  • Research Progression of Zinc Deficiency after Gastric Bypass Surgery

    Objective To summarize and analyze the research progression of zinc deficiency in patients with type 2 diabetes mellitus or obesity after Roux-en-Y gastric bypass (RYGB) surgery. Method The domestic and international published literatures about zinc deficiency after RYGB in recent years were reviewed. Results There was a degree of zinc deficiency after RYGB surgery, its mechanisms had not been fully clarified, which were related to reduced intake and absorption of zinc, protein malnutrition, dietary factors, and specific ways of surgery and the zinc supplementation programmes after operation would also affect the postoperative zinc nutritional status. Conclusions Reasons for zinc deficiency after RYGB surgery are multifaceted and have not been clarified. Further research is needed to provide experimental and theoretical basis for management of zinc nutritional status after RYGB surgery.

  • Effect of Recombinant Human Growth Hormone on Growth of Human Colonic Cancer Line COLO-320 Cells in Vitro

    Objective To evaluate the effect of recombinant human growth hormone(rhGH) on growth of human colonic cancer cells (COLO-320) in vitro. Methods Human COLO-320 cells in logarithm growing period were cultured for 24 h,48 h or 72 h with variant concentrations of rhGH,camptothecine (CPT) or rhGH combined CPT in calf serum(serum group) or calf serum-free (serum-free group). Light density of cells were determined by MTT method, so that cellular inhibition rate were calculated.Results No influence on cell growth or inhibition rate was observed from cultures with variant concentrations and different acting times of rhGH (P>0.05). Inhibition rate of single CPT or CPT combined rhGH were much more increased than single rhGH used (P<0.01) with no statistical significance (P>0.05).Conclusion The results show that rhGH has neither direct COLO-320 cells stimulation nor any evidence of COLO-320 cells inhibition, and has no influence of CPT on COLO-320 cells inhibition in vitro.

  • Study of Relationship of Indoleamine 2,3-Dioxygenase to Immune Escape of Gastric Cancer

    Objective To explore the research status and development tendency of relation between indoleamine 2,3-dioxygenase (IDO) and immune escape of gastric cancer. Methods The related literatures about IDO, immune escape of gastric cancer, and their the relationship at domestic and international in recent ten years were collected and reviewed. Results Gastric cancers induced that dendritic cells expressed IDO by the CD4+ CD8+ regulatory T cells, which made the microenviroment tryptophan starvation, thus inhibited T cell proliferation. While tryptophan metabolites existed T cell cytotoxicity that inhibited T cell proliferation. IDO-specific inhibitor 1-MT combinated with chemotherapy drugs in the treatment for gastric cancer was a synergistic effect. Conclusions IDO as an immune modulating enzymes may play a key role in the process of immune tolerance that induced by gastric body. IDO may become a new target for inhibition of gastric malignancy formation and enhance the effectiveness of tumor immune treatment.

  • Detecting Free Cancer Cells in Peritoneal Cavity of Colorectal Cancer Patients by Branched-Chain DNA and SqRT-PCR

    Objective To evaluate branched-chain DNA (b-DNA) signal amplification and semi-quantitative (Sq) RT-PCR in detection of free cancer cells in peritoneal flushing fluid of colorectal cancer patients during surgery. Methods The CEA mRNA in peritoneal flushing fluid in 48 cases of colorectal cancer were detected by b-DNA and SqRT-PCR. Peritoneal flushing fluid cytology (PLC) was conformed simultaneously to detect the free cancer cells. The peritoneal flushing fluid of 12 cases with colorectal benign disease were taken as negative control, GAPDH mRNA as internal control. Results In colorectal cancer patients, positive rate of free cancer cells by bDNA and SqRT-PCR (43.8%, 31.3%) was higher than that by PLC (4.2%). The relative quantitative expressions of CEA mRNA were related to the Dukes staging, depth invasion and differentiation degree (Plt;0.05), but irrelevant to tumor size,the patients’ age and gender (Pgt;0.05).Conclusion Both b-DNA and SqRT-PCR technologies have advantages and disadvantages to detect free cancer cells in peritoneal flushing fluid, which are related to clinicopathological factors.

  • Clinical analysis of left-sided appendicitis: report of 2 cases and review of 212 published cases

