  • Therapeutic Effect of Methotrexate Combined Mifepristone on Early Ectopic Pregnancy

    【摘要】 目的 探讨甲氨蝶呤(MTX)联合米非司酮治疗早期异位妊娠(EP)的临床效果。 方法 收集2006年2月-2010年2月收治的早期未破裂型EP患者126例,随机分为MTX联合米非司酮组62例,单独应用MTX组64例, MTX治疗采用小剂量分次肌肉注射给药进行。 结果 126例患者中,MTX联合米非司酮组和单独应用MTX治疗组的成功率为分别为88%和65%,两组差异有统计学意义(Plt;0.05)。 结论 MTX小剂量分次给药联合米非司酮治疗早期未破裂型EP效果优于MTX的单独使用。【Abstract】 Objective To observe the therapeutic effect of methotrexate (MTX) combined with mifepristone on early ectopic pregnancy. Methods A total of 126 patients with early ectopic pregnancy diagnosed from February 2006 to Febrary 2010 were randomly divided into two groups. In 126 patients, 62 treated with MTX combined with mifepristone were in the treatment group,and 64 treated independently with MTX were in the control group. MTX was administrated at a low dose in several times. Results In 126 patients,the success ratio of the treatment was 88% in treatment group groups and 65% in the control group; the difference between the two groups was significant (Plt;0.05). Conclusion Low-dose MTX in separate times combined with mifepriston is effective on the early ectopic pregnancy, and the therapeutic effect of the combined administration of MTX and mifepriston is better than that of the single administration with MTX.

  • Surgical Treatment for Complex Congenital Heart Disease of Newborn Babies

    Objective To summarize the experience of the surgical treatment of complex congenital heart diseases in 29 newborns. Methods Twenty-nine newborns were operated on for various congenital heart diseases, ages were from 3 to 28 d. There were ventricular septal defect 3 cases, D-transposition of the great arteries (D-TGA) 10 cases, pulmonary atresia (PA) 1 case,tricuspid atresia 3 cases, single ventricle 1 case, tetralogy of Fallot 6 cases, endocardial cushion defect 4 cases and truncus arteriosus 1 case. All patients were combined with atrial septal defect and patent ductus arteriosus. All operations were performed under hypothermic cardiopulmonary bypass. Results The operative mortality was 13.8%(4/29). One case with D-TGA and 1 case with PA succumbed due to low cardiac output syndrome, and 2 cases due to acute respiratory distress syndrome and low blood oxygen saturation. Twenty-five cases were discharged. Follow-up was completed in 19 patients,with a duration of 1-31 months, all patients have been normal. Conclusion Anesthesia and cardiopulmonary bypass should be performed smoothly and satisfactorily in newborn babies for cardiac surgery. Surgical manipulation should be accurate with less trauma and well protection of myocardium and lung is important.

  • Relationship Between Characteristic of Expression of Facilitative Glucose Transporter-1 in Bronchial Aveolar Carcinoma and Fluorine-18 Fluorodeoxyglucose Uptake

    Objective To investigate the characteristic of the expression of facilitative glucose transporter 1 (Glut1) in bronchial aveolar carcinoma (BAC) and the relationship between expression of Glutl and fluorine-18 fluorodeoxyglucose (18F-FDG) uptake. Methods Twenty patients with BAC were imaged with 18F-FDG positron emission tomography (PET) before surgery. Maximum standard uptake value (SUVmax) and mean standard uptake value (SUVmean ) of tumor and standard uptake value of normal lung (SUVIung) were measured. The expression of Glutl was studied in paraffin sections by streptavidin peroxidase (SP) immunohistochemistry. Results All tumors of the patients could be detected by ^18F-FDG-PET. 18F-FDG uptake of tumor was higher than that of normal lung (SUV SUV and SUVlong were 3.09± 1.35, 2.37±1.03 and 0.39±0.09 respectively). The expression of Glutl were positive in all tumors (73. 8%± 14. 8% of positive cell rate, 2. 8 ± 0. 9 grade of staining intensity). Predominantly cytoplasm positive with weak staining intensity were observed in many tumors. Glutl negtive were observed in normal bronchial and lung parenchyma. Positive correlations were found among 18F-FDG uptake, Glut1 expression and tumor size (P〈0. 01). Conclusion Glutl expression with peculiarity in BAC is correlate with 18F-FDG uptake.

