  • Progress of Research on the Mechanism of Inflammatory Response in Pathogenesis of Lung Contusion

    Abstract: As the most common blunt thoracic injury, lung contusion may develop into acute lung injury, adult respiratory distress syndrome or ventilation associated pneumonia, which can cause a high mortality. However, the pathogenesis and pathophysiology of lung contusion is not well understood yet. Stress is laid by many researchers on inflammatory response in the pathogenesis of lung contusion. We review the potential role of inflammatory response in the pathogenesis and pathophysiological changes of lung contusion. Emphasis is put on studies of inflammatory cells, mediators, receptors, surfactant dysfunction, and the potential role of epithelial cell or neutrophil apoptosis. The animal models are essential to the study of lung contusion and the studies examining secondary injuries exacerbating lung contusion are also noted.

  • Efficacy of Radiofrequency Modified Maze Procedure Combined with Open-heart Surgery for Atrial Fibrillation

    Objective To evaluate the efficacy of Radiofrequency (RF) modified maze procedure combined with open-heart surgery for atrial fibrillation (AF). Methods From January 2003 to October 2004, 66 patients underwent the RF modified maze Ⅲ procedure for AF combined with open-heart surgery. The preoperative and postoperative indexes of electrocardiogram and echoeardiogram were compared through retrospective analysis and follow-up. Postoperative cardiac function and thromboembolie events were evaluated through telephone and mail. Results The time needed for RF modified maze Ⅲ was 18.61±3.56 min. There were no hospital deaths and the complications was 15.15%(10/66). Follow-up duration was 14.25±6.47 months with 95.45%(63/66) completion. At the lastest follow-up, the rate of freedom from AF was 80.95% (51/63)and the rate of restoration to sinus rhythm was 74.60%(47/63). No thromboembolic events was seen. 77.78%(49/63) of patients were in NYHA class Ⅰ. Significant decrease was seen in both left atrial dimension (LAD) and left ventricular dimension (LVD)(P〈0.01) more than 6 months after operation. Conclusion RF modified maze Ⅲ procedure as an adjunctive procedure is safe, time-sparing and effective in eliminating AF.

  • Diagnosis and Treatment for Intramural Esophageal Dissection: Report of One Case and Literature Review

    Abstract: Objective To discuss the probable pathogenesis, clinical manifestations, diagnostic and treatment methods, and prognosis of intramural esophageal dissection (IED), in order to improve diagnostic and therapeutic levels for IED. Methods We retrospectively analyzed the clinical data of one patient suffering from circumferential intramural dissection of whole thoracic esophagus with inflammation of false lumen and localized esophageal perforation treated in the First People’s Hospital Affiliated to Shanghai Jiaotong University in February 2010. The 56 years female underwent right exploratory thoracotomy through a standard posterolateral incision in the fifth intercostal space with the whole diseased esophagus resected and the stomach anastomosed through retrosternal tunnel to the cervical intact esophagus in the left neck. Case reports with integral clinical data in recent 10 years’ literature were reviewed through PubMed searching system with the keyword being intramural esophageal dissection or intramural esophageal hematoma. Results The patient was finally cured by whole thoracic esophagectomy and discharged at postoperative day 14. Halfyear followup result was satisfactory. Thirteen cases with integral clinical data were reviewed. The major manifestations were mainly chest and dorsal pain, odynophagia and dysphagia, and occasional hematemesis. Diagnosis was mainly based on esophagography, endoscope and CT. Twelve patients were cured or remitted after conservative therapy, endoscopic therapy or surgical therapy. One patient died after surgical exploration. Conclusions IED is arare disease, and esophagography, endoscope and CT are important diagnostic methods. IED is widely regarded as benign process which responds to conservative managements and endoscopic treatments. However, in some severe cases, we suppose that removal of the diseased esophagus is more reliable.

  • Analysis of Risk Factors of Pulmonary Complications Following Lung Resection

    Abstract: Objective To analyze possible associated risk factors of postoperative pulmonary complications (PPC) after lung resection in order to decrease the incidence and mortality of PPC. Methods We reviewed the data of 302 patients including 228 males and 74 females undergoing lung resection from January 2007 to December 2009 in our department. The age of the patients ranged from 23 to 91 years old with an average age of 63.38 years. Based on the present definition of PPC, we recorded the related information and data before, during and after the operation, and observed the rate of PPC. The independent risk factors of PPC were evaluated by multiple logistic regression analysis. Results A total of 22 patients (7.28%) died during the operation and 75 patients (24.83%) experienced 110 times of PPC, the majority of which were prolonged air leak/bronchopleural fistula (8.94%, 27/302), nosocomial pneumonia (6.95%, 21/302) and acute respiratory failure (6.29%, 19/302). The results of logistic regression analysis showed that an American Society of Anesthesiology (ASA) score ≥3 (OR=2.400,P=0.020) and prolonged duration of immediate postoperative mechanical ventilation (OR=1.620,P=0.030) were independent factors associated with the development of PPC.Conclusions The ASA score based on the patients’ general condition and the function status of the main organs, and the prolonged duration of immediate postoperative mechanical ventilation are independent risk factors of PPC. In order to decrease the PPC rate, more attention should be paid to perfecting preoperative preparation, improving the function and condition of the organs, preserving pulmonary function and decreasing the duration of immediate postoperative mechanical ventilation for patients with high risk factors.

  • Clinical pathological characteristics and prognosis of 468 thymoma patients

    ObjectiveTo assess the correlation of WHO pathological classification and Masaoka stage of thymomas with its prognosis.MethodsA total of 468 patients with thymomas who received surgeries during 2009-2019 in Huashan Hospital, Fudan University, were collected. There were 234 males and 234 females with an average age of 21-83 (49.6±18.7) years. A total of 132 patients underwent video-assisted thoracic surgery (VATS) and 336 patients underwent thymectomy with median sternal incision. The follow-up time was 5.7±2.8 years. The clinical data of the patients were analyzed.ResultsThe amount of intraoperative bleeding was 178.3±133.5 mL in the median sternal incision group, and 164.8±184.1 mL in the VATS group (P=0.537). The operative time was 3.3±0.7 h in the median sternal incision group and 3.4±1.2 h in the VATS group (P=0.376). Postoperative active bleeding, phrenic nerve injury and chylothorax complications occurred in 8 patients, 9 patients and 1 patient in the VATS group, respectively, and 37 patients, 31 patients and 7 patients in the median sternal incision group, respectively. There was no statistical difference between the two groups (P=0.102, 0.402, 0.320). The 5-year cumulative progression free survival (PFS) rates of patients with WHO type A, AB, B1, B2, B3 and C thymomas were 100.0%, 100.0%, 95.7%, 81.4%, 67.5% and 50.0%, respectively (P<0.001). The 5-year PFS rates of patients with Masaoka stageⅠ-Ⅳ thymomas were 96.1%, 89.2%, 68.6% and 19.3%, respectively (P<0.001). The 5-year PFS rate was 87.3% in patients with myasthenia gravis (MG) and 78.2% in patients without MG (P<0.001). The 5-year PFS rates of patients with different surgeries were 82.4% and 83.8%, respectively (P=0.904). ConclusionWHO pathological classification and Masaoka stage have significant clinical prognosis suggestive effect. Thymoma patients combined with MG have better prognosis, which suggests early diagnosis and treatment of thymoma are important.
