  • Subfascial Endoscopic Communicating Branch Vein Ablation in Treating Communicating Branch Vein Insufficiency (Report of 57 Cases)

    bjective To study the effectiveness of subfascial endoscopic communicating branch vein ablation in treating communicating branch vein insufficiency.MethodsFiftyseven cases of primary lower extremity vein valve insufficiency complicated by communicating vein insufficiency diagnosed by venography and color duplex were treated with subfascial endoscopic communicating branch vein ablation, and their information was analyzed retrospectively.ResultsSwelling was observed in leg of 7 cases after operation, and disappeared within one week spontaneously. Incision infection occurred in 2 cases and skin ambustion happened in 2 cases. Superficial vein varicosis disappeared in all case, skin pigmentatin declined significantly and ulcers healed in 2-3 weeks.Conclusion Subfascial endoscopic communicating branch vein ablation is an excellent choice in treating communicating branch vein insufficiency.

  • PROGRESS OF ROBOTIC SYSTEM APPLICATION IN VASCULAR SURGERY

    Objective To review the progress of the robotic applications in vascular surgery. Methods Recent literature about the robotic applications in vascular surgery was reviewed and analyzed. Results Robotic system is composed of surgery robotic system and endovascular interventional robotic system. The time of aortic clamping and anastomosis is reduced considerably during the robotic-assisted aorta bypass surgery, and the dissection of aorta is completed successfully in totally robotic approach. Endovascular interventional robotic system has good performance in navigation and stability, and shows apparent advantages in passing special anatomical segment and complicated lesion. However, the robotic systems are still limited in application for high cost. The problem of tactile feedback should also be solved quickly. Conclusion Robotic systems have apparent advantages and good prospect in vascular surgery. Nevertheless, it still require many clinical trials to formulate the indication and contraindication, to establish standard procedure, to assess the long-term effectiveness of the robotic systems and so on.

  • SUBSTITUTE VALVE AT POPLITEAL VEIN IN TREATING DEEP VENOUS VALVE INSUFFICIENCY OF LOWER EXTREMITIES

    Objective To study the effectiveness of substitute valve at the popliteal vein in treatment of deep venous valve insufficiency of lower extremities. Methods From January 1996 to August 2002, 27 patients were diagnosed having deep venous valve insufficiency of lower extremities by color Doppler and radiography with an average disease course of 17.4 years.All 27 patients had varicose vein, 25 pain, 22 swelling,25 pigmentation in ankle area and 19 chronic ulcerations.Two cases had been treated with great saphenous vein ligation and striping.Averagevein pressure in resting position was (11.00±0.73)kPa,and the ambulatory venous pressure was (9.14±0.68)kPa.All patients were treated with substitute valve at the popliteal vein,and great saphenous vein ligation and stripping, some were treated with subfascial endoscopic perforating veins ablation. Results The average ambulatory venous pressure after operation was (5.94±0.82)kPa,were significantly different from that before operation(P<0.01). The curative results were satisfactory,and all symptom and physical sign disappeared.After a mean follow-up period of2-6 years,21 cases had satifactory results. Conclusion Substitute valve at the popliteal vein have the value of widespread application.

  • Current Status of da Vinci Surgical System in Vascular Surgery

    ObjectiveTo review the current status of da Vinci surgical system in vascular surgery. MethodsRecent literatures published in Chinese or English about the application of da Vinci surgical system in vascular surgery were reviewed and analyzed. Resultsda Vinci surgical system was mainly applied in diseases such as aortoiliac occlusive disease, aneurysm, typeⅡ endoleak after endovascular aortic repair, celiac artery compression syndrome, and so on. It was used to expose arteries, or assist to ligate and dissect arteries. da Vinci surgical system showed apparent advantages in shorting the time of aortic clamping, however, the high cost, the loss of tactile feedback, and the long learning curve may be its shortcomings. Conclusionda Vinci surgical system is safe and effective in vascular surgery, while much more experience is required to evaluate its long-term effect and complication.

  • The feasibility study of bilingual teaching rounds in vascular surgery teaching

    Objective To analyze the feasibility, possible problems and application of bilingual teaching rounds in vascular surgery teaching. Methods Ninety participants included internship, interns and residents of standardized training from 2015 to 2016 in vascular department of West China Hospital of Sichuan University were included. The questionnaire named the feasibility of bilingual rounds in surgery teaching was done and analyzed in all of these students. The results of the evaluation score from different teachers on their teaching process were also analyzed. Results Three groups could accept bilingual teaching rounds of this teaching form, and all of them were willing to be involved in, but their English levels and acceptance participate times were different. The CET4 levels of three groups did not have any difference; however, the CET6 levels had significant difference. There were significant differences in the preparation times and the review times (all P values <0.05), and there were no difference in acceptance, value and promote degree (all P values >0.1). In different teaching teachers' self assessment on the same objective, there was significant differences (all P values <0.05). Conclusion Bilingual rounds in surgery teaching have certain feasibility. Different adjustments are needed according to the English level and the grade, and higher request of the teacher is necessary.

  • The role of artificial intelligence robot in the development of vascular surgery technology

    Artificial intelligence in robot system is mainly divided into two types: endoscopic robot system and intracavitary robot navigation system. The endoscopic robot system can effectively shorten the time of vascular anastomosis and occlusion during vascular bypass surgery, while the intracavitary robot navigation system has good localization and real-time observation function. Moreover, it has significant advantages in complex lesions and special anatomical locations. High cost and complicated equipment debugging are the main factors that limit the wide application of robot systems. Artificial intelligence represented by robot system has obvious advantages and broad prospects in the field of vascular surgery, but more research is needed to improve its shortcomings and to further clarify its standard operation and long-term results.
