  • RELATED ISSUES IN CLINICAL TRANSLATIONAL APPLICATION OF ADIPOSE-DERIVED STEM CELLS

    Objective To introduce the related issues in the clinical translational application of adipose-derived stem cells (ASCs). Methods The latest papers were extensively reviewed, concerning the issues of ASCs production, management, transportation, use, and safety during clinical application. Results ASCs, as a new member of adult stem cells family, bring to wide application prospect in the field of regenerative medicine. Over 40 clinical trials using ASCs conducted in 15 countries have been registered on the website (http://www.clinicaltrials.gov) of the National Institutes of Health (NIH), suggesting that ASCs represents a promising approach to future cell-based therapies. In the clinical translational application, the related issues included the quality control standard that management and production should follow, the prevention measures of pathogenic microorganism pollution, the requirements of enzymes and related reagent in separation process, possible effect of donor site, age, and sex in sampling, low temperature storage, product transportation, and safety. Conclusion ASCs have the advantage of clinical translational application, much attention should be paid to these issues in clinical application to accelerate the clinical translation process.

  • TREATING SACROILIAC JOINT TUBERCULOSIS WITH RIFAMPICIN-LOADED OsteoSet

    ObjectiveTo assess the safety, feasibility, and effectiveness of medical calcium sulfate (OsteoSet) mixed with powder for injection of rifampicin after radical debridement in the treatment of sacroiliac joint tuberculosis. MethodsA retrospective analysis was made on the clinical data from 27 patients with sacroiliac joint tuberculosis who underwent debridement surgery and local bone graft of OsteoSet artificial bone impregnated with powder for injection of rifampicin between August 2006 and August 2010. There were 10 males and 17 females with an average age of 35.2 years (range, 16-64 years). The mean disease duration was 7.5 months (range, 1.5-16 months). The left sacroiliac joint was involved in 16 cases, the right side in 10 cases, and both sides in 1 case; Of them, 18 cases had iliac fossa abscess and 6 cases of buttocks abscess. According to the classification system by Kim, there were 9 cases of type Ⅲ and 18 cases of type ⅠV. The preoperative Majeed score was 61.23±6.49, including good in 4 patients, general in 19 patients, and poor in 4 patients. ResultsAll patients achieved wound healing by first intention; no complications of pelvic infection, peripheral nerve injury, and lower extremity deep vein thrombosis occurred. Twenty-six patients were followed up 16 months on average (range, 12-24 months). All the patients had normal sense and movement of both lower limbs; no perineum sensory disturbance was found. One case of multi-drug resistant had local recurrence at 3 months after operation, which was cured after adjusting anti-tuberculosis drugs, nutritional support, enhancing immunity with thymopentin, and second operation. Bone union was observed at 10.5 months on average (range, 9-12 months) in 25 patients, and clinical symptoms disappeared with no recurrence or complication, and they returned to previous work. The Majeed score at last follow-up was 92.31±3.36, showing significant difference when compared with preoperative score (t=-32.76, P=0.00). The results were excellent in 22 patients, good in 4 patients, and the excellent and good rate was 100%, showing significant follow-up was 92.31±3.36, showing significant difference when compared with preoperative score (t=-32.76, P=0.00). ConclusionBone graft interbody fusion surgery with rifampicin loaded OsteoSet is one of the effective methods to treat sacroiliac joint tuberculosis.

  • Construction of Medical Professionalism in the Era of Translational Medicine: from Research to Application

    This article briefly introduces the status and problems of research and application on medical professionalism in China; proposes the concept of translational medicine and the challenges of introduction of translational medicine to physician professionalism research and translation. Some measurements or approaches to promote research translation on medical professionalisms are proposed to shorten the distance from research outcomes to practical application, in order to carry out evidence-based construction of modern medical professionalism of Chinese characteristics and build a harmonious doctor-patient relationship.

It consistently provides the latest evidence to target customers according to its strict criteria in the specific fields. Clearinghouse is objectively and scientifically designed to assess the scientific nature and the practical applicability of evidence to provide the best current evidence to different service practitioners. It emphasizes much on the process from "knowledge production" to "knowledge utilization", which effectively fills the gap between scientific research and practice. It will play an increasing important role in the knowledge translation in the future. This article specifically introduces its history of development, characteristics and application and sets the construction of Chinese Clearinghouse for Evidence Translation in Child & Aging Health (CCET) as an example, fully elaborating the application status of Clearinghouse and providing assistance for evidence informed decision making.

