  • Slow Release Microspheres of Tannic Acid Attenuate Postinfarction Remodeling in Rats

    Abstract: Objective To investigate the effect of intramyocardial injection of slow release microspheres of tannic acid (TA) on ventricular remodeling after acute myocardial infarction (AMI) in rats. Methods Slow release microspheres of TA were prepared and the release parameters in vitro were detected. AMI model in rats was induced. Eighty rats were enrolled and divided into 4 groups by random digital table:poly (lactic-co-glycolic acid) (PLGA) microspheres injection (PLGA group, n=24), PLGA-TA microspheres injection (PLGA-TA group, n=24), TA injection group (TA group, n=16) and normal saline injection group (NS group, n=16). Heart function was evaluated by echocardiography after the injection. The structure of cardiac extracellular matrix (ECM) in the infarcted borderline area was evaluated at 4th week after the injection. Collagen content in the infarcted area was evaluated by hydroxyproline colorimetry assay at 2nd and 4th week after the injection. Results TA release was maintained at a constant rate from the microspheres for one month in vitro. Two weeks after the injection, left ventricular ejection fraction(LVEF), left ventricular fraction shortening(LVFS), left ventricular end-diastolic diameter(LVEDD) and left ventricular end-systolic diameter(LVESD) in PLGA-TA group and TA group were significantly better than those in the other two groups(P<0.05). Four weeks after the injection, LVEF, LVFS, LVEDD and LVESD in PLGA-TA group were significantly better than those in the other three groups (P<0.05). Four weeks after the injection, slow release microspheres of TA in the PLGA-TA group effectively improved the arrangement of ECM compared with TA group. Four weeks after the injection, collagen content in the infarcted area of PLGA-TA group was significantly higher than that in TA group(88.88±7.28 μg/mg dry weight vs. 72.43±9.02 μg/mg dry weight), PLGA group(88.88±7.28 μg/mg drg weight vs. 71.97±6.06 μg/mg dry weight) and NS group(88.88±7.28 μg/mg dry weight vs. 68.86±7.55 μg/mg dry weight, F=7.162,P=0.003), but there was no statistical difference in the collagen content of the infarcted area among TA group, PLGA group and NS group (P>0.05) . Conclusion Intramyocardial injection of slow release microspheres of TA can maintain a constant release of TA for a comparatively long period, inhibit collagen matrix degradation, and effectively attenuate ventricular remodeling after AMI in rats.

  • Conservative in the Treatment of Protrusion of Cervical Vertebra Intervertebral Disc: A Systematic Review

    Objective To evaluate the effect and safety of the conservative treatment for the protrusion of cervical vertebra intervertebral disc. Methods We searched CBM disk (1978~2007), CNKI (1979~2007), VIP (1989~2007). Telephone interviewed the original authors of claimed randomized controlled trials(RCTs) to identify whether they are truly RCTs. Only true RCTs and quasi-randomized controlled trials were included and the quality of them was critically assessed. Results Five RCTs and eleven quasi-randomized controlled trials were identified and included. It showed that lifting-massage, massage combining with traction or acupuncture, articulus mobilization combining with traction and electrotherapy, traction combining with computer intermediate frequency therapeusis and injectio ad acumen combining with massage have marked improvement. But it could hardly draw a conclusion that all the combining therapies had better curative effect. Conclusions Weak evidence shows that combining conservative treatment can improve the curative effect of the protrusion of cervical vertebra intervertebral disc. The results need to be supported by well designed and high quality trials.

  • Dominant diseases of acupuncture in clinical efficacy: an overview of systematic reviews

    ObjectiveTo overview the systematic reviews of dominant diseases of acupuncture in clinical efficacy.MethodsPubMed, The Cochrane Library, Web of Science, CNKI, CBM, VIP and WanFang Data databases were electronically searched to collect systematic reviews or meta-analyses of dominant diseases of acupuncture in clinical efficacy from inception to December 2020. Two reviewers independently screened literature, extracted data and assessed the quality of the included studies. Then, Excel 2010 and VOS viewer were used for data analysis.ResultsA total of 263 systematic reviews were included. Acupuncture techniques involved electroacupuncture (n=29), point thread-embedding (n=25), dermal needle (n=15), acupoint injection (n=12), spoon needle (n=12), fire needle (n=8), laser (n=8), intradermal needle (n=5), filiform needle (n=4), pricking blood therapy (n=2) and round-sharp needle (n=1). A total of 94 kinds of diseases were identified, and their total effective rate ranged from 4% (bladder cancer) to 98% (bladder stones, renal colic), 72 kinds of which were above 85%.ConclusionsAcupuncture is currently widely used in clinical practice. Based on the clinical effectiveness evidence, this study finally identifies 94 types of dominant diseases. However, the total sample size and total effective rate vary considerably, and the types of acupuncture are not yet specified in this study, which requires to be focused in future research.