    Objective To investigate the clinical features, diagnosis, and surgical methods of left-sided appendicitis (LSA). Methods We retrieved LSA-related literatures through Pubmed, Google Scholar English databases, Wanfang, CNKI, VIP, and SinoMed databases (published from January 1981 to June 2017), as well as 2 cases of LSA who treated in Beibei Traditional Chinese Medical Hospital, to analyze the clinical characteristics of LSA and its diagnosis and treatment methods. Results There were 92 articles in a total of 212 LSA patients were retrieved, and 2 cases treated in Beibei Traditional Chinese Medical Hospital, a total of 214 LSA patients were included in the analysis. Pain fixed position of LSA: 139 cases (65.0%) located in left-lower quadrant, 30 cases (14.0%) located in right-lower quadrant, 8 cases (3.7%) located in peri-umbilical, 15 cases (7.0%) located in mid-lower abdomen, 15 cases (7.0%) located in left-upper quadrant, 3 cases (1.4%) located in right-upper abdomen, 2 cases (0.9%) located in mid-upper abdomen, 2 cases (0.9%) located in pelvic cavity, respectively. LSA had occurred in association with several types of abnormal anomalies: 131 cases (61.2%) suffered from situs inversus totalis (SIT), 53 cases (24.8%) suffered from midgut malrotation (MM), 21 cases (9.8%) suffered from cecal malrotation, 4 cases (1.9%) suffered from long appendix, 2 cases (0.9%) suffered from free ascending colon, and 3 cases (1.4%) were unclear. The diagnosis of 114 LSA cases (53.3%) before operation was correct, in which the correct diagnosis rates of SIT-LSA and MM-LSA were 74.8% (98/131) and 22.6% (12/53), respectively. Three patients (1.4%) underwent conservative treatment, and 211 patients (98.6%) underwent surgical treatment, including 25 cases (11.7%) of laparoscopic surgery, 145 cases (67.8%) of open abdominal surgery, and unknown of 41 cases (19.1%). Laparotomy incision: abdominal incision in 74 cases (51.0%), ventral midline incision in 16 cases (11.0%), the left side of the anti McBurney incision in 43 cases (29.7%), right McBurney incision in 12 cases (8.3%). Conclusions LSA mainly occurs in association with 2 types of congenital anomalies: SIT and MM. There is some difficult to make diagnosis for abnormal anatomy and inaccurate pain location of LSA, so it is easy to cause the delay in diagnosis or misdiagnosis. For LSA, the choices of laparoscopy or laparotomy operation methods are applicable.

  • Effects of CO2 pneumoperitoneum on bacterial growth and translocation of acute peritonitis rats

    Objective To study the effects of different carbon dioxide pneumoperitoneum pressure and time on abdominal cavity infection bacteria of peritonitis in rats, including bacteria growth and bacterial translocation. Methods Sixty Sprague Dawley rats were injected with Eseherichia coli into the abdominal cavity to establish models of intra-abdominal infection. To give 3 types of pneumoperitoneum pressure for the experimental group: 15 mm Hg (1 mm Hg=0.133 kPa) for high pressure group, 5 mm Hg for low pressure group, and blank control group for no-pneumoperitoneum. To give 2 types of experimental period: 1 h and 3 h. These 60 Sprague Dawley rats were randomly divided intomoperi 6 groups by random number table. They were treated by different pneumoperitoneum pressure and time. All rats were killed at the end of the carbon-dioxide pneumo-peritoneum experiment. Peritoneal lavage fluids and portal vein blood were taken for microbiological examinations and culture. The endotoxin content in portal vein blood was detected too. Results ① Bacteria content: bacteria counts of different pneumoperitoneum pressure groups were obviously different (F=9.02, P=0.020), bacteria counts of different experimental period groups were obviously different (F=8.47, P=0.003), the effect of time was different in different pneumoperitoneum pressure groups (F=8.07, P=0.020). ② Bacterial translocation: Bacterial translocation occurred in all 6 groups. Blood culture positive rates were similar between 1 h group and 3 h group at 3 types of pneumoperitoneum pressure groups (P>0.05). The positive rate of blood culture in high pneumoperitoneum group was significantly higher compared with the no-pneumoperitoneum group (P<0.05). ③ The endotoxin content: the endotoxin content of different pneumoperitoneum pressure groups were obviously different (F=14.70, P<0.01), the endotoxin content in plasma increased obviously in high pressure group compared with low pressure group (P=0.018) and no-pneumoperitoneum group (P<0.01), the endotoxin content in plasma increased obviously in low pressure group compared with no-pneumoperitoneum group (P=0.005). The endotoxin content of different experimental period groups were obviously different (F=148.90, P<0.01), the endotoxin content in plasma increased obviously in 3 h group compared with 1 h group. There were no significant difference in the effect of time with different pneumoperitoneum pressure groups (F=0.14, P=0.874). Conclusion CO2pneumoperitoneum promoted intestinal bacterial endotoxin and bacterial translocation in peritonitis of rats, which increased with the pressure and time.

  • Pathological and Clinical Effects of Intraoperative Regional Chemotherapy of Rectal Cancer

    Objective To observe the histopathological changes of tumor tissue after intraoperative regional chemotherapy of rectal cancer. Methods After the treatment of intraoperative regional chemotherapy with 5-FU (15 mg/kg), the histopathological changes of tumor tissue were detected. Results Slight changes with cancer cells in all the cases accepted intraoperative regional chemotherapy were found under light microscope such as karyopyknosis, nuclear swelling, coagulation and necrosis of cytoplasm, hydropsia of intercellular substance, invasion of inflammatory cells, and 9/15 cases with slight inflammation of vessels were observed; While those changes were found in individual cells of the cases without regional chemotherapy. The more enlarged intercellular space of cancer cell was observed under electron microscope in the case with regional chemotherapy. Conclusion The intraoperative regional chemotherapy of rectal tumor can change the histopathological appearance of tumor tissue, that is significant in preventing cancer cells diffusing during operation and relapsing after operation.