  • EXPERIMENTAL STUDY OF MECHANISM AND MEASURES OF PREVENTION AND TREATMENT OF HYPOTHERMAL VASOCONSTRICTION

    OBJECTIVE: To investigate the mechanism and to explore the measures of prevention and treatment of hypothermal vasoconstriction. METHODS: By the techniques of endothelial cell culture and scanning electron microscopy, and vasomotor functional test of isolated vascular vessels, the relation of hypothermal vasoconstriction and the release of endothelium-derived contractile and vasodilative factors were observed. RESULTS: Hypothermia obviously induced vasoconstriction of isolated vascular vessels, whether endothelium was intact or removed, the lower the temperature, the higher the vascular tension. Removal of endothelium could decrease the effect of vasoconstriction by hypothermia. The conditioned medium of bovine aortic endothelial cell could induce significantly vasoconstriction of isolated rat common neck arterial ring in hypothermia. It indicated that the bovine aortic endothelial cells secreted contractile factors into the medium. Reheating to 37 degrees C or vasodilator or reheating plus vasodilator did not obviously influence the hypothermia-induced vasoconstriction within 2 hours. When reheating to 50 degrees C, vascular tension was decreased, but only changed in range of 28% to 42%. CONCLUSION: Hypothermia vasoconstriction is relative to vasoconstrictor factors secreted by endothelium. Reheating to 37 degrees C or vasodilator does not antagonize the constriction of vascular vessels. Reheating to 50 degrees C only partially eliminates the constrict effect of blood vessels, so the prevention of hypothermia vasoconstriction should be emphasized.

  • Progress of generation mechanisms of auditory steady-state response

    The scalp-recorded auditory steady-state response (ASSR) is a periodically evoked potential in response to the stimulation with the acoustical property in the same period. The ASSR can be readily induced in comparison with transient responses for specific conditions. The clinical utility of ASSR may be unjustified for the ambiguity of the genesis. With the advance of relevant research, it is considered that the main generation hypotheses of the ASSR are conceived to be pertinent with the linear superposition or neural entrainment mechanism. Based on current findings and our contributions in this field, we introduce recent progresses of the two mechanisms with comments, and suggest the benefit of the rapid stimulation technology in this regard.

  • The necessity of gastrointestinal decompression after Ivor Lewis esophagectomy: A randomized controlled trial

    Objective To explore the feasibility and safety of non-gastrointestinal decompression after esophagectomy and the necessity of gastric tube or the time to remove gastric tube. Methods Thirty patients with esophageal cancer who underwent surgical treatment in the Department of Thoracic and Cardiovascular Surgery, Nanjing Drum Tower Hospital, were included in the trial from June to October 2017. The patients were randomly and equally assigned to a trial group (non-gastrointestinal decompression) or a control group (gastrointestinal decompression). There was no significant difference in age (P=1.000), sex (P=1.000), tumor location (P=0.732), pathological type (P=1.000), pathological stage (P=0.507), and operation time (P=0.674) between the two groups. The clinical effect between the two groups were compared. Results There was no statistical difference in incidences of anastomotic leakage (P=1.000), anastomotic bleeding (P=1.000), gastroesophageal reflux (P=1.000) between the two groups. And there was no statistical difference in time of the first flatus (P=0.629) and the first bowel movement (P=0.599) after operation between the two groups. Conclusion Without gastrointestinal decompression after Ivor Lewis esophagectomy does not increase the incidences of anastomotic leakage, anastomotic bleeding and gastroesophageal reflux, and has no significant effect on the recovery of gastrointestinal function. Without gastrointestinal decompression after Ivor Lewis esophagectomy is safe and feasible. Removing gastric tube on the second day after operation is reasonable and feasible.

In recent ten years, with the development of surgical techniques and medical instruments, tubular stomach has been widely used. Although the advantages of tubular stomach are becoming more and more obvious, there are still many details and problems for the function and application of tubular stomach worthy of further discussion and study. In this paper, the technical progress, advantages, functions and applications of tubular stomach are reviewed and discussed, and the future prospect is predicted.

  • Efficacy of exercise intervention in adolescents with mild to moderate idiopathic scoliosis: a meta-analysis

    ObjectiveTo systematically review the efficacy of exercise intervention in adolescents with mild to moderate idiopathic scoliosis (AIS). MethodsPubMed, Web of Science, The Cochrane Library, WanFang Data and CNKI databases were electronically searched to collect randomized controlled trials (RCTs) on the efficacy of exercise intervention in adolescents with mild to moderate AIS from inception to November 2021. Two reviewers independently screened the literature, extracted data and assessed the risk of bias of the included studies. Meta-analysis was then performed using Stata 16.0 software. ResultsA total of 11 RCTs involving 638 patients were included. The results of meta-analysis showed that exercise intervention significantly improved the Cobb angle of the spine in patients with AIS (MD=−1.65, 95%CI −2.63 to −0.68, P<0.01), reduced the angle of trunk rotation (ATR) (MD=−0.68, 95%CI −0.96 to −0.40, P<0.01), and increased their forced vital capacity (FVC) (MD=0.63, 95%CI 0.10 to 1.15, P=0.02). However, there was no statistically significant improvement in the forced expiratory volume in the first second (FEV1) or the ratio of FEV1/FVC between the two groups. ConclusionCurrent evidence shows that the exercise intervention could improve the Cobb angle of the spine, reduce ATR and enhance FVC. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusion.

