOF yields good surgical results. Transatrial repair of intracardiac pathology and retaining pulmonary valve annulus can be safely applied to yield good postoperative right ventricular function.

  • Longterm Results of Tetralogy of Fallot in Adults

Abstract: Objective To evaluate the longterm results of surgical treatment of tetralogy of Fallot (TOF) in adults and discuss the perioperative treatment skills. Methods From January 2000 to March 2008, 149 patients older than 14 years with tetralogy of Fallot received surgical treatment in Changhai Hospital. Among the patients, there were 78 males and 71 females with ages ranged from 14 years to 53 years and the average age was 26.3 years. Twenty patients had previous pulmonary arterial shunts before radical treatment. A total of 129 patients underwent primary radical treatment. Thirtyeight patients received a right ventricular outflow tract patch, 107 patients had transannular patch, and 4 patients had homograft aorta with valves. Results Hospital mortality was 4.0%(6/149). Four patients died of low cardiac output syndrome (LCOS), and multiple organ failure, and 2 patients died of acute renal failure. The postoperative complications included pleural effusion in 11 patients, pulmonary edema in 10 patients, severe LCOS in 9 patients, severe cardiac arrhythmia in 7 patients, reoperation for excessive bleeding in 7 patients, reintubation in 6 patients, and residual ventricular septal defect (VSD) in 5 patients (two of them had reoperation for residual VSD repair and 2 received transcatheter closure of VSD). One hundred and thirtyfour patients were followed up for 3 to 102 months (47.2±28.6 months) with a followup rate of 93.7%(134/143). Late death occurred in 2 patients, one of whom died of secondary infective endocarditis and the other had a sudden death 29 months after operation. During the followup, one patient had residual VSD (2 mm), but had a normal life. The peak systolic right ventricletopulmonary artery pressure gradient exceeded 40 mmHg in 4 patients. Two patients had severe pulmonary regurgitation. A total of 132 patients survived and had an improved life. One hundred and twentyone patients had class Ⅰ heart function (NYHA), and 11 patients in class Ⅱ. Conclusion The pathophysiologic conditions of the patients with tetralogy of Fallot in adults are very complicated due to longterm right ventricle outlet stricture and chronic hypoxia. Preoperative evaluations and postoperative treatment of complications are necessary. The systemicpulmonary arterial shunts should be performed when hypotrophy of the pulmonary arteries or left ventricles exists. Repair of tetralogy of Fallot in adults has acceptable morbidity and mortality rates with goodlongterm outcomes.

  • Staging and OneStop Hybrid Approach for Surgical Treatment of Tetralogy of Fallot with Aortopulmonary Collateral Artery

Objective To improve the operative effects of patients who had tetralogy of Fallot with aortopulmonary collateral arteries (TOF-APCAs) and evaluate the clinical effects of staging and onestop hybrid approach for TOFAPCAs. Methods From January 2003 to December 2007, thirty patients with TOF-APCAs had undergone combined therapy of APCAs embolization and complete surgical repair. Fifteen patients had APCAs embolization therapy before or after TOF radical operation(staging hybrid group ); Fifteen had onestop hybrid treatment(onestop hybrid group). Results Angiography revealed that there were 19 APCAs in staging hybrid group, and of which 15(78%) were embolized successfully. Five cases had complications and one died from respiratory circulating failure. The rest all recovered and discharged. And 22 APCAs were found in one-stop hybrid group, eighteen (82%) of them were embolized successfully. Only one case had pulmonary effusion. The time of hospitalization(median 37 d vs. 22 d, P=0.011),ICU staying(median 7.0 d vs. 4.7 d,P=0.029)and endotracheal intubation(median 131 h vs. 19 h,P=0.009) was obviously longer, and the hospitalization expenses(median 64 101 [CM(159mm]yuan vs. 48 021 yuan, P=0.033)were obviously higher in staging hybrid group than that in one-stop hybrid group.And there was no statistical significance in cardiopulmonary bypass time(P=0.126) and aortic clamping time(P=0.174) between two groups. Conclusion In comparison with traditional staging hybrid approach, one-stop hybrid approach can simplify the operative process for patients who have TOFAPCAs, improve the operative successful rate and cut down expenses.

Objective To compare perioperative results between transventricular and transatrialtransventricular approaches in repairing tetralogy of Fallot (TOF), and to improve the surgical results. Methods The data of 1 423 consecutive patients who underwent complete repair of TOF between January 1998 and December 2007 were reviewed. 736 patients were repaired by the transventricular approach, and 687 patients by the transatrialtransventricular approach. Results Patients repaired by transventricular approach decreased from 100% in 1998 to 65% in 2002, and by transatrialtransventricular approach increased from 35% in 2002 to 79% in 2007. Aortic clamping time, cardiopulmonary bypass (CPB) time, mechanical ventilation time, and intensive care unit (ICU) stay in patients repaired by transatrialtransventricular approach had less than those in patients repaired by transventricular approach. No difference in transvalve patch ratio. There was lower morbidity in patients repaired by transatrialtransventricular approach in one to two organ systems dysfunction than that in patients repaired by transventricular approach. No difference in three or more organ systems dysfunction between them. Rate of residual ventricular septal defect(VSD), right ventricule to mean pulmonary artery (MPA) pressure gradient, tricuspid regurgitation, pulmonary artery regurgitation and arrhythmia in patients repaired by transatrialtransventricular approach were less than those in patients repaired by transventricular approach. Reoperative rate and mortality in patients repaired by transatrialtransventricular approach were less than those in patients repaired by transventricular approach. Conclusion TOF repair by the transatrialtransventricular approach fits to the actual conditions in China.