  • Thoughts on the construction and management of academic societies of medical students

    Medical education is undergoing the transition from traditional classroom teaching to more diversified " inside-and-outside-class collaborative teaching”, that is, extracurricular teaching has become an important supplement to medical education. The academic societies of medical students are important platforms in medical education. With the increasing of academic societies in medical colleges, the coordination and balance between community construction and student development requires deep thinking and investigation. The society should develop under the ideas of " following the initial intention - realizing transformation - promoting innovation”. The society-based extracurricular teaching should be performed in the manner of capacity-directing " translational teaching”. The society needs to carry out the " personalized and advanced” training to improve the scientific research or innovation capacity of individual medical student. This paper aims to discuss key points on the construction and management of academic society of medical students at the micro-level, to provide references for single society’s development.

  • Open innovation of national infrastructure for translational medicine

    The Chinese government invested National Major Science and Technology Infrastructure for Translational Medicine (NITM) is a research platform with large-scale science facilities designed for opening and sharing. NITM is required to share its research facilities to domestic and international users and to facilitate interdisciplinary cooperation and innovation, it is also required to cooperate with other national major science and technology infrastructures so that these infrastructures can work together to enhance the nation’s international competitiveness in science and technology. This paper proposes to apply the concept of open innovation and designs an innovation model for the management of NITM.

  • Construction and application of clinical evidence database of traditional Chinese medicine

    To describe the construction and application of clinical evidence database of traditional Chinese medicine (TCM-CED) so as to provide evidence for TCM research. The construction process primarily includes: expert team building, TCM-CED function module design, evidence collection and quality control. The applications of TCM-CED primarily include the following aspects: automatic generation of systematic review/meta-analysis in TCM, automatic generation of evidence reports on dominant diseases of TCM, automatic generation of evidence index of Chinese patent medicine, optimizing the selection of outcomes in TCM research, tracking methodological and reporting quality of TCM research, and promoting international dissemination of TCM evidence. With the rapid development of information technology and artificial intelligence, TCM-CED will be combined with artificial intelligence to achieve the construction of all-dimensional TCM evidence chain and the automation of the whole process.

  • Analyzing and advising on clinical outcome assessment measurements translation and cross-cultural adaptation

    With the transformation of modern medical models, patient-reported outcomes, clinician-reported outcomes, observer-reported outcomes, and performance outcomes have become internationally recognized clinical outcome assessment indicators, and scales have also become important evaluation tools, among which translation and cross-cultural adaptation are one of the important sources of scales. However, at present, there are fewer guidelines for scale translation in China. At present, domestic scale translation has not yet been unified and standardized in clinical reporting. Most translation reports provide readers with incomplete information, which affects the development of scale translation, and the methodology related to the translation of clinical outcome assessment scales still focuses on patient-reported outcome scales, which creates a gap in terms of the recommendations for the rest of the types of translations, a gap which leads to inconsistencies in the translation methodology and process. In this paper, we will develop specific translation methods and processes for each of the four current types of clinical outcome assessments by combining scale translation guidelines to support a standardized approach to translation, cross-cultural adaptation, and linguistic validation for use in standardizing the process of recommending translations of patient-reported outcome scales, clinical-reported outcome scales, observer-reported outcome scales, and behavioral outcome scales.

  • An emerging discipline: brain-computer interfaces medicine

    With the development of brain-computer interface (BCI) technology and its translational application in clinical medicine, BCI medicine has emerged, ushering in profound changes to the practice of medicine, while also bringing forth a series of ethical issues related to BCI medicine. BCI medicine is progressively emerging as a new disciplinary focus, yet to date, there has been limited literature discussing it. Therefore, this paper focuses on BCI medicine, firstly providing an overview of the main potential medical applications of BCI technology. It then defines the discipline, outlines its objectives, methodologies, potential efficacy, and associated translational medical research. Additionally, it discusses the ethics associated with BCI medicine, and introduces the standardized operational procedures for BCI medical applications and the methods for evaluating the efficacy of BCI medical applications. Finally, it anticipates the challenges and future directions of BCI medicine. In the future, BCI medicine may become a new academic discipline or major in higher education. In summary, this article is hoped to provide thoughts and references for the development of the discipline of BCI medicine.