  • Early and mid-term results of Fontan operation versus anatomic correction for congenitally corrected transposition of the great arteries: A retrospective cohort study in a single center

    Objective To compare the early and mid-term results between Fontan operation and anatomic correction for congenitally corrected transposition of the great arteries (ccTGA). Methods The clinical data of 53 patients with ccTGA who underwent anatomic correction and Fontan operation from January 2009 to September 2021 in our hospital were reviewed, including 41 males and 12 females with a mean age of 55.02 (3-168) months. They were divided into an anatomic correction group (16 patients) and a Fontan operation group (37 patients) according to the operation. The hospitalization mortality, survival rate, postoperative complications, and free rate from re-intervention between the two groups were compared. Another 180 healthy children were recruited as a control group, and 14 children were matched with the propensity score matching method as a Fontan control group. The results of cardiopulmonary exercise testing (CPET) between the Fontan operation group and the Fontan control group were compared. Results There were 2 (12.5%) early deaths and 3 (18.8%) early re-intervention in the anatomic correction group, while 1 death and 2 re-intervention in the Fontan operation group. In addition, there were 9 patients (56.3%) in the anatomic correction group and 6 (16.2%) patients in the Fontan operation group suffering from arrhythmia after operation, respectively. Compared with the anatomic correction group, cardiopulmonary bypass time, aortic cross-clamping time, intubation time and ICU stay were significantly shortened in the Fontan operation group (P<0.05). CPET results showed that, percent predicted max VO2 in the Fontan operation group was lower than that in the Fontan control group (0.84±0.11 vs. 0.99±0.12, P<0.05). The patients were followed up for 0.5-126.0 months. Two patients were lost in the Fontan operation group. There was no death and 1 re-intervention in the anatomic correction group, while no death or re-intervention in the Fontan operation group. The 1-year, 5-year and 10-year transplant-free survival rate of the anatomic correction group and the Fontan operation group was 87.5%, 87.5%, 87.5% and 97.3%, 97.3%, 97.3%, respectively (P>0.05). The 48 patients were classified as grade Ⅰ-Ⅱ in cardiac function in the last follow-up. Conclusion There is no statistical difference in the transplant-free survival rate between the anatomic correction and the Fontan operation group. The postoperative complications in the Fontan operation group are decreased than those in the anatomic correction group. The Fontan operation is also a good choice, even though the patients with ccTGA meet the condition of the procedure of anatomic correction.

  • Research progress of NETs in hepatic ischemia-reperfusion injury

    ObjectiveTo summarize the mechanism of neutrophil extracellular traps (NETs) in hepatic ischemia-reperfusion injury (HIRI) and the research progress in targeting NETs to reduce HIRI, providing valuable reference for reducing HIRI. MethodThe related literatures at home and abroad about the role of NETs in the pathogenesis of HIRI and target NETs to alleviate HIRI were retrieved and reviewed. ResultsHIRI usually appeared in the process of liver surgery and was a common clinical problem, which occured in situations such as liver surgery, organ transplantation, liver ischemia and so on. This kind of injury would lead to tissue necrosis, inflammatory response and oxidative stress, which was a major cause of hepatic dysfunction and multiple organ failure after hepatic surgery, greatly increases the complications and mortality after hepatic surgery. NETs played a crucial role in the aseptic inflammatory response induced by hepatic ischemia/reperfusion. During hepatic ischemia-reperfusion, neutrophils promoted inflammatory cascade reactions and cytokine storms by forming NETs, exacerbating damage caused by hepatic ischemia-reperfusion. At present, some experimental and clinical studies had shown that inhibiting the formation of NETs or eliminating the formed NETs could alleviate the hepatic ischemia-reperfusion injury and improve the prognosis. ConclusionsTargeting NETs may become a new method for treating hepatic ischemia-reperfusion injury. In the future, it is foreseeable that more experiments and clinical trials will be conducted on targeted NETs for the treatment of hepatic ischemia-reperfusion injury. And gradually establish more comprehensive and effective treatment strategies, thereby providing new ways to improve the prognosis of hepatic surgery patients in clinical practice.