  • Distribution and Expression of Estrogen Receptor β Protein in Sigmoid Colon of Female Patients with Slow Transit Constipation

    Objective To investigate the express of ERβ protein in female slow transit constipation (STC) patients. Methods Immunohistochemistry and Western blot technique were used to detect the distribution and expression of estrogen receptor β (ERβ) protein of 20 patients with STC and 20 aged-matched controls. Results ERβ expressions were detected in mucous layer, myenteric nerve plexus and submucous nerve plexus in two groups. In comparison with the control group, the expression of ERβ protein of STC group was much lower (Plt;0.01). The expression of ERβ protein of sigmoid colon in STC group was significantly lower than that in control group (Plt;0.05). Conclusion The expression of ERβ protein decreased in myenteric and submucous nerve plexus of sigmoid colon tissues may involve in the pathogenesis of STC.

  • Clinical Toxic and Adverse Reaction and Pathological Observation of Regional Arterial Perfusion Chemotherapy During Radial Resection of Gastric Cancer

    Objective To observe the drug distributional characteristics after regional arterial perfusion chemotherapy (RAC) during gastric cancer radical resection, postoperative histopathological change and clinical toxic and adverse reactions. Methods According to the indications of RAC, 60 patients admitted in this department from September 2007 to November 2008 were included and divided into treated group and control group randomly. Treated group underwent the treatment of RAC with the 100 ml perfusion fluid including 5-FU (1 000 mg/m2), MMC (10 mg/m2) and 2 ml methylene blue injection by which the control group were not treated. Then the methylene blue distributional characteristics during operation, postoperative histopathological changes of tumors and clinical toxic and adverse reactions were observed. Results In the treated group, after RAC with injection contained methylene blue by primary supply arterial, the tumor region colored immediately and then dropped slowly, but it presented blue during whole operation. After operation, light microscope examination revealed a mild change of karyopyknosis, nuclear swelling, coagulation of cytoplasm in cancer cells, mild hydropsia of intercellular substance, invasion of inflammatory cells and mild vasculitis in some cases. Transmission electron microscope showed that nuclear swelling or coagulation, nuclear heterochromatin agglutination, nuclear-week gap expansion, mitochondrial swelling, endoplasmic reticulum expansion, and Golgi complex expansion. AST of treated group increased apparently on the first day (Plt;0.01), and recovered normal on the third day (Pgt;0.05). There was no significant difference between the two groups in renal function, ALT, ALP, GGT, LDH of liver function, medullary restraining, ECG by bed or reaction of gastrointestinal tract (Pgt;0.05). And stomal leak was not found in two groups. Conclusions The RAC during radical resection of gastric cancer enables gastric tumor to expose to therapeutics during whole operation and depresses the activity of cancer cells. Its clinical toxicity is little, so it can be used as an important supplementary means to prevent intraoperational extension and postoperative recurrence.

  • Role of Vaspin in Roux-en-Y Gastric Bypass on Type 2 Diabetes Mellitus Rats

    Objective To study the therapeutic effect of Roux-en-Y gastric bypass (RYGB) on type 2 diabetes mellitus (T2DM) rats and explore the possible mechanism of vaspin in RYGB on T2DM. Methods Twenty SD rats with T2DM and 20 age- and sex-matched normal SD rats were randomly divided into 4 groups according to the random digits table:T2DM-RYGB group, T2DM-sham operation (SO) group,RYGB group,and SO group,10 rats in each group. Fasting plasma glucose (FPG) level,serum insulin (INS) level,vaspin level,and homeostasis model of insulin resistance (HOMA-IR) were determined before operation and on week 4,8 after operation,respectively.At the same time,the correlation between vaspin and the indicators (FPG,INS,or HOMA-IR) was analyzed.Results Compared the indicators after operation with before operation,the FPG level,INS level,vaspin level,and HOMA-IR were not significantly different between the T2DM-RYGB group and T2DM-SO group (P>0.05) or between the RYGB group and SO group (P>0.05),but the FPG level,INS level,vaspin level,and HOMA-IR in the T2DM-RYGB group and T2DM-SO group were significantly higher than those in the RYGB group (P<0.05) and SO group (P<0.05),respectively. On week 4 after operation,the FPG level,INS level,vaspin level,and HOMA-IR decreased in the T2DM-RYGB group,except for the FPG level,the other indexes had no significant differences as compared with the values before operation. On week 8 after operation,the FPG level,INS level,vaspin level,and HOMA-IR further decreased in the T2DM-RYGB group,there were significant differences of these indicators between before operation and on week 8 after operation. Compared the indicators after operation with before operation,the FPG level,INS level,vaspin level,and HOMA-IR were not statistically significant (P>0.05) in the T2DM-SO group,RYGB group,or SO group. The changes in serum vaspin level correlated positively with those in INS and HOMA-IR before operaion and on week 4,8 after operaion in the T2DM-RYGB group and T2DM SO group rats (P<0.05),respectively. Conclusions RYGB surgery has a therapeutic effect on T2DM rats,and serum vaspin level decreases and insulin resistance is improved after RYGB surgery,which may be one of the mechanisms of the treatment for T2DM.
